In high-risk patients undergoing surgery for colorectal cancer with organ invasion, a thorough evaluation of surgical indications and careful selection of optimal approaches is essential. We report a case of advanced transverse colon cancer with gastric invasion in a 58-year-old male with Child-Pugh grade B liver cirrhosis. The patient underwent laparoscopic transverse colectomy combined with gastric wedge resection. Intraoperative upper gastrointestinal endoscopy facilitated precise gastric resection. The primary tumor was excised with a clear margin, and the postoperative course was uneventf

Falls are a leading cause of injuries to the temporomandibular joint (TMJ) in the paediatric age group. In low- and middle-income nations, after injury to a child's chin, a careful clinical examination of the mandible and TMJ is sometimes omitted. Paediatric mandibular condylar fractures tend to shatter the relatively narrow condylar head, leading to ankylosis to the skull base, which leads to debilitating progressive deformity.1 Early surgical release is then the sole modality of treatment and needs to be followed up by several weeks of active mouth-opening exercises to prevent re-ankylosis,

Pediatric neuroimaging is a complex and specialized field that uses magnetic resonance (MR) imaging as the workhorse for diagnosis. Standard MR techniques used in adult neuroimaging are suboptimal for imaging in pediatrics because there are significant differences in the child's developing brain. These differences include size, myelination and sulcation. MR protocols need to be tailored to the specific indication and reviewed by the supervising radiologist in real time, and the specialized needs of this population require careful consideration of issues such as scan timing, sequence order, sed

Post-licensure surveillance for adverse events following immunizations (AEFI) can identify rare complications of vaccinations and rigorous vaccine adverse event causality assessments can help to identify possible causal relationships. We report the development of arm paralysis after varicella vaccination in a 1-year-old child. Paralysis was initially presumed to be due to vOka because of the temporal relationship between vaccination and onset of arm weakness; however, molecular studies identified wild-type varicella zoster virus VZV (WT-VZV) in the CSF, leading the authors to conclude that WT-

Vitamin B1 (thiamine) plays a fundamental role in the proper functioning of the nervous and cardiovascular systems and in glucose metabolism. Because of the inability of the human body to store large amounts of vitamin, continuous restoration through diet is necessary. In the pediatric population, thiamine deficiency has a broad spectrum of clinical presentations, making diagnosis difficult. In this article, we report the case of a 7-month-old child with thiamine deficiency presented with neurological symptoms and simulation of acute abdomen. The patient received thiamine replacement, which re

Tuberous sclerosis complex (TSC) and nail-patella syndrome (NPS) are autosomal dominant pleiotropic disorders with full penetrance that can both involve kidneys. TSC1 and NPS genes are located on chromosome 9q3. In a large family with the two disorders with two novel frameshift TSC1 and LMX1B mutations, we describe the phenotypes. The father, who has both disorders, has passed on TSC to three of his children, NPS to another three, and both TSC and NPS to one child. Patients carrying both mutations appear to show an additive phenotype and no obvious epistatic effects. The segregation of two dom

Max was treated with SABA using an MDI and spacer with facemask and responded well to the initial treatment. You explained to the parents that nebulisers are neither required nor recommended in the treatment of wheezing in their child's situation. You advised the parents on the proper technique of MDI use with spacer and facemask, as well as care of the equipment. You also gave them a clearly written action plan regarding the efficient management of the next episode of wheeze with MDI and spacer. You further explained the side effects of oral bronchodilators and nebulisers, and why you refrain

IgG4-related disease was first described in adults with autoimmune pancreatitis but is now known to affect multiple organs. Lung involvement has never been described in children to our knowledge. Here, we report an adolescent presenting with recurrent dry cough and hemoptysis who was found to have venous ectasia in the left upper lobe, and diffuse bronchiectasis. Sustained high levels of IgG4 (1,090 mg/dL) were found, and the endobronchial biopsy revealed a marked infiltration of plasma cells producing IgG4 (ratio of IgG4 plasma cells to IgG plasma cells >50%). This unique case highlights t

Cerebellar infarction is a known possible complication of extreme prematurity, and usually results in severe disability. This report presents a 20-month-old girl (16 months corrected), who was born in a toilet at 24 weeks gestational age. She had multiple neonatal predictors of poor outcome, including unilateral cerebellar hemispheric infarction, grade 3 intraventricular hemorrhage, periventricular leukomalacia, fungal sepsis, patent ductus arteriosus, pulmonary valve stenosis, lack of prenatal care, and poor social situation. Despite all these poor prognostic factors, she was close to meeting

Henoch-Schönlein purpura (HSP) is the most common vasculitis of childhood. In this review, the main clinical features and complications are described. Although most features are self-limiting, renal disease is the most likely to result in long-term morbidity. Treatment of HSP nephritis is controversial, and the evidence for both prevention and treatment of established disease is reviewed. Follow-up for children presenting with HSP should be for at least 6 months and should include regular urine testing for proteinuria and haematuria and a blood pressure measurement. Women with a history of HSP

