Outcomes of hematopoietic stem cell transplantation for Hurler's syndrome in Europe: a risk factor analysis for graft failure.

Hurler's syndrome (HS), the most severe form of mucopolysaccharidosis type-I, causes progressive deterioration of the central nervous system and death in childhood. Allogeneic stem cell transplantation (SCT) before the age of 2 years halts disease progression. Graft failure limits the success of SCT. We analyzed data on HS patients transplanted in Europe to identify the risk factors for graft failure. We compared outcomes in 146 HS patients transplanted with various conditioning regimens and grafts. Patients were transplanted between 1994 and 2004 and registered to the European Blood and Marro

Comparison of ketorolac and morphine for postoperative pain after major surgery.

This study was designed to determine the relative analgesic efficacy and safety of single intramuscular injections of ketorolac (10 mg or 30 mg) and morphine (10 mg) in patients of either sex with moderate to severe pain after major surgery. In a single-dose, randomised, double-blind study of parallel design, pain was assessed immediately before injection of test medication and at regular intervals for 8 h thereafter. One hundred and seventeen patients (109 undergoing cardiac surgery; 8 lung surgery) were randomized to one of the three treatment groups. Pain intensity was assessed using a 5-po

Safety and tolerance of single oral doses of trandolapril (RU 44.570), a new angiotensin converting enzyme inhibitor.

The safety and tolerance of single oral doses of a new angiotensin converting enzyme (ACE) inhibitor, trandolapril have been examined in 90 healthy male volunteers, in a randomised, double blind, placebo-controlled study. The subjects were divided into 10 groups, each of 9 subjects and treatments (6 subjects on trandolapril and 3 on placebo per group) were allocated by unbalanced randomisation. Ten single, increasing oral doses were tested: 0.125, 0.25, 0.5, 1, 2, 4, 8, 16, 24 and 32 mg. The assessment criteria were clinical (monitoring of blood pressure, heart and respiratory rate, electrocar

Spontaneous Intracranial Hypotension and Multi-Level Cervical and Lumbar Epidural Blood Patches: A Case Report.

Spontaneous intracranial hypotension (SIH) is a neurologic condition where the intracranial pressure is reduced due to a loss of cerebrospinal fluid from its reservoir, the intrathecal space, to surrounding tissues. It is commonly characterized by an incapacitating headache, phono-photophobia, nausea, and vomiting, commonly refractory to medical treatment and requires further investigation. We describe the case of a healthy young man who presented to the emergency room with a postural headache, accompanied by nausea, vomiting, and phono-photophobia. Brain computed tomography (CT) imaging study

Vacuum-assisted venous drainage, angel or demon: PRO?

Vacuum-assisted venous drainage (VAVD) was proposed to optimize venous drainage during bypass through femoral venous cannulation. It is currently used in both adult and pediatric surgery when siphon gravity venous drainage is suboptimal. In pediatric surgery, the major advantages of VAVD are a significant decrease in cardiopulmonary bypass prime volume and an improved drainage with all collateral benefits. To limit gravity drainage, we use a two-level heart-lung machine dedicated to pediatric perfusion. The top level of the cardiotomy reservoir is positioned at the patient atrial level, making

Reimagining Brain Drug Delivery: Mechanistic and Translational Advances in Carbon Nanotube Nanomedicine.

Neurological and neurodegenerative disorders (NDDs) present ongoing therapeutic challenges attributed to the structural complexity of the central nervous system (CNS) and the restrictive properties of the blood-brain barrier (BBB), which inhibit the effective penetration of most drugs into the brain. Thus, carbon nanotubes (CNTs) possess a high aspect ratio, tunable surface chemistry, and exceptional electrical and mechanical properties, establishing them as versatile nanoplatforms that can address significant challenges in CNS drug delivery. Despite the exploration of various nanocarriers, a

Phytocompounds Targeting Signaling Pathways in Stroke: Preclinical and Clinical Insights into Therapeutic Mechanisms and Neurotoxicity.

Stroke, primarily due to ischemic injury and neurodegeneration, is a significant global cause of death and permanent neurological impairment in adults. Phytocompounds are promising candidates for stroke care because of their effects on key molecular signaling pathways that promote neuroprotection in stroke. This review highlights the therapeutic potential of phytocompounds, such as curcumin, resveratrol, quercetin, berberine, and epigallocatechin gallate, which target signaling pathways, including PI3K/Akt, Nrf2/ARE, MAPK, NF-κB, and JAK/STAT. These substances have neuroprotective properties b

Preparation, antioxidant and antibacterial properties of PCL/APS composite nanofiber films embedded with CS/TPP-CV nanospheres for blueberry preservation.

At present, active cling films for food preservation face a core bottleneck: the encapsulation materials for active substances have limited effectiveness, failing to achieve a balance between material safety and sustained-release performance. In this study, carvacrol (CV) was encapsulated in chitosan (CS)/sodium tripolyphosphate (TPP) ionic gelation nanospheres, with an encapsulation efficiency of 89.3 ± 1.1%. The gel nanospheres were integrated into polycaprolactone/astragalus polysaccharide (PCL/APS) nanofibers via electrospinning (ELS) technology to fabricate active cling films. Characteriz

Regional versus general anesthesia for hip fracture surgery in older adults: A focused research review.

Hip fractures in the elderly population are associated with considerable complications such as morbidity, mortality, and enduring functional deterioration. Thus, they render perioperative management an essential element of strategies aimed at fostering healthy aging. Regional anesthesia (RA) and general anesthesia (GA) constitute the two predominant methods for the surgical intervention of hip fractures; however, the most effective technique remains a subject of ongoing scholarly debate. This research review seeks to systematically assess and compare regional anesthesia to general anesthesia i

Pharmacological effects and molecular targets of L-theanine in cardiovascular diseases and comorbidities.

L-theanine (Thea), a non-protein amino acid derived from Camellia sinensis (green tea) has emerged as a promising nutraceutical for the prevention and treatment of cardiovascular diseases (CVDs) and their comorbidities, thanks to its favorable safety profile, oral bioavailability, and natural origin. While extensive literature exists on green tea polyphenols, this review provides the first comprehensive synthesis of Thea's protective functions in specific cardiovascular contexts: attenuation of myocardial ischemia/reperfusion injury and heart failure, reduction of blood pressure, improvement o

Centripetal Axonal Transport as a Gateway to the CNS for Veterinary Antiparasitics: Bypassing the Blood-Brain Barrier, Clinical Impact in Vulnerable Age Groups, and the Potential Facilitating Role of PFAS.

The widespread use of veterinary antiparasitics, including neonicotinoids, isoxazolines, avermectins, and pyrethroids, is essential for canine health but raises concerns regarding potential neurotoxicity, particularly in young and geriatric animals. While the blood-brain barrier (BBB) offers significant protection, this review confronts a central "Safety Paradox": how can a drug class with a demonstrated high therapeutic index in conventional safety studies be associated with a significant and persistent number of real-world neurological adverse events? We propose that centripetal (retrograde)

SGLT2 inhibitors in hemodialysis or peritoneal dialysis patients: rationale and state-of-the art.

Sodium-glucose cotransporter-2 inhibitors (SGLT2i) reduce the risk of end-stage kidney disease (ESKD), cardiovascular events, and all-cause mortality in chronic kidney disease (CKD), regardless of diabetes or baseline renal function. However, patients with severely impaired kidney function or on dialysis have been excluded from landmark randomized clinical trials (RCTs). Several off-target mechanisms of SGLT2i could be involved in the cardiac protection of patients with ESKD in which the reduced nephron mass strongly diminishes the tubular effects of SGLT2i. However, available evidence in pati
