Skin microcirculation in patients with sequelae from local cold injuries.

The microcirculation in the skin was assessed in 31 patients with sequelae from local cold injuries (LCI) in the extremities. All patients reported cold intolerance 3-4 years after the primary cold injury, which they sustained during military service. The used methods were laser Doppler fluxmetry, transcutaneous oxygen tension (TCpO2) and vital capillaroscopy. Neurovascular reflexes were stimulated by deep inspiration, digital cuff occlusion of venous and arterial circulation, neck cooling with an ice bag and water immersion at 5 and 15 degrees C. Unaffected lower or upper extremities were als

A novel radioligand [125I]BQ-3020 selective for endothelin (ETB) receptors.

A linear endothelin (ET) analog, N-acetyl-LeuMetAspLysGluAlaValTyrPheAlaHisLeu-AspIleIleTrp (BQ-3020), is highly selective for ETB receptors. BQ-3020 displaces [125I]ET-1 binding to ETB receptors (nonselective to ET isopeptides) in porcine cerebellar membranes (IC50: 0.2nM) at a concentration 4,700 times lower than that to ETA receptors (selective to ET-1) on aortic vascular smooth muscle cells (VSMC) (IC50: 940nM). BQ-3020 as well as ET-1 and ET-3 elicits vasoconstriction in the rabbit pulmonary artery. The ETA antagonist BQ-123 failed to inhibit this BQ-3020-induced vasoconstriction. Further

Tissue pressure and blood flow in pulpal inflammation.

The initial vascular reactions during inflammation are vasodilation and increased vessel permeability. Both these basic reactions cause increased pulpal fluid volume. In the dental pulp the inflammatory vascular reactions take place in a rigid enclosed dentin chamber, which to some extent makes the pulp vulnerable. Due to this lack of distensibility any gain in pulpal volume will necessarily increase the pulpal tissue pressure. If the tissue pressure rises to the same level as the blood pressure it will compress the pulpal vessels, thus counteracting a beneficial blood flow increase during inf

Effects of desflurane on the central nervous system.

Desflurane causes dose-dependent decreases in cerebrovascular resistance and cerebral metabolic rate of oxygen consumption (CMRO2), suggesting that desflurane is a cerebral arteriolar dilator with global flow-metabolism coupling similar to halothane and isoflurane. Desflurane is also similar to isoflurane in that cerebrovascular responsivity to carbon dioxide appears to be maintained. In the dog, arterial hypotension to 40 mm Hg induced with 2.4 MAC desflurane resulted in global decreases in cerebral blood flow of 60% and CMRO2 of 20%. Concentrations of cerebral metabolites of high-energy phos

Cerebral blood flow during experimental epidural bleeding in swine.

Regional cerebral blood flow (rCBF) was studied during an aggressive epidural bleed, using a ventilated swine model. rCBF, regional organ blood flow and cardiac output were measured using the radioactive microsphere technique. Blood flows were measured prior to the start of bleeding (Stage 1), when intracranial pressures had reached a plateau and supratentorial perfusion pressure was reduced by about 50% (Stage 2), and at isoelectric EEG (Stage 3). Supratentorial rCBF did not change significantly between stages 1 and 2 while rCVR decreased, implying autoregulatory activity. Cerebral ischaemia

[Quality of life of patients with hypertension treated with converting enzyme inhibitors].

Converting enzyme inhibitors (CEIs) are widely used in treatment of essential hypertension. Large-scale clinical studies have shown that CEIs are well tolerated and cause fewer side effects than most other antihypertensive agents. The latter observation is fundamental for compliance with long-term treatment. There do exist, however, some side effects which although rare are not negligible. It is necessary though to distinguish between side effects linked to the class of therapeutic agents and those associated with particular structural features. Three types of side effects have been seen: 1) m

Pharmacological reversibility of experimental cerebral vasospasm.

