Diffuse Lewy body disease.

Diffuse Lewy body disease, also called dementia with Lewy bodies (DLB), is defined as progressive dementia and pathological Lewy bodies distributed in the central and autonomic nervous systems. The clinical features are dementia, cognitive fluctuations, visual hallucinations, parkinsonism, and REM sleep behavior disorder (RBD). Confirmatory techniques include dopamine transporter imaging, meta-iodobenzylguanidine (MIBG) myocardial scintigraphy, and polysomnography. The pathology finding in DLB is misfolded alpha-synuclein, the main component of Lewy bodies, propagating in the central nervous s

Plasma catecholamine levels before and after paroxetine treatment in patients with panic disorder.

Catecholamines such as norepinephrine, epinephrine, and dopamine are closely related to the autonomic nervous system, suggesting that panic disorder may involve elevated catecholamine levels. This study investigated basal and posttreatment catecholamine levels in patients with panic disorder. A total of 29 patients with panic disorder and 23 healthy controls participated in the study. Panic disorder patients received paroxetine treatment for 12 weeks after clinical tests and examination had been conducted. We investigated the difference in basal levels of catecholamine and measured the changes

[Nerve growth factor and diabetic neuropathy].

INTRODUCTION: The progress made by contemporary neurobiology opens new horizons both for study and for treatment of disorders of the nervous system. At the present time we are in the age of growth factors. These are molecules which affect survival, development and the normal functioning of cell populations. DEVELOPMENT: One of the most widely studied growth factors is nervous growth factor (NGF) which is necessary for normal life of various types of neurones, including sensory nerves and nerves derived from the neural crests. Diabetic neuropathy, worldwide a major neurological disorder, is pr

Central leptin stimulates corticosterone secretion at the onset of the dark phase.

Leptin, a hormone secreted by adipose tissue in proportion to body adiposity, is proposed to be involved in the central nervous regulation of food intake and body weight. In addition, evidence is emerging that leptin regulates neuroendocrine and metabolic functions as well, presumably via its action in the central nervous system (CNS). To investigate this regulatory effect of leptin, we infused 3.5 microg of human leptin directly into the third cerebral ventricle (i3vt) of lean male Long-Evans rats, 90 min before the onset of their dark phase. Before and after infusion, blood samples were with

Technical Note: Modulation of fMRI brainstem responses by transcutaneous vagus nerve stimulation.

Our increasing knowledge about gut-brain interaction is revolutionising the understanding of the links between digestion, mood, health, and even decision making in our everyday lives. In support of this interaction, the vagus nerve is a crucial pathway transmitting diverse gut-derived signals to the brain to monitor of metabolic status, digestive processes, or immune control to adapt behavioural and autonomic responses. Hence, neuromodulation methods targeting the vagus nerve are currently explored as a treatment option in a number of clinical disorders, including diabetes, chronic pain, and d

Diabetes and postural stability: review and hypotheses.

Among the complications associated with diabetes mellitus is postural control. The authors reviewed 28 studies in the literature that focused on the magnitudes of postural sway that people with and without diabetes exhibit. The general observation is that postural sway is greater for people with diabetes, especially if their condition includes neuropathy. Peripheral sensory neuropathy seems to be the primary factor, but the available evidence does not rule out diabetes per se, other neuropathies (central, autonomic, motor), or an inability to exploit fully optical and inertial information abou

Effects of autonomic neuropathy on coronary blood flow in patients with diabetes mellitus.

BACKGROUND: C ardiac sympathetic signals play an important role in the regulation of myocardial perfusion. We hypothesized that sympathetically mediated myocardial blood flow would be impaired in diabetics with autonomic neuropathy. METHODS AND RESULTS: We studied 28 diabetics (43+/-7 years old) and 11 age-matched healthy volunteers. PET was used to delineate cardiac sympathetic innervation with [(11)C]hydroxyephedrine ([(11)C]HED) and to measure myocardial blood flow at rest, during hyperemia, and in response to sympathetic stimulation by cold pressor testing. The response to cardiac autonom

Cardiac sympathetic nervous disintegrity is related to exercise intolerance in patients with chronic heart failure.

The aim of this study was to clarify the relationship between cardiac sympathetic nervous activity assessed by 123I-MIBG imaging and exercise capacity in patients with congestive heart failure. The subjects were 24 patients with heart failure (NYHA class II to III) and 7 normal controls. A dose of 148 MBq of 123I-MIBG was administered intravenously, and 5-min anterior planar images were obtained 20 min (early) and 4 h (delayed) after the injection of 123I-MIBG. Regions of interest were placed over the left ventricle and mediastinum, and the heart-to-mediastinum ratio (H/M ratio) was calculated

Relationship between density of muscarinic receptors and nasal hypersecretion in a nasal allergic model.

