Heavy gravity combined with light gravity local anesthetic in subarachnoid anesthesia for cesarean section did not reduce the incidence of intraoperative hypotension in maternal women: a prospective cohort study.

BACKGROUND: Subarachnoid anesthesia is the primary anesthetic method for elective cesarean section surgery, characterized by rapidly taking effect and reliable analgesia. However, subarachnoid anesthesia is prone to cause a high block level, resulting in a high incidence of maternal hypotension. How to reduce the incidence of maternal hypotension under subarachnoid anesthesia is a practical problem that needs to be solved urgently in clinical practice. METHODS: This prospective cohort study was performed at Nantong Maternal and Child Health Care Hospital in China between January and July 2023

Compliance with ESAIC guidelines for fibrinogen concentrate prescription in cardiac surgery: a multicentre French cohort study.

BACKGROUND: Fibrinogen concentrate may reduce allogeneic blood product transfusion in cardiac surgery patients with bleeding associated with acquired hypofibrinogenemia. The European Society of Anaesthesiology and Intensive Care (ESAIC) has issued guidelines, but compliance to these guidelines has not been studied yet. METHODS: This multicentre observational cohort study was aimed at evaluating the compliance of fibrinogen prescription with ESAIC. Adult patients undergoing cardiac surgery with cardiopulmonary bypass in 13 French cardiac surgery centres were recruited from March 2017 to April

Intraoperative hypotension prediction in cardiac and noncardiac procedures: is HPI truly worthwhile? A systematic review and meta-analysis.

BACKGROUND: Intraoperative hypotension (IOH), defined as a mean arterial pressure (MAP) below 65 mmHg, is a common complication during surgery and is associated with significant postoperative morbidity, including acute kidney injury, myocardial injury, stroke, and increased mortality. Despite the availability of traditional monitoring techniques, predicting and preventing IOH remains a challenge. The Hypotension Prediction Index (HPI), a machine-learning algorithm developed by Edwards Lifesciences, aims to predict IOH by analyzing real-time arterial waveform data, offering an opportunity for p

Ultrasonography evaluation of the inferior vena cava collapsibility index and the superior vena cava distensibility index for predicting hypotension during general anesthesia in atrial fibrillation patients: a prospective observational study.

BACKGROUND: Atrial fibrillation (AF) is a prevalent arrhythmia, the ineffective contraction of the atria leads to a decrease in effective cardiac output. AF patients are prone to hypotension during anesthesia, especially in the early stages of general anesthesia. We explored whether the inferior vena cava collapsibility index (IVCCI) or the superior vena cava distensibility index (SVCDI) could predict the occurrence of post-induction hypotension (PIH) and early intraoperative hypotension (eIOH) in AF patients. METHODS: A total of 77 AF patients undergoing left atrial appendage occlusion under

Hypotension prediction index smart alerts and trends to improve compliance with goal-directed hemodynamic therapy: a feasibility study.

BACKGROUND: Goal-directed hemodynamic therapy (GDHT) and Hypotension Prediction Index (HPI, Edwards Lifesciences, Irvine, CA) technology are used to improve post-operative patient outcomes. Trials evaluating GDHT are frequently limited by poor algorithm compliance. The Acumen HPI Software incorporates smart trends and alerts that highlight the probable mechanism(s) of hypotension and facilitate compliance with the GDHT algorithm. We hypothesized that prompts from the Acumen HPI Smart Alerts would improve compliance to a GDHT algorithm during moderate-to-high-risk elective noncardiac surgery.

Limited effect of leg elevation in preventing intraoperative hypotension during total shoulder arthroplasty in the beach chair position: a randomized controlled trial.

