A 67-year-old man with a history of melanoma, treated surgically 7 years before, was referred for vomiting and right flank pain after three intravesical instillations of bacillus Calmette-Guérin (BCG) for superficial bladder cancer. OBJECTIVES: To conduct a trial using three different doses of bacillus Calmette-Guérin (BCG) to determine whether lowering the dose of BCG could reduce toxicity without compromising its efficacy in the treatment of superficial bladder cancer.

Intravesical Bacillus Calmette-Guerin (BCG) is usually a well-tolerated treatment strategy for transitional cell cancer of the bladder. We report a patient with penile and bladder mucosal lesions with inguinal adenopathy after six instillations of intravesical BCG. Pathology demonstrated inflammation with granulomatous changes. The lesions and adenopathy resolved with antituberculous drug treatment. We describe the patient's clinical presentation and review the complications of intravesical BCG treatment.

We evaluated the need for transurethral biopsy at first follow-up after intravesical bacillus Calmette-Guerin (BCG) therapy for superficial bladder cancer. The records of 84 patients with superficial bladder cancer who received a 6- or 8-week course of BCG were reviewed. Pathological results before BCG, cystoscopic findings, urinary cytology, and biopsy results for evaluation of BCG therapy were reviewed. PURPOSE: Advanced age is considered a risk factor for complications in patients receiving intravesical bacillus Calmette-Guérin (BCG) therapy. However, there is no clear delineation of BCG-related complication rates relative to patient age.
MATERIALS AND METHODS: We reviewed the clinical course of 58 consecutive men receiving maintenance BCG therapy from December 1999 to July 2004 for transitional cell carcinoma. Intravesical bacillus Calmette-Guerin (BCG) is used in patients with urinary bladder carcinoma. Although it is generally well tolerated, granulomatous hepatitis is a rare but serious complication. We report a 42-year-old man and a 56-year-old man who developed granulomatous hepatitis following intravesical BCG. One of them was treated successfully with antitubercular therapy; the other died because of BCG sepsis and multi-organ failure.

Besides the commonly seen side effects of intravesical BCG instillations, very rare complications have been reported including new tumor formation other than transitional cell carcinoma of the bladder. Malignant melanoma is resected with curative intent in 80-85% of the patients. In case of tumor thickness according to Breslow of > 4 mm and metastatic regional lymph nodes a high relapse rate and mortality of 50-90% is observed.
