Lipid Transporter TMEM24/C2CD2L Plays Essential Roles in Retinal Photoreceptor Survival by Regulating Calcium Signaling and Synaptic Transmission.

Retinal neurotransmission is fundamental to vision, relying on the precise calcium-dependent release of glutamate at photoreceptor ribbon synapses. Disruptions in this pathway underlie multiple visual disorders in humans. TMEM24/C2CD2L, an SMP domain-containing phospholipid transporter enriched at endoplasmic reticulum (ER)-plasma membrane (PM) contact sites, regulates phosphoinositide homeostasis and calcium signaling. However, its role in the retina remains unknown. Here, we show that C2CD2L is predominantly expressed in retinal photoreceptors. Using a rod-specific conditional knockout mouse

Retinoid biology from embryonic patterning to adult tissue homeostasis: metabolism, RAR signaling, disease mechanisms, and clinical translation.

Vitamin A gives rise to functionally distinct retinoids that regulate vision, embryonic patterning, and adult tissue homeostasis. The principal endogenous transcriptional ligand, all-trans-retinoic acid (ATRA), activates RARα, RARβ, and RARγ within RAR-RXR heterodimers; this mechanism is distinct from direct RXR activation by synthetic rexinoids. Tissue retinoic acid homeostasis is maintained through tightly coordinated regulation of vitamin A absorption, storage, transport, intracellular metabolism, and degradation. We integrate these pathways across embryonic development and adult physiology

Inherited Retinal Disease as a Predisposing Factor for Paclitaxel Maculopathy.

Purpose: To describe 3 cases of angiographically silent cystoid macular edema (CME) associated with paclitaxel therapy in patients with clinical or genetic features suggestive of an underlying inherited retinal disease (IRD). Methods: A series of 3 patients was reviewed. Results: All 3 patients presented with decreased vision and bilateral, angiographically silent CME shortly after starting paclitaxel. Clinical findings in each case raised suspicion of a concurrent IRD. Genetic testing revealed a pathogenic mutation in the NR2E3 gene, consistent with enhanced S-cone syndrome in 1 patient, seve

Stem cell-based therapies for neuroretinal degeneration: current landscape and future perspectives.

Retinal degenerations, comprising a heterogeneous group of disorders culminating in neuroretinal dysfunction, such as inherited retinal dystrophies (IRDs) and age-related retinal degeneration, are among the leading causes of irreversible vision loss worldwide. Restoring visual function through cell replacement therapy represents an attractive, yet challenging, strategy. Candidate retinal cell sources include retinal progenitor cells (RPCs), Müller glia, human neuroretinal stem-like cells (hNRSCs), and pluripotent stem cells (PSCs). In this review, we discuss their translational advantages and

Distinct developmental trajectories shape human sensitivity to rhythms in the environment.

Rhythm is an omnipresent feature of our environment. Repetitive temporal patterns in sound and vision influence how we pay attention to the world, move, and speak. Grasping these regularities is critical for development. Humans can track surrounding rhythms explicitly-like dancing to the beat of music-or implicitly, when rhythms guide perception and behavior without deliberate attention. Whether these abilities follow different developmental trajectories remains unknown. Here, we tested 98 children aged 7-13 using a novel gamified task measuring implicit rhythm processing, alongside assessment

Retinal Pigment Epithelium Phagocytosis and Retinal Degenerative Diseases.

In the mammalian retina, photoreceptors rely on the continuous renewal of their outer segments to preserve their function as light-sensing cells, thereby ensuring lifelong vision. This process entails the routine phagocytosis of shed photoreceptor outer segments (POS) by the retinal pigment epithelium (RPE). Phagocytosis of POS by RPE is a highly circadian-regulated process that is dependent on intricate and tightly controlled cellular signaling pathways. This article provides a systematic review of the research on the regulation and functional implications of the RPE phagocytic signaling syst

Gene Therapy for Inherited Retinal Disease: Current Strategies, Personalized Medicine, and Future Implications-A Comprehensive Review.

Gene therapy represents a transformative frontier in ophthalmology, offering the potential to address inherited and acquired retinal diseases at their genetic origin rather than through symptomatic management. By introducing exogenous genetic material to restore or modulate gene expression, gene therapy aims to preserve or even restore vision in patients with mutations that disrupt normal retinal function. The eye's small, compartmentalized structure, relative immune privilege, and direct accessibility through subretinal or intravitreal routes make it an ideal target for localized delivery wit

Use of Sialendoscopy to Diagnose Sialodochitis Fibrinosa: A Case Report.

First reported by Kussmaul in 1879, sialodochitis fibrinosa is a disease of the parotid and/or submandibular glands that is characterized by pain and diffuse swelling. It is believed to be caused by obstruction of the salivary duct by mucofibrinous plugs and is often misdiagnosed as other types of sialadenitis, such as Sjögren's syndrome, or sialadenitis caused by bacterial or viral infection. Although specific diagnostic criteria have not been defined, the typical pathological finding of sialodochitis fibrinosa is infiltration of lymphocytes and eosinophils into the interstitium around the sa

Preventive and Protective Effects of Nicotinamide Adenine Dinucleotide Boosters in Aging and Retinal Diseases.

