Perceived need, barriers to and facilitators of mental health care among HIV-infected PWID in Hanoi, Vietnam: a qualitative study.

BACKGROUND: The HIV epidemic in Vietnam has been primarily driven by injection drug use. HIV-infected people who inject drugs (PWID) in Vietnam have very high rates of mental health problems, which can accelerate progression to AIDS and increase mortality rates. No research has explored the barriers and facilitators of mental health care for HIV-infected PWID in Vietnam. METHODS: We conducted 28 in-depth interviews among HIV-infected PWID (n = 16), HIV and MMT (methadone maintenance treatment) providers (n = 8), and health officials (n = 4) in Hanoi. We explored participants' perceptions of m

[Prehospital analgesia performed by paramedics: quality in processes and effects under medical supervision].

BACKGROUND: In the context of the new education program for German paramedics, a discussion has been fueled regarding the emergency care competencies of paramedics. In 1999 a system for training and qualification of paramedics was established in the emergency medical service (EMS) area of Reutlingen (Germany), including an algorithm for analgesia using ketamine and midazolam. Under defined circumstances analgesia may be administered to patients in the absence of a physician. AIM: The objectives of the study were to evaluate the effectiveness of pain reduction measured by a recognized assessme

Factors associated with late antiretroviral therapy initiation in Cameroon: a representative multilevel analysis.

OBJECTIVES: Many people living with HIV/AIDS in resource-limited settings begin antiretroviral therapy (ART) at low CD4 counts. Here, we investigated the simultaneous effect of individual-, facility- and regional-level factors on late ART initiation. METHODS: We conducted a survey in a nationally representative sample of 55 HIV treatment facilities in Cameroon. Medical records of 4935 patients >15 years of age who initiated ART in the month of October during the period 2007-10 were reviewed to gather individual characteristics. Late ART initiation was defined as CD4 count ≤ 100 cells/mm(3). F

Validation of Criteria to Guide Prehospital Naloxone Administration for Drug-Related Altered Mental Status.

INTRODUCTION: We aimed to validate previously derived clinical criteria to predict successful prehospital response to naloxone in patients with altered mental status treated by EMS. We hypothesized that prehospital naloxone criteria would have high sensitivity for effective antidote response, but would be underutilized, in patients with drug-related altered mental status (DRAMS). METHODS: This study was a secondary data analysis of a prospective cohort of acute DRAMS at an urban ED. Naloxone criteria (respiratory rate (RR) <12, miotic pupils, or drug paraphernalia) and mental status, graded b

Oral antidiabetics use among diabetic type 2 patients with chronic kidney disease. Do nephrologists take account of recommendations?

BACKGROUND: There is an increasing prevalence of diabetes type 2 and chronic kidney disease, challenging appropriate prescribing of oral anti-diabetic drugs (OADs). METHODS: We have described the practice patterns of 13 nephrologists in 4 centers, in a cohort of 301 consecutive adult type 2 diabetic patients. Among oral anti-diabetic prescriptions, we have detailed drugs dosage for each subject, with 3 different formulae for estimating glomerular filtration rate (GFR) and its adequation according to the latest ERBP recommendations (2015). As individuals were mostly obese in this work, we also

Weekend/holiday versus weekday hospital discharge and guideline adherence (from the American Heart Association's Get with the Guidelines--Coronary Artery Disease database).

Most hospitals have reduced medical staff on weekends. Furthermore, a recent study on acute myocardial infarction suggested that weekend admissions were associated with higher mortality compared with weekday admissions. We sought to determine if compliance with guideline recommendations for acute coronary syndrome performance measures would be worse on weekends/holidays compared with weekdays. We utilized the American Heart Association's Get with the Guidelines (GWTG)--Coronary Artery Disease database. This study included 154,910 patients admitted to 515 various hospitals from January 14, 2000

Suicide rates in relation to health care access in the United States: an ecological study.

OBJECTIVE: We tested the hypothesis that suicide rates in the United States are associated with indicators of access to health care services. METHOD: With an ecological study design, we compared age-adjusted suicide rates for men and women with demographic, socioeconomic, and other indices of access to health care, by state (N = 51, including the District of Columbia). The most recently available information from the National Statistics Reports at the U.S. Census Bureau, the U.S. Centers for Disease Control and Prevention National Center for Health Statistics, and the American Board of Medica

Planetary Health in Nursing Scholarship: A Bibliometric Mapping of Climate Change, Sustainability, and Health-System Transformation.