In guinea pigs actively sensitized with ovalbumin, we have observed that nasal hyperreactivity and hypersensitivity to methacholine causes nasal secretion accompanied by an increase in the density of muscarinic acetylcholine receptor (m-ACh.R) following the appearance of nasal symptoms induced by the ovalbumin challenge. Therefore in the present investigation, we studied the relationship between the density of m-ACh.R and nasal hypersecretion in actively sensitized guinea pigs. There was no relationship between the quantity of nasal secretion and serum Ig G1 or Ig E levels. On the other hand,

Cardiac beat-to-beat variation as an indicator of neuropathy and the frequency of its abnormality in diabetes mellitus.

The cardiac beat-to-beat variation and the maximum constriction velocity and acceleration of the pupillary light reflex have been considered indices of parasympathetic neuropathy. The abnormality of these indices was evaluated in 191 non-diabetics and 151 diabetics. The cardiac beat-to-beat variation decreased gradually with age in non-diabetics as well as diabetics. It was small in diabetics compared to non-diabetics in corresponding age group between 10 and 59 years. In diabetics it was still smaller in the presence of peripheral neuropathy than in its absence. The overall prevalence of this

Treatment of neurogenic orthostatic hypotension with a monoamine oxidase inhibitor and tyramine.

Six patients with neurogenic orthostatic hypotension were treated with a chemical preparation of tyramine and tranylcypromine ("Parnate"), a monoamine oxidase inhibitor (M.A.O.I.). Four had autonomic failure with no other neurological deficit (idiopathic orthostatic hypotension), and in two patients other neuronal systems were also involved (Shy-Drager syndrome). Previous therapy with fludrocortisone, ephedrine, elastic garments, postural training, and, in one patient, an anti-G suit was unsatisfactory. Tyramine given orally with tranylcypromine produced a moderate rise in blood-pressure which

Differences in skin sympathetic involvements between two chronic autonomic disorders: multiple system atrophy and pure autonomic failure.

OBJECTIVE: Certain stimuli evoke increased sweat secretion (sympathetic sweat response; SSwR) and reduced skin blood flow (skin vasomotor reflex; SkVR) in the palm/sole. We evaluated SSwR and SkVR in patients with multiple system atrophy (MSA) and pure autonomic failure (PAF). METHODS: SSwR and SkVR on the palm in response to deep inspiration and mental arithmetic were recorded in 11 MSA patients, 11 PAF patients, and 11 healthy controls. In addition, the head-up tilt test was performed, and the coefficient of variation of R-R intervals (CV(R-R)) was obtained. RESULTS: SSwR amplitudes were s

Aging alteration of cardiac vagosympathetic balance assessed through the tone-entropy analysis.

BACKGROUND: Age-associated alterations of cardiac autonomic functions have been investigated intensively through heart rate variability analyses. However, changes with age in cardiac vagosympathetic balance remain to be elucidated. METHODS: We studied 142 male subjects (30-69 yr) at a health care center in Kyoto, Japan. Electrocardiographic data were collected from subjects in a recumbent position for 10 min in the morning. Analysis was done by classifying the subjects into four age groups. In a pharmacological experiment carried out in December 1996, tone was verified to reflect the cardiac

Supportive care for systemic amyloidosis: International Society of Amyloidosis (ISA) expert panel guidelines.

Systemic amyloidosis refers to a group of protein misfolding disorders resulting in organ deposition with amyloid, leading to organ dysfunction, ultimately resulting in organ failure and death if not successfully treated. Treatment is type-specific and aimed at the underlying source of the misfolded protein. In the past decades, treatments have become increasingly available across the various amyloidosis types with improved response rates and longer survival. Supportive care measures are an integral part of care for patients with systemic amyloidosis to improve symptom burden and quality of li

Biological mechanisms of different exercise modalities in improving sleep disorders: A review.

Sleep disorders such as insomnia, sleep apnea, and circadian rhythm dysfunctions constitute a major public health challenge due to their high prevalence and associated risks for physical and mental health. In response to the limitations and potential side effects of pharmacological treatments, non-pharmacological interventions have become increasingly central to management strategies. Among these, structured physical exercise has emerged as a particularly prominent and evidence-supported approach, valued for its demonstrated efficacy and favorable safety profile. This narrative review aims to

Wilkie syndrome combined with nutcracker syndrome presented as syncopal episodes: A case report.

Combined aortomesenteric compression is an extremely rare vascular condition where both the duodenum (Wilkie) and the left renal vein (nutcracker) are compressed between the aorta and the superior mesenteric artery (SMA). The causal factor is the abnormal course of the SMA originating from the abdominal aorta with reduced angle. Here we describe the case of a young male patient presenting with repetitive episodes of loss of consciousness, arrhythmias and hyperamylasemia, not associated with other symptoms. Color Doppler revealed an aortomesenteric angle of 15°, compression of the left renal ve
