Clinical aspects and causes of rickets in a Kenyan population.

Twenty nine patients with rickets were studied in a one year period. The majority of patients (17/29) were below 2 years of age. Most of them had nutritional rickets resulting from a combination of factors. Premature delivery, nonexposure to sunlight, nutritional marasmus and inappropriate dietary intake. Some had familial hypophosphataemic rickets, others had renal tubular acidosis while the rest had rickets with a familial tendency. Both the previous hospital records and the present study indicate that rickets is a persistent problem in children in the community and should be suspected in ch

[Hematologic and clinico-chemical research findings in calves and young cattle with ileus].

The impairment of fluid, electrolyte and acid-base status which is observed in calves and juvenile cattle as a result of ileus and subileus is not only dependent on form and site of the obstruction, but is also influenced considerably by the duration of the condition and any coexisting disease. This publication reviews the most important blood, urine and ruminal fluid parameters in such patients. The diagnostic significance of the laboratory values under discussion is not so great with obstructions to the caudal part of the digestive tract. However, markedly raised ruminal fluid chloride level

Cardiogenic shock and L-carnitine: clinical data and therapeutic perspectives.

Research experiences on the use of L-carnitine in conditions of acute hypoxia underline the protective role of this molecule on the cellular enzymic complex. To obtain unconfutable clinical data at this regard, the survival rate in two groups of patients affected by cardiogeic shock was evaluated. The first group (80 patients) was treated with L-carnitine while the second group (36 patients) received sodium bicarbonate. The results showed a significant response to L-carnitine treatment, indicating the role of this molecule on the metabolic acidosis due to shock. The sum of these data confirmed

Deficiency of subunits of Complex I and mitochondrial encephalomyopathy.

Enzymic activities of the respiratory chain and content of immunochemically detectable subunits in NADH-ubiquinone oxidoreductase (Complex I) were measured in mitochondria from the skeletal muscles of 4 patients with mitochondrial myopathy, encephalopathy, lactic acidosis, and strokelike episodes (MELAS). The rotenone-sensitive NADH-cytochrome c reductase activity was extremely decreased, ranging from 0% to 27% of the control value. In all patients, the content of subunits of Complex I was also reduced in parallel with the rotenone-sensitive NADH-cytochrome c reductase activity. It is suggeste

Acute salicylate self-poisoning in 177 consecutive patients treated in ICU.

The course of 177 consecutive patients with severe salicylate self-poisoning treated in an intensive care unit (ICU) during a period of 15 years is presented. On admission, cerebral depression was observed in 61% respiratory failure was present in 47%, acidosis in 36% and cardiovascular function was impaired in 14%. A mortality rate of 15% was observed, which was proportionally higher in patients more than 40 years old and in patients with delayed diagnosis. Twenty-seven patients died and an autopsy was performed on 26 patients. The main autopsy diagnosis was ulcers of the gastrointestinal tra

[Approaches to the creation of a model of cerebral thrombocytic microembolism and to research on the effects of drugs].

It is shown that the introduction of collagen into the carotid artery brings forth an increase in perfusion pressure during cat cerebral autoperfusion due to blockade of micro-vessels by platelet aggregates. This increase is of a transitory character and is accompanied by changes in the acid-base balance of the arterial blood towards acidosis. It has been established that papaverine and cavinton (vinpocetine) do not cause recanalization of the blocked microvessels, and in case of verapamil it is of a partial character. The infusion of prostacyclin brings about an almost total recanalization of

Epileptiform activity induced by alkalosis in rat neocortical slices: block by antagonists of N-methyl-D-aspartate.

The effects of changing extracellular pH on epileptiform activity induced by the removal of magnesium ions from the perfusing medium were studied. The proportion of bicarbonate in the artificial cerebrospinal fluid and of CO2 in the gas mixture were altered to mimic metabolic and respiratory acid-base disturbances. Changes in pH of 0.2 unit from control produced marked effects. Epileptiform activity was enhanced by alkalosis and diminished by acidosis. In normal magnesium-containing medium metabolic alkalosis (pH greater than 7.8) induced spontaneous epileptiform activity that was blocked by s

Malignant pneumomediastinum: successful tube mediastinostomy in the neonate.

Five critically ill neonates underwent tube mediastinostomy in the neonatal intensive care nursery for tension pneumomediastinum. All of the neonates showed immediate clinical improvement, with a decrease in peak airway pressure and elevation of arterial oxygen pressure levels. There were no complications directly attributable to the procedure. We believe that pneumomediastinum in association with severe hypoxia, metabolic acidosis, and high ventilation pressures indicates clinically significant tension in the mediastinum. This results in a decrease in systemic blood pressure and pulmonary ven

Anesthesia in Göttingen miniature swine used for experimental surgery.

This contribution describes a modern technique of producing general anesthesia in the Göttingen minipig. The method of anesthesia selected is dependent on the duration and type of surgery. The drugs used to produce anesthesia should not interfere with the experimental aims, therefore only drugs with well known actions and a standardized anesthesiological procedure should be used. Climazolam, Ketamine and atropine are preferred for induction of anesthesia. After endotracheal intubation, inhalation anesthetic mixed with oxygen and nitrous oxide is used to maintain general anesthesia. Controlled

Use of sodium bicarbonate to treat tricyclic antidepressant-induced arrhythmias in a patient with alkalosis.

