BACKGROUND: Ghana adopted the WHO-recommended Seasonal Malaria Chemoprevention (SMC) in 2016 following a pilot study as a vital strategy for malaria control. SMC is the intermittent administration of a preventive and curative dose of anti-malarial medicine (Sulfadoxine-Pyrimetamine + Amodiaquine) during four months of the malaria season. This study monitored the implementation of SMC to ensure the intervention is achieving its target.
METHODS: This was a longitudinal study in four administrative districts of the Upper East Region of Ghana. Children aged between 3 and 59 months were sampled an

The 1,2,5-oxadiazol MMV665805 from Medicines for Malaria Venture's Malaria Box Project is the lead compound of herein presented study. It exhibits very promising activity against the blood stages of the strain NF54 of Plasmodium falciparum. Twenty-five new derivates were prepared by replacing the initial left-hand side 3,4-diethoxyphenyl ring as well as introducing various benzamido and benzenesulfonamido substituents in position 3 of the furazan. Thereby, an insightful series of valuable structure-activity-relationships was elaborated. Compounds were furthermore analyzed for their compliance

BACKGROUND: Preventing malaria is crucial, especially given that vulnerable groups such as pregnant women and neonates bear a disproportionate malaria burden and such illness can have long-term consequences for their health. Zimbabwe faces a high burden of malaria, which falls heavily on pregnant women and their neonates, contributing towards maternal and neonatal mortality, maternal anaemia and low birthweight. We contribute to the existing literature by examining the correlation between malaria prophylaxis stock-outs and health outcomes in this vulnerable group in Zimbabwe. Women who do not

BACKGROUND: Most malaria-endemic countries, including Cambodia, use a total dose of 3·5 mg/kg of primaquine to eliminate Plasmodium vivax hypnozoites and prevent relapses. There are, however, indications that the lower dose of 3·5 mg/kg is insufficient for tropical P vivax isolates, particularly in southeast Asia, and WHO now recommends a total dose of 7·0 mg/kg in most countries. We aimed to determine the most effective regimen to eliminate P vivax hypnozoites to support elimination efforts of this malaria parasite.
METHODS: We conducted an open-label, randomised controlled trial in Kampong

The malaria parasite Plasmodium falciparum regularly confronts orchestrated changes in frontline drug treatment that drastically alter its selection landscape. When this has occurred, the parasite has successfully adapted to new drugs through novel resistance mutations. These novel mutations, however, emerge in a genetic background already shaped by prior drug selection. In some instances, selection imposed by different drugs targets the same loci in either synergistic or antagonistic ways, which may leave genomic signatures that are hard to attribute to a specific agent. Here, we use two appr

Mefloquine is an antimalarial drug routinely used for prophylaxis and in the treatment of malaria. Approximately 50% of mefloquine metabolism, both in vitro and in vivo, is mediated by CYP3A4 with the remaining contributions by other CYP450 isoforms unaccounted for. This study aimed to determine the contribution of CYP450s to mefloquine metabolism and incorporate this knowledge into a physiologically-based pharmacokinetic model. The data in human liver microsomes demonstrated the involvement of CYP3A4/5 as well as the previously unreported contribution of CYP1A2 to mefloquine metabolism. The f

BACKGROUND: In pregnancy, treating malaria with quinine and reducing its adverse effects on pregnant women has posed a significant challenge to clinicians in malaria-endemic areas such as Ghana for over 2-decades. Clinicians' experiences, therefore, play a significant role in clinicians' choices of drugs to safeguard pregnant women. Thus, this study aims to explore clinicians' experiences with quinine-based treatment (QBT) of malaria in early pregnancy in the Tamale Metropolis of Ghana.
METHODS: An exploratory qualitative approach, using a phenomenological design, was employed to allow for an

INTRODUCTION: Artemisinin-based combination therapies (ACTs) remain the WHO-recommended treatment for uncomplicated Plasmodium falciparum malaria. However, the emergence and spread of artemisinin resistance (ART-R) threatens ACT efficacy. ART-R is phenotypically expressed as delayed parasite clearance, which can facilitate ACT partner drug resistance. ART-R has been causally linked to specific mutations in the Pfkelch13 gene.
METHODS AND ANALYSIS: The systematic review and associated meta-analysis aim to determine the correlation between Pfkelch13 (alleles present in the Kelch13 gene region o

BACKGROUND: Malaria remains a significant public health concern, particularly in resource-constrained settings and regions affected by ongoing humanitarian crises, such as Northen Bahr el Ghazal in South Sudan. For successful implementation and uptake of malaria interventions, it is important to understand community perceptions in these contexts. Therefore, we present the findings of the qualitative study examining the feasibility of implementing seasonal malaria chemoprevention (SMC) and acceptability of the intervention by the community in this complex setting.
METHODS: Five monthly cycles

Sulfadoxine-pyrimethamine plays a key role in Plasmodium falciparum chemoprevention across Africa, yet the protective efficacy of SP is undermined by mutations conferring resistance in the genes encoding dihydrofolate reductase (pfdhfr) and dihydropteroate synthase (pfdhps). The emergence and spread of the pfdhps 431V mutation suggests that this may confer resistance and be selected by drug use. Here, we report a non-coding mutation a548383t, which expands a di-nucleotide repeat in the first intron of pfpppk-dhps. The first intron and second exon of the pfdhps gene were analysed by target ampl