Using a morphometric technique, the pharmacological reversibility of luminal narrowing after experimental subarachnoid hemorrhage (SAH) was investigated. For vasodilation, a "cocktail" consisting of 10(-4) M papaverine, 2 x 10(-4) M sodium nitroprusside, and 10(-5) M adenosine was administered intra-arterially. Forty-two rabbits were divided into six groups: control (normal animals); control plus cocktail (normal animals perfused with the cocktail before fixation); SAH (animals sacrificed 48 hours subsequent to intracisternal injection of 1.5 ml/kg of arterial blood); SAH plus cocktail (SAH pl

Effects of thromboxane synthesis inhibitor triflusal on renal hemodynamics in microalbuminuric diabetic patients.

Triflusal (2-acetoxy-4-trifluormethylbenzoic acid) is a platelet-antiaggregant drug that selectively inhibits thromboxane synthesis with little effect on prostacyclin production. In this study, we evaluated the effect of 5-day administration of 900 mg/day triflusal on glomerular filtration rate (GFR), renal plasma flow (RPF), urinary albumin excretion (UAE), thromboxane B2 (TXB2), 6-ketoprostaglandin F1 alpha (PGF1 alpha), and PGE2 in nine normotensive insulin-dependent diabetic patients with UAE between 30 and 103 micrograms/min. Plasma TXB2 and plasma renin activity (PRA) were also determine

Vasodynamic and angiogenic effects of eicosanoids in the eye.

In the eye, the main vascular response to PGs is vasodilation. Although its effect has not been studied in detail, TxA2 can be expected to cause vasoconstriction in the eye, as it does in other tissues. When applied in high doses, PGs also increase the permeability of the microvasculature in the anterior segment, especially in rabbits. This may contribute to protein leakage associated with disruption of the BAB. The peptidoleukotrienes seem to reduce blood flow in the anterior segment, although more data will be required to substantiate this observation. It has been suggested over the years th

Studies on the salivary secretion induced by substance P in perfused submandibular gland of rat.

The purpose of these experiments was to clarify the effects of substance P (SP) on the salivary secretion in comparison with those of autonomic agents. Salivary secretion from rat submandibular gland (SMG) was induced by intravenous infusion into the femoral vein or partial perfusion of SMG with isotonic solution containing SP or autonomic stimulants. Blood and salivary flow rates, and electrolyte concentrations of saliva were measured. The relationship between electrolyte concentrations and salivary flow rate after intravenous infusion of SP was similar to that obtained using parasympathomime

Increased circulating atrial natriuretic factor (ANF) release during induced ventricular tachycardia.

During sustained ventricular tachycardia (VT), there is usually a rise in mean atrial pressure. Atrial natriuretic factor (ANF) is a peptide released by the atria in response to increased atrial pressure or distension. ANF causes arterial vasodilation and natriuresis in man, and may contribute to the polyuria noted during tachycardia. We hypothesized that induction of sustained VT would cause elevation of right atrial pressure and lead to increased release of ANF. We measured mean arterial pressure, right atrial pressure, VT cycle length, atrial cycle length, plasma norepinephrine, and central

Direct and adaptive cytoprotection.

Cytoprotection confers increased cellular resistance to various damaging challenges. For example, the administration of certain prostaglandins (PG) prevents injury or facilitates recovery from injury in tissues exposed to noxious substances, such as ethanol, aspirin, and indomethacin. In addition, naturally occurring PG (E and I types) may play a physiological role in protecting the gastroduodenal mucosa against corrosion by gastric juice. Within a responsive tissue, not all cells may be protected by PG against severe damage. Thus, while PG will not prevent necrosis of the gastric epithelial m

5-HT, alpha-adrenoceptors, and blood pressure. Effects of ketanserin in essential hypertension and autonomic insufficiency.

