Acute palsy of the recurrent laryngeal nerve complicating Epstein-Barr virus infection.

We report an 11-year-old boy with active Epstein-Barr virus (EBV) infection who developed acute aphonia and had signs and symptoms of recurrent laryngeal nerve palsy. The association of isolated recurrent laryngeal nerve palsy and EBV infection has not previously been reported. This case report expands the spectrum of neurologic complications of EBV infection, and suggests that infectious mononucleosis should be considered in the differential diagnosis of acute aphonia in children.

[Role of type I thyroplasty in the treatment of unilateral laryngeal paralysis. Apropos of 19 cases].

Authors report an analysis of different vocal parameters of 19 patients operated of type I thyroplasty. Thyroplasty was performed in conjunction with cervical surgery mandating vagal nerve section in 12 cases and for unilateral vocal paralysis in 7 cases. No post-surgical complications were observed. Sixteen patients (84%) had satisfactory voice although 15 cases (79%) presented residual glottic gap. Computer-assisted vocal evaluation was performed on 6 patients following surgery. It recorded normal vocal pitch and intensity values. In 5 patients, however, phonatory oral air-flow remained abno

Thyroplasty to improve the voice in patients with a unilateral vocal fold paralysis.

Unilateral vocal fold paralysis may cause incomplete closure of the glottis and a poor voice. Thyroplasty is a relative new operation to improve the voice by 'medialization' of the paralysed vocal fold. In our series of 29 patients 24 (83%) were satisfied and 26 (90%) had a better voice. After the operation the voice was louder, clearer and easier to understand. The dynamic and melodic ranges on the phonetogram were wider; maximum loudness and maximum phonation time were improved. There were no complications during the follow-up of 4 months to 5 years. In the three patients whose voice was not

Comparison of aerodynamic and electroglottographic parameters in evaluating clinically relevant voicing patterns.

The purpose of the present study was to identify one or more aerodynamic or electroglottographic measures that distinguish among voicing patterns that are clinically relevant for nodule pathogenesis and regression: a presumably pathogenic pattern (pressed voice), a neutral pattern (normal voice), and two presumably therapeutic patterns (resonant voice and breathy voice). Trained subjects with normal voices produced several tokens of each voice type on sustained vowels /a/, /i/, and /u/. For each token, maximum flow declination rate, alternating current flow, and minimum flow were obtained from

Effects of arytenoid adduction on laryngeal function following ansa cervicalis nerve transfer for vocal fold paralysis in an in vivo canine model.

Laryngeal reinnervation with the ansa cervicalis has been proposed as a treatment for human unilateral vocal fold paralysis (UVFP). This study tested the assumption that results from reinnervation could be improved if combined with medialization surgery. Six canine subjects underwent recurrent laryngeal nerve section and reinnervation with a branch of the ansa cervicalis. After reinnervation, vocal function was assessed before and after arytenoid adduction. Although laryngeal function improved significantly following reinnervation, results were significantly enhanced by the addition of mediali

The dysphagia diagnostic procedure as a treatment efficacy trial.

In all of these case reports, the patient's swallow function and the effect of therapy procedures on this function could be observed and measured from the videofluoroscopic assessment. In some cases, effects could be measured using other imaging procedures. The effects of each swallow therapy procedure can be measured by examining specific physiologic elements of the swallow. Table 3 presents the various therapy procedures and appropriate efficacy measures for each. In some cases, introduction of therapy procedures into the diagnostic evaluation can immediately enable the patient to begin eati

Complications of laryngeal framework surgery (phonosurgery).

The rising popularity of surgery involving the laryngeal framework (surgical medialization of immobile vocal folds, vocal fold tightening, pitch variation, etc.) has resulted in increasing case experience. Little has appeared in the literature regarding complications or long-term results of this type of surgery. Several years' experience in a major referral center with various types of laryngeal framework surgery has led to a small number of complications. These have included late extrusion of the prosthesis and delayed hemorrhage. A review of these complications and recommendations for modifi

Electronic pacing of incapacitated head and neck structures.

Paralysis in the head and neck can affect any motor or mixed cranial nerves and the cervical roots. Most conspicuous deficits, however, involve the larynx and the face. The capacity for denervated striated muscle to undergo reinnervation, and the presence of remaining sources of information, have allowed coordinated rehabilitation of incapacitated cervical neuromuscular systems. The object of further related research should focus on the long-term efficacy of the reinnervated muscle machinery and the potential complexities of electronic integration.

Effect of superior laryngeal nerve stimulation on phonation in an in vivo canine model.

We investigated the effect of variation in superior laryngeal nerve stimulation (SLNS) on vocal fold vibration. Photoglottography (PGG), electroglottography (EGG), and subglottic pressure (Psub) were measured in seven mongrel dogs using an in vivo canine model of phonation. The PGG, EGG, and Psub signals were examined at three SLNS frequencies (100 Hz, 130 Hz, and 160 Hz) using a constant rate of air flow. Increasing SLNS, which causes a contraction of the cricothyroid muscle, produced a marked increase in Fo, little change in Psub, an increase in the open quotient, and a decrease in the close

Neuromuscular pedicle graft for restoration of arytenoid abductor function in dogs with experimentally induced laryngeal hemiplegia.

Left laryngeal hemiplegia was induced by resection of the left recurrent laryngeal nerve in 12 dogs. A neuromuscular pedicle graft formed from the first cervical nerve and sternothyroideus muscle was transplanted after 1 week to the denervated cricoarytenoideus dorsalis muscle in 8 dogs. The remaining 4 dogs served as controls. Left arytenoid abduction was blindly evaluated by laryngoscopy with video photography at time 0, at 1 week, and at 19 weeks in all dogs. At 19 weeks, biopsy specimens of the left cricoarytenoideus dorsalis muscle and the neuromuscular pedicle were taken from 4 of the tr

Different endogenous analgesia systems are activated by noxious stimulation of different body regions.

Noxious pinch of the neck and the base of the tail can produce equipotent analgesia as measured by the tail flick method. However, noxious stimulation of the neck can suppress pain responsiveness both at the site of stimulation and at sites remote from the stimulated area while noxious stimulation of the tail produces analgesia only at sites remote from the stimulated area. Thus, neck pinched animals are immobile and completely unresponsive to the noxious pinch whereas pinch to the base of the tail, which results in tail flick suppression, causes vocalization and well organized biting behavior