Sodium bicarbonate has been recommended for the treatment of arrhythmias induced by tricyclic antidepressants. It is unclear, however, whether this therapy is effective only in the presence of acidosis. A case is presented in which there was an immediate response to sodium bicarbonate in three episodes of ventricular tachycardia despite the presence of alkalosis on two of the three occasions. Given the poor response to conventional therapy of arrhythmias induced by tricyclic antidepressants the use of sodium bicarbonate may be reasonable even in the presence of alkalosis. However, in the prese

Complete atrioventricular block, shock, and hyperkalemia induced by toxic adenoma of the thyroid gland.

The case of a 74-year-old woman with toxic thyroid adenoma, presenting as complete heart block, is described. Upon admission the patient was in a state of circulatory collapse with metabolic acidosis and hyperkalemia. After initiation of right ventricular pacing the circulatory and metabolic disorders rapidly normalized without any other specific therapy. Since treatment and attainment of the euthyroid state the patient has had no conduction delays or arrhythmias. Complete heart block in thyrotoxicosis is uncommon and previous reports relate to its occurrence in Graves' disease, usually compli

Acute toxicology of ethanol ingestion. Role of the clinical laboratory.

Information from the clinical laboratory can contribute in several ways to the evaluation and management of the acute consequences of alcohol ingestion. If a blood alcohol analysis is not available, measurement of serum osmolality can be helpful. An elevated ethanol level can explain central nervous system depression. Evidence of acute or chronic alcohol use can explain atypical responses to various drugs. Severe acidosis in association with an increased anion gap may suggest ingestion of other toxic volatiles, such as methanol or ethylene glycol. Alcoholic ketoacidosis, also associated with a

Severe hypophosphatemia may be observed after the ingestion of certain antacids which complex phosphorus in the intestinal lumen. In severe burn cases, during realimentation after denutrition, during intense and prolonged hyperventilations responsible for respiratory alkalosis, in diabetic ketoacidosis and in alcoholics. Hypophosphatemia lead to erythrocyte abnormalities which may eventually approach hemolysis, to phagocytosis and platelet function disorders and to neurological troubles which suggest a metabolic encephalopathy, a myopathy, a metabolic acidosis or a change in hepatic functionin

The effect of pH on platelet and coagulation factor activities.

After observing that lavage of the stomach with alkaline solutions has a beneficial effect on the control of gastric hemorrhage, we examined the platelet and coagulation factor activities under conditions of lower and elevated pH. The results showed that change of the pH from acidosis to 7.0 and even to slight alkalosis induces platelet aggregation, platelet calcium and serotonin release, as well as platelet factor II availability. The prothrombin and partial thromboplastin times approached normal levels, whereas the fibrinogen level did not change significantly. The results obtained may serve

Treatment of diabetic coma with continuous low-dose infusion of insulin.

Thirty-eight patients in diabetic coma from four different centres were treated with a continuous low-dose intravenous infusion of insulin at an average dose of 7.2 IU/hr. All patients recovered rapidly except for one profoundly shocked patient who died. The mean fall in plasma glucose was 58% four hours after the start of insulin. Blood ketone bodies and plasma free fatty acids showed a similar response. There was no significant difference in plasma glucose response according to severity of acidosis or previous treatment with insulin. Hypokalaemia was uncommon. In the treatment of diabetic co

Effects of temperature management on postoperative myocardial injury in patients undergoing laparoscopic radical resection for colorectal cancer: A randomized controlled trial.

BACKGROUND: Intraoperative hypothermia is common in patients undergoing laparoscopic radical resection for colorectal cancer. This study evaluated the effectiveness of maintaining 2 different intraoperative core temperature targets (36°C and 37°C) compared with routine thermal management in reducing postoperative myocardial injury in this patient population. METHODS: In this randomized controlled trial, A total of 144 patients were randomly assigned to 1 of 3 groups: routine (target nasopharyngeal temperature ≥35°C), aggressive (≥36°C), or intensive (37°C) thermal management, utilizing a forc

A toothache gone wrong: an unexpected case of perioperative lactation ketoacidosis.

BACKGROUND: Lactation ketoacidosis is a rare metabolic complication of breastfeeding, characterised by high anion gap metabolic acidosis due to elevated levels of ketone bodies in the blood or urine. Common symptoms include nausea, vomiting, abdominal pain and general malaise. The typical triggers for lactation ketoacidosis are intensive exercise, a ketogenic diet, reduced food intake or fasting, often associated with an attempt to lose weight. CASE PRESENTATION: We report the case of a 31-year-old woman who had been breastfeeding continuously for two years, including throughout her most recen

Starvation Ketosis Versus Euglycemic Diabetic Ketoacidosis in Pregnancy: A Case Report Highlighting the Diagnostic Overlap.

INTRODUCTION: Starvation ketosis in pregnancy, though rare, is increasingly recognized, underscoring the need for a better understanding of its pathophysiology. Prolonged fasting or inadequate carbohydrate intake leads to elevated ketone levels, which can adversely affect maternal and fetal health. Early diagnosis via ketone monitoring and clinical assessments, is essential. The clinical presentation and laboratory profile of starvation ketosis overlap with euglycemic diabetic ketoacidosis (DKA), often creating a diagnostic dilemma. CASE PRESENTATION: A 28-year-old female with gestational dia

Analysis of the fecal microbiome and metabolome in dairy cows with different body condition scores.

Holstein Friesian is the most popular breed of dairy cows worldwide due to its exceptional milk production capabilities. In dairy cow management, the body condition score (BCS) is a useful tool, serving as a reliable indicator of a cow's nutritional status and overall health. It is determined via a subjective visual and tactile assessment of fat cover and muscle mass. A low BCS is associated with decreased milk production and fertility. While genetic and nutritional factors have previously been associated with BCS, their effects are often moderate. In this study, we compared the fecal microbio