BACKGROUND: Planetary health is an increasingly important transdisciplinary approach in nursing because it links human health and equity to the integrity of Earth's natural systems. Climate change is a major component of planetary health, but it is not synonymous with the broader approach, which also encompasses biodiversity loss, pollution, resource pressures, environmental sustainability, and the transformation of health systems. Although nursing scholarship has increasingly engaged with climate change and sustainability, it remains unclear whether planetary health has emerged as a distinct

Development and psychometric validation of a self-report instrument assessing perceived disaster preparedness among Spanish nurses.

AIM: To develop and validate SN-READY, a context-specific, self-reporting instrument that measures perceived disaster preparedness and self-efficacy among Spanish registered nurses. BACKGROUND: In light of the escalating frequency and severity of disasters, there is an imperative for a nursing workforce that can respond in an effective and coordinated manner. While the World Health Organization recognises the indispensable role of nurses in disaster health systems, there was previously no validated instrument to measure perceived preparedness specifically within the Spanish nursing context.

Acceptability, Usability, and Perceived Quality of Care of a Digital Decision Support System in Tele-Primary Health Care in Sindh, Pakistan: Sequential Explanatory Mixed Methods Pilot Study.

BACKGROUND: Primary health care (PHC) delivery in Pakistan is constrained by persistent workforce shortages, which are further exacerbated by the attrition of trained female physicians following marriage or childbirth. Telehealth platforms, such as Sehat Kahani, have emerged as one response to this gap, enabling female physicians to provide remote primary care from home. Within this model, a digital decision support system (DDSS) was recently piloted for selected febrile illnesses to strengthen clinical decision-making. However, evidence on how such systems are perceived by female PHC provider

The Implementation of Infection Prevention and Control Procedures in Primary Care During the COVID-19 Pandemic: A Qualitative Study of Nursing Roles.

Introduction: During the COVID-19 pandemic, primary care practices felt poorly supported by existing infection prevention and control (IPAC) guidelines, which focused primarily on acute care facilities. This issue was further complicated by insufficient provision of personal protective equipment in primary care settings, which limited clinic capacity and the ability of primary care to provide in-person services. Nurses play an integral role in the implementation of IPAC procedures and the provision of ongoing primary care during a health crisis; however there is limited literature related to n

Implementation of a Virtual Hospital in the Home Service for Patients With COVID-19 in Queensland, Australia: Mixed Methods Evaluation Using the RE-AIM Framework.

BACKGROUND: Hospital in the home (HITH) provides home-based care as an alternative to traditional hospitalization. In response to the COVID-19 Omicron wave, a public hospital in the rural Western portion of Southeast Queensland implemented a virtual HITH service to support adults, maternity patients, and children with moderate COVID-19 symptoms and additional health concerns. Although the pandemic accelerated the uptake of virtual care within HITH models, existing literature has focused on clinical outcomes, with limited evidence on key implementation outcomes. OBJECTIVE: Using the RE-AIM (re

Musculoskeletal Injury in Australian Infantry Personnel: A Cross-sectional Study to Understand Prevention Priorities.

INTRODUCTION: Musculoskeletal injury patterns are under-investigated in the Royal Australian Infantry Corps. Subsequently, more evidence is needed to support injury prevention processes in this population. One difficulty in collecting injury information to monitor injury patterns within combat populations accurately is known injury concealment behaviors in such populations. This study aims to examine musculoskeletal injury epidemiology within Australian infantry battalions using a tailored approach to mitigate reporting avoidance. MATERIALS AND METHODS: A cross-sectional study using an anonym

FERTILITY CARE IN LOW- AND MIDDLE- INCOME COUNTRIES: The future use of AI to improve accessibility of assisted reproductive technology in low- and middle-income countries.

ABSTRACT: People in low- and middle-income countries face many obstacles when trying to access fertility treatments. These challenges include high costs, the need for specialized medical facilities, and cultural beliefs that may discourage seeking help. This paper explores how artificial intelligence (AI) and automation could help overcome some of these barriers and make fertility treatments more widely available. It examines how AI may improve the accuracy, efficiency, and consistency of different steps in fertility treatments, such as choosing the healthiest embryos, analyzing sperm, evaluat
