The cases of two low-weight children are reported, who became dialysis patients at the age of 2.5 and 5.5 years, respectively, and were hemodialyzed by way of a bovine artgraft A-V fistula at the thigh. One year later, both patients were successfully transplanted with a cadaveric kidney. Our experience with the management of two patients with life-threatening aortic disease during pregnancy is presented with a review of the literature. Four patients developed duodenal obstruction after 161 abdominal aortic reconstructions, an incidence (2.5%) rivaling that of graft infection and arterioenteric fistula. The diagnosis is easily confirmed by gastrointestinal contrast studies. Duodenal obstruction is usually caused by perigraft collagenous adhesions and is probably less likely to occur if the mobilized duodenum is not replaced directly over the aorta during resuture of the retroperitoneum. Undetected duodenal obstruction leads to rapid dehydration and electrolyte and caloric depletion.

Cryopreserved homograft pulmonary valves were used as a valved external conduit between the right ventricle and native pulmonary arteries in 21 patients with complex congenital heart disease. The technique used to construct the conduit was modified after incompetence of the valve was detected early in 3 of the first 9 conduits. The next 12 conduits remained competent. This experience suggests that early valve competency can be achieved if the valve is not distorted during construction of the conduit.

A retrospective analysis of 75 patients with 103 atherosclerotic popliteal aneurysms, seen at Princess Alexandra Hospital, Brisbane, was undertaken to assess the results of management and the most appropriate approach to the contralateral popliteal aneurysm. A 13-year experience consisting on 16 consecutive patients submitted to axillary-to-contralateral artery by-pass in the treatment of obstructive lesions of the subclavian or innominate artery is reported. Nine patients complained of typical subclavian steal syndrome. Extended patency with very satisfactory functional result was achieved in twelve patients. An extensive survey is made of the results of such procedure, as reported in the literature. The theoretical advantages in comparison with other kinds of extrathoracic procedures aiming at the same goal are briefly outlined.

Between 1979 and 1985 11 selected patients underwent axilloaxillary and carotid-subclavian bypass grafting [dacron or Gore-Tex (USA), 8 mm inner diameter]. From 1978 through 1983, 206 patients had 236 polytetrafluoroethylene (PTFE) grafts inserted in vascular wounds. More than 85% of injuries were due to gunshot wounds, shotgun wounds, or stab wounds. Arterial grafts were inserted into vessels of the upper extremity (38.8%), lower extremity (46.1%), neck and chest (8.8%), and abdomen (6.3%). We compared CT findings with endoscopic, angiographic and surgical results in ten patients suspected of having an aortoenteric fistula (AEF) because of gastrointestinal bleeding (seven) or recurrent sepsis (three). CT correctly diagnosed AEF in six patients and excluded it in the other four. CT findings of AEF consisted of perigraft fluid (PGF) (5/6) and/or gas within the bed of the graft (4/6) later than three months after graft surgery. From 1968 through 1982, 129 homologous vein grafts were used in 91 patients in the following positions: 75 in the femoropopliteal, tibial, or peroneal artery; 38 in the aortocoronary artery; 13 in the aortopulmonary artery; and one each in the atrioventricular fistula, carotid-subclavian artery, and brachial-radial artery. Easy access to the vascular system is vital to any long-term dialysis program.
