Hypothermia for Stroke: call to action 2010.

The European Hypothermia Stroke Research Workshop was held in January 2010, in response to the alarming prospects of a significant increase of stroke expected in the coming years globally. Considering that a minority of patients (around 10%) are currently eligible for thrombolytic treatment, there is a need for an efficacious, cost-effective novel therapy that can be implemented broadly within European health care systems. Accordingly, the primary objective of the workshop was the definition of a research agenda aiming to assess the therapeutic benefits of hypothermia in patients with acute is

Ischemic stroke and hyperhomocysteinemia: truth or myth?

Hyperhomocysteinemia is generally acknowledged as a treatable risk factor for atherotrombotic diseases, but a causal relationship between both is not yet definitively established. Hyperhomocysteinemia originates from a deviation in the methionine-homocysteine metabolism including disturbances of enzymes, vitamin deficiencies and different other factors. Observational studies, genetic polymorphism studies and several meta-analyses implicate already a causal relation between homocysteine and cerebrovascular diseases. It is useful to determine homocysteine levels for stroke who present no clue fo

Antithrombotic medication for stroke prevention.

This article provides the reader with an overview and an update on antithrombotic stroke-prevention strategies. Relevant journals were hand-searched by the authors to compile a broad, but not comprehensive, summary of innovative and clinically relevant studies. These findings were then summarized to provide an expert opinion on selected fields of medical stroke prevention. We conclude that aspirin, clopidogrel or dipyridamol-aspirin are the cornerstone therapies in patients with noncardioembolic stroke. More potent antiplatelet drugs or the combination of aspirin and clopidogrel prevent more i

Clinical aspects of induced hypothermia in full term neonates with perinatal asphyxia.

Moderate hypothermia is a novel neuroprotective therapy for full term neonates with severe perinatal asphyxia. Although the therapy appears to be safe, admission to a level III neonatal intensive care unit of these patients is justified. Potential complications include hypotension, tube obstruction due to sticky secretions, severe bradycardia, and thrombocytopenia. Furthermore, doses of commonly used drugs such as sedatives, anticonvulsants and antibiotics should be adjusted during hypothermia and on rewarming, and should be monitored carefully. Further studies aiming at optimizing onset, dura

[Neonatal hypoxic-ischemic encephalopathy: Incidence and prevalence in the first decade of the 21st century].

AIM: To examine the incidence and the prevalence of neonatal hypoxic-ischemic encephalopathy (HIE) in a tertiary Spanish center over a 9-year period, before the implementation of a hypothermia program. METHODS: All infants > or =34 weeks gestation, born between 2000 and 2008 with evidence of perinatal asphyxia and neonatal encephalopathy were identified. HIE was classified as mild, moderate or severe. Joinpoint regression model was used to identify changes in the trends of HIE incidences. RESULTS: A total of 90,963 live infants were born in La Paz Hospital between 2000 and 2008, and 23.3% of

[MELAS syndrome as a differential diagnosis of ischemic stroke].

Mitochondrial encephalomyopathy, lactacidosis and stroke-like episode (MELAS) syndrome is a phenotypically and genetically heterogeneous mitochondrial disorder with a clinical onset between the first and third decade. The clinical hallmark is the stroke-like-episode, which mimicks ischemic stroke but is usually transient and non-disabling in nature. The morphological equivalent on MRI is a T2-hyperintensity, predominantly over the temporo-parieto-occipital region, not confined to a vascular territory, which is also hyperintense on diffusion weighted imaging and on apparent diffusion coefficien

Antioxidative and thrombolytic TMP nitrone for treatment of ischemic stroke.

Ischemic stroke results from brain blood vessel blockage by thrombus, and produces neuronal cell damage and death. While thrombolytic therapy with tPA has achieved some success in clinic, the strategy of using neuroprotective agents to treat ischemic stroke has been disappointing thus far. In the present work, we synthesized TBN, a derivative of the clinically useful stroke drug TMP armed with a powerful free radical-scavenging nitrone moiety. TBN retains the thrombolytic activity of the parent TMP and possesses strong antioxidative properties. TBN demonstrates significant activity in the rat

[The progress of therapeutic hypothermia for patients with comatose survivors of out-of-hospital cardiac arrest this century].

Two randomized clinical trials were reported using mild therapeutic hypothermia following cardiac arrest in the 2002. One is the multicenter randomized clinical trial projected by The Hypothermia after Cardiac Arrest Study Group. The other one was performed by four centers in Australia. Two trials significantly showed better outcome in the hypothermia group compared with the normothermia group. There were some differences between Europe study and Australia study, although their outcome was doing very well. We will discuss cooling techniques (blanket or ice pack or cold saline intravenously), s

[Early evaluation of brain injury by electroencephalogram in neonates with asphyxia].

OBJECTIVE: To explore the value of electroencephalogram (EEG) in early diagnosis of brain injury in neonates with asphyxia. METHODS: EEG examination was performed in 49 neonates with asphyxia (mild: n=9; severe: n=40) within 6 hrs of their births. Of the 49 asphyxiated neonates, 33 had concurrent HIE, including 20 cases of mild, 9 cases of moderate and 4 cases of severe HIE. RESULTS: Twenty-one (63.6%) out of the 33 patients with HIE showed abnormal EEG, but only one (6.3%) in the asphyxia group without HIE. All of 13 patients with moderate-severe HIE showed abnormal EEG. The degree of EEG a

MRI of bilateral internal carotid artery agenesis.

