Cognitive Outcomes Following Paediatric Mild Traumatic Brain Injury: A Systematic Review and Meta-Analysis of Studies up to 2025.

The purpose of this review was to provide a systematic and quantitative summary of recent literature relating to cognitive outcomes following paediatric mild traumatic brain injury (mTBI). Variable findings and diverse methodologies have thus far made it difficult to meaningfully integrate the extant literature. In this context, there is a need to examine the factors that define heterogeneity in this population and to critically evaluate the methodologies from which variable outcomes are derived. Studies identified in this review (PROSPERO, identification number CRD42015030098) were obtained t

Epidemiology, Clinical Profiling, Management, and Functional Outcome in Moderate-to-Severe Traumatic Brain Injury in Children: A Single-center Experience.

BACKGROUND: Pediatric traumatic brain injury (TBI) is a leading cause of morbidity and mortality, with children being more vulnerable due to thinner skulls, immature autoregulation, and lower seizure thresholds. In India, they account for up to 30% of TBI cases. The outcomes remain variable, especially in the low-resource settings, highlighting the need to understand local epidemiology and management strategies. MATERIALS AND METHODS: A prospective study of 70 children aged 1-14 years admitted with moderate-to-severe TBI at a tertiary neurosurgical center between August 2023 and February 2025

Bedside transcranial Doppler as a diagnostic bridge for suspected intracranial hypertension after traumatic brain injury surgery: A case report.

BACKGROUND: Early recognition of postoperative neurologic deterioration in patients with traumatic brain injury (TBI) is essential but remains challenging, as clinical assessment may lack sensitivity and computed tomography (CT) imaging is not always immediately available. Transcranial Doppler (TCD) provides a noninvasive bedside method to detect early hemodynamic abnormalities, including impaired perfusion and increased vascular resistance. This case report highlights the role of TCD in recognizing early indicators of intracranial hypertension and prompting expedited CT evaluation. CASE DESC

Seawater immersion reshapes the temporal dynamics of traumatic brain injury and reveals mitochondrial oxidative stress as a modifiable therapeutic target.

Traumatic brain injury (TBI) evolves through time-dependent secondary injury, but whether seawater (SW) immersion merely amplifies pathology or reshapes the temporal trajectory of post-traumatic biology remains unclear. Here, we applied time-resolved proteomics to mouse brains after controlled cortical impact (CCI) with or without artificial SW immersion at 1, 3, 7, and 28 days post-injury. Trajectory-based proteomic analysis revealed that SW immersion altered the direction, magnitude, timing, persistence, and recovery of protein responses, rather than simply intensifying TBI-induced changes.

Hemodynamic characterization of cardiac dysfunction after traumatic brain injury using a controlled cortical impact model in male rats.

Traumatic brain injury (TBI) is associated with cardiovascular dysfunction that worsens clinical outcomes, yet the underlying mechanisms are poorly understood. Clinical studies are limited by heterogeneity, highlighting the need for controlled experimental models. Controlled cortical impact (CCI) provides precise control of injury severity, and invasive pressure-volume (PV) loop analysis allows detailed hemodynamic assessment. Adult male Wistar rats underwent sham operation or mild or severe CCI-induced TBI. PV-loop assessment and transthoracic echocardiography were performed at 8 or 24 h afte

Age-stratified outcomes after small-volume isolated blunt traumatic brain injury: A national ACS-TQIP analysis.

BACKGROUND: Small-volume traumatic brain injuries (TBIs) are often considered low risk clinically, but older adults may face disproportionate adverse outcomes. Evidence outlining age-specific risks related to limited subdural hematomas (SDH), epidural hematomas (EDH), and cerebral contusions remains limited. METHODS: We performed a retrospective cohort study using data from the American College of Surgeons Trauma Quality Programs Participant Use File (ACS-TQIP-PUF), 2017-2022. Adults aged 40 years and older with isolated blunt TBI were included, excluding skull fractures and multisystem traum

Limited clinical utility of risk stratification for CT decision-making in patients ≥ 75 years with mild traumatic brain injury.

BACKGROUND: Older adults account for an increasing proportion of emergency department visits for mild traumatic brain injury (mTBI). Although age is incorporated as a high-risk criterion in major clinical decision rules, the operational impact of risk stratification on computed tomography (CT) utilization in very elderly patients remains uncertain. METHODS: We conducted a retrospective cohort study including 3088 consecutive patients aged ≥ 14 years presenting with mTBI (Glasgow Coma Scale 13-15) between 2019 and 2022. The primary outcome was the presence of an acute intracranial lesion on he

Early (<24 hours) venous thromboembolism prophylaxis in traumatic brain injury: development of the EViTBI algorithm using a national cohort.