PURPOSE OF REVIEW: An estimated 0.5-4% of pregnant patients have cardiac disease, such as rheumatic disease, which is decreasing in Western countries, uncorrected congenital heart disease, cardiomyopathy and ischaemic heart disease. There has been an increase in maternal mortality due to cardiac causes. Congenital heart disease is becoming the most common source of cardiac problems in the pregnant patient, because patients are increasingly likely to survive to childbearing age with the improvement of surgery.
RECENT FINDINGS: The increasing age of patients conceiving their first child is also

Three women, aged 28, 27 and 25 years, two primigravidae and one woman who had given birth to a hydropic child previously, had four pregnancies with hydrops of the foetus and the placenta: two (in the same patient) due to foetal cardiomyopathy and two with foetal heart block. The patients developed severe oedema and preeclampsia. After the delivery they recovered well. Since the introduction of anti-D rhesus immunoprophylaxis, foetal hydrops has become less frequent. The resulting maternal complications (the maternal hydrops syndrome and enlarged polycystic ovaries) are therefore less well-kno

The pediatrician plays a pivotal role in the initial diagnosis of food allergy. Alternative diagnoses are considered as a careful history, physical examination, and directed laboratory tests determine the type of adverse reaction and the responsible food. Through elimination diets in infants, appropriately selected tests for specific IgE, and, in some cases, supervised oral food challenges, a diagnosis is secured. Treatment consists of strict dietary elimination with provisions for emergency management of accidental ingestions. Referral to an allergist and dietitian is made as warranted by the

PURPOSE: To report the association of tonic pupil, congenital neuroblastoma, Hirschsprung disease, and central hypoventilation syndrome.
METHODS: Case reports.
RESULTS: Two infants with central hypoventilation syndrome and Hirschsprung disease were evaluated for dilated, nonreactive pupils present since birth. In both cases, pharmacologic testing with dilute pilocarpine confirmed denervation supersensitivity consistent with tonic pupil. The diagnosis of congenital neuroblastoma was subsequently established in both infants.
CONCLUSIONS: The close association of these conditions in these two

Spina bifida and other congenital abnormalities of the spinal cord are relatively common (1 per 1000 births). Early urologic evaluation to determine the extent of neurologic involvement of the lower urinary tract is essential. Urodynamic studies are important in determining voiding pressure and leak pressure and in classifying the type of detrusor and sphincter dysfunction. Therapy is directed toward preservation of the upper tracts. Many nonsurgical (clean intermittent catheterization and anticholinergic drugs) and surgical procedures allow the child to be continent and maintain a normal uppe

This is a case report of a two-year-old boy who was operated electively for a blind-loop syndrome of the proximal jejunum. Because of the appearance of chylous ascites, parenteral nutrition was carried out postoperatively. The boy developed a severe uncompensated acidosis and paralytic ileus. Relaparotomy on suspicion of ischemic bowel did not explain the cause of the acidosis and ileus. Postoperatively, the child's condition worsened, requiring intensive care. The drastically elevated lactate levels corroborated the eventually suspected diagnosis of a vitamin B1 deficiency syndrome. The admin

Malnutrition due to lack of nutritional resources unfortunately remains an endemic which ravages Third World countries. Not only is malnutrition a result of continuous droughts and wars, but it is a reflection of the difficult economic circumstances which persist in these countries. Malnutrition should be specifically treated in each region with the least costly locally available means. The experience of the Protestant Hospital of Dabou proves that this manner of care is possible in the midst of a rural area with very limited resources. Hospital surveillance, nutritional education and a stay i

In a group of 8-12-year-old children from two areas of Bratislava with a different degree of air pollution the authors investigated repeatedly the growth of the children. It was revealed that in boys and girls from the exposed area growth was retarded, as compared with children from a relatively clean area as well as when compared with the Bratislava population. After improvement of the living environment in the exposed area children who grew up in an environment, where the two areas did not differ as to contamination of the atmosphere, achieved the same growth level at the age of 10 years. The effects of human immunodeficiency virus (HIV) disease, with its progressive and multifaceted manifestations, have not yet been fully expressed in the hemophilic population. However, even at this phase of the disease, the effects of the viral illness on the growing hemophilic child are extremely complex and have only recently been recognized and studied. The management of these unfortunate children is difficult and demands understanding and insight to obtain maximum benefit from the limited therapeutic armamentarium at hand. This unsatisfactory situation will continue at least for the near

Reports about recent famine victims and refugees have described the occurrence of xerophthalmia and resultant blindness related to severe vitamin A deficiency. These populations are subject to high prevalences of childhood protein-energy malnutrition and infectious diseases, pre-existing marginal vitamin A status, and inadequate levels of vitamin A in relief rations. In order to prevent unnecessary morbidity and mortality when any of these risk factors arise, famine victims or refugees should receive vitamin A supplements as an early and essential component of the nutritional support provided
