A case of a middle fossa arachnoid cyst which spontaneously disappeared is reported. A 16 year old boy visited our out-patient clinic complaining of headache after a blunt head trauma by a motorcycle accident. The neurological examination was negative except for mild neck pain on the flexion and extension of neck. Plain x-ray of skull and neck were negative. Medial dislocation of the patella is a previously unreported entity. This disorder can be disabling to the patient and may require a hospital visit for reduction. Three cases are presented in this article in which computed tomography demonstrated the dislocation. All three patients had undergone a lateral retinacular release to the involved knee for treatment of chronic knee pain or recurrent lateral patellar subluxation.

Juvenile intervertebral disc calcification is an uncommon disorder of childhood, characterized by calcification of the nucleus pulposus of one or more intervertebral discs. Calcification may remain dormant or subsequently become symptomatic. Actinomyces species are rare but treatable causes of central nervous system infection. A case of actinomycotic cerebral abscess is reported. The primary source of infection was probably the lung based on radiological findings although culture of the bronchial brushings was negative. Emphysematous pyelonephritis is a rare condition which in most cases occurs as a result of Escherichia coli infection of the urinary tract in patients known as being diabetic. The gas-filled abscesses carry a high mortality rate, especially when the antibiotic treatment is not combined with early surgery. Progression represents the qualitative tumor evolution during the time. Recently it received great consideration, mainly since the introduction of the neuroradiological imaging (CT and MRI). A series of 224 supratentorial astrocytic tumors of the juvenile and adult age have been analyzed: we found the morphological features of progression in 26% of them. These data introduce, perhaps, the most important information about progression: the "crisis" of the concept that low-grade tumors are stable lesions during the time.

An adult female patient presented with an exclusively spinal leptomeningeal infestation by cysticercosis (restricted to the cauda equina), verified in surgery. The rôle of the vertebral vein system in the spread of cysticercosis larvae is discussed. A commentary on the possibility that cysticercosis of the basal cisterns may be due to ascending migration of primarily spinal cysticerel, as originally proposed by Isamat de la Riva, is stated.

Retroperitoneal fibrosis is the formation of a fibrotic plaque in the prelumbar or presacral area which becomes clinically significant when it produces ureteral obstruction. It can occur as a result of taking certain drugs, retroperitoneal fluid collections, inflammatory disease of various intraabdominal organs, previous surgery or radiation therapy, and various metastatic neoplasms. The majority of the causes of retroperitoneal fibrosis are considered idiopathic.

We report the case of a rapidly expanding mediastinal and chest wall lipoblastoma in an 11-month old infant. Clinical presentation was respiratory distress and chest radiographs revealed a large soft tissue mass in the anterior mediastinum without specific features. CT scanning demonstrated a mass of principally fat attenuation, with enhancing hyperdense soft tissue whorls, which had a compressive effect on normal intrathoracic structures. At surgery the tumour was removed without complication and histology demonstrated an encapsulated, well differentiated lipoblastoma.

Particular difficulties arise when trying to classify mediastinal fibrosis and determining its etiology and clinical evolution. In this paper, the Authors describe a case of mediastinal fibrosis which was characterized by a specific clinical origins (mild fever-anemia). It was not possible to establish the real etiology of the mediastinal lesion which in the end led to an unfavourable outcome that was linked to the local development of the illness, in spite of its benign histological characteristics.

A case of tuberculosis initially involving the vertebral arch is presented, and this rare mode of presentation is contrasted to more typical radiographic presentation of Pott disease. The role of CT in delineating the extent of disease and in detecting complications such as paravertebral abscesses and cord compression is illustrated specifically and through a literature review.

Maffucci's syndrome is a particularly rare clinical form of mesodermal dysplasia. It consists of angiomatosis, chondromatosis and sometimes dyschromic cutaneous lesions. The syndrome is usually associated with a high incidence of malignancy, originating primarily from mesodermal derivatives. The present case is the second in the literature in which the syndrome is associated with a frontal lobe astrocytoma. This patient and those reported in the literature, suggest the potential involvement of non-mesodermal tumors in the Maffucci's syndrome.

The combination of a parapharyngeal abscess and torticollis in an adult, due to a spastic contraction of the sterno-cleido-mastoid muscle is an extremely rare condition. We have recently been exposed to such case, in which it was very difficult to make the diagnosis and where CT of the neck gave us the answers without any difficulty. The indication for CT at an early stage in unexplained neck infections is stressed.

Whipple's disease is a rare protean disease. Cerebral involvement occurs in ten percent of the cases. CCT findings in two patients with cerebral symptoms are presented. There was an unspecific atrophy in one patient. Patient two had hydrocephalus occlusus and a temporal lesion enhanced by contrast agent. A specific diagnosis on the sole basis of the CCT without additional clinical data does not seem possible.