BACKGROUND: The beach chair position, often used in shoulder surgery, increases the risk of hypotension and cerebral hypoperfusion. This randomized controlled trial evaluated the efficacy of leg elevation in preventing hypotension during total shoulder arthroplasty in the beach chair position. METHODS: Fifty patients scheduled for total shoulder arthroplasty in the beach chair position were randomly assigned to the control (25 patients) or treatment (25 patients) groups. The treatment group elevated their legs during surgery, whereas the control group maintained a neutral leg position. The pr

Evaluating the effect of caudal epidural block on optic nerve sheath diameter in pediatric patients: randomized controlled study.

INTRODUCTION: Caudal epidural block is a widely performed procedure for postoperative pain control of pediatric patients. As the local anesthetic acts by spreading cranially after caudal block, it may lead to several effects on the cerebrospinal fluid and intracranial region. METHOD: Children aged 1-7, ASA I-II were included in this study. The patient population was assigned into two groups as the Caudal Block Group (Group CB) and the Control Group (Group C) Caudal block with 0.25% bupivacaine 1 ml/kg was performed on patients in Group CB. Optic nerve sheath diameter was measured at the follo

Elevated right ventricular systolic pressure and outcomes after emergency hip surgery: a case control study.

Patients at risk of pulmonary hypertension frequently present for emergency orthopedic surgery. A right ventricular systolic pressure of 35 mmHg or above, calculated from a tricuspid regurgitant jet on transthoracic echocardiogram, is considered an appropriate screening test for pulmonary hypertension. The aim of this study was to evaluate the impact of an elevated right ventricular systolic pressure detected on a preoperative transthoracic echocardiogram, on outcomes after emergency hip surgery. We undertook a retrospective, single centre, case control study of 98 adult patients who had a tra

Early urea-to-creatinine ratio to predict rapid muscle loss in critically ill patients with sepsis: a single-center retrospective observational study.

BACKGROUND: Patients with sepsis in the intensive care unit (ICU) often experience rapid muscle loss. The urea-to-creatinine ratio (UCR) is thought to reflect muscle breakdown (creatinine) and catabolism (urea) and is commonly used to assess nutritional and metabolic status. This study aimed to investigate whether changes in UCR (ΔUCR) can predict the development of rapid muscle loss in patients with sepsis. METHODS: This retrospective observational study was conducted in a university ICU between 2014 and 2021, involving adult patients (≥ 18 years) diagnosed with sepsis. The primary outcome w

Intravenous lidocaine decreased oxygen-desaturation episodes induced by propofol-based sedation for gastrointestinal endoscopy procedures: a prospective, randomized, controlled trial.

BACKGROUND: As a popularly used analgesic adjuvant, intravenous (IV) lidocaine could reduce the consumption of propofol in painless gastrointestinal (GI) endoscopy. However, whether IV lidocaine could affect the incidence of oxygen-desaturation episodes (ODE) during painless GI endoscopy is still unknown. Therefore, we tested the hypothesis that IV lidocaine could decrease the incidence of propofol-induced ODE and involuntary movements in patients during GI endoscopy. METHODS: Three hundred twenty-two patients scheduled for GI endoscopy were randomly divided into lidocaine group and control g

Hypotension prediction index: comparison between invasive and non-invasive pressure inputs.

BACKGROUND: The Hypotension Prediction Index (HPI) derived using an Acumen™ arterial pressure transducer can decrease the incidence of intraoperative hypotension and possibly decrease perioperative complications. The HPI can also be obtained from the ClearSight™ continuous non-invasive blood pressure monitor. Concurrent comparison of HPI values obtained from these two pressure inputs is limited and additional comparisons could increase clinician confidence in non-invasive hemodynamic monitoring, expand its applications and improve patient outcomes. METHODS: Simultaneous hemodynamics were reco

Sedoanalgesia with dexmedetomidine in daily anesthesia practices: a prospective randomized controlled trial.

BACKGROUND: Patient safety is important in daily anesthesia practices, and providing deep anesthesia is difficult. Current debates on the optimal anesthetic agents highlight the need for safer alternatives. This study was justified by the need for safer and more effective anesthetic protocols for outpatient hysteroscopic procedures, particularly those conducted outside the operating room. Propofol, while widely used, has significant limitations, including respiratory depression, hemodynamic instability, and delayed recovery when higher doses are required for adequate sedation. The addition of

Role of the inferior vena cava collapsibility index in predicting propofol-induced hypotension in patients undergoing colonoscopy.

