Distortion-product otoacoustic emissions in nonacoustic tumors of the cerebellopontine angle.

OBJECTIVES: The goal of the present study was to compare the frequencies of cochlear and non-cochlear patterns of distortion-product otoacoustic emissions (DPOAEs) in patients with nonacoustic (non-AN) tumors of the cerebellopontine angle (CPA)/internal auditory canal with those in patients with acoustic neuroma (AN). STUDY DESIGN AND SETTING: We conducted a prospective study of patients with non-AN internal auditory canal/CPA tumors treated between 1992 and 1999 in a tertiary care setting. We collected data on behavioral pure tone average, speech discrimination score, acoustic immittance tes

Effects of discotheque music on audiometric results and central acoustic evoked neuromagnetic responses.

Audiograms and auditory evoked magnetic fields (AEFs) were observed in young male and female adults at different ages before and after being exposed to discotheque music for 4 hours. Sound pressure levels (SPLs) ranged from 95 dB (SPL) up to 130 dB (SPL). After exposure, subjects had temporary threshold shifts up to 20-25 dB, which almost disappeared after 2 hours. The majority of the subjects suffered from tinnitus that lasted approximately as long as the temporary threshold shift. Correspondingly, a transient delay and prolongation of the main component of the acoustically evoked magnetic fi

Electrocochleogram after transection of vestibulo-cochlear nerve in a patient with a large acoustic neurinoma.

This study reports pre- and post-operative compound action potentials (CAPs) that were recorded from a 27-year-old woman with an acoustic neurinoma. During surgery it was necessary to totally sever her vestibulo-cochlear nerve to excise a large tumor. A pure tone audiogram changed to the scale-out pattern immediately after operation. However, CAP, the waveform of which was broadened, has been recorded 3 years post-operatively with a threshold elevation of 10 dB over the pre-operative threshold. This phenomenon suggests that CAP may originate from the extreme periphery of the auditory nerve wit

Hearing changes and questionnaire responses in patients with paramedian suboccipital vestibular neurectomy.

In order to determine the effectiveness of paramedian suboccipital transmeatal vestibular neurectomy, pre- and postoperative neurotological studies were conducted and questionnaire responses of 30 patients who underwent surgery were evaluated. Hearing was maintained within 10 dB of the preoperative level or improved in 54% of patients 2 years after surgery. Questionnaire responses indicated that all patients suffering from intractable vertigo experienced no vertiginous symptoms after surgery. Based on this study, we conclude that paramedian suboccipital transmeatal vestibular neurectomy is a u

Tympanometric findings in cleft palate patients: influence of age and cleft type.

Tympanometry was analysed according to cleft type with respect to age in 239 cleft palate patients (57 with bilateral cleft lip and palate (BCLP)), 122 with unilateral cleft lip and palate (UCLP) and 60 with isolated cleft palate (ICP). The frequency of type B tympanograms was 46.5 per cent in the BCLP group, 50.6 per cent in the UCLP group and 58.3 per cent in the ICP group. Type A tympanograms were more frequently observed in older patients (median age 11 years) in comparison to type B (median age five years; p < 0.001) and type C (median age six years; p < 0.001). The total sample showed an

Duration, compression, and the aided loudness discomfort level.

OBJECTIVE: The purpose of this investigation is to determine how the unaided and aided loudness discomfort level (LDL) varies with the duration of the input signal and whether the electroacoustic characteristics of compression circuits affect this relationship in a manner that may alter the listener's dynamic range for short duration sounds. DESIGN: Ten hearing-impaired and 20 normal-hearing listeners participated. LDLs were determined for noise bursts of durations ranging in six steps from 32 to 1024 msec, using a two-alternative, forced-choice adaptive tracking procedure in which input leve

Conductive hearing loss in individuals with fibrodysplasia ossificans progressiva.

Fibrodysplasia ossificans progressiva (FOP) is a very rare genetic disorder that is characterized by progressive heterotopic ossification of soft tissues and congenital malformation of the great toes. Although previous case studies have reported hearing loss in individuals with FOP, there have been no large-scale studies regarding the nature or cause of the hearing loss. Here, we report the findings of a two-part study. In Part I, we report the findings of a postal survey regarding hearing loss that was sent to 102 individuals with FOP. In Part II, we report the findings of on-site hearing eva

Tympanoplasty type II with ionomeric cement and titanium-gold-angle prostheses.

OBJECTIVE: The pronounced isolated erosion of the long incus process is a challenging situation during tympanoplasty. Here, we report two methods for reconstruction of the ossicular chain as tympanoplasty type II with the incus remaining in situ. (1) lonomeric cement is used, resulting in two characteristics: a direct link between stapes head and the long incus process is achieved. At the stapes head the link possesses a joint allowing physiological movements between incus and stapes. (2) Titanium-gold-angle prosthesis according to Plester were crimped to the long process of the incus and posi

Perinatal asphyxia, hypoxia, ischemia and hearing loss. An overview.

Birth hypoxia, asphyxia and ischemia have often been thought to be major causes of early hearing loss or deafness. The purpose of the present review is to focus on the role of these particular factors for perinatal auditory disorders. On the whole, only a small proportion of neonatal hearing loss is caused by perinatal factors. The exact etiology of neonatal hearing loss in children with complicated deliveries is difficult to evaluate due to the large number of causative factors that might be involved. After reviewing the literature covering the past 15-20 years, it is not possible to say that

[Toxic inner ear damage in topical treatment of psoriasis with salicylates].

BACKGROUND: Whereas cochlear impairment after intravenous ingestions of salicylates is well known, reports of cochlear symptoms after topical application are quite rare. The extent of the ototoxic salicylate impact is increased by diabetes, renal insufficiency, and alcoholism. PATIENT: This study presents a case report of a female patient who suffered a repeated, symmetric, pancochlear, reversible inner ear impairment after two treatments with salicylate containing ointment for psoriasis. The correlation of the salicylate therapy with the observed inner ear lesions is obvious due to the close

Proposed algorithm to aid the diagnosis of cerebellopontine angle tumors.

Hearing and/or balance disorders are symptoms that may be associated with tumors of the cerebellopontine angle (CPA). Between March 1988 and May 1995, 22 patients had diagnosis or evaluation of CPA tumors at Cleveland Clinic Florida. The most common presenting signs or symptoms included unilateral low-frequency tinnitus, unsteadiness, and/or asymmetric hearing loss. On the basis of our experience, we have constructed an algorithm to help identify and manage acoustic nerve dysfunction. We compare our findings with national epidemiologic data and current medical reports.
