ESBL-producing Enterobacteriaceae: controlling the spread of infection.

Extended spectrum beta-lactamase (ESBL)-producing Enterobacteriaceae were first reported in Europe in the 1980s and have since become a worldwide problem. This has resulted in increased morbidity, mortality and cost in treating the infections they cause (Paterson et al, 2001; Bisson et al, 2002). In the UK the prevalence of ESBL rose from 5.4% in 1997 to 22.7% in 1999 (Goossens, 2000). ESBL are not the only beta-lactamases to present resistance to 1st, 2nd and 3rd generation cephalosporins but are the most important. New strains produce a particular type of ESBL, the CTX-M type, which is able

Linkage of ciprofloxacin resistance with a single genotypic cluster of Klebsiella pneumoniae.

The objective of this study was to examine the epidemiology of ciprofloxacin-resistant, extended-spectrum beta-lactamase (ESBL)-producing Klebsiella pneumoniae strains. Sixty-nine unique patient isolates of K. pneumoniae isolated from a variety of clinical specimens submitted to the clinical bacteriology laboratories of The Royal Infirmary of Edinburgh and associated General Practices were identified and susceptibility testing was performed with the Vitek system. Strains flagged as ESBL-positive by the Vitek system were subjected to isoelectric focusing. The results suggested that all 69 isola

Amoxicillin-clavulanate therapy increases childhood nasal colonization by methicillin-susceptible Staphylococcus aureus strains producing high levels of penicillinase.

We examined factors associated with penicillinase production by nasal carriage Staphylococcus aureus strains in 648 children aged 3 to 6 years attending 20 randomly sampled playschools. The children were prospectively monitored for drug use and medical events for 6 months and were then screened for S. aureus carriage. Isolates were tested for their susceptibility to penicillin G and methicillin, and penicillinase production by methicillin-susceptible, penicillin-resistant strains was quantified. S. aureus was isolated from 166 children (25.6%). Exposure to amoxicillin-clavulanate during the pr

Genetic characterization of gut-associated Clostridium perfringens isolates from Austrian animals using a combination of established and novel methods.

INTRODUCTION: Clostridium perfringens (C. perfringens) is a pathobiont of the intestinal microbiota that can cause severe systemic and enteric diseases. C. perfringens colonizes a variety of hosts and environmental niches, which is the theorized reason for its large genome, low guanine-cytosine content, and small stable core genome. Over 20 putative virulence factors have been described, but only six are included in the used typing scheme. Epidemiological analysis is limited, and to the authors' knowledge, this is the first study for Austria. Furthermore, C. perfringens frequently harbors diff

Catheter-associated bacteremia caused by Asaia lannensis in an immunosuppressed patient: a case report.

BACKGROUND: Asaia spp. are an environmental Gram-negative bacterial species rarely implicated in human infections. Only a handful of cases have been reported, primarily involving catheter-associated bloodstream infections or infections related to intravenous drug abuse. We present here a case of bacteremia caused by Asaia lannensis in an immunocompromised patient with hairy cell leukemia. CASE PRESENTATION: An 84-year-old man with a history of hairy cell leukemia presented with relapse and was treated with cladribine via a subclavian central venous catheter. The patient developed a febrile epi

Safety, effectiveness, and acceptability of antenatal penicillin allergy evaluation: a systematic review.

Approximately 8% to 13% of pregnant patients report a penicillin allergy. Penicillins and other beta-lactams are widely used in pregnancy but are often avoided in these patients, resulting in suboptimal therapy, antimicrobial resistance, higher costs, and increased morbidity for patients and neonates. True penicillin allergy is rare, and 95% of unverified penicillin allergies are delabeled upon evaluation. Although penicillin allergy evaluation is safe and recommended for pregnant patients, few patients are assessed. Research concerning antenatal penicillin allergy evaluation is accelerating,

Antimicrobial Utilization, Adverse Drug Reactions, and Associated Cost of Care in a Tertiary Care Hospital in Eastern India.

Background Antimicrobials considerably reduce infectious conditions, but their overuse and misuse contribute to various adverse drug reactions (ADRs) and antimicrobial resistance. In 2019, India adopted a national program to reduce antibiotic resistance for 2019-2021. Assessing antibiotic consumption among the out-patient and in-patient departments is paramount because it is the foundation for implementing and assessing antibiotic stewardship initiatives. This study aims to evaluate the usage pattern, ADR of antimicrobials and cost of treatment due to the use of antibiotics in patients admitte

Antibiotic Use and Subsequent Cognitive Decline and Dementia Risk in Healthy Older Adults.

BACKGROUND AND OBJECTIVES: Antibiotics rapidly reduce intestinal bacterial diversity, leading to dysbiosis that persists for months to years. Although emerging evidence from retrospective and claims-based studies has linked dysbiosis to cognitive function, prospective data are lacking. We aim to examine the prospective association of antibiotics with cognitive aging among initially healthy older adults. METHODS: We leveraged data from prospective follow-up and observational extension of ASPirin in Reducing Events in the Elderly, a completed randomized trial of community-based Australian older
