Early Life BMI Trajectories and Associations With Childbirth and Infertility Among Women.

IMPORTANCE: Concomitant with the global obesity epidemic, fertility has declined in Western countries with birth rates falling below replacement levels. A higher body mass index (BMI) among women is associated with reduced fertility, but associations with body size earlier in life are underexplored. OBJECTIVE: To investigate whether trajectories of childhood BMI are associated with timing of first childbirth and infertility in women. DESIGN, SETTING, AND PARTICIPANTS: This prospective, population-based cohort study used linked national register and school health examination data of women age

Establishing adherence-concentration-efficacy thresholds of TDF-FTC pre-exposure prophylaxis for HIV prevention in African women: a protocol for the Women TDF-FTC Benchmark Study.

BACKGROUND: Oral pre-exposure prophylaxis (PrEP) using co-formulated emtricitabine (FTC) and tenofovir disoproxil fumarate (TDF) is a potent HIV prevention method for men and women, with its efficacy highly dependent on adherence. A pivotal HIV efficacy study combined with a directly observed pharmacological study defined the thresholds for HIV protection in men who have sex with men (MSM), which are the keys to PrEP promotion and development of new PrEP agents. For African women at risk for HIV and belonging to a priority group considered due to disproportionately high incident HIV infections

Associated factors of pelvic organ prolapse among patients at Public Hospitals of Southern Ethiopia: A case-control study design.

BACKGROUND: Pelvic organ prolapse (POP) is the descent of the vaginal wall, cervix, uterus, bladder, and rectum downward into the vaginal canal. It occurs owing to the weakness of the structures supporting and keeping pelvic organs in anatomic position. Prolapse occurs due to exposure to risk factors; women in developing countries are highly predisposed to the risk factors of the prolapse. No study assesses POP in Southern Ethiopia. METHODS: A case-control study design was employed in 369 participants (123 cases and 246 controls) of seven randomly selected Public Hospitals of Southern Ethiopi

Provision of interpreting support for cross-cultural communication in UK maternity services: A Freedom of Information request.

BACKGROUND: Language, communication and understanding of information are central to safe, ethical and efficient maternity care. The National Health Service (NHS) commissioning board, NHS England, describes how healthcare providers should obtain language support through professionally trained interpreters. Providers of interpreters are commissioned to deliver remote/face to face interpretation across the NHS. Services can be booked in advance or calls can be made in real time. However, women report infrequent use of professionally trained interpreters during their maternity care, often relying

Race, the Vaginal Microbiome, and Spontaneous Preterm Birth.

Previous studies have investigated the associations between the vaginal microbiome and preterm birth, with the aim of determining whether differences in community patterns meaningfully alter risk and could therefore be the target of intervention. We report on vaginal microbial analysis of a nested case-control subset of the Pregnancy, Infection, and Nutrition (PIN) Study, including 464 White women (375 term birth and 89 spontaneous preterm birth, sPTB) and 360 Black women (276 term birth and 84 sPTB). We found that the microbiome of Black women has higher alpha-diversity, higher abundance of L

Standardized Cesarean Risk Counseling with Induction: Impact on Racial Disparities in Birth Satisfaction.

OBJECTIVE: Our prior work demonstrated decreased birth satisfaction for Black women undergoing labor induction. We aimed to determine if implementation of standardized counseling around calculated cesarean risk during labor induction could reduce racial disparities in birth satisfaction. STUDY DESIGN: We implemented use of a validated calculator that provides an individual cesarean risk score for women undergoing induction into routine care. This prospective cohort study compared satisfaction surveys for 6 months prior to implementation (preperiod: January 2018-June 2018) to 1 year after (pos

Project20: Does continuity of care and community-based antenatal care improve maternal and neonatal birth outcomes for women with social risk factors? A prospective, observational study.

BACKGROUND: Social factors associated with poor childbirth outcomes and experiences of maternity care include minority ethnicity, poverty, young motherhood, homelessness, difficulty speaking or understanding English, migrant or refugee status, domestic violence, mental illness and substance abuse. It is not known what specific aspects of maternity care work to improve the maternal and neonatal outcomes for these under-served, complex populations. METHODS: This study aimed to compare maternal and neonatal clinical birth outcomes for women with social risk factors accessing different models of

Peripheral monocyte culture supernatants of menopausal women can induce bone resorption: involvement of cytokines.

Increased bone resorption is a mechanism contributing to bone loss in the postmenopausal period. Cytokines are involved in osteoclastic differentiation and, therefore, may play a role in the regulation of bone resorption. Several previous works showed the implication of interleukin-1 (IL-1), IL-6, and tumor necrosis factor-alpha (TNF alpha) in the modulation of bone remodeling. This study determines the concomitant production of the three cytokines and tests the bone-resorbing activity of peripheral monocyte supernatants. Four groups of women were studied: premenopausal women (n = 13; mean age

Development of the Healthy Women Intervention to Increase Women's Engagement in Medication Treatment for Opioid Use Disorder: Mixed Methods, User-Centered Design Approach.

BACKGROUND: Rates of opioid use disorder (OUD) have increased among women over the past 2 decades. Medication treatment for opioid use disorder (MOUD) is effective but underused. Gender-specific treatments for women have been associated with improved substance use outcomes. However, these treatments have not specifically targeted women's engagement in MOUD, and the impact of existing gender-specific treatments is restricted by in-person delivery. OBJECTIVE: The aim of this study was to develop a digital intervention to feasibly deliver gender-specific care that addresses the individualized ne

Depressive symptoms partially mediate the association between femoral neck T-score and all-cause mortality in older adults: a sex-stratified analysis.

