Divergent fertility responses to cyclones and floods: a mixed-methods study from rural Bangladesh.

Over the past decade, Bangladesh has experienced substantial demographic change alongside increasing climate-related disruption. This study examines the links between extreme climatic events and fertility among married Bangladeshi women of childbearing age who already have children, focusing on how cyclones and floods relate to fertility behaviour in cyclone- and flood-prone villages. A mixed-methods design was used, drawing on 544 household surveys, 74 in-depth interviews with women, 10 focus group discussions with men, and 8 key informant interviews. Comparing two ecologically distinct sites

The Affordable Care Act Improves Access, Survival, and Racial Disparities of Patients With Liver Disease: A Systematic Review.

BACKGROUND & AIMS: Given the evolving epidemiology of liver disease, escalating healthcare costs, and persistent racial health disparities, evaluating the Affordable Care Act (ACA)-the most substantial U.S. healthcare reform to date-is critical for informing future policy. We conducted a systematic review to assess the impact of the ACA and its Medicaid Expansion (ME) provision on access to care, survival, and racial and ethnic disparities among patients with chronic liver disease (CLD). METHODS: We reviewed studies published between 2010 and 2025 that included adults with CLD and evaluated A

Variation in Hypertension Control by Race and Ethnicity, and Geography in US Veterans.

BACKGROUND: Hypertension control and related cardiovascular outcomes among Americans remain suboptimal, and differ by race, ethnicity, and geography. Healthcare access is one of multiple critical factors in hypertension control. Understanding the degree to which healthcare access, versus other factors, produce these outcomes can inform policies and interventions to improve cardiovascular outcomes and reduce disparities. Department of Veterans Affairs Healthcare System data provide a unique opportunity to understand residual racial and ethnic differences in hypertension control after accounting

Foot-and-mouth disease (FMD) vaccine market survey at agro-veterinary stores in rural and peri-urban areas of Nigeria.

Globally, the annual economic impact of foot-and-mouth disease (FMD) is estimated at USD 11 billion in endemic settings, with the impact most profound among smallholder farmers. Farmers and the agro-veterinary stores that supply them are marginalized populations with limited to no access to veterinary care, and paucity of information on the availability and accessibility of vaccines. The objective of this study was to assess the accessibility and distribution channels of FMD vaccine, and the challenges related to the FMD vaccine market in these regions where livestock farming is highest and is

Bridging Needs and Expectations of Individuals With Physical Disabilities and Community Services Stakeholders for the Cocreation of an Adapted Physical Activity Platform in Virtual Reality: Qualitative Study.

BACKGROUND: Physical activity supports the health and well-being of individuals with physical disabilities. Despite the significance of engaging in physical activity, barriers faced by individuals with disabilities, such as limited access to adapted facilities and lack of transportation, can restrict their participation. Community organizations play a role in addressing these challenges, but virtual reality (VR) also offers a way to diversify adapted activities. In some situations, VR can help overcome the resource limitations of organizations by providing accessible, engaging, and highly pers

Breast Cancer Barriers to Education and Screening in People Experiencing Homelessness.

BACKGROUND: Each year in the United States, approximately 42,211 women and men die from breast cancer. Individuals experiencing homelessness are disproportionately affected, facing a higher incidence of the disease. People experiencing homelessness (PEH) have a higher incidence of breast cancer and mortality than domiciled individuals. Across all 50 states, the Centers for Disease Control and Prevention provides free breast cancer screenings for eligible individuals with limited financial resources. However, PEH have screening rates that remain below the national average. Early detection is cr

"I just don't know what will be better other than an apartment which I can't afford": Experiences of housing quality following homelessness in Ontario, Canada.

Having access to good quality housing is a key determinant of well-being. Little is known about experiences of housing quality following homelessness from the perspectives of persons with lived experience. To build on existing literature, we conducted a secondary analysis of qualitative interviews with 19 individuals who had experiences of transitioning to housing following homelessness. Interview transcripts were drawn from a community-based participatory research study exploring the conditions needed for thriving following homelessness in Ontario, Canada. We analyzed these transcripts using

Designing and Implementing a Web-Based Platform for Accurate and Reliable Clinical Outcome Measures and Global Certification for Evaluating Charcot-Marie-Tooth disease.

BACKGROUND AND AIMS: Accurate, reliable and sensitive clinical outcome measures in rare neurologic conditions, such as Charcot-Marie-Tooth disease (CMT), are essential for monitoring disease progression and evaluating treatment efficacy. Ensuring measures such as the CMTPedS, CMTInfS and CMT-FOM are easily accessible removes a barrier to implementation. The aims of this project were to: (1) design a web-based platform to enable real-time scoring of CMT outcome measures; (2) implement co-designed training and quality assurance resources; and (3) establish a Global Certification Standard for cli

Living Well: Protocol for a web-based program to improve quality of life in rural and urban ovarian cancer survivors.