The serotonin (5-hydroxytryptamine, 5-HT) antagonist, ketanserin, has a high affinity for 5-HT2-receptors but it also binds to alpha 1-adrenoceptors. The compound (10 mg i.v.) lowered mean arterial pressure by 22% +/- 2% (mean +/- SEM, p less than 0.001) in 30 patients with essential hypertension. Measurements of heart rate, cardiac output, cardiac filling pressures, forearm blood flow, renal blood flow, and glomerular filtration rate revealed a hemodynamic pattern compatible with vasodilation of both resistance and capacitance vessels. This was accompanied by moderate reflex cardiostimulation

Furosemide fails to alter plasma active or inactive renin in conscious sheep but does so in anaesthetized animals.

Regulation of plasma levels of active and inactive renin was investigated using sheep with indwelling artery, vein and bladder catheters. Control and experimental studies were carried out in the same animals on different days. Volume depletion during any single experiment was limited to a maximum of 50 ml. Despite large changes in sodium and water excretion, the diuretic furosemide at two dose levels, 1 and 10 mg/kg, failed to alter plasma levels of either active or inactive renin in conscious sheep. Induction of pentobarbitone anaesthesia in the sheep did not, per se, alter either plasma acti

The cortisol fluctuations in plasma in relation to human regulatory nonshivering thermogenesis.

It is known that many stressful situations are accompanied by an increase in plasma cortisol concentration. This study was undertaken to see if a standardized and moderate exposure to cold under simultaneous peripheral vasodilation, which is known to result in increased oxygen consumption and plasma concentration of noradrenaline, can be considered a stress, based on changes in plasma cortisol concentration. Since the nonshivering thermogenesis induced is dependent on normal thyroid function, nine of the 19 adult humans exposed to cold were normo-, four hyper-, and six hypothyroid. The cold ex

Intra-operative haemodynamic findings and their prognostic significance in femoropopliteal reversed saphenous vein graft bypass operations.

Post-reconstructive graft blood flow (basal flow') was measured by means of an electromagnetic flowmeter in 72 consecutive femoropopliteal reversed saphenous vein grafts performed for occlusive atherosclerosis causing leg ischaemia. In the last 48 operations, post-reconstructive pre- and post-graft intra-arterial pressures were recorded, and graft blood flow after papavarine-induced vasodilation ('maximal flow') was measured. Mean 'basal flow' was 132 ml/min, mean 'maximal flow' 285 ml/min and mean flow increment after papavarin 136%. Mean 'basal flow', mean 'maximal flow' and mean flow increm

Interaction of adrenergic antagonists with prostaglandin E2 and tetrahydrocannabinol in the eye.

Both alpha- and beta-adrenergic antagonists have been utilized in an atempt to discern the site of action of prostaglandin (PG) and tetrahydrocannabinol (THC) in the eye. Both alpha- and beta-adrenergic antagonists (alpha-antagonists, phentolamine and phenoxybenzamine; beta-antagonists, propranolol and sotalol) cuased a dose-dependent reduction in intraocular pressure and blood pressure and increased total outflow facility. Beyond Oral Therapies: A Practical Approach to Overcoming Barriers to Intracavernosal Injection Therapy in Clinical Practice.

The objective of this manuscript is to present intracavernous therapy to readers, serving as a model and guide for physicians on how to introduce and manage it in their patients. Furthermore, we propose a strategy designed to ensure maximum safety with minor complications and provide an effective implementation of intracavernosal injection (ICI) therapy for the management of erectile dysfunction (ED). While phosphodiesterase type 5 inhibitors (PDE5i) are recognized as first-line therapy, approximately one-third of patients do not achieve satisfactory clinical outcomes, highlighting the importa

Should Caffeine Be Avoided Following Free Flaps: Fact or Fiction?

Many microsurgeons recommend that their patients avoid all compounds containing caffeine after free tissue transfer, however, there is little in the literature to implicate caffeine as a contributor to flap loss. While caffeine has historically been viewed as a vasoconstrictor that could theoretically impair flap perfusion, its physiological effects are complex, involving both vasoconstrictive and vasodilatory mechanisms. This review aims to determine whether caffeine consumption may increase flap-related complications including ultimate failure.A narrative review was conducted through PubMed