Bilateral internal carotid artery (ICA) agenesis is rare. A patient presented with symptoms of a transient ischaemic attack. Magnetic resonance imaging with magnetic resonance angiography showed multiple foci of chronic cerebral ischaemic change and bilaterally absent ICA. Cerebral supply was derived from the posterior circulation. Differentiation has to be made between congenital agenesis/hypoplasia and acquired ICA occlusion. Computed tomography through the skull base showed the absence of the carotid canals bilaterally confirming congenital ICA agenesis.

Developmental changes and injury induced disruption of the radial organization of the cortex in the immature rat brain revealed by in vivo diffusion tensor MRI.

During brain development, morphological changes modify the cortex from its immature radial organization to its mature laminar appearance. Applying in vivo diffusion tensor imaging (DTI), the microstructural organization of the cortex in the immature rat was analyzed and correlated to neurohistopathology. Significant differences in apparent diffusion coefficient (ADC) and fractional anisotropy (FA) were detected between the external (I-III) and deep (IV-VI) cortical layers in postnatal day 3 (P3) and P6 pups. With cortical maturation, ADC was reduced in both cortical regions, whereas a decrease

Hypothermia as a treatment for birth asphyxia.

This chapter will report to the frequency of neonatal hypoxic-ischemic encephalopathy. The pathophysiology and the childhood outcome of encephalopathy due to hypoxia-ischemia will be examined. The limitations of current therapy for this condition and new therapies will be evaluated. Hypothermia seems to offer the most promise as a therapy for neuroprotection in hypoxic-ischemic encephalopathy. The evidence-based trials of hypothermia will be reviewed along with recommendations regarding clinical applications for this therapy and need for long-term follow-up of children receiving this therapy.

High urinary concentrations of activin A in asphyxiated full-term newborns with moderate or severe hypoxic ischemic encephalopathy.

BACKGROUND: Hypoxic ischemic encephalopathy (HIE) is a major cause of permanent neurological disabilities in full-term newborns. We measured activin A in urine collected immediately after birth in asphyxiated full-term newborns, and assessed the ability of the measurements to predict the occurrence of perinatal encephalopathy. METHODS: We studied 30 infants with perinatal asphyxia and 30 healthy term neonates at the same gestational age. We recorded routine laboratory variables, cranial assessments by standard cerebral ultrasound, and the presence or absence of neurological abnormalities duri

Cerebral ischemia as an apparent complication of anterior cervical discectomy in a patient with an incomplete circle of Willis.

A 58-yr-old patient sustained a cerebral ischemic injury in the distribution of the carotid artery ipsilateral to retraction during an anterior cervical discectomy. Relative hypotension was permitted during the anesthetic. Angiography revealed an anatomic variant of the circle of Willis that resulted in minimal collateralizaton of the left internal carotid artery territory. The combination of that vascular variant with relative hypotension and some degree of carotid compression appears to have resulted in clinically significant cerebral ischemia.

An uncommon case of spontaneous resolution of mural thrombus causing symptomatic severe stenosis of the extracranial part of the internal carotid artery.

OBJECTIVES: Etiology of ischemic stroke(IS) remains sometimes unexplained in younger subjects. Its well-known causes are atherosclerosis(AS), embolization, arterial dissection, congenital defects, vasculitides, smoking, use of oral contraceptives, coagulopathy and arterial dissection, or vasospasm. CASE REPORT: A 54-year old male subject was examined because of motor disturbance in his left upper extremity. Computerized tomography(CT) of the brain did not detect any focal pathological changes. Utrasonographic(USG) examination revealed severe stenosis of the right internal carotid artery(ICA)

[Changes of serum and cerebrospinal fluid insulin-like growth factor-II levels in neonates with hypoxic-ischemic encephalopathy].

OBJECTIVE: Many studies have demonstrated that low levels of insulin-like growth factor-I (IGF-I) may be associated with the hypoxic-ischemic brain damage (HIBD) and that IGF-I has a neuroprotective effect. The role of IGF-II, a structurally and functionally homologous polypeptide with IGF-I, is unclear in HIBD. This study was designed to observe the changes of serum and cerebrospinal fluid (CSF) IGF-II levels in neonates with hypoxic-ischemic encephalopathy (HIE) and to investigate its effects on HIE. METHODS: Serum and CSF IGF-II levels in 41 neonates with HIE were measured by radioimmunoas

Monitoring of neurological parameters in newborns with hypoxic-ischemic encephalopathy.

Asphyxia i a condition caused by lack of oxygen in tissues and organs. The basic pathogenic mechanisms of asphyxia are: 1)hypoxemia, 2) ischemia. The effects of perinatal asphyxia on the brain of a neonatal baby are critical in development of hypoxic-ischemic encephalopathy. The diagnosis of hypoxic-ischemic encephalopathy is based on clinical data including course of pregnancy and delivery (Apgar score) and especially on the neurological status of the newborn (consciousness, tonus, convulsions, reflexes, vegetative functions, etc.) and it can be confirmed by biochemical analysis and neurologi
