Machine learning-based prediction of early invasive mechanical ventilation in ICU patients with pneumonia: Development and external validation.

BACKGROUND: Pneumonia is a common critical illness in the intensive care unit (ICU), and a subset of patients rapidly progresses to respiratory failure requiring invasive mechanical ventilation (IMV). Current decisions often rely on fragmented clinical indicators, highlighting the need for integrated, data-driven tools for early risk assessment. METHODS: We conducted a retrospective cohort study using the Medical Information Mart for Intensive Care Ⅳ (MIMIC-Ⅳ) database as the development cohort. The cohort was randomly split into training (80%) and test (20%) sets. Candidate predictors at ICU

Risk factors for mechanical ventilation and mortality among hospitalized Medicaid patients with COVID-19 in Kentucky, 2020-2021.

This retrospective cohort study examined patient characteristics and comorbidities associated with mechanical ventilation and mortality among 28,128 hospitalized Kentucky Medicaid patients aged 18-64 years with COVID-19 in 2020-2021. Logistic regression estimated adjusted odds ratios (aORs) for mechanical ventilation and Cox regression estimated adjusted hazard ratios (aHRs) for mortality, controlling for demographics, rurality, and comorbidities. Mechanical ventilation was required in 18.8% of patients, and 8.4% died. Older age (45-54: aOR = 1.25; aHR = 1.28; 55-64: aOR = 1.35; aHR = 1.61), m

Ventilation-Associated Differences in Lower Airway Microbial Signatures and Peripheral Blood Transcriptome Among Critically Ill COVID-19 Patients.

BACKGROUND: Invasive mechanical ventilation (IMV) is a critical intervention for severe respiratory failure and has been widely used in the clinical management of COVID-19 patients. The relationship between such microbiota changes and the host transcriptome in IMV patients remains poorly documented. METHODS: We prospectively enrolled 69 critically ill COVID-19 patients, among whom 41 received IMV. Correlation analyses were conducted to investigate the relationship between the lung microbiome and host immune status in IMV COVID-19 patients. RESULTS: Compared to the Non-mechanical ventilation

Ultra-low versus low tidal volume ventilation in acute respiratory distress syndrome: a systematic review and meta-analysis.

BACKGROUND: Low tidal volume (LTV) ventilation improves outcomes in acute respiratory distress syndrome (ARDS), whereas ultra-low tidal volume (ULTV) ventilation may further reduce lung stress but often requires extracorporeal life support (ECLS) to maintain gas exchange. Existing studies differ in ARDS severity, ECLS modality, and indications for ECLS initiation. We aimed to compare ULTV versus LTV ventilation in patients with ARDS. METHODS: A systematic review and meta-analysis of studies published up to April 2024 was conducted using PubMed, EMBASE, Cochrane, and Web of Science databases.

The Relative Contributions of Arousal Presence and Arousal Intensity to Post-Respiratory-Event Ventilation in Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is characterised by cyclical respiratory events followed by hyperpneic breaths that frequently coincide with cortical arousals. Whilst this post-event hyperventilation partly reflects accumulated respiratory stimuli, arousal itself appears to contribute independently through both its presence and its intensity. This study aimed to quantify the relative contributions of arousal presence and arousal intensity to post-event ventilation, independent of chemoreflex-driven responses. Two retrospective polysomnography data sets were analysed: a community-based cohort (Mu

Evaluation of total VOC exposure and ventilation controls in funeral home preparation rooms: A case study from Grenada.

Funeral home (FH) preparation rooms pose potential occupational health hazards due to exposure to volatile organic compounds (VOCs) emitted during embalming and autopsy procedures. This analytical observational study assessed indoor air quality (IAQ) hygiene conditions in all three funeral homes operating in Grenada using total volatile organic compound (TVOC) concentrations as an exposure indicator. Air sampling was conducted during 18 embalming and autopsy sessions between February and June 2025 using a GrayWolf AdvancedSense Pro monitor with a photoionization detector. Concurrent measuremen

Equivalent cumulative energy yields unequal lung injury: effects of tidal volume and ventilation duration in experimental lung injury.

Mechanical ventilation contributes to lung injury in acute respiratory distress syndrome, yet whether cumulative mechanical energy, the time-integrated delivery of ventilatory power, adequately reflects the risk of ventilator-induced lung injury (VILI) remains uncertain. Because lung tissue exhibits nonlinear stress-strain behavior, the rate and amplitude of energy delivered may be as relevant as its magnitude. We tested whether different combinations of tidal volume (VT) and ventilation duration, matched for cumulative energy, produce distinct patterns of VILI following endotoxin-induced lung

Effect of salbutamol during one-lung ventilation in patients with chronic obstructive pulmonary disease: a randomized controlled trial.

PURPOSE: Ventilation-perfusion mismatch during one-lung ventilation (OLV) for thoracic surgery increases the risk of hypoxemia. Salbutamol may selectively dilate pulmonary vessels and enhance perfusion of the ventilated lung. We sought to investigate whether selective salbutamol nebulization to the ventilated lung during OLV improves gas exchange and respiratory mechanics in patients with chronic obstructive pulmonary disease (COPD). METHODS: In this prospective randomized controlled trial, we randomly allocated 90 patients scheduled for lung resection to receive salbutamol or placebo nebuliz

Penehyclidine hydrochloride alleviates one-lung ventilation-induced lung injury by disrupting PLAT2-mediated glycerophospholipid metabolism and macrophage polarization.

