First successful allogeneic hematopoietic stem cell transplantation in STING-associated vasculopathy of infancy-A case report.

Stimulator of interferon genes (STING)-associated vasculopathy with onset in infancy (SAVI) is a monogenic autoinflammatory disorder caused by gain-of-function mutations in TMEM173, leading to constitutive STING activation and persistent type I interferon signaling. Affected children develop early-onset cutaneous vasculopathy, systemic inflammation, and progressive interstitial lung disease, often refractory to standard immunosuppressive treatments. Janus kinase (JAK) inhibitors offer partial transient control, with risk of irreversible organ damage. Allogeneic hematopoietic stem cell transpla

Systemic lupus erythematosus complicated by thrombotic microangiopathy with atypical HUS features: A case report.

RATIONALE: Systemic lupus erythematosus (SLE)-associated thrombotic microangiopathy with features of atypical hemolytic uremic syndrome (aHUS) is a rare but life-threatening complication. Early recognition is challenging because of its overlapping clinical manifestations with other thrombotic microangiopathies. PATIENT CONCERNS: A 49-year-old man with SLE presented with anemia, thrombocytopenia, and multi-system involvement. Despite initial immunosuppressive therapy, his hematologic parameters progressively deteriorated, with hemoglobin decreasing to 89 g/L and the platelet count to 54 × 109/

Disease-related complications and risk factors in hemoglobin H disease in a Thai multicenter registry.

Hemoglobin H (Hb H) disease is a prevalent hereditary hemolytic anemia worldwide. Clinical severity varies by genotype, particularly between deletional and non-deletional forms, as well as the presence of co-inherited Hb E. However, disease-related complications remain underreported. We analyzed a Thai multicenter registry of 1,298 patients with Hb H disease to evaluate disease-related complications and risk factors. Deletional Hb H disease accounted for 55.9% of cases, whereas 44.1% had non-deletional disease. Hb E co-inheritance was present in 253 patients (19.5%). Patients with non-deletion

A Case of Hemolysis, Elevated Liver Enzyme, and Low Platelet Count (HELLP) Syndrome-Precipitated Acute Thrombotic Thrombocytopenia Purpura.

Hemolysis, elevated liver enzyme, and low platelet count (HELLP) syndrome and thrombotic thrombocytopenic purpura (TTP) are both thrombotic microangiopathies that share overlapping clinical and laboratory features, making differentiation during pregnancy particularly challenging. We present a case of a 27-year-old woman at 21 weeks gestation who initially presented with hypertensive emergency and right middle cerebral artery (MCA) ischemic stroke requiring embolectomy and whose subsequent intensive care unit (ICU) course was complicated by fever, severe thrombocytopenia, microangiopathic hemol

Laboratory characteristics of direct antiglobulin test-positive anemia in patients with newly diagnosed multiple myeloma.

INTRODUCTION: This study retrospectively characterized the laboratory features of direct antiglobulin test (DAT)-positive anemia in newly diagnosed multiple myeloma (MM) and explored associated immunologic alterations to better understand MM-related immune dysregulation. METHODS: We enrolled 77 patients with newly diagnosed MM admitted to our hospital between 2021 and 2025. Based on DAT results, patients were assigned to a DAT-positive group (n = 52) or a DAT-negative group (n = 25). Clinical characteristics, laboratory parameters, and peripheral blood T-cell subsets were compared between gro

Pharmacological readthrough and base editing for ANK1 nonsense mutations in an erythroid model of hereditary spherocytosis.

Hereditary spherocytosis (HS) is the most common inherited chronic hemolytic anemia and results from defects in proteins of the erythrocyte membrane skeleton. Nonsense mutations in ANK1 are a common cause of HS, yet current treatments remain largely supportive without addressing genetic defects. In this study, we identified two ANK1 nonsense mutations (p.R281X and p.Q744X) in unrelated HS pedigrees and generated K562 erythroid differentiation models using cytosine base editor-mediated knockin to evaluate potential targeted therapies. Functional analyses confirmed that both mutations markedly r

Molecular epidemiology and multilocus characterization of canine hemoplasmas in Thailand reveal the first 23S rRNA gene sequences of 'Candidatus Mycoplasma haematoparvum'.