BACKGROUND: Impaired bone health is recognized as a contributor to mortality in later life, yet the pathways connecting these conditions are incompletely understood. Existing work has emphasized fractures and functional decline, whereas the contribution of depressive symptoms, and whether that contribution differs between men and women, has received less attention. METHODS: Participants were 5823 adults aged 60 years or older from five NHANES cycles (2005-2018), including 2773 women and 3050 men. We evaluated femoral neck T-score as a categorical and continuous exposure using Cox models-incor

A longitudinal study examining how self-injection social norms are associated with contraceptive self-injectable interest and use in rural Uganda.

BACKGROUND: There is mounting evidence that social norms affect attitudes, decision-making, and behaviors related to contraceptive use. Integral to the self-care movement for women to have more control over their reproductive health, subcutaneous depot medroxyprogesterone acetate (DMPA-SC) is a contraceptive that can be safely administered by women themselves after training. DMPA-SC was introduced in Uganda in 2017 and is slowly gaining traction, especially among women who value a convenient and private method. However, only a small percentage of DMPA-SC users choose to self-inject, perhaps mi

Indoor residual spraying household visits as a platform for engaging pregnant women in malaria prevention on Bioko Island, Equatorial Guinea.

BACKGROUND: Pregnant women are particularly vulnerable to many health conditions, including infectious diseases, which place them at elevated risk of severe outcomes. In particular, malaria poses a significant burden during pregnancy, and as such there has long been a strong focus on delivering malaria prevention interventions to pregnant women during antenatal care (ANC), including long-lasting insecticide-treated nets (LLINs) and intermittent preventive treatment in pregnancy (IPTp). However, outreach to drive uptake of these interventions presents some distinct challenges. For example, preg

The psychosocial impacts of skin-neglected tropical diseases (SNTDs) as perceived by the affected persons: A systematic review.

BACKGROUND: Neglected Tropical Diseases (NTDs) disproportionately affect marginalised groups within impoverished communities, conferring devastating physical, financial and psychosocial effects. Skin-NTDs (SNTDs) are uniquely stigmatising due to their visible nature, rendering affected individuals vulnerable to psychosocial risk and the associated decline in social participation, quality of life and mental health. In response to knowledge gaps identified by current global efforts for integrated control of SNTDs this review gathers existing evidence on the psychosocial effects of SNTDs, with co

Oncogenic human papillomavirus infection (HPV 16/18) and associated factors among women in East Gojjam Zone, NorthWest Ethiopia 2021.

BACKGROUND: Human papillomavirus is a common pathogen that infects the skin and mucosal epithelium, is transmitted sexually; causes condylomas or squamous cell carcinomas. Two (16 and 18) of the 150 HPV serotypes are oncogenic types. Studies have been done on the infection by oncogenic human papillomavirus 16/18 and associated factors are found to be very limited in Ethiopia. This study aimed to assess the prevalence of oncogenic human papillomavirus infection (HPV 16/18) and associated factors among women. METHODS: An institutional-based cross-sectional study was conducted among 337 women sc

Changes in the Vaginal Microbiome and Associated Toxicities Following Radiation Therapy for Gynecologic Cancers.

Postmenopausal women often suffer from vaginal symptoms associated with atrophic vaginitis. Additionally, gynecologic cancer survivors may live for decades with additional, clinically significant, persistent vaginal toxicities caused by cancer therapies, including pain, dyspareunia, and sexual dysfunction. The vaginal microbiome (VM) has been previously linked with vaginal symptoms related to menopause (i.e. dryness). Our previous work showed that gynecologic cancer patients exhibit distinct VM profiles from healthy women, with low abundance of lactobacilli and prevalence of multiple opportuni

Characteristics of the vaginal microbiome in women with and without clinically confirmed vulvodynia.

BACKGROUND: Vulvodynia (idiopathic vulvar pain) affects up to 8% of women by age 40 years, has a poorly understood etiology, and has variable treatment efficacy. Several risk factors are associated with vulvodynia from a history of yeast infections to depression and allergies. Recent work suggests an altered immune inflammatory mechanism plays a role in vulvodynia pathophysiology. Because the vaginal microbiome plays an important role in local immune-inflammatory responses, we evaluated the vaginal microbiome among women with vulvodynia compared with controls as 1 component of the immune syste

The reduction in anemia through normative innovations (RANI) project: study protocol for a cluster randomized controlled trial in Odisha, India.

BACKGROUND: More than half of women in India are anemic. Anemia can result in fatigue, poor work productivity, higher risk of pre-term delivery, and maternal mortality. The Indian government has promoted the use of iron-folic acid supplements (IFA) for the prevention and treatment of anemia for the past five decades, but uptake remains low and anemia prevalence high. Current programs target individual-level barriers among pregnant women and adolescents, but a more comprehensive approach that targets multiple levels among all women of reproductive age is needed to increase uptake of IFA and iro

The relationship between severe maternal morbidity and psychological health symptoms at 6-8 weeks postpartum: a prospective cohort study in one English maternity unit.

BACKGROUND: The incidence of severe maternal morbidity is increasing in high-income countries. However, little has been known about the impact on postnatal morbidity, particularly on psychological health outcomes. The objective of this study was to assess the relationship between severe maternal morbidity (ie. major obstetric haemorrhage, severe hypertensive disorders or intensive care unit/obstetric high dependency unit admission) and postnatal psychological health symptoms, focusing on post-traumatic stress disorder (PTSD) symptoms at 6-8 weeks postpartum. METHOD: A prospective cohort study
