Practical guidelines for venous thromboembolism chemoprophylaxis in elective plastic surgery.

Venous thromboembolism is a complication that results in a wide range of patient morbidity and potential mortality. Awareness of this risk has led to recent advances in both the perioperative planning of chemoprophylaxis and the stratification of risk factors. Together, these modifications have helped reduce the incidence and severity of venous thromboembolism. Major hospital-based initiatives to decrease the overall rate of venous thromboembolism and halt the progression of severe complications, including pulmonary embolism, are one such change. However, surgeon reluctance based on the potent

Laparoscopic sleeve gastrectomy as first stage or definitive intent in 300 consecutive cases.

BACKGROUND: Laparoscopic sleeve gastrectomy (SG) was originally used as a bridge to definitive surgery in high-risk patients. Recently it has been considered as a stand-alone procedure due to its effectiveness on weight loss and comorbidities resolution. This study was designed to evaluate the results of SG on complications, body mass index (BMI), and comorbidities resolution in 300 consecutive obese patients and to analyze the lesson learned from this experience. METHODS: From October 2002 to November 2009, 300 patients underwent SG. In the first 100 cases (group 1: mean BMI, 54.4±9.3), SG w

Anemia and iron deficiency in gastrointestinal and liver conditions.

Iron deficiency anemia (IDA) is associated with a number of pathological gastrointestinal conditions other than inflammatory bowel disease, and also with liver disorders. Different factors such as chronic bleeding, malabsorption and inflammation may contribute to IDA. Although patients with symptoms of anemia are frequently referred to gastroenterologists, the approach to diagnosis and selection of treatment as well as follow-up measures is not standardized and suboptimal. Iron deficiency, even without anemia, can substantially impact physical and cognitive function and reduce quality of life.

Incidental gastric mesenchymal tumors identified during laparoscopic sleeve gastrectomy.

BACKGROUND: Laparoscopic sleeve gastrectomy (SG) is a well-tolerated and effective procedure for sustained weight loss and amelioration of weight-related co-morbidities. Rarely, unexpected pathology may be identified intraoperatively, which may alter the surgical plan. Gastrointestinal stromal tumors (GISTs) are among the more frequently encountered tumors and pose a particular concern because of their malignant potential. We review our findings of incidental tumors encountered during 1415 consecutive SGs. METHODS: Abnormal pathology records from all patients who underwent SG at our instituti

Obesity and diabetes: newer concepts in imaging.

Quantifying body fat is currently an area of active research. Recent studies have shown that the quantity and location of fat in different compartments have varying clinical significance. This information can now be obtained from computed tomography (CT) or magnetic resonance (MR), and it can inform clinical decision making for patient management. Diabetes patients with insulin resistance and hyperinsulinemia have nonalcoholic fatty liver disease (NAFLD) ranging from steatosis to nonalcoholic steatohepatitis (NASH) and cirrhosis, typically diagnosed by liver biopsy or serum markers. There is n

Involvement of TNF-alpha in abnormal adipocyte and muscle sortilin expression in obese mice and humans.

AIMS/HYPOTHESIS: Insulin resistance is caused by numerous factors including inflammation. It is characterised by defective insulin stimulation of adipocyte and muscle glucose transport, which requires the glucose transporter GLUT4 translocation towards the plasma membrane. Defects in insulin signalling can cause insulin resistance, but alterations in GLUT4 trafficking could also play a role. Our goal was to determine whether proteins controlling GLUT4 trafficking are altered in insulin resistance linked to obesity. METHODS: Using real-time RT-PCR, we searched for selected transcripts that wer

Personality Traits Predict Weight Loss Following Endoscopic Sleeve Gastroplasty: A Cohort Study with External Validation.

BACKGROUND: Endoscopic sleeve gastroplasty (ESG) is a safe and effective treatment for obesity, but weight loss varies widely among patients. Personality traits may influence behavioral adherence after bariatric interventions. We evaluated whether Big Five personality traits predict weight loss after ESG and validated findings across two demographically distinct cohorts. METHODS: Two retrospective cohorts were analyzed. The primary cohort included self-pay patients who underwent ESG at a tertiary center in New York (2013–2019). The external cohort included predominantly uninsured or underinsur

Ten-Year Follow-Up for Sleeve Gastrectomy Versus Roux-en-Y Gastric Bypass: A Systematic Review and Meta-Analysis.

BACKGROUND: Laparoscopic sleeve gastrectomy (SG) and Roux-en-Y gastric bypass (RYGB) are the most performed bariatric procedures worldwide. Trials and observational studies up to five years show RYGB yields slightly greater weight loss and better metabolic control, notably for dyslipidemia and reflux. Ten-year cohorts and recent randomized trials give perspective on durability differences, but small sample sizes and single-center designs limit their reach. No meta-analysis has yet pooled these decade-long data, leaving clinicians without clear guidance on the comparative long-term performance

Maximizing Metabolic Synergy: A Review of Dual Incretin Therapy as a Step-Up Strategy Following Glucagon-Like Peptide-1 (GLP-1) Monotherapy Failure or Optimization.

While single-pathway glucagon-like peptide-1 (GLP-1) receptor agonists (RAs) have transformed metabolic medicine, patients encounter therapeutic ceilings, glycemic plateaus, and weight-loss stagnation despite ongoing treatment. The emergence of dual glucose-dependent insulinotropic polypeptide (GIP) and GLP-1 receptor (GLP-1R) co-agonists offers a novel multi-receptor mechanism that can overcome these single-pathway limitations. This review synthesizes current clinical evidence supporting the transition from single-pathway GLP-1 monotherapy to dual-pathway co-agonism and outlines practical, ev

Protein-truncating variants in APBA1 in minors with severe, early-onset obesity: implications for genetic testing.

BACKGROUND: Severe, early-onset obesity is a monogenic disorder in a subset of patients. Recently, rare protein-truncating variants (PTVs) in BSN and APBA1 have been implicated as monogenic causes of obesity. Truncating variants in these two genes have been associated with adult-onset monogenic obesity. Here, we aimed to investigate the occurrence of rare PTVs in APBA1 and BSN in minors with severe obesity manifesting in early childhood or adolescence. METHODS: Previously obtained exome data of 209 children and adolescents (0–18 years) with severe obesity manifesting in early childhood or adol

Mechanisms of Obesity-Related Kidney Disease: From Adipose Depot Biology to the Chymase-Aldosterone and Ghrelin-Leptin Axes.

Obesity is an increasingly important and modifiable driver of chronic kidney disease (CKD), with effects that extend well beyond its associations with type 2 diabetes, hypertension, and dyslipidemia. To synthesize the evidence that excess adiposity is a causal and modifiable determinant of kidney disease, and to examine how specific adipose depots injure the glomerulus and the tubulointerstitium, and then map these mechanisms onto established and emerging therapies. Throughout, obesity-related kidney disease (ORKD) denotes the full spectrum of diposity-driven renal injury, whereas obesity-rela

Impact of Obesity on In Vitro Fertilization (IVF) Outcomes: A Systematic Review.

Obesity is a prominent health issue across the globe that causes negative reproductive consequences like decreased fertility. Even its effects on the success of in vitro fertilization (IVF) have drawn interest, with some studies indicating that BMI was linked to IVF failure. In this systematic review, we attempt to bring together the data regarding the impact of obesity on IVF outcomes. It also intends to quantify the adverse effects of female partner obesity on key IVF measures such as implantation, clinical pregnancy rate (CPR), live birth rate (LBR), and miscarriage rate (MR), and to unders