BACKGROUND: Optimal timing of venous thromboembolism (VTE) prophylaxis after traumatic brain injury (TBI) remains uncertain because of competing concerns regarding thromboembolism prevention and hemorrhagic progression. National recommendations remain inconsistent, and prophylaxis is frequently delayed beyond 72 hours. OBJECTIVE: To develop and internally validate the early VTE prophylaxis in TBI (EViTBI) algorithm for identifying a reproducible low-risk cohort potentially eligible for early (<24 hours) pharmacologic prophylaxis. METHODS: A retrospective cohort study was performed using the

Sex and Gender Differences in Physical Activity, Sedentary Behavior, and Sleep During Traumatic Brain Injury Recovery: A Narrative Review.

Traumatic brain injury (TBI) is a complex neurological condition with enduring consequences that extend well beyond the initial insult. Once regarded primarily as an acute event, TBI is now increasingly recognized as a chronic disorder associated with cognitive, emotional, and physical impairments. These sequelae contribute to diminished quality of life and elevated long-term risks for adverse health outcomes. Despite growing awareness of its multifaceted nature, TBI recovery remains heterogeneous and is shaped by a range of individual-level determinants that remain insufficiently understood.

Admission whole-blood transcriptomic characterization of a neutrophil-predominant systemic immune response in patients with acute traumatic brain injury.

BACKGROUND: Acute traumatic brain injury (TBI) is accompanied by systemic immune responses, but their whole-blood transcriptomic features at hospital arrival remain incompletely characterized. We aimed to characterize these features in patients with acute TBI compared with healthy controls. METHODS: In this single-center prospective observational study, we performed whole-blood RNA sequencing on hospital-arrival samples from 42 patients with acute TBI and 21 healthy controls. Analyses included differential expression (limma-voom; FDR < 0.05, |log2FC| > 0.7), functional enrichment, Ingenuity P

Deep Learning-Based Classification of Temporal Stages of AT8-Labeled Tau Pathology After Experimental Traumatic Brain Injury.

Tauopathies are characterised by a progressive accumulation of hyperphosphorylated tau. However, early and intermediate stages remain challenging to quantify due to subtle and heterogeneous morphological characteristics. This study evaluates a deep learning framework for classifying multiple temporal stages of tauopathy progression using AT8 (anti-phospho-tau antibody)-stained cortical micrographs in a controlled traumatic brain injury mouse model - an underexplored application. Three convolutional neural network (CNN) architectures were examined: a custom CNN and two transfer-learning models

Evaluation of long-term quality of life in traumatic brain injury patients early treated with biperiden - a randomized controlled trial.

BACKGROUND: Traumatic brain injury (TBI) is a leading cause of disability, with post-traumatic epilepsy (PTE) being a major complication. Biperiden has shown experimental potential in preventing epileptogenesis, however its impact on outcomes, such as quality of life (QoL), remains unknown. This study evaluated long-term QoL in TBI survivors treated with biperiden and identified clinical and socioeconomic predictors of their QoL. METHODS: We analyzed QoL data from a randomized, double-blind, placebo-controlled trial involving 48 adults with moderate to severe TBI, assessed on average 32 month

Utilization of Noninvasive Intracranial Pressure Monitoring in Patients with Traumatic Brain Injury Treated in Low-Resource Settings: The Intrigo Study.

BACKGROUND: Invasive intracranial pressure (ICP) monitoring is not universally available in low-resource settings, mainly owing to high costs. In this scenario, noninvasive ICP (nICP) estimation could be integrated in therapeutic protocols. The objective of this international survey was to describe the utilization of nICP monitoring in this setting. METHODS: The survey comprised 23 items. Target respondents were doctors treating patients with severe traumatic brain injury (TBI) in low-resource settings without the availability of invasive ICP monitoring. RESULTS: In total, 126 physicians fro

A Modular Prussian Blue Nanozyme-Based Microneedle Platform With Flexible Therapeutic Formulations for Localized Treatment of Traumatic Brain Injury.

Secondary injury after traumatic brain injury (TBI) is characterized by excessive reactive oxygen species (ROS) production and persistent neuroinflammation, which remain difficult to control using a single therapeutic agent. Herein, a dissolvable microneedle platform based on Prussian blue (PB) nanoparticles was developed to locally deliver curcumin (Cur) or edaravone (EDA) into injured brain tissue through a transiently disrupted blood-brain barrier. The microneedles were fabricated from hyaluronic acid/carboxymethyl chitosan (HCMN) using a low-temperature multilayer drying process while main

Neutrophil extracellular traps promote neuronal ferroptosis through STING-mediated AMPK dysregulation after traumatic brain injury.

Neuronal ferroptosis exacerbates neurological dysfunction after traumatic brain injury (TBI), yet the upstream signals driving this process remain poorly defined. Here, we identify neutrophil extracellular traps (NETs) as critical mediators of post-TBI ferroptotic injury. In patients, NET accumulation closely accompanied markers of neuronal ferroptosis and correlated with neurological severity. In a mouse TBI model, genetic ablation of peptidylarginine deiminase 4 (PAD4) or enzymatic degradation of NET-derived DNA markedly attenuated neuronal lipid peroxidation, suppressed ferroptosis-associat
