Association of frontal sinus development with somatic and skeletal maturation in Aboriginal Australians: a longitudinal study.

There have been very few studies of frontal sinus development and its association with other growth parameters in humans. The aim of this study was to investigate the relationship of frontal sinus development with somatic and skeletal maturation in a sample of Aboriginal Australians. The sample comprised 31 individuals, of whom 17 were males and 14 females. For the selected subjects the following records were available: lateral head radiographs, hand-wrist radiographs and stature recordings, generally covering the age range from 7 to 18 years. Descriptive statistics for frontal sinus size, ske

Patterns of joint involvement at onset differentiate oligoarticular juvenile psoriatic arthritis from pauciarticular juvenile rheumatoid arthritis.

OBJECTIVE: To compare the patterns of joint involvement of patients with oligoarticular onset juvenile psoriatic arthritis (Oligo-JPsA) and pauciarticular onset juvenile rheumatoid arthritis (Pauci-JRA) in order to estimate the predictive performance of specific patterns for the diagnosis of Oligo-JPsA. METHODS: Twenty-three children who fulfilled the diagnostic criteria for JPsA (Vancouver criteria) and who had fewer than 5 joints involved in the first 6 months of disease (Oligo-JPsA), and 64 children with Pauci-JRA (ACR criteria) were enrolled. Patients were also classified with respect to

Improvement of periarticular osteoporosis in postmenopausal women with rheumatoid arthritis by beta-alanyl-L-histidinato zinc: a pilot study.

The effect of zinc on bone metabolism in patients with rheumatoid arthritis (RA) is unknown. In the present pilot study, we investigated the effect of two antiulcer drugs, beta-alanyl-L-histidinato zinc (AHZ) and cimetidine, on bone metabolism in postmenopausal women with RA who had bilateral wrist pain. Eight patients were enrolled in a prospective, single-blind study consisting of 6-month cimetidine treatment (400 mg/day) followed by 6-month AHZ treatment (300 mg/day). Biochemical markers and bone mineral density (BMD) by dual energy X-ray absorptiometry were measured at baseline, 6 months,

Posttraumatic ulnar translation of the carpus.

Ulnar translation of the carpus is a rare posttraumatic carpal instability pattern, in which the entire carpus is displaced ulnarward on the radioulnar surface. No previous formal reports of this problem have been located although it has been described in general discussions of wrist instability. Seven men and one woman were diagnosed with this condition although the diagnosis was delayed from 2 to 23 months, an average of 7.3 months. Three patients were treated surgically before diagnosis; the other five patients were treated conservatively before diagnosis. At the time of definitive diagnosi

The palmar intra-articular extended window approach for distal radial fractures: a biomechanical cadaveric study.

A palmar intra-articular extended window approach can be used during surgical treatment of intra-articular fractures of the distal radius. A biomechanical study was carried out on 10 fresh-frozen cadaver wrists to measure the impact of the approach on proximal carpal row stability during simulated grip loading. The null hypothesis was that this approach would not generate a displacement of more than 2 mm of the scaphoid or lunate in any direction. After incising the palmar capsule, no significant increase in ulnar translation of the lunate or scaphoid was observed. However, after additionally

Muscular branches from the ulnar artery are the basis for a pedicled partial flexor digitorum superficialis muscle flap.

The feasibility of a pedicled flexor digitorum superficialis muscle flap was studied in 10 fresh cadavers. The number, length and distance from the flexion wrist crease of muscular branches from the ulnar artery in the distal 10 cm of the forearm were recorded. The mean number of muscular branches was 2.7 (range 1-4). The mean distance of the most distal branch was 35 mm (range 26-40) from the proximal wrist flexion crease. Its mean length was 20 mm (range 16-26). A partial muscle flap was raised on the most distal branch and transposed over the median nerve in the distal forearm. Dissection a

A novel case of ulnar nerve compression neuropathy with co-existing accessory flexor carpi ulnaris and accessory abductor digiti minimi.

There are numerous cases reported of the accessory muscles of the hand and wrist in surgical, cadaveric, and imaging-based studies. Anatomical muscle variations in the flexor compartment of the wrist and forearm can present as a pseudo mass or space-occupying lesion causing external compression on the traversing nerves. Guyon's canal is a compact space with a high potential for nerve entrapment. Common etiologies include ganglion cysts, osteophytes, or soft tissue masses. This rare case illustrates the combined existence of two accessory muscles, an accessory flexor carpi ulnaris, and an acces

[An «all around» version of the Mac Gregor free flap].

