Decline in coronary heart disease mortality. Old questions and new facts.

By 1979, mortality rates for coronary heart disease had declined for 14 consecutive years in the United States. Preliminary data indicate a continued decline. This review of reports published in the last five years documents the consistency of most data with the following hypotheses: (1) Reductions in population levels of hypertension and cigarette smoking have contributed to the decline in mortality from coronary heart disease. (2) Improved medical care for acute myocardial infarction has also contributed to the decline. Data are lacking to specify the contributions of changes in other risk f

Evaluation of concomitant peripheral arthritis in early axial spondyloarthritis: results from a 24 month follow-up (Italian arm of SPACE study).

OBJECTIVE: Although peripheral arthritis (PA) is common in early axial spondyloarthritis (axSpA), its influence on spinal/pelvic structural damage and disease activity remains unclear. We aimed to assess associations between PA, clinical and disease activity indices, and imaging features in early axSpA. METHOD: Baseline data analysis of the Italian SPACE cohort, including patients with chronic back pain (duration ≥3 months and ≤2 years; age of onset <45 years), was conducted. axSpA was diagnosed by magnetic resonance imaging and X-rays of the sacroiliac joints. Clinical features, disease acti

Analysis of Hematological Indices in Patients of Systemic Lupus Erythematous and Its Correlation with SLEDAI-2K.

Background: Systemic lupus erythematosus (SLE) is an autoimmune disease which is characterized by chronic multisystem inflammation and end-organ damage. In recent times there has been a need for new hematological markers to assess disease activity in SLE patients specifically, which can be easily available like eosinophil, basophil, neutrophil, monocytes, and platelet to lymphocyte ratios (ELR, BLR, NLR, MLR, and PLR, respectively). Materials and methods: The present investigation determines the use of a different peripheral hematological marker to assess SLE activity in 106 patients attended

[Contra indications of sclerotherapy, update 2005].

BACKGROUND: Modern sclerotherapy, using ultrasound guidance and foam sclerosants, is a reference treatment of varices of the lower limbs. Like all other techniques it should be employed in the safest, precisely described and reproducible conditions. Therefore, its contra indications must be clearly known. METHODS: Contra indications of sclerotherapy can be defined in legal notices provided by the manufacturers in compliance with regulatory standards, developed empirically, extrapolated from theoretical mechanisms, or analyzed from controlled studies. RESULTS: Contra indications can be patien

Micro RNAs as Emerging Therapeutic Targets in Liver Diseases.

MicroRNAs (MiRNAs) are endogenous non-coding small RNA molecules that regulate gene expression in plants, animals and some viruses. Both normal and pathological liver processes are regulated by miRNAs. Recent research indicated that miRNAs have been implicated in liver diseases caused by viral hepatitis (Hepatitis B and Hepatitis C), metabolic problems, alcohol and drug abuse. Because altered miRNA expression is linked to liver metabolic dysregulation, liver damage, liver fibrosis, and tumour growth, miRNAs are promising therapeutic targets for the detection and treatment of liver diseases. An Overview of Hepatoprotective Effects of Thymoquinone.

BACKGROUND: Liver diseases are globally recognized as major medical problems. Several species of herbal medicine have been investigated for the management of liver-related diseases. OBJECTIVE: In this context, Thymoquinone (TQ) as the main constituent of Nigella sativa has long been focused for the treatment of liver diseases in recent years. To suppressing liver diseases, it was reported that TQ exhibits hypolipidic effects, which prevented the fatty acid accumulation in the hepatocytes. The other activity of TQ, not only in the management of chronic liver diseases but also regarding the car

Innate lymphoid cells in autoimmunity: emerging regulators in rheumatic diseases.

Innate lymphoid cells (ILCs) are important in the regulation of barrier homeostasis. These cells do not express T cell receptors but share many functional similarities with T helper cells and cytotoxic CD8+ T lymphocytes. ILCs are divided into three groups, namely group 1 ILCs, group 2 ILCs and group 3 ILCs, based on the transcription factors they depend on for their development and function, and the cytokines they produce. Emerging data indicate that ILCs not only have protective functions but can also have detrimental effects when dysregulated, leading to chronic inflammation and autoimmune

Mycobacterial infections in the era of modern biologic agents.

