[Hypotension and Generalized Edema Due to Plasma Leakage: A Case Report].

Generalized edema is commonly encountered in everyday clinical practice, usually due to heart, liver and renal disease. On the other hand, edema related to fluid extravasation associated with hypotension, hemoconcentration and hypalbuminemia is a rare clinical disorder. We present a case of a 61-year-old man with 3 subsequent endothelium permeability alteration episodes with increasing severity related to Sars-Cov-2 infection. During the last episode he was admitted to the intensive care unit, a monoclonal gammopathy was diagnosed and acute kidney injury requiring continuous renal replacement

A suspected case of hyponatraemia induced rhabdomyolysis: a case report.

BACKGROUND: Hyponatraemia is a documented but under-recognised cause of rhabdomyolysis, with the contrasting treatment strategies for the two conditions posing a unique challenge. Balancing the need for aggressive fluid replacement for the treatment of rhabdomyolysis, with the risk of rapidly correcting hyponatraemia is imperative. CASE PRESENTATION: A 52-year-old gentleman with a background of HIV infection and hypertension presented with seizures following methamphetamine use, acute water intoxication, and thiazide use. He was found to have severe hyponatraemia, and following initial correc

Post-Surgical Peritoneal Scarring and Key Molecular Mechanisms.

Post-surgical adhesions are internal scar tissue and a major health and economic burden. Adhesions affect and involve the peritoneal lining of the abdominal cavity, which consists of a continuous mesothelial covering of the cavity wall and majority of internal organs. Our understanding of the full pathophysiology of adhesion formation is limited by the fact that the mechanisms regulating normal serosal repair and regeneration of the mesothelial layer are still being elucidated. Emerging evidence suggests that mesothelial cells do not simply form a passive barrier but perform a wide range of im

Morphological characteristics in peritoneum in patients with neutral peritoneal dialysis solution.

Peritoneal dialysis solution (PDS) plays a role in functional and morphological damage to the peritoneum. This study aimed to clarify the effect of neutral PDS in preventing morphological changes by assessing peritoneal damage and comparing morphological alterations between PD patients treated with neutral PDS and acidic PDS. Sixty-one patients participated from seven hospitals. All patients were treated with neutral PDS excluding icodextrin, during their entire PD treatment, and experienced no episode of peritonitis. The thickness of submesothelial compact (SMC) zone and the presence of vascu

A case of splenic rupture: a rare event after laparoscopic cholecystectomy.

BACKGROUND: Laparoscopic cholecystectomy (LC) is generally safe and well-accepted. In rare cases, it is associated with complications (intra- e postoperative bleeding, visceral injury and surgical site infection). Splenic lesion has been reported only after direct trauma. We report an unusual case of splenic rupture presenting after "uncomplicated" LC. CASE PRESENTATION: A 77-year-old woman presented with distended abdomen, tenderness in the left upper quadrant and severe anemia 12 hours after LC. Clinical examination revealed hypovolemic shock. Abdominal computed tomography confirmed the dia

Aortic injury during diagnostic pediatric laparoscopy.

Major vascular injury during laparoscopic procedures is a rare but catastrophic complication. We report a pediatric case of aortic laceration during the setup phase of diagnostic laparoscopy in a 5-year-old girl with recurrent abdominal pain. The Veress needle inserted below the umbilicus confirmed the proper placement by use of a saline-filled syringe. The abdomen was insufflated without difficulty. The first trocar was inserted at the same point as Veress needle. The video laparoscope was introduced, and a small amount of blood was seen in the abdomen. We converted the procedure to laparotom

Sutureless laparoscopic extraperitoneal inguinal herniorrhaphy using reusable instruments: two hundred three repairs without recurrence.

Laparoscopic extraperitoneal hernia repair has several distinct advantages over the anterior repair and the laparoscopic transabdominal preperitoneal method. Laparoscopic extraperitoneal hernia repair allows detection and repair of occult contralateral defects with minimal risk of intraabdominal injury or adhesion formation and is associated with less pain and a quicker recovery. However, there are disadvantages. Circumferential mobilization of the spermatic cord and the use of staples to secure the mesh have been associated with injury to the spermatic cord and nerves. The cost of the laparos

Non-steroidal anti-inflammatory drugs associated with gastroduodenal injury and Helicobacter pylori.