Hemotropic Mycoplasma spp., also known as hemoplasmas, are blood-borne pathogens that infect red blood cells and can cause hemolytic anemia in mammals, including dogs. Mycoplasma haemocanis (Mhc) and 'Candidatus Mycoplasma haematoparvum' (CMhp) are the two most common canine hemoplasma species. However, molecular epidemiological data and information on their genetic diversity remain scarce in Thailand. This study investigated hemoplasma PCR positivity in 472 client-owned dogs from Bangkok and Nakhon Pathom and 199 community dogs from Chiang Mai using a polymerase chain reaction (PCR) assay tar

Lecithin-cholesterol acyltransferase deficiency with the finding of "zebra bodies": A diagnostic challenge in the context of suspected Fabry disease.

Lecithin-cholesterol acyltransferase (LCAT) deficiency is a rare autosomal recessive disorder resulting from mutations in the LCAT gene, which leads to abnormal lipoprotein metabolism. This results in markedly reduced high-density lipoprotein cholesterol and the accumulation of lipoprotein X, leading to renal, corneal, and hemolytic damage. Two clinical variants have been described: familial LCAT deficiency (FLD) and fish-eye disease (FED). We report the case of a 41-year-old male with a history of hypertension, tinnitus, and progressive hearing loss, who presented with bilateral corneal opaci

Immune cell-derived membrane nanovesicles: A promethean fire for autoimmune disease therapy through immune cell mimicry.

Autoimmune diseases (AIDs) constitute a heterogeneous group of disorders characterized by immune dysregulation, loss of self-tolerance, and chronic inflammation, which leads to tissue damage and organ dysfunction. Current therapies for AIDs are often limited by their lack of specificity, systemic side effects, and insufficient restoration of immune tolerance. Recent advances in nanotechnology and bioengineering have introduced immune and associated cell-derived membrane vesicles (IACMVs) as a promising therapeutic platform. Derived from macrophages, dendritic cells, neutrophils, platelets, or

Challenges in managing iTTP: insights into ADAMTS13 inhibitor boosting during caplacizumab therapy.

Immune-mediated thrombotic thrombocytopenic purpura (iTTP) is a rare but life-threatening disorder characterized by severe thrombocytopenia, hemolytic anemia, and end-organ ischemic damage. The introduction of caplacizumab, an anti-von Willebrand factor A1 nanobody, has revolutionized the treatment of patients with iTTP by preventing fatal thrombotic events and shortening the time to platelet normalization. Despite its benefits, caplacizumab does not address the challenge of anti-ADAMTS13 autoantibody production, posing a risk of ADAMTS13 inhibitor boosting and delayed recovery of ADAMTS13 act

Multiple congenital anomalies in two fetuses with glutathione-synthetase deficit (GSS).

Glutathione synthetase deficiency is a rare inborn metabolic disease usually caused by biallelic variants in GSS. Clinical severity varies from isolated hemolytic anemia, sometimes associated with chronic metabolic acidosis and 5-oxoprolinuria, to severe neurological phenotypes with neonatal lethality. Here we report on two fetal siblings from two pregnancies with glutathione synthetase deficiency exhibiting similar multiple congenital anomalies associating phocomelia, cleft palate, intra-uterine growth retardation, genito-urinary malformations, and congenital heart defect. Genome sequencing s

A Rare Case of Atypical Hemolytic Uremic Syndrome Presenting as Chronic Interstitial Nephritis.

Atypical hemolytic uremic syndrome commonly presents as rapidly progressive renal failure and is histologically characterized by thrombotic microangiopathy (TMA). TMA presenting with acute renal failure requires aggressive medical management. Here, we present a case of a 30-year-old man who presented with a history of accelerated hypertension and a strong family history of end-stage renal disease, in September 2023. Upon evaluation, he was found to have a creatinine level of 2 mg/dl, bland urine and normal-sized kidneys; a renal biopsy revealed chronic interstitial nephritis. Genetic analysis

The impact of autoimmune cytopenias on the clinical course and survival of Hodgkin lymphoma.

The characteristics of autoimmune cytopenias (AICP) associated with Hodgkin lymphoma (HL) are not thoroughly defined. We retrospectively assessed the clinical features of HL-associated AICPs in 563 HL patients diagnosed over a period of 28 years. We identified 8 cases of autoimmune hemolytic anemia (AIHA) and 8 cases of autoimmune thrombocytopenia among 14 patients altogether. Four (26%) AICPs were present at lymphoma diagnosis, while 11 (74%) cytopenias occurred during follow-up after first-line therapy. The overall incidence of HL-associated AICPs was 2.8%. Nine (75%) cytopenias responded to
