Serious infections of the central nervous system: encephalitis, meningitis, and brain abscess.

Central nervous system infections in adolescents range from the diffuse cerebritis of encephalitis to the regional inflammation of meningitis, and very focal disease of brain abscess. Clinical presentations reflect this wide spectrum, with encephalitis primarily characterized by altered mental status, meningitis by fever, headache, and neck stiffness, and brain abscess manifesting localizing findings. Encephalitis and viral meningitis are frequently caused by the seasonal enteroviruses and arboviruses, while most adolescent bacterial meningitis is due to Neisseria meningitidis and Streptococcu

Spinal epidural abscess in preverbal children: A case report with Currarino triad.

Spinal epidural abscess is rare in preverbal children and leads to permanent neurologic deficits if not treated promptly. Currarino triad (anorectal malformation, sacral bony abnormality and presacral mass) is also rare in children. We report the association of extensive spinal epidural abscess and Currarino triad in a young child.

Obturator internus muscle abscess in children: report of seven cases and review.

Obturator internus muscle (OIM) abscess is an uncommon entity often mistaken for septic arthritis of the hip. We describe seven children with OIM abscess and review seven previously reported cases. The most common presenting symptoms were hip or thigh pain (14 patients), fever (13), and limp (13). The hip was flexed, abducted, and externally rotated in 11 patients. Magnetic resonance imaging and computed tomography (CT) were diagnostic for OIM abscess in the 14 patients. Associated abscesses were located in the obturator externus muscle (5 patients), psoas muscle (2), and iliac muscle (1). The

Detection of circulating antigen in amoebic liver abscess by counter-current immunoelectrophoresis.

Fifty serum samples from patients with amoebic liver abscess and 50 from subjects who had not suffered from the disease (25 from patients with other, chiefly parasitic, infections and 25 from healthy blood donors, staff or students) were tested for circulating amoebic antigen by counter-current immuno-electrophoresis (CIEP). Amoebic antigen was detected in 38 sera (76%) from cases of amoebic liver abscess, but in none of the other sera. Although CIEP is only moderately sensitive, the high specificity suggests that this simple test may be useful in the diagnosis of amoebic liver abscess.

Appendiceal abscess mimicking infected urachal cyst in a child with intestinal malrotation.

An appendiceal abscess with intestinal malrotation can occur anywhere in the abdomen, not only in the right lower quadrant. We report a case presenting a midline mass of the lower abdomen whose computed tomography (CT) and ultrasonography (US) findings mimicked a urachal abscess. A retrospective review of CT findings led to the correct diagnosis by showing malposition of the ascending colon.

Spontaneous evacuation of cerebellar abscess through the middle ear. A case report.

Otogenic brain abscesses are a significant cause of mortality and morbidity in developing countries. Usually they occur either by distant thrombophlebitis of cerebral veins or by direct extension in an unsafe type of chronic suppurative otitis media. This case of cerebellar abscess was prepared for surgical excision, when sudden evacuation of pus through the left middle ear resolved the abscess and clinical features in the patient. No surgical intervention was required in this patient. The case under discussion is unique and unreported for its spontaneous evacuation through the middle ear.

Medically sound, cost-effective treatment for pelvic inflammatory disease and tuboovarian abscess.

OBJECTIVE: Our purpose was to determine the clinical effectiveness and cost-effectiveness of three antibiotic regimens for the treatment of pelvic inflammatory disease and tuboovarian abscess. STUDY DESIGN: A review of all patients' hospitalized at Hutzel Hospital, Detroit, Michigan, for treatment of pelvic inflammatory disease and tuboovarian abscess between Jan. 1, 1993, and April 30, 1997, was performed. Demographic data, antibiotic choices, changes in therapy, operative interventions, and cost of therapy were assessed. RESULTS: Two hundred three patients were admitted for treatment of pe

Recurrent parotid abscess formation 8 years after an episode of cancrum oris.

Recurrent parotid abscess formation as a consequence of isolated right parotid duct osteal stenosis was a very unusual sequela of successfully treated early cancrum oris. Interestingly, it manifested after an uneventful interval of 8 years and required a near-total conservative parotidectomy for cure.

Immunohistochemical detection for Actinomyces sp. in swine tonsillar abscess and granulomatous mastitis.

The tonsils of eleven pigs and the mammary glands of a sow were used to investigate actinomycotic lesions due to Actinomyces sp. infection. At necropsy, there was no abnormality on these tonsils, on the other hand, numerous abscesses containing sulfur granules were found in the mammary. Histopathologically, the Actinomyces sp. lesions were noted as crypt abscesses in the tonsils and as pus-forming granulomas in the mammary glands. The microorganisms in both lesions were composed of bead-like cocci, bacillary cells and short, branching filaments, those cells being positive by the Gram's and Gro

Cholecystogastrocolonic fistula with intrahepatic abscess: a rare complication of biliary stone disease.

A 52-year-old woman exhibited both a cholecystogastrocolonic fistula and intrahepatic abscess. Biliary-enteric fistulas are unusual complications of gallstone disease, and multiple, complex fistulas are extremely uncommon. Hepatic abscesses are also unusual complications of gallstone disease. The combination of a complex biliary-enteric fistula and hepatic abscess arising from gallstone disease has not been reported. In our patient, surgery was prompted by the hepatic abscesses; the fistula was not recognized preoperatively. We elected to resect gallbladder, gastric antrum, and transverse colo

Iliopsoas abscess following catheterization of the femoral artery: diagnostic and treatment strategies.

The following case is presented to illustrate the necessity of arthrotomy of the hip when an iliopsoas abscess lies adjacent to the hip joint capsule. Arthrotomy of the hip through a separate incision adds minimal morbidity, does not expose the hip to the abscess, and assures the surgeon that the hip has been debrided. This is important considering the 15% incidence of communication of the iliopsoas bursa with the hip joint.

[A case of urachal abscess resected electively after successful drainage from umbilicus].

A 22-year-old woman presented to our hospital complaining of high fever and a painful fist-sized mass in the lower abdomen. From the results of imaging studies, including abdominal urtrasonography and computerized tomography, the patient was diagnosed as having a urachal abscess. Despite intensive antibiotic therapy, her high fever persisted. Drainage via the umbilicus was then performed with a pigtail catheter being inserted into the abscess through a fistula. Her temperature normalized within a few days and the size of the abscess gradually decreased. Total excision of the urachus and partia

Syndrome of inappropriate secretion of antidiuretic hormone (SIADH) in amoebic liver abscess.

We report here three cases of amoebic liver abscess with biochemical abnormalities of syndrome of inappropriate secretion of antidiuretic hormone (SIADH). All three patients were febrile and had pain in the right upper abdomen radiating to the right shoulder. Liver function tests were disturbed. Chest X-ray showed raised right hemidiaphragm with obliteration of the right costophrenic angle. On ultrasound, an abscess 710 cm in diameter was located in the upper right lobe. SIADH was transient and improved with treatment. We believe this to be the first reported association of SIADH with amoebic

Tophaceous gout of the lumbar spine mimicking an epidural abscess: MR features.

We present the MR findings in a case of tophaceous gout of the lumbar spine with cauda equina compression, mimicking an epidural abscess. The diagnosis was clinically unsuspected and confirmed at laminectomy. Tophus material in the paraspinal soft tissues appeared intermediate in signal intensity and showed heterogeneous contrast enhancement.