OBJECTIVE: To clarify the relationship between non-steroidal anti-inflammatory drug (NSAID)-associated gastroduodenal mucosal injury and Helicobacter pylori infection. DESIGN AND METHODS: The incidence of H. pylori infection was determined in a group of patients treated with NSAID for rheumatoid arthritis for > or = 3 months and in a control group of patients with mainly abdominal symptoms but without rheumatoid arthritis and not being treated with NSAID. The incidence of H. pylori infection was also determined in patients treated with different NSAID and antirheumatic drugs. In addition, the

Bile duct disruption after laparoscopic cholecystectomy.

The reported prevalence of biliary tract disruption following laparoscopic cholecystectomy has ranged from 0% to 7% in early reports. We have reviewed the first 823 laparoscopic cholecystectomies performed at our institution and found 13 symptomatic biliary complications necessitating further therapy (prevalence 1.6%). This finding represents a decrease from the 2.7% prevalence found in our earlier series. The incidence of biliary complications will likely continue to vary depending on patient selection, operator experience, and new developments in laparoscopic technique. Bile duct injury and

New insight for management of blunt splenic trauma: significant differences between young and elderly.

BACKGROUND/AIMS: Although highly successful in children and young patients, the non-operative management of blunt splenic injury in the elderly has yet to be clearly studied. The purpose of this study was to determine whether or not a relationship exists among the mechanism of injury, the grade of splenic injury, the associated injuries, and whether patterns of injury differ between the young group (younger than 60 years old) and the elderly group (60 years and older than 60 years). METHODOLOGY: One hundred and sixty-seven patients (116 young patients including 30 early deaths and 51 elderly

An Anatomical Variant of the Accessory Right Hepatic Artery Arising From the Superior Mesenteric Artery: A Cadaveric Case Report.

The right hepatic artery (RHA) provides blood flow to the right lobe of the liver. In typical anatomy, the RHA branches from the proper hepatic artery, which branches from the common hepatic artery originating from the celiac trunk of the abdominal aorta. During a standard cadaveric dissection, a large accessory right hepatic artery (aRHA) was noted arising directly from the superior mesenteric artery. The lumen of the identified aRHA measured approximately 1 cm in diameter. The artery also appeared darker in color than the surrounding vasculature, with no evidence of prior surgery or procedur

Septic Shock and Rhabdomyolysis as Rare Complications Following Rectal Perforation: A Case Report.

Rectal perforation leads to massive translocation of intestinal bacteria and enteric fluid into the peritoneal cavity, which can cause severe intra-abdominal infection and may progress to sepsis. Beyond prompt surgical intervention and drainage, proactive prevention of potential postoperative complications is critically important. This report describes the case of a patient with septic shock secondary to rectal perforation complicated by rhabdomyolysis, who was managed in our intensive care unit. During treatment, HA380 hemoperfusion was utilized as an adjunctive therapy to assist in controlli

A Magnetic Hazard: A Case of Multiple Foreign Body Ingestion in a Two-year-old Child.

Magnetic foreign body ingestion is an increasingly recognized hazard in young children and poses a significant risk for severe gastrointestinal injury, particularly when multiple magnets are swallowed. Such cases can result in pressure necrosis, perforation, fistula formation, and bowel obstruction. We report the case of a 2-year-old girl who presented with a 3-4-day history of abdominal pain and vomiting. Imaging studies demonstrated multiple foreign bodies within the ileum, and the child was initially managed conservatively. Due to persistent symptoms and imaging findings, surgical explorati

A Case Series on Unilateral Elevation of Diaphragm.