BACKGROUND: Adding single low-dose (0.25 mg/kg) primaquine (SLDPQ) to block Plasmodium falciparum transmission is now a WHO recommendation. Whether SLDPQ increases haemolysis in glucose-6-phosphate dehydrogenase deficient (G6PDd) patients, leading to increased folate demand and impaired haemoglobin (Hb) recovery is unknown. This study sought to answer this question.
METHODS: This randomized, placebo-controlled trial measured serial plasma folate concentrations [Day (D) 0, 3, 7 and 28] in falciparum-infected Ugandan and Congolese children (6 months to 11 years), treated with age-dosed SLDPQ/pl

BACKGROUND: Antimalarial resistance, especially artemisinin resistance, in Plasmodium falciparum threatens global malaria control. Molecular surveillance of Plasmodium falciparum drug resistance is critical for monitoring the efficacy of artemisinin-based combination therapies (ACTs). Current molecular surveillance tools for Plasmodium falciparum artemisinin resistance are restricted to predefined polymorphism hotspots in limited loci (Pfk13 domain), failing to capture novel mutations in emerging resistance genes (Pfcoronin, Pfubp1, Pfap2μ) or cover complete coding regions. This study addresse

Recent reports from East African countries indicate the emergence and spread of artemisinin partial resistance (ART-R), posing a significant threat to malaria control efforts in the region. The presence of critical Plasmodium falciparum kelch13 (k13) resistance markers, including C469Y, P553L and A675V, have been detected in Kenya, although their clinical significance remains unclear. This highlights an urgent need to closely monitor the prevalence of these mutations. A total of 24,227 dried blood spot (DBS) samples were collected from 82 primary schools across eight counties in Western Kenya

Plasmodium multi-resistance, including against artemisinin, seriously threatens malaria treatment and control. Hence, new drugs are urgently needed, ideally targeting different parasitic stages, which are not yet targeted by current drugs. The SUB1 protease is involved in both hepatic and blood stages due to its essential role in the egress of parasites from host cells, and, as potential new target, it would meet the above criteria. We report here the synthesis as well as the biological and structural evaluation of substrate-based α-ketoamide SUB1 pseudopeptidic inhibitors encompassing positio

In the twenty-first century, we are experiencing persistent waves of diverse pathogen variations, contributing significantly to global illness and death rates. Within this varied spectrum of illnesses, malaria and oxidative damage emerge as prominent obstacles that have persistently affected human health. The motivation for exploring the antioxidant potential of transition metal (II) complexes with tridentate Schiff base ligands is driven by the need for effective treatments against malaria and oxidative stress-related conditions. Both malaria and oxidative damage are significant global health

BACKGROUND: Drug resistance in Plasmodium falciparum is a major threat to malaria control efforts. Pathogen genomic surveillance could be invaluable for monitoring current and emerging parasite drug resistance.
METHODS: Data from two decades (2000-2020) of continuous molecular surveillance of P. falciparum parasites from Senegal were retrospectively examined to assess historical changes in malaria drug resistance mutations. Several known drug resistance markers and their surrounding haplotypes were profiled using a combination of single nucleotide polymorphism (SNP) molecular surveillance and

Malaria remains a global health concern as drug resistance threatens treatment programs. We identified a piperidine carboxamide (SW042) with anti-malarial activity by phenotypic screening. Selection of SW042-resistant Plasmodium falciparum (Pf) parasites revealed point mutations in the Pf_proteasome β5 active-site (Pfβ5). A potent analog (SW584) showed efficacy in a mouse model of human malaria after oral dosing. SW584 had a low propensity to generate resistance (minimum inoculum for resistance [MIR] >109) and was synergistic with dihydroartemisinin. Pf_proteasome purification was facilitated

Diagnostic routine and knowledge about the therapy regimes of infectious diseases like malaria gain in importance due to globalization, global warming, and increasing numbers of refugees. We report a case of a 66-year-old patient who presented with severe abdominal pain, most prominent in the left upper abdomen. He was recently hospitalized with severe falciparum malaria, diagnosed after returning from a trip around the world. Upon readmission, laboratory results showed post-artesunate delayed hemolysis. The ultrasound examination was highly suspicious of splenic rupture, confirmed by the imme

BACKGROUND: Perennial malaria chemoprevention (PMC) aims to protect children at risk from severe malaria by the administration of anti-malarial drugs to children of defined ages throughout the year. Sulfadoxine-pyrimethamine (SP) has been widely used for chemoprevention in Africa and a child-friendly dispersible tablet formulation has recently become available.
METHODS: This qualitative non-interventional observational study was conducted in Benin, Côte d'Ivoire, and Mozambique between February and June 2022. Prototype blister packs, dispensing boxes and job aids designed to support dispersib

BACKGROUND: The use of artemisinin-based combination therapy (ACT) is recommended by the World Health Organization for the treatment of uncomplicated falciparum malaria. Artemether-lumefantrine (AL) is the most widely adopted first-line ACT for uncomplicated malaria in sub-Saharan Africa (SSA), including mainland Tanzania, where it was introduced in December 2006. The WHO recommends regular assessment to monitor the efficacy of the first-line treatment specifically considering that artemisinin partial resistance was reported in Greater Mekong sub-region and has been confirmed in East Africa (R