Given the appearance of a good number of cover flaps in recent decades, the Mac Gregor flap in its free form is used less and less but still finds its place today in certain complex cases with extensive loss of substance. This report describes an unusual case of wide tissue coverage by «all around» flap by Mac Gregor technique in a victim of an extensive burn of the left upper limb which occurred on 08/08/1997 and was responsible for a progressive flessum of the wrist and a hyperextension of the fingers with a complete loss of their mobility. The intervention is done in one step with a wide re

A rare case of a median nerve schwannoma: Case report.

A median nerve schwannoma is an uncommon type of tumor that develops from Schwann cells in the peripheral nerves. We present a case report of a young prisoner with a rare median nerve schwannoma presenting as a swelling on the anterior aspect of the right wrist along the radial edge, with associated paresthesia and a positive Tinel's sign. The case was diagnosed using MRI which showed the target sign of biphasic contrast enhancement in both the mass's center and periphery as well as distinct encapsulation. The mass was managed with surgical excision which confirmed the mass to be a neurilemmom

Application of ultrasound in a case of eosinophilic fasciitis mimicking stiff-person syndrome.

Eosinophilic fasciitis (EF) is a rare disorder characterized by muscle stiffness mimicking other neuromuscular diseases. The diagnosis of EF is made on the basis of typical skin lesions. We report a case of a 36-year-old male patient with suspected stiff-person syndrome (SPS), who presented with progressive limb muscle stiffness and limited mobility of both wrists without obvious skin changes. Ultrasound revealed fascial thickening of bilateral upper and lower limb muscles and enlargement of hypoechoic tissues around the flexor digitorum tendons of the wrist. Skin and fascia biopsy confirmed t

Cross-sectional area reference values of the median nerve at the palm using ultrasound.

BACKGROUND: The most common neuromuscular ultrasound abnormality in carpal tunnel syndrome is enlargement of the median nerve near the distal wrist crease, but sometimes the median nerve enlarges in the palm, just distal to the transverse carpal ligament. This study was conducted to devise a protocol for measuring the median nerve in the palm and to generate reference values for nerve cross-sectional area at this site. METHODS: A systematic protocol was developed, 30 healthy individuals underwent ultrasound of the median nerve in the palm bilaterally, and cross-sectional area was recorded. R

Chronic bursitis and tenosynovitis revealing Whipple's disease.

Joint complaints, most commonly intermittent arthritis, are the initial manifestation in about three-fourths of Whipple's disease cases. We herein report on two cases wherein Whipple's disease manifested itself as chronic bursitis and tenosynovitis at several sites. A 42 year-old man had bilateral olecranon bursitis, a right patellar bursitis and an extensor tenosynovitis on the left wrist and a 54 year-old man had extensor tenosynovitis at both wrists and a bilateral tenosynovitis of the extensors at both ankle. Methotrexate in both patients and etanercept in one of them were not effective. P

Effectiveness of second corticosteroid injections for carpal tunnel syndrome.

INTRODUCTION: A single local corticosteroid injection is an effective treatment for carpal tunnel syndrome. No study has specifically examined the effectiveness of a second injection on relapse after primary injection. METHODS: We identified a cohort of patients who had received an initial corticosteroid injection into 1 wrist and then, at a later date, a second injection into the same wrist. We compared the change in the Boston Symptom Severity Scale (SSS) and Functional Status Scale (FSS) between first and second injections. RESULTS: In 229 patients who received 2 injections the mean impro

Carpal tunnel: Normal anatomy, anatomical variants and ultrasound technique.

The carpal tunnel is an osteofibrous canal situated in the volar wrist. The boundaries are the carpal bones and the flexor retinaculum. In addition to the medial nerve, the carpal tunnel contains nine tendons: the flexor pollicis longus, the four flexor digitorum superficialis and the four flexor digitorum profundus. Ultrasound (US) study of the carpal tunnel generally involves short-axis imaging of the tendons, and in the presence of disease, long-axis imaging and dynamic maneuvers are added. There are numerous reports of anatomical variants of the wrist involving vessels, nerves, tendons and

Atypical variant of lichen planus mimicking normal skin histology.

We present the case of a 69-year-old Caucasian and non-smoker patient with erythematous, itchy, violaceous lesions on the ankles, wrists, lower legs, forearms and trunk developed within 15 months. Her condition was diagnosed as prurigo and she was treated for a long period of time with antihistaminic drugs, with no resolution of lesions. In October 2008, she presented to a Private Practice of Dermatology in Timisoara. The dermatologist noticed the development of violaceous lesions on her trunk. The patient had similar lesions on the ankles, wrists, lower legs, and forearms for the last eight m
