Replacement of the common carotid artery by a plastic prosthesis. Report of a case with forty years of follow-up.

An 18-year-old man suffered a 38 caliber gunshot wound to the right side of the neck on 25 September 1957. On 16 October 1957, a traumatic arteriovenous aneurysm between the common carotid artery and the internal jugular vein was resected. The jugular vein was ligated and the common carotid was replaced by a nylon prosthesis. On 16 October 1960. the dilated nylon graft was replaced by a dacron prosthesis. The patient has been followed-up during more than 40 years and his condition is normal. To the best of our knowledge, this operation was the first of its kind performed in the world and has t

Light-induced amaurosis associated with carotid occlusive disease.

A 74-year-old man with the isolated complaint of uniocular transient visual loss after exposure to bright light was found to have severe ipsilateral, atherosclerotic carotid occlusive disease. Signs of ocular ischemia that were present included slightly reduced visual acuity, mild afferent pupillary defect, lowered intraocular pressure, increased photostress recovery time, and reduced opthalmodynamometry values on the affected side. After a subclavian to internal carotid artery bypass procedure, the patient's symptoms resolved completely and his ocular signs returned to normal. This patient's

Tissue factor pathway inhibitor, vascular risk factors and subclinical atherosclerosis: the Multi-Ethnic Study of Atherosclerosis.

BACKGROUND: Tissue factor pathway inhibitor (TFPI) is an endothelial membrane-associated anticoagulant protein. Higher circulating levels might reflect endothelial damage. OBJECTIVE: We hypothesized an association of higher total TFPI with subclinical atherosclerosis. PATIENTS/METHODS: Total TFPI was measured in 1000 participants of the Multi-Ethnic Study of Atherosclerosis, a cohort of 6814 men and women without clinical vascular disease, aged 45-84, from four ethnic groups. Subclinical atherosclerosis measures were coronary artery calcium (CAC), carotid intima-media thickness (IMT) and ank

Bihemisphere Ischemia Due to a Unilateral Lesion: A Case Report.

Bihemispheric ischemic strokes secondary to unilateral vessel disease are uncommon. We present the case of a 70-year-old man with multiple acute/subacute bilateral infarcts. The patient was found to have stenosis of the left internal carotid artery secondary to herpes zoster ophthalmicus vasculopathy, with involvement of the left proximal middle and anterior cerebral arteries. Angiographic studies also revealed A1 segment aplasia of the right anterior cerebral artery (ACA), thus indicating dependence on the left-sided circulation for perfusion of the bilateral ACA vascular territory. This case

Simultaneous occurrence of internal capsule infarct and cerebellar haemorrhage in a patient with hemiplegia.

A 68-year-old woman with hypertension with no history of cerebrovascular events presented with a left-sided hemiplegia which had developed acutely 2 days ago. She was not on maintenance therapy with antiplatelets or anticoagulants. A CT scan showed acute ischaemic infarction of the right internal capsule and cerebellar haemorrhage. Cardiac evaluation was normal. Doppler ultrasonography of the extracranial carotid and vertebral arteries showed diffuse arteriosclerotic changes, but did not reveal any haemodynamic occlusion. The simultaneous development of dual strokes was considered to be an ext

Anterior approaches in juvenile nasopharyngeal angiofibromas with intracranial extension.

Although surgery is regarded as the mainstay of treatment for juvenile nasopharyngeal angiofibromas (JNAs), ancillary treatment modalities such as radiotherapy and on rare occasions chemotherapy are still recommended by many for intracranial extension with apparent radiologic involvement of the cavernous sinus and internal carotid artery. Further, most authors undertaking surgical excision of this subgroup of patients would recommend a lateral or combined frontal and lateral approach for its removal. In a series of 49 cases of JNA, 14 were found during surgery to have intracranial extradural e

Carotid artery reconstruction for Takayasu's arteritis the necessity of all-autogenous-vein graft policy and development of a new operation.

BACKGROUND: We have adopted an all-autogenous-vein-graft policy in carotid reconstruction for Takayasu arteritis, namely an ascendo-right carotid and right subclavian (axillary) arteries bypass using a pantaloon vein graft for patients all of whose arch branches are occluded, and an extra-anatomical bypass from the right subclavian artery for patients whose brachiocephalic artery is the only arch branch that remains patent. This report is to elaborate on these operations and to assess the long-term outcome. METHODS: Six patients were operated on according to this policy; (5 women, 1 man, age

[Prevalence of carotid arteries atherosclerotic changes in ischemic stroke patients].

UNLABELLED: The aim of our study was to investigate prevalence, characteristics and extensiveness of CA plaques in acute stroke using high-resolution B-mode USG. METHODS: USG of CA was performed with a 7.0-MHz Duplex-type scanner Acuson 128 XP/10 C. We examined 149 patients aged 26-95 years, with acute ischemic stroke, consecutively admitted to our Department. Atheromatous plaques morphology was determined using the HSP Classification (H--Hemodynamic characteristics, S--Surface plaque, P--Plaque echogenics). RESULTS: An extracranial carotid disease (CCA, ICA, ECA) was found in 85 (57%) patie

Disease progression in contralateral carotid artery is common after endarterectomy.

