Angiography in malignant and chronic inflammatory lesions of the gallbladder.

Angiography findings in 18 patients with carcinoma of the gallbladder and 17 with cholecystitis indicate that similar abnormalities may appear in both lesions. Irregularities and obstruction of arteries were more common in malignant disease and somewhat more severe. Accumulation of contrast medium and prominent veins occurred in both groups. Displacement of adjacent vessels was more common in malignant disease. As chronic inflammatory lesions cannot with certainty be differentiated angiographically from malignant disease, cholecystectomy is recommended when the abnormalities described are demo

Percutaneous Cholecystostomy: Procedural Guidance and Future Directions for Clinical Management.

Percutaneous cholecystostomy (PC) is a recognized treatment option for the management of acute cholecystitis and is an integral component of the treatment algorithm in the 2018 Tokyo Guidelines. The utilization of PC has significantly increased over the past 30 years, particularly in the setting of critically ill patients and those with extensive comorbidities who are poor surgical candidates. The indications, complications, patient selection considerations, and technical complexities of the procedure will be discussed. Postprocedural drain management and the potential for shortened indwelling

Harry Truman's Complicated Cholecystectomy.

Harry S. Truman, the 33rd President of the United States, developed right-sided abdominal pain the year after he left office. Misdiagnosed with appendicitis, Truman underwent an appendectomy before a cholecystectomy treated the underlying cholecystitis. This error was concealed at the time from the American people. His postoperative course was closely followed by Americans through newspapers and was complicated by a bout of Clostridium difficile colitis. Truman survived this episode to die of heart failure decades later.

Unusual fistula demonstrating the need for an early cholecystectomy.

A 79-year-old man developed a spontaneous cholecystocutaneous fistula 12 months after an initial episode of acute cholecystitis. A laparoscopic cholecystectomy procedure was twice abandoned due to extensive adhesions and active disease, limiting safe dissection of Calot's triangle. Abdominal collections formed and a spontaneous cholecystocutaneous fistula developed. Imaging revealed an 11 cm calculus and erosion of the fundus of the gall bladder through the sheath. Definitive management was achieved with a laparoscopic assisted open cholecystectomy.

Rare case of cystic artery pseudoaneurysm.

A patient with a cystic artery pseudoaneurysm (CAP) presented to the emergency department with upper abdominal and back pain. The patient also had clinical signs of sepsis. CT revealed gallstones with acute suppurative cholecystitis with a gallbladder perforation. In addition, a CAP was also suspected and subsequently diagnosed on CT angiography. The pseudoaneurysm was treated with embolisation and a cholecystostomy was performed for the gallbladder perforation. Following her acute admission, the patient underwent an elective cholecystectomy and made a good recovery post surgery.

Peril of missing a double gall bladder: report of a patient who needed cholecystectomy two times over.

Gall bladder (GB) duplication is a rare anomaly, not often seen in regular clinical practice. Though a vestigial organ, the presence of calculi within the GB can cause the patient to be acutely symptomatic with right hypochondriac pain, which can even be life threatening if not treated. The presence of two GBs means this pathology can be seen in both the GBs, highlighting the importance of diagnosing this condition, lest the patient returns years after a cholecystectomy with cholecystitis yet again!

[Minimally-invasive surgery: even less invasive? Oncological surgery: multidisciplinary first].

Despite advertising for NOTES in 2009, single trocart laparoscopic surgery is about to become a new standard in selected indications. As other important topics, the limits of oncological surgery are extended due to a systematic multidisciplinary approach. To discuss every publication would be difficult and our review will focus on a selected number of papers of importance for daily practice. As examples, the management of acute calculous cholecystitis, gastro-esophageal reflux, inguinal and incisional hernia repair as well as colorectal surgery are presented.

[Method of diagnosing liver tumors].

The investigation is concerned with developing a model for using blood serum from patients with hepatic tumors to induce the activity of a gamma-glutamyl transferase enzyme in homogenized bovine hepatic tissue culture. An immunoglobulin protein was found to be responsible for the effect. The technique under discussion (Patent No. 2027997) provides a means for differential diagnosis of hepatoma and liver metastasis, on the one hand, and cholecystitis, pancreatitis, hepatitis, echinococcosis and hemangioma of the liver, on the other. The effectiveness of the method was 94.7; sensitivity--90.8 an

Laparoscopy for Acute Abdominal Conditions in Children.

Laparoscopy for acute abdominal conditions if one area where laparoscopy is being used in children. However, because of the reduced abdominal surface area of small children, special concerns, are present and must be appreciated. The most common acute abdominal in children for which laparoscopy is used is appendicitis. Other indications include cholecystectomy for acute cholecystitis, Meckel's diverticulectomy, evaluation for trauma, and correction or excision of adnexal torsions. In addition, the future of laparoscopy for acute abdominal conditions is speculated.

[Cholecystolithotomy with cholecystojejunostomy as surgical solution for cholelithiasis in portal hypertension caused by hepatic cirrhosis].