Nicotinamide adenine dinucleotide (NAD+) boosting can sustain energy metabolism and neurovascular stability in the retinal tissue. Depletion of NAD+ is linked to the development of pathological retinal conditions, such as age-related macular degeneration (AMD) and diabetic retinopathy (DR). Mitochondrial dysfunction, oxidative stress, and inflammation occur in these diseases. This review summarizes substantial evidence of therapeutic NAD+ boosters, including nicotinamide, nicotinamide mononucleotide, or nicotinamide riboside. They help improve mitochondrial function and lessen neurovascular in

The immunopathology of keratoconus: tear film biomarkers and immune pathways.

PURPOSE OF REVIEW: This review explores established risk factors for keratoconus and current insights into tear film immune biomarkers, with a focus on the role of chronic low-grade inflammation in disease pathogenesis and emerging targeted therapies. RECENT FINDINGS: Growing evidence supports immune dysregulation as a key driver in the onset and progression of keratoconus. Genetics establish a foundation for a dysregulated inflammatory response in the ocular surface and corneal microenvironment, which sustains and accelerates disease progression. Elevated tear levels of IL-1, IL-6, TNF-α, HM

Piperine alleviates retinal ischemic injury by mediating the APE1 signaling.

Ischemic retinal damage is the most common cause of severe vision impairment and blindness. Anti-vascular endothelial growth factor (anti-VEGF) agents have transformed the treatment of retinal ischemic disorders and have become the cornerstone therapy for these conditions. Nonetheless, the risk for systemic and ocular adverse effects necessitates careful consideration. Meanwhile, the therapeutic potential of natural compounds for ischemic retinal injury is increasingly attracting attention. In this study, piperine (PIP), a natural compound derived from pepper, was found to reduce apoptosis by

Layers of Rarity: An Unusual Concurrence of Keratoconus, Salzmann's Nodular Degeneration, Ptosis, and Congenital Retinal Macrovessel.

We report a highly unusual case involving the coexistence of four rare ophthalmic conditions (keratoconus, Salzmann nodular degeneration (SND), congenital ptosis, and congenital retinal macrovessel) in a single patient. A young woman in her late adolescence presented with bilateral progressive vision loss and a history of left upper eyelid drooping since childhood. She was recently diagnosed as being in the early stages of pregnancy. Ocular examination revealed signs consistent with keratoconus, including corneal thinning, conical protrusion, and a scissoring reflex on retinoscopy. Two subepit

Giant Cell Arteritis Mimicking Temporomandibular Disorder: Diagnostic Value of Temporal Artery Halo Sign.

Giant-cell arteritis (GCA) involving the temporal arteries primarily affects the older adults, with symptoms that include head pain and tenderness. GCA may mimic temporomandibular disorders by causing jaw opening difficulty. Herein, we report an 89-year-old man who presented with jaw fatigue during meals, frontal headache, and low-grade fever. Examination revealed tenderness and dilatation of the right superficial temporal artery, jaw claudication, an elevated C-reactive protein (8.62 mg/dL), and erythrocyte sedimentation rate of 97 mm/h. Magnetic resonance imaging of the brain showed a small

Thyroid Hormone Signaling in Retinal Development and Function: Implications for Diabetic Retinopathy and Age-Related Macular Degeneration.

Thyroid Hormones (THs) play a central role in the development, cell growth, differentiation, and metabolic homeostasis of neurosensory systems, including the retina. The coordinated activity of various components of TH signaling, such as TH receptors (THRs) and the TH processing enzymes deiodinases 2 and 3 (DIO2, DIO3), is required for proper retinal maturation and function of the adult photoreceptors, Müller glial cells, and pigmented epithelial cells. Alterations of TH homeostasis, as observed both in frank or subclinical thyroid disorders, have been associated with sight-threatening disease

Hypertensive states of pregnancy.

Hypertensive disorders in pregnancy (HDP) remain a leading cause of pregnancy-related maternal and foetal morbidity and mortality worldwide, including chronic hypertension, gestational hypertension, and pre-eclampsia. Affected women and newborns also have an increased risk of cardiovascular disease later in life, independent of traditional cardiovascular disease risks. Despite these risks, recommendations for optimal diagnosis and treatment have changed little in recent decades, probably due to fear of the foetal repercussions of decreased blood pressure and possible drug toxicity. Genetics of inherited peripheral neuropathies and the next frontier: looking backwards to progress forwards.

Inherited peripheral neuropathies (IPNs) encompass a clinically and genetically heterogeneous group of disorders causing length-dependent degeneration of peripheral autonomic, motor and/or sensory nerves. Despite gold-standard diagnostic testing for pathogenic variants in over 100 known associated genes, many patients with IPN remain genetically unsolved. Providing patients with a diagnosis is critical for reducing their 'diagnostic odyssey', improving clinical care, and for informed genetic counselling. The last decade of massively parallel sequencing technologies has seen a rapid increase in

Giant Intracranial Xanthoma with Cloudy Vision as the First Symptom: A Case Report and Literature Review.

A 31-year-old male with mild dizziness complained of cloudy vision in his right eye for 5 days. The visual acuity of both eyes was normal, while the visual contrast sensitivity of both eyes slightly reduced. Fundus examination showed the swollen and radial superficial hemorrhage of his both optic nerves. Brain MRI scan indicated a huge tumor in the right temporal lobe with clear boundary, close to the skull. The midline structure shifted to the left. Blood tests indicated no hyperlipidemia or lipid disorders. The patient then received tumor resection. The size of the tumor was 5.6 cm × 7.5 cm
