Optical imaging of the optic nerve: beyond demonstration of retinal nerve fiber layer loss.

Although we are still early in the evolution of optical imaging of the optic nerve, the available techniques already play an important role in clinical decision making. I would summarize our findings to date as follows: For acute ON: Presentation: OCT shows RNFL swelling, normal GCL + IPL by OCT; 1 month: OCT and SLP show RNFL thinning and swelling, GCL + IPL thinning by OCT; 3 months or later: OCT and SLP show RNFL thinning, further mild GCL thinning by OCT; 6 months: RNFL and GCL + IPL thinning finished. For acute NAION: Presentation: OCT shows RNFL swelling and SLP shows loss of birefringen

[Electrophysiological methods in the diagnosis and monitoring of glaucoma].

Primary open-angle glaucoma (POAG) is characterized by chronic progressive damage to the retinal ganglion cell layer (GCL) and their axons, leading to gradual visual function loss. Currently, the gold standards for structural and functional assessment of the retina in glaucoma are static automated perimetry (SAP) and optical coherence tomography (OCT). However, in clinical practice, data from SAP and OCT may be insufficient to reliably determine the stage of glaucomatous optic neuropathy, monitor its progression, or differentiate it from other causes of visual dysfunction. For these purposes w

If at first you don't succeed, try, try again.

A previously healthy 15-year-old Asian boy presented with progressive bilateral painless vision loss, dyschromatopsia, and visual field defects. Optical coherence tomography showed diffuse retinal nerve fiber layer thinning. Work-up including neuroimaging, lab testing for acquired bilateral optic atrophy, extensive mitochondrial and nuclear genetic testing yielded negative results. Several years after initial presentation, his paternal uncle tested positive for an autosomal dominant variant in the Ubiquitin C-terminal hydrolase L1 (UCHL1) gene. This led to reanalysis of the patient's previous

Methanol poisoning: two case studies of blindness in Indonesia.

We report two recent cases of methanol intoxication in French patients living in Bali. These intoxications were secondary to the consumption of adulterated liquor. Both patients presented acute bilateral loss of vision a few days after methanol ingestion with no sign of recovery. The fundus showed bilateral optic atrophy that was well correlated with retinal nerve fibre layer thickness measured with spectral-domain optical coherence tomography and compatible with toxic optic neuropathy. In one of the patients, macular swelling in one eye was observed. Electroretinograms were normal contrasting

Performance of Artificial Intelligence-Based Models for Epiretinal Membrane Diagnosis: A Systematic Review and Meta-Analysis.

TOPIC: Epiretinal membrane (ERM) can impair central vision by forming a pre-retinal fibrous layer on the inner retina. Artificial intelligence (AI)-based tools may streamline ERM diagnosis, but their overall performance and factors affecting accuracy require evaluation. CLINICAL RELEVANCE: With an aging population, ERM prevalence is expected to rise, placing increased demands on clinical resources. Early detection via AI models could expedite diagnosis, reduce subjective errors, and guide timely surgical intervention. This systematic review and meta-analysis evaluates the pooled diagnostic pe

Retinal thinning in epilepsy: A meta-analysis.

BACKGROUND AND OBJECTIVES: Epilepsy has been linked with neurodegenerative changes, including neuronal and axonal loss. Epilepsy-related neuronal loss may be reflected on a retinal level as retinal thinning. Several Optical Coherence Tomography (OCT) studies have reported varied and contradicting results with regard to retinal thickness changes in epilepsy. We conducted a meta-analysis to elucidate retinal layer thickness changes in adult and pediatric epilepsy as assessed by OCT and to examine the relationship of these changes with disease, patient, and treatment-related factors. METHODS: Us

Comparative Evaluation of Field of View Across Widefield Retinal Imaging Systems.

PURPOSE: Ultra-widefield retinal imaging is critical for diagnosing peripheral retinal diseases, but discrepancies in field of view (FOV) definitions hinder system comparisons. This study aims to evaluate the FOV performance of six widefield or ultra-widefield retinal imaging systems using a simplified eye phantom model and in vivo demonstration in a healthy subject. METHODS: A custom eye phantom model with concentric ring patterns and geometric markers was used to assess FOV and distortion across six imaging systems, including Nikon Optos, Natus RetCam, Intalight Dream OCT, Siloam Vision's c

Pathologic Myopia.

Pathologic myopia is defined by a refractive error exceeding -6.00 diopters with associated degenerative retinal changes due to axial elongation. Key risk factors include increased age, axial length, and high myopic spherical equivalent. Clinical presentation often begins in childhood and may include visual symptoms like scotomas and metamorphopsias. Fundus findings may include posterior staphyloma, tilted optic disc, and chorioretinal atrophy, among others. Advanced complications include lacquer cracks, Fuchs spots, and myopic traction maculopathy. Imaging modalities like OCT, FA, and OCT-A a

Clinical histopathology and pathogenesis of macular telangiectasia type 2.

Macular telangiectasia type 2 (MacTel) is a rare neurodegenerative retinal disease defined by a unique combination of reduced macular pigment, a characteristic angiographic pattern, and a localized clinical presentation. The condition typically affects the temporal perifovea and leads to progressive visual impairment. No definitive treatment exists. Current management is limited to intravitreal anti-vascular endothelial growth factor (VEGF) injections for end-stage neovascular complications and the recently approved Encelto implant (Neurotech Pharmaceuticals, Inc.), which delivers ciliary neur

Choroidal vascularity index as a marker of health and disease: systematic review and meta-analyses.

The Choroidal Vascularity Index (CVI), derived from optical coherence tomography (OCT), has emerged as a potential biomarker for detecting vascular changes. Understanding its variability across physiological states, ocular conditions, and systemic diseases is crucial for its integration into clinical practice. We evaluated variations in CVI across different physiological states (e.g., first-trimester pregnancy), ocular conditions (e.g., age-related macular degeneration[AMD]), and systemic diseases (e.g., diabetes mellitus) compared to healthy controls. From 1210 identified articles, 63 studies

Current advances in dome-shaped macula and associated conditions.

Dome-shaped macula (DSM) is a distinctive anatomical entity characterized by an inward convexity of the macula, initially described in highly myopic eyes within posterior staphyloma, but is now recognized as occurring across a broader spectrum of refractive conditions, including mild myopia and even emmetropia. Since its initial description in 2008, advances in imaging technologies and longitudinal studies have significantly improved our understanding of DSM. We analyzed the recent literature, focusing on publications from the last 10 years. DSM affects about 0.2 % of the general population, w