Patients with rheumatologic disease are at increased risk for mycobacterial infections because of both immunocompromising therapy and preexisting lung damage from the disease itself. Tuberculosis is uncommon in the United States; however, it is relatively more prevalent in immigrants, minorities, elderly, persons with acquired immune deficiency syndrome and among healthcare workers. In the United States, the nontuberculous mycobacteria (NTM) are now more prevalent than tuberculosis. Although initially noted in men with chronic obstructive pulmonary disease, recent observations indicate increas

No evidence for XMRV association in pediatric idiopathic diseases in France.

Retroviruses have been linked to a variety of diseases such as neoplastic and immunodeficiency disorders and neurologic and respiratory diseases. Recently, a novel infectious human retrovirus, the xenotropic murine leukemia virus-related virus (XMRV), has been identified in cohorts of patients with either a familial type of prostate cancer or chronic fatigue syndrome. The apparent unrelatedness of these diseases raised the question of the potential involvement of XMRV in other diseases. Here, we investigated the presence of XMRV in a selection of pediatric idiopathic infectious diseases with sy

Proteinuria in autoimmune thyroid disease.

We have investigated the prevalence of proteinuria in patients with Graves' disease and chronic autoimmune thyroiditis attending a routine thyroid clinic. Using the urine protein creatinine index, proteinuria was found in 29.8% of patients with autoimmune thyroid disease and in 9.5% of patients attending the same clinic but without these conditions. When patients with Graves' disease were treated with 131I, proteinuria measured by 24 h collections developed in 9 of 14 patients without pre-existing proteinuria and appeared to diminish in 4 patients in whom proteinuria had been present before tr

Perioperative management of obstructive sleep apnea: a survey of Puerto Rico anesthesia providers.

BACKGROUND: Obstructive sleep apnea (OSA) prevalence has been increasing in the past years adding significant morbidity. Perioperative management is controversial and few studies have addressed this matter. The American Society of Anesthesiology (ASA) and the American Academy of Sleep Medicine (AASM) have developed clinical practice guidelines for the perioperative management of patients with OSA. Existing evidence suggest an increase in early postoperative complications in patients with OSA. Nevertheless, data about perioperative management of OSA is limited. To our knowledge, only two studie

Management of mineral metabolism in hemodialysis patients: discrepancy between interventions and perceived causes of failure.

BACKGROUND: Mineral and bone disorders (MBD) in patients undergoing hemodialysis (HD) are a major clinical complication. Current therapeutic strategies do not attain the expected results. The Italian audit on mineral metabolism was implemented to investigate MBD management through a "patient-oriented" approach. METHODS: Clinical and laboratory data pertinent to MBD from 509 prevalent adult patients on chronic HD were recorded and examined (audit), after which individual strategies were elaborated to improve MBD control. Their effectiveness was evaluated 6 months after the audit (Post-6). RES

Bidirectional negative relationship between thyrotropin and kidney function during alcohol intoxication in males.

INTRODUCTION: Despite a well-established direct toxic effect of alcohol on renal cells, there is a salutary dose-dependent effect of alcohol consumption on common laboratory parameters related to kidney performance. Alcohol also impacts thyroid hormones, while thyroid status modulates kidney function. The modulation of kidney parameters with thyrotropin (TSH) and thyroid status indicates a possible interaction between alcohol, kidney, and thyroid functions. This retrospective study was conducted to test the hypothesis that the positive effect of alcohol use on the estimated glomerular filtrati

Endoscopic Features of Chronic Rhinosinusitis in Patients with Gastroesophageal Reflux Disease.

Chronic rhinosinusitis (CRS) is a complex inflammatory condition affecting the nasal and paranasal sinus mucosa. Gastroesophageal reflux disease (GERD) has been implicated as a potential exacerbating factor in CRS, but the specific endoscopic features of nasopharyngeal pathology in this context remain poorly understood. Background and Objectives: Chronic rhinosinusitis is a multifactorial disease with various underlying etiologies, including inflammation, anatomical factors, and environmental triggers. While gastroesophageal reflux disease has been suggested as a potential contributor to chron

Case Report: Variable Pharmacokinetic Profile of Eculizumab in an aHUS Patient.

BACKGROUND: With the introduction of eculizumab, a C5-inhibitor, morbidity and mortality improved significantly for patients with atypical hemolytic uremic syndrome (aHUS). In view of the high costs, actual needs of the drug, and increasing evidence in literature, aHUS patients can be treated according to a restrictive eculizumab regimen. We retrospectively analyzed the pharmacokinetic and dynamic parameters of eculizumab in one patient in time, emphasizing various factors which could be taken into account during tapering of treatment. CASE PRESENTATION: A nowadays 18-year-old male with a sev