BACKGROUND: Hypotension is a common side effect of propofol induction, and when severe, it is associated with adverse outcomes. Ultrasonography of the inferior vena cava (IVC) is a reliable indicator of the intravascular volume. This study investigated whether preoperative IVC ultrasound measurements could predict hypotension after propofol induction in patients undergoing colonoscopies. METHODS: Sixty-two adult patients with American Society of Anesthesiologists physical status (ASA) I-II scheduled for colonoscopy after propofol induction were recruited. The Ultrasound Maximum IVC diameter (

Effect of improving sleep quality the night before surgery with zolpidem on postoperative gastrointestinal function in patients undergoing laparoscopic partial colorectal resection: a randomized, double-blind, controlled trial.

BACKGROUND: Sleep is one of the basic physiological needs of human beings. Preoperative sleep disorders are associated with poor prognosis in surgical patients, and sleep disorders have been shown to be one of the risk factors for gastrointestinal dysfunction. However, there are now few studies to investigate whether improving preoperative sleep disorders can promote the recovery of postoperative gastrointestinal function. This study aimed to investigate the effects and significance of improving preoperative sleep quality with zolpidem on postoperative gastrointestinal function. METHODS: In t

Reticulocyte hemoglobin content: a new frontier in iron deficiency diagnostics for major surgical patients.

BACKGROUND: Iron deficiency (ID) is the most common nutritional deficiency among patients undergoing major surgery. Treatment of ID is straightforward, however implementing a comprehensive anemia management strategy within clinical routines is complex. Recently, reticulocyte hemoglobin content (Ret-He) has been evaluated as an early marker for ID diagnosis. METHOD: In this retrospective study, 2,966 major surgical patients from two University Hospitals were screened for the presence of ID and the significance of Ret-He in diagnosis ID was evaluated in both non-anemic and anemic patients. Development and validation of a nomogram to pediatric postoperative pulmonary complications following thoracic surgery.

BACKGROUND: Since the respiratory anatomy and physiology of children differ from those of adults, they are more susceptible to postoperative pulmonary complications after thoracic surgery that requires one-lung ventilation. Hypothesizing that the incidence of postoperative pulmonary complications could be predicted using easily accessible perioperative variables, we aimed to develop a nomogram specifically for children receiving thoracic surgery with one-lung ventilation. METHODS: A total of 361 children undergoing thoracic surgery with one-lung ventilation were randomly assigned to two group

Impact of perioperative dexmedetomidine on long-term outcomes in older patients following cardiac surgery: follow-up of a randomized trial.

BACKGROUND: Perioperative dexmedetomidine is reported to reduce complications and even in-hospital mortality after cardiac surgery. We therefore tested the hypothesis that perioperative dexmedetomidine may improve long-term outcomes after cardiac surgery. METHODS: This was long-term follow-up of a randomized trial. We enrolled 285 patients aged 60 years or older who were scheduled for elective cardiac surgery. Patients were randomized to receive either dexmedetomidine or placebo (normal saline) during and early after surgery. Follow-up was conducted for up to 6 years post-surgery. The primary

Esketamine combined with low-dose propofol induction strategy for category-1 cesarean section: a case series.

BACKGROUND: General anesthesia (GA) is the most accepted option for category-1 emergency cesarean sections (CSs). A low dose of esketamine has been used as an excellent adjunct to neuraxial anesthesia (NA) with little effect on newborns. However, literature on the use of esketamine for GA induction in emergency CS is limited. This case series describes our experience with an esketamine-based combined low-dose propofol induction strategy for category-1 CS. METHODS: We retrospectively analyzed esketamine-based anesthesia induction for category-1 emergency CS at our hospital between November 202