BACKGROUND: Ovarian cancer (OC) survivors commonly experience chronic symptoms including anxiety, depression, sleep disturbances, fatigue, physical symptoms, poor health-related quality of life (HRQOL), and a generally poor prognosis. Additionally, factors such as social isolation, stress, and depression are associated with key biological processes promoting tumor progression and poorer survival. Accessible psychosocial interventions to improve HRQOL and clinical outcomes are needed. This need is particularly true in rural settings where survivors may have less access to clinic-based support s

A qualitative study of how structural vulnerability shaped COVID-19 testing behaviors in Portland, Maine.

BACKGROUND: People with structural vulnerabilities (including immigrants, people who use drugs, and those who are unhoused or uninsured) are more likely to experience COVID-19 testing disparities relative to other groups. We documented barriers and facilitators to COVID-19 testing and explored how structural vulnerabilities created and/or exacerbated COVID-19 testing barriers. METHODS: Between 2021 and 2022, we conducted semi-structured interviews with 34 members of structurally vulnerable populations and 27 key informants who provide health and social services to them. Our abductive analysis

Arteriovenous access creation and hazards of hospitalization and death in patients starting hemodialysis.

BACKGROUND: Recent evidence suggests an overestimation of the benefits associated with arteriovenous (AV) fistula versus graft in certain populations. We assessed hazards of all-cause and cause-specific hospitalization and death associated with AV access type in patients who started hemodialysis with a catheter in France, overall and by subgroups of age, sex and comorbidities. METHODS: We performed a target trial emulation including patients who initiated hemodialysis with a catheter from 2010 through 2018 and were followed by the REIN Registry. We identified first-created fistula or graft th

Digital screening method using social media advertising for the remote assessment of trigeminal neuralgia.

OBJECTIVE: Global trends, such as improving accessibility to healthcare services through the Internet, and the COVID-19 pandemic are among the driving factors in the adoption of digital health. This study hypothesized that digital solutions can reach and gather data from a large number of patients with trigeminal neuralgia (TN), a commonly misdiagnosed neuropathic facial pain syndrome, and quickly and fast-track their diagnosis by suggesting them to consult a neurologist. We developed an accessible digital screening tool based on patient symptoms and history to test this hypothesis and used so

Assessment of barriers for people with disability to enjoy national parks.

There is increasing awareness of the importance of nature-based recreation to human wellbeing. Given its significant benefits, natural-protected area management has not always provided equitable access to all potential users. Limited research till now has been focused on how the outdoor barriers impact the equal enjoyment of people with mobility/motor disabilities (PwMDs) of nature when promoting sustainable nature-based tourism. This study intends to shed light on those aspects which constrain the PwMD's enjoyment of the great outdoors at national parks (NPs). The study was carried out in Can

Genetic differentiation of a southern Africa tepary bean (Phaseolus acutifolius A Gray) germplasm collection using high-density DArTseq SNP markers.

Genetic resources of tepary bean (Phaseolus acutifolius A. Gray) germplasm collections are not well characterized due to a lack of dedicated genomic resources. There is a need to assemble genomic resources specific to tepary bean for germplasm characterization, heterotic grouping, and breeding. Therefore, the objectives of this study were to deduce the genetic groups in tepary bean germplasm collection using high-density Diversity Array Technology (DArT) based single nucleotide polymorphism (SNP) markers and select contrasting genotypes for breeding. Seventy-eight tepary bean accessions were g

A Cross-Sectional Analysis of Self-Reported Needs and Health Service Utilization Among Transgender Women in Lima, Perú.

Purpose: Globally, transgender women (TGW) experience wide-ranging barriers to health and care, with disproportionately high risks of infectious and chronic diseases. Yet, research on transgender populations' access to care in low- and middle-income countries remains limited, focused on human immunodeficiency virus (HIV) infection, and assesses TGW as a homogenous group. We analyzed morbidity and health service uptake patterns among TGW in Lima, Perú, to understand health outreach and service needs to inform targeting and design of community-level interventions. Methods: This cross-sectional s

Contraceptive utilization at publicly funded clinics before and after introduction of low-cost levonorgestrel intrauterine system: a retrospective case study analysis.

Cost can be a barrier to accessing safe and affordable contraception. Therefore, we sought to understand how the availability and utilization of a low-cost levonorgestrel intrauterine system (hormonal IUD) impacts uptake relative to other contraceptive methods. Another objective was to determine if the percentage of self-pay women who obtained a hormonal IUD changed over time. Electronic health record (EHR) data was collected from five public health organizations enrolled in the 340B drug-pricing program (30 individual health centers) from April 15, 2014, through December 31, 2017, with most s