Cholecystolithotomy and cholecystojejunostomy has been carried out on 11 patients with severe hypertension from cirrhosis. The indications were frequently recurring attacks of biliary cholic or acute cholecystitis at onset. There was 1 postoperative death from cardiac infarction and only minor in-hospital morbidity. None of the remaining patients has to date developed post-cholecystojejunostomy sequelae. Except the case of extensive inflammation on gangrena, this procedure appears to be a safe and definitive operation, alternative to subtotal cholecystectomy.

[Microcholecystostomy].

Treatment efficacy was compared for transcutaneous transhepatic microcholecystostomy controlled by laparoscopy (75 patients) against that controlled by ultrasound scanning (67 patients) principally in acute cholecystitis. Basic indications for microcholecystostomy are a high operative risk in elderly patients and decompression of the bile ducts for obstructive jaundice. Four patients (2.8%) developed postmicrocholecystostomy complications treated by urgent surgery. The manipulation is short-term, simple and is well tolerated even in grave condition. This makes microcholecystostomy an alternati

Acute Gallbladder Volvulus in a 93-Year-Old High-Risk Patient: A Case Report.

BACKGROUND A gallbladder volvulus is a rare and life-threatening emergency requiring immediate surgery to prevent gallbladder perforation and sepsis. A correct preoperative diagnosis is often difficult. CASE REPORT We describe the case of a 93-year-old woman with an acute gallbladder volvulus that was initially misdiagnosed as acalculous cholecystitis. She was started on i.v. antibiotics for conservative management due to a symptomatic, high-grade aortic valve stenosis. After her clinical condition worsened, an emergency cholecystectomy was scheduled. After an interdisciplinary discussion with

Evaluation of Risk Factors Leading to Conversion from Laparoscopic Cholecystectomy to Open Surgery: A Retrospective Controlled Study.

Background: Laparoscopic cholecystectomy (LC) is one of the most frequently performed operations in surgical practice. Despite increasing experience and technological advances, conversion to open surgery is still necessary in some cases. Although several studies have identified factors associated with increased risk of conversion, debates continue in the literature. This study aimed to evaluate the factors affecting the risk of conversion to open cholecystectomy. Materials and Methods: Patients who underwent surgery for cholelithiasis between 2020 and 2023 were analyzed retrospectively. Patien

[Risk factors of post-ERCP complications: a single-center retrospective study].

OBJECTIVE: To analyze standalone and synergistic risk factors of early post-ERCP adverse events. MATERIAL AND METHODS: We retrospectively reviewed case records of patients with choledocholithiasis who underwent endoscopic lithoextraction (n=876) between 2018 and 2023. Significant risk factors, odds ratios (OR) and threshold values were identified for postoperative complications. Combinations of factors and their cumulative effect were obtained using regression analysis. RESULTS: There were the following postoperative complications: hyperamylasemia (5.8%), acute pancreatitis (5.5%), cholangit

Assessment of Risk Factors for Conversion in Laparoscopic Cholecystectomy Performed Due to Symptomatic Cholecystolithiasis.

INTRODUCTION: Laparoscopic cholecystectomy is the current gold standard in the treatment of symptomatic cholecystolithiasis. The occurrence of the conversion is related to higher perioperative mortality or prolonged hospitalization. The aim of this study was to evaluate selected risk factors for conversion of laparoscopic cholecystectomy among patients undergoing surgery due to symptomatic cholelithiasis. PATIENTS AND METHODS: A retrospective analysis of patients operated on for symptomatic cholelithiasis from November 2021 to June 2023 was performed. Correlations between selected factors and

Concurrent adrenal and extra-adrenal myelolipoma: A case report.

INTRODUCTION: Myelolipoma, a benign tumor characterized by mature fat cells and hematopoietic cells, is predominantly found in the adrenal glands, accounting for 6-16 % of all adrenal tumors. These tumors are often asymptomatic and discovered incidentally during imaging. We present a rare case of concurrent adrenal and extra-adrenal myelolipomas, contributing to the limited research in this area. CASE PRESENTATION: A 65-year-old female with a history of Steven-Johnson syndrome presented with epigastric pain, initially diagnosed with emphysematous cholecystitis. Predictive Factors for the Development of Gallbladder Necrosis.

Introduction Acute cholecystitis is a common complication of gallstone disease. Likewise, gallbladder necrosis is a complication of cholecystitis associated with higher risks of morbidity and mortality. Identification of risk factors which portend to gallbladder necrosis is key in prioritizing the management of higher-risk patients. This study aimed to identify such factors that predict the development of gallbladder necrosis. Method A retrospective review of all patients undergoing emergency cholecystectomy in a tertiary hospital over a two-year period was performed. Gallbladder necrosis was

Gallbladder hemorrhage associated with segmental arterial mediolysis: a case report.

BACKGROUND: Gallbladder hemorrhage is a rare but fatal condition. The reported causes of gallbladder hemorrhage include iatrogenesis, atherosclerotic changes in the cystic arteries, acute cholecystitis or cholelithiasis, malignancy, trauma, hemophilia, pseudoaneurysm, and the use of oral anticoagulant medications. Recently, segmental arterial mediolysis (SAM) has been reported as a possible etiology of life-threatening abdominal, retroperitoneal, and intracranial hemorrhages. However, no previous reports have described the association between gallbladder hemorrhage and SAM. CASE PRESENTATION:
