Hypersomnia in dystrophia myotonica: a neurophysiological and immunogenetic study.

Ten patients with dystrophia myotonica (8 adults and 2 prepubertal children), from three unrelated families, were investigated for diurnal sleepiness, using a sleep questionnaire and multiple sleep latency test (MSLT). Immunogenetic study was also carried out to assess the involvement of HLA region genes in modulating susceptibility to excessive diurnal sleepiness (EDS). EDS was reported by 5 patients and confirmed in each case by MSLT. In the whole patients group, mean daytime sleep latency was significantly shorter than in healthy controls matched for age and sex. At clinical or neurophysiol

Lactose malabsorption and calcium intake as risk factors for osteoporosis in elderly New Zealand women.

OBJECTS: to evaluate the prevalence of lactose malabsorption and to assess dietary calcium intake, in elderly New Zealand women with hip fractures, in age matched case controls without hip fractures drawn from general practice, and in healthy young women. METHODS: lactose malabsorption was assessed by measuring breath hydrogen after a 50 g oral lactose tolerance test. Dietary calcium was estimated from a food frequency questionnaire. RESULTS: the elderly fracture cases (n = 15), and their elderly controls (n = 16), showed a similar (60% and 63% respectively), but significantly higher prevale

Effect of enprostil on gastric emptying, intestinal transit time and post-prandial release of gastro-intestinal peptides.

The effect of an oral dose of 70 micrograms enprostil (a prostaglandin E2 analogue) on the post-prandial hormone response to a test breakfast was examined in a double-blind, placebo-controlled, cross-over study in 10 healthy volunteers. Enprostil markedly reduced the post-prandial rises in insulin and glucose-dependent insulinotropic peptide (GIP) but plasma glucose remained unchanged. To study the effects on gut motility 8 healthy volunteers ingested a liquid meal containing 75 g glucose, 20 g lactulose and 99mTc colloid after taking placebo or 70 micrograms enprostil. Gastric emptying, measu

Hyperventilation and anxiety in panic disorder, social phobia, GAD and normal controls.

Patients with DSM-III Agoraphobia, Panic Disorder, GAD, Social Phobia and normal controls underwent a series of experimental procedures and measures to determine whether panic attack patients show a greater tendency towards hyperventilation that is independent from their anxiety levels. Contrary to expectations, the Agoraphobia and Panic Disorder patients did not show significantly lower levels of expired pCO2 at rest than the other anxious or non-anxious groups. However, the panic attack patients did show significantly higher levels of anxiety and hyperventilatory symptoms during a hyperventi

Normal cardiovascular autonomic function in patients with systemic sclerosis (CREST variant).

Sixteen consecutive patients with systemic sclerosis (CREST variant (calcinosis, Raynaud's phenomenon, oesophageal dysmotility, sclerodactyly, telangiectasia)) were included in this study. Standard non-invasive clinical and electrocardiographic procedures for the detection of cardiovascular autonomic insufficiency were performed in all patients and included heart rate (HR) response to deep breathing, Valsalva manoeuvre, and standing, and blood pressure (BP) response to hand grip and standing. None of the patients had overt cardiovascular disease or were receiving cardiovascular treatment at th

A summary of preoperative and postoperative cardiorespiratory performance in patients undergoing pectus excavatum and carinatum repair.

Preoperative and postoperative pulmonary function tests and progressive work exercise performances were compared for fourteen pectus excavatum and five pectus carinatum patients who underwent operative repair. No changes could be documented in the pectus carinatum patients. The excavatum patients demonstrated a small improvement in total lung capacity (P less than .02) and a significant improvement in maximal voluntary ventilation (P less than .001). In addition, the exercise performances were improved for the excavatum patients postoperatively as quantitated both by the total exercise time an

Comparative tolerance of elderly from differing ethnic backgrounds to lactose-containing and lactose-free dairy drinks: a double-blind study.

Milk intolerance was investigated in 87 healthy elderly individuals with a mean age of 77 years who were given 240 ml of a chocolate dairy drink twice in one week with a light lunch. No significant differences in symptomatic responses distinguished the subjects consuming a lactose-free (LF) drink from those consuming a drink containing 4.5% lactose (LC) under double-blind study conditions. Breath hydrogen analysis during lactose tolerance testing identified 23 malabsorbers, none of whom responded exclusively to the LC drink, although five were symptomatic on both days, and two had symptoms onl

Inadvertent contamination of anaesthetic circuits with halothane.

All halothane vaporizers tested for leakage when turned off, leaked significant amounts of halothane and this may represent a hazard to patients liable to develop halothane hepatitis or malignant hyperpyrexia. The hazard from leaking vaporizers may be reduced considerably by the use of well-designed bypass units. Circuit contamination by halothane may still result from such sources as neoprene seals around flowmeters, breathing bags and anaesthetic hose which have had previous contact with halothane vapour, whether or not an apparatus is in use. The hazard from contaminated hoses and bags may

Antepartum evaluation of the high-risk fetus: problems and prospects.