We report a case series of seven patients aged 24 to 78 years who presented with unilateral elevation of the diaphragm secondary to diverse underlying mechanisms. The diagnoses included sarcoidosis-related pulmonary inflammation, gallbladder adenomyomatosis with sepsis and metabolic derangement, pulmonary tuberculosis with Chilaiditi's syndrome, acute pulmonary embolism with diaphragmatic eventration, superior vena cava syndrome due to non-small cell lung carcinoma, cryptogenic organizing pneumonia with an incidental thymic cyst and pyogenic liver abscess. This series emphasizes that unilatera

Emerald effluent in severe pancreatitis: Acute PD as dual-action support?

A 50-year-old man with severe presumed gallstone pancreatitis, multiorgan failure, and oliguric stage 3 acute kidney injury underwent bedside acute peritoneal dialysis (PD) for haemodynamically tolerated kidney support and intra peritoneal lavage. Catheter insertion drained a large volume of ascites that rapidly became vivid emerald green. Cultures remained sterile, whereas effluent amylase (11,041 U/L) and lipase (50,750 U/L) were markedly elevated, favouring inflammatory pancreatic leakage over infectious peritonitis or biliary perforation. During PD, effluent enzyme concentrations and cell

Sarcoidosis Presenting With Nephritis and Pancreatitis in a Pediatric Patient: A Rare Case.

Sarcoidosis is a multisystem granulomatous disorder of undetermined etiology that usually affects the lungs. It is a rare presentation to have an association between sarcoidosis and pancreatitis. We present a challenging case of acute pancreatitis and kidney dysfunction which had been diagnosed later to be sarcoidosis. A 12-year-old boy with abdominal pain and weight loss for one month, however, had no respiratory manifestations. Investigations revealed elevated lipase and creatinine. The patient was managed for acute kidney injury with an initial suspicion for autoimmune pancreatitis. Further

Budd-Chiari syndrome following abdominal trauma.

We report a case of Budd-Chiari syndrome (BCS) in a 44-year-old man with inferior vena cava (IVC) thrombosis and nephrotic syndrome. This case was complicated by right atrial clot and pulmonary emboli. Endothelial injury of the IVC was the likely mechanism, following a kick from a donkey. Abdominal ultrasonography revealed a large thrombosis located in a segment of IVC near its orifice in the right atrium. Transthoracic echocardiography (TTE) revealed IVC thrombosis that extended to the right atrium; however, pulmonary emboli (PE) were not documented in TTE. Intraoperative exploration revealed

Efficacy of Hyaluronic Acid in Radiation Proctitis: A Case Series.

Gastrointestinal toxicities of radiation might develop after exposure, thus leading to conditions such as abdominal pain, rectal bleeding, diarrhea, anemia, and weight loss. The development of drugs to reduce mucosal damage progression has been a focus on managing radiation proctitis. Radiation proctitis resulting from exposure to pelvic radiotherapies is effectively managed with topical administration on the anal mucosa. The clinical use of hyaluronic acid offers an innovative approach to managing radiation injury. Hyaluronic acid has multiple beneficial properties, such as regulating immune

Trans-anal Minimally Invasive Surgery Combined With a Robotic Anterior Approach for Sleeve Resection of a Huge Rectal Gastrointestinal Stromal Tumor.

Due to anatomical complexity, large rectal gastrointestinal stromal tumors (GISTs) in the pelvis at the anterior aspect often require extended abdominal surgery to obtain clear surgical margins. Here, we show our trans-anal minimally invasive surgery combined with a robotic anterior approach for a huge low rectal GIST that was widely in contact with the prostate and urethra. By performing lateral dissection first, we can identify the orientation of critical organs such as the prostate, urethra, and neurovascular bundles, facilitating anterior anorectal dissection without urethral injury. Uretero-vaginal fistulas - clinical presentation, treatment and literature overview.

A uretero-vaginal fistula (UVF) describes an abnormal connection between the ureter and vagina causing urinary incontinence, frequent infection, and discomfort. Although UVF might be diagnosed after vaginal delivery, infertility treatment or pelvic radiation therapy, gynecological operations, especially total abdominal hysterectomy, remain the leading cause of ureteral injury and formation of UVF. Traditional ureteroneocystostomy was usually the treatment of choice in patients with UVF. Nevertheless, it is now frequently replaced by less invasive endoscopic and percutaneous procedures which ar