OBJECTIVE: Although the North American Symptomatic Carotid Endarterectomy Trial (NASCET) and the Asymptomatic Carotid Atherosclerosis Study (ACAS) have helped to define the role of carotid endarterectomy (CEA) for both symptomatic and asymptomatic lesions, the role of surveillance of the contralateral carotid artery remains unclear. The purpose of this study was to determine the progression of contralateral carotid artery disease with serial duplex ultrasound scans after CEA compared with the recurrent stenosis rate for the carotid artery ipsilateral to the CEA. METHODS: From January 1990 to

Endovascular embolization of intermittent massive epistaxis secondary to posttraumatic carotid artery pseudoaneurysms.

Epistaxis is a very common medical problem in otolaryngology practice and can usually be controlled with conservative interventions. Rarely, uncontrolled and life threatening epistaxis occurs. We present the case of a 29-year-old male who developed intractable intermittent epistaxis due to post-traumatic pseudoaneurysms arising from the cavernous segment of the left internal carotid artery. The patient was successfully treated with endovascular embolization.

Bilateral traumatic caroticocavernous fistulae: total resolution following unilateral occlusion.

Balloon occlusion is the accepted treatment for direct posttraumatic caroticocavernous fistula. We present a case of bilateral traumatic fistulae associated with a pseudoaneurysm. Resolution of both fistulae occurred following treatment of one of them by balloon occlusion of the internal carotid artery. This case highlights the importance of considering a more conservative approach to bilateral fistulae or those associated with a pseudoaneurysm. We review other treatment options.

[Parapharyngeal tumors: apropos of 14 cases].

The author reports on fourteen parapharyngeal tumours, of which 7 are of parotidean salivary origin. The other etiologies, in particular 1 chemodectoma and 1 aneurysm of the internal carotid artery, justify a systematic arteriography with a CT scan. In surgery, 9 operations were performed via the purely cervical approach, and 5 via a combined cervical and transvelar approach. The endobuccal vault is limited and dangerous.

[Isolated unilateral hypoglossal nerve paralysis in carotid dissection].

A 41-year-old man presented with a unilateral atrophy and paresis of his tongue which were not explained by any pathological finding in the brainstem or at the base of the skull. The diagnosis of right internal carotid artery dissection was suggested by magnetic resonance imaging and ultrasound examination and confirmed by arterial angiography. During anticoagulant therapy clinical recovery and angiographical improvement occurred.

Carotid-Threatening Penetrating Oropharyngeal Trauma in a Pediatric Patient: A Case Report and Literature Review.

Penetrating oropharyngeal injury is a relatively common occurrence in pediatric patients; however, cases involving close proximity to critical vascular structures, such as the internal carotid artery (ICA), are exceedingly rare and pose significant risks. This case report describes a seven-year-old male who sustained penetrating oropharyngeal trauma with startling proximity to the ICA after being pushed into a locker with a pencil in his mouth. Initial evaluation showed the wooden pencil protruding from the soft palate without active bleeding. CT angiography revealed that it was less than 1 mm

Bilateral ophthalmic segment "kissing" aneurysms presenting with subarachnoid hemorrhage--case report.

A 31-year-old woman presented with bilateral ophthalmic segment "kissing" aneurysms causing subarachnoid hemorrhage manifesting as sudden severe headache and nausea 3 days before admission. Cerebral angiography demonstrated bilateral internal carotid-superior hypophyseal artery aneurysms, both projecting medially from the medial surface of the internal carotid arteries and appearing to touch each other. Both aneurysms had to be clipped in the same operation, because of uncertainty over which aneurysm had bled. She underwent bilateral frontotemporal craniotomy on the day after admission. Intrao

Early hemodynamic characteristics of eversion and patch carotid endarterectomies.

OBJECTIVE: Carotid endarterectomy (CEA) is currently the gold standard in the operative management of carotid artery stenosis. While eversion and patch CEAs vary greatly in technique, various studies have determined equivalence with regard to clinical outcomes. However, the hemodynamic differences following each procedure are not known. This study aimed to investigate any early hemodynamic differences between eversion and patch CEAs. METHODS: All CEAs performed at our institution from March 2012 to June 2018 were aggregated in a retrospective database by querying the 35301 CPT code from the e

Combined carotid endarterectomy with transluminal angioplasty and primary stenting of the supra-aortic vessels.

BACKGROUND: Carotid endarterectomy (CEA) is the standard of care for patients with high-grade carotid artery stenosis who are acceptable surgical candidates. Focal occlusive lesions of the origin of aortic arch vessels can be effectively and safely treated with balloon angioplasty and primary stenting. The purpose of this study was to retrospectively review results of carotid endarterectomy for high-grade carotid bifurcation stenosis combined with intraoperative retrograde transluminal angioplasty and primary stenting of a hemodynamically significant stenosis at the origin of a proximal ipsila