The chief obstetrical problems encountered today in the prenatal evaluation of the high-risk fetus are presented. Advantages and pitfalls or recent techniques utilized in the management of the high-risk pregnancy are discussed. They include: a prenatal scoring system for identifying the high-risk population; examination of the karyotype of cells in amniotic fluid, and quantitation of alpha-fetoprotein levels in maternal plasma and amniotic fluid for the early prenatal detection of birth defects; ultrasonography for the intrauterine diagnosis of fetal growth retardation and assessment of fetal

Complete ventilation failure due to a defective heat and moisture exchange filter.

Heat and moisture exchange filters are routinely used in anaesthesia to provide passive humidification and protect the breathing circuit. We report a case of a 3-year-old patient where a complete circuit obstruction due to a defective heat and moisture exchange filter occurred. This resulted in a total inability to ventilate the patient's lungs, initially attributed to bronchospasm, followed by the need for cardiopulmonary resuscitation. Manufacturing defects resulting in such critical incidents are rare but remain an important potential differential when managing difficulty in ventilation. Th

Speech therapy for exercise-induced laryngeal obstruction.

INTRODUCTION: Exercise- Induced Laryngeal Obstruction (EILO) can lead to disabling exercise related dyspnea and hamper participation in physical activity. In this study, we aimed to investigate the effects of a standardized speech therapy protocol as treatment for EILO. METHODS: Patients diagnosed with EILO at our institution were invited to participate. We compared laryngeal findings obtained during a continuous laryngoscopy exercise (CLE) test and questionnaire based self-reported breathing symptoms, before vs. after the treatment intervention. The laryngeal obstruction was characterized us

Development of a low-fructose carbohydrate gel for exercise application.

This study aimed to develop a low-fructose (<3 g/serve) carbohydrate (CHO) gel for athletes. Various prototypes with 30 g CHO/serve and differing water content (12 %, 21 %, 32 %, 39 % w/v) were created and evaluated for sensory attributes. The final gel contained 62.1 ± 0.2 g CHO/100 g with 0.17 % w/w fructose. Endurance athletes (n = 20) underwent a feeding-challenge protocol, ingesting 30 g gel every 20 min during 2 h of running (60 % V ˙ O2max), followed by a 1 h self-paced distance test. Blood glucose increased significantly from baseline (4.0 ± 0.9 vs. 6.6 ± 0.6 mmol/L, p < 0.001) and

Approach to the patient with Childhood Interstitial and Diffuse Lung Disease.

Childhood Interstitial and Diffuse Lung Disease (chILD) encompasses a group of rare, chronic lung disorders in infants and children with overlapping clinical features but diverse etiologies. The clinical presentation of chILD is of chronic or recurring respiratory signs and symptoms, often including increased work of breathing and hypoxia, with diffuse radiographic abnormalities on chest imaging. Recognition can be challenging since some clinical features overlap with those of more common pediatric respiratory diseases including asthma and recurrent viral infections, among others. chILD should

Prevalence of chronic respiratory symptoms among cement factory workers in Gauteng Province, South Africa.

Cement workers are exposed to various kinds of occupational hazards, dust being the most hazardous. Despite certain exposure limits on the emission of air pollutants in place, several people die each year due to complications from respiratory disease. This study aimed to assess the prevalence of chronic respiratory symptoms among workers exposed to cement dust. A quantitative, descriptive cross-sectional design was employed among 81 workers from two cement production companies in Gauteng, South Africa in 2018. A self-administered questionnaire, anthropometric measurements, and a spirometry tes

An Investigation on Self-Medication Practice During the COVID-19 Pandemic Among the Indian Population.

BACKGROUND: Self-medication has both negative and beneficial effects on people's health, as the COVID-19 epidemic has demonstrated. The goal of the study is to look into the epidemiology of self-medicated medications used for respiratory symptoms, as a COVID-19 preventive, for its symptoms, or after a positive COVID-19 test, and to see how symptom relief is viewed in India, as well as what demographic factors encourage self-medication. METHODS: Using a trial version of Qualtrics Core XM software to prototype 24 surveys, a webbased questionnaire was built, tested, and disseminated in several I

A Rare Case of Pneumocystis Pneumonia in HIV Patient on Glucocorticoid.

Pneumocystis pneumonia (PCP) is an opportunistic infection caused by Pneumocystis jirovecii. PCP due to immunosuppressive drugs is rarely reported in the literature. Herein we present a case of PCP in a 49-year-old patient who presented with progressive shortness of breath, dry cough, and low-grade fever. History revealed that he was taking prednisolone daily for his hyperactive airway disease. His temperature was 99oF, and he had bilateral crackles in the lungs with resonant wheezing. High-resolution computed tomography showed diffuse ground-glass haze and cystic lesions in the middle and upp