BACKGROUND: One-lung ventilation (OLV), essential in thoracic surgery, frequently induces acute lung injury characterized by inflammation and impaired oxygenation. Penehyclidine hydrochloride (PHC) has demonstrated organ-protective potential, but its efficacy against OLV-induced injury and the underlying mechanisms, particularly concerning macrophage polarization and metabolic reprogramming, remain poorly defined. METHODS: A murine OLV model was established. Mice received intravenous PHC (0.15 mg/kg) or saline 30 min prior to OLV. Lung injury was assessed via PaO₂/FiO₂ ratio, wet/dry weight r

The efficacy and safety of extraluminal bronchial blockers in pediatric patients during one-lung ventilation for thoracoscopic surgery.

BACKGROUND: This study aimed to evaluate the impact and safety of extraluminal bronchial blockers on ventilation parameters in pediatric patients undergoing one lung ventilation (OLV) for thoracoscopic surgery. We hypothesized that, compared with endotracheal tube ventilation alone, the use of a bronchial blocker positioned outside the endotracheal tube can enhance lung collapse quality and improve ventilation efficiency without increasing the risk of complications. METHODS: In this prospective randomized controlled trial, a total of 128 pediatric patients who underwent OLV thoracoscopic surg

Prognosis of neonates receiving invasive mechanical ventilation in low-resource settings: a systematic review and prognostic meta-analysis.

Provision of invasive mechanical ventilation (IMV) in the neonatal intensive care has seen a steady rise in low-esource settings (LRS). However, outcomes among those exposed to IMV remain under-reported, with the current evidence base being restricted to single-centre observational studies, thus limiting comparative analyses and effective healthcare planning. This study aims to estimate the pooled proportion of mortality and morbidity among neonates exposed to IMV in low-resource settings. Medline, Embase, and CENTRAL were searched until 22 August 2025. Randomised and non-randomised studies we

Electrical impedance tomography-guided individualized ventilation strategy in patients with trauma-related and postoperative acute respiratory distress syndrome.

OBJECTIVE: Optimal positive end-expiratory pressure (PEEP) selection in acute respiratory distress syndrome (ARDS) is challenging because of marked regional lung heterogeneity. Electrical impedance tomography (EIT) provides bedside assessment of regional ventilation and may support individualized ventilation by informing PEEP titration, but evidence in trauma- and postoperative-associated ARDS remains limited. METHODS: In this single-center randomized controlled trial, 86 adults with moderate-to-severe trauma- or postoperative-associated ARDS were randomized to an EIT-guided individualized ve

Impact of mechanical ventilation on oxidative stress, neuroinflammation, memory and anxiety related behavior: Insights from an experimental model.

BACKGROUND: Mechanical ventilation is an essential intervention for patients with acute respiratory failure. However, ventilatory strategies with high tidal volumes can be harmful, and are strongly associated with ventilator-induced lung injury. Growing evidence suggests that ventilator-induced lung injury may also trigger oxidative stress and contribute to systemic inflammation and neuronal impairment, particularly in regions such as the hippocampus, which plays a fundamental role in cognitive functions. OBJECTIVE: Investigate the impact of high tidal volume mechanical ventilation on anxiety

Staged whole lung lavage in severe pulmonary alveolar proteinosis-Anaesthetic strategies for one-lung ventilation: A case report.

Pulmonary alveolar proteinosis causes severe hypoxaemia. Whole lung lavage is standard therapy, but anaesthetic management of staged sequential bilateral lavage in patients with partial pressure of oxygen (PaO2)/fraction of inspired oxygen (FiO2) <150 is challenging. We describe a 51-year-old female with autoimmune pulmonary alveolar proteinosis and respiratory failure who underwent staged lavage 3 days apart under general anaesthesia. A 35-Fr left-sided double-lumen tube (DLT) was placed with fibreoptic confirmation. Lung-protective one-lung ventilation was used with a tidal volume of 300 mL,

Postoperative Controlled Ventilation in a Patient With Montgomery's T-Tube In Situ After Laryngotracheal Reconstruction: A Case Report.

Positive pressure ventilation through a Montgomery T-tube remains a clinical challenge. This report highlights a successful case of positive pressure ventilation through a Montgomery tube with the aid of a cuffless endotracheal tube inserted into the proximal end of the T-tube. In cases where removal of the T-tube is not possible due to surgical indications, this technique can be used to maintain effective ventilation.

Early Risk Stratification for Mechanical Ventilation in Acute Ischemic Stroke: Development and Validation of a Simplified Clinical Score.

Background and PurposeMechanical ventilation (MV) occurs in a substantial subset of acute ischemic stroke (AIS) hospitalizations and is associated with worse outcomes, including prolonged hospital stay. Large national studies evaluating clinical predictors of MV and comparing regression with machine learning-based risk stratification in AIS remain limited. We examined clinical factors associated with MV in AIS and compared logistic regression with gradient-boosting approach for early respiratory risk stratification.MethodsWe analyzed 949,916 AIS hospitalizations from National Inpatient Sample
