AI-Augmented Fundus Disease Screening by Non-Ophthalmologist Physicians: A Paired Before-After Study.

Screening for retinal disease is increasingly performed by general practitioners and other non-ophthalmologist clinicians in primary care, especially where access to ophthalmology is limited and diagnostic accuracy may be suboptimal. To investigate the role of an automated fundus-interpretation support solution in improving general physicians' screening accuracy and referral decisions, we conducted a paired before-after study evaluating an AI-based decision support tool. Four non-ophthalmologists who have been involved in screen fundus images in clinical practice reviewed 500 de-identified col

Iniencephaly: radiologic and pathomorphologic perinatal observation.

Iniencephaly (IE) is a rare neural tube malformation involving severe head retroflexion and deformity of the spine. IE is typically accompanied with other congenital abnormalities and carrying a poor fetal prognosis. This report presents radiological findings in a rare case of IE associated with multiple malformations of the skull, spine, face, heart, and body. A 44-year-old pregnant female underwent an obstetric ultrasound examination on the 26th week of gestation followed by fetal magnetic resonance imaging on the 36th week. Imaging revealed complex developmental anomalies, which led to the

Legacies and Relationships: Diverse Social Networks and BRCA1/2 Risk Management Decisions and Actions.

In families with hereditary breast/ovarian cancer, complex disease histories challenge established patterns of family communication and influence decision-making for clinical surveillance, genetic testing, and risk management. An interdisciplinary team examined longitudinal interview data from women with identified BRCA1/2 mutations to assess interactions within family and social networks about risk information communication and management. We used interpretive description to identify motivation, content, and derived benefit of these interactions. Participants discussed risk information and ma

Animal models of efficacy to accelerate drug discovery in malaria.

The emergence of resistance to artemisinins and the renewed efforts to eradicate malaria demand the urgent development of new drugs. In this endeavour, the evaluation of efficacy in animal models is often a go/no go decision assay in drug discovery. This important role relies on the capability of animal models to assess the disposition, toxicology and efficacy of drugs in a single test. Although the relative merits of each efficacy model of malaria as human surrogate have been extensively discussed, there are no critical analyses on the use of such models in current drug discovery. In this art

Structured decision-making: using personalized medicine to improve the value of cancer care.

Cancer care is often inconsistently delivered with inadequate incorporation of patient values and objective evidence into decision-making. Utilization of time limited trials of care with predefined decision points that are based on iteratively updated best evidence, tools that inform providers about a patient's experience and values, and known information about a patient's disease will allow superior matched care to be delivered. Personalized medicine does not merely refer to the incorporation of genetic information into clinical care, it involves utilization of the wide array of data points r

Challenges for healthcare providers in treating women with psychiatric disorders during pregnancy.

There continues to be a stigma surrounding mental illness, which includes women with psychiatric illnesses who become pregnant. In addition, both women and their health care providers often have an increased risk perception surrounding the safety of psychotropic drug use in pregnancy, resulting in physicians hesitant to prescribe and women afraid to take. Consequently, this creates many challenges and barriers when it comes to treating women pharmacologically during pregnancy and lactation. The treatment of mental illness in the perinatal period is an important clinical decision, as it involves

A Proposed Parameter-Based Planning Framework for Inferior Alveolar Nerve Repositioning.

Background: Rehabilitation of the severely atrophic posterior mandible remains challenging when the residual bone height above the inferior alveolar nerve (IAN) is insufficient for conventional implant placement. In such cases, IAN lateralization or transposition may facilitate placement of longer implants and support fixed prosthetic rehabilitation. However, these procedures require careful case selection because of their technical complexity and the risk of neurosensory complications. Objective: This narrative review presents a literature-informed, expert-derived, parameter-based clinical de

Dynamic Prediction of Clinical Endpoints in Survival Analysis With Machine and Deep Learning: A Systematic Review and Regulatory Perspective.

Accurate prediction of time-to-event outcomes is essential for clinical decision-making and model-informed drug development (MIDD). Conventional survival models may inadequately represent complex longitudinal patient trajectories, time-dependent treatments, and evolving biomarker or pharmacokinetic/pharmacodynamic (PK/PD) profiles. Recent advances in machine learning (ML) and deep learning (DL) have enabled dynamic survival models that update individualized risk predictions as new longitudinal information becomes available. This report systematically reviews dynamic survival modeling framework

Beyond the bone flap: integrating biomarkers, imaging, and emerging technologies in the surgical continuum of acute subdural hematoma (ASDH).

Acute subdural hematoma (ASDH) is a life-threatening neurosurgical emergency associated with high mortality and long-term functional disability. Despite advances in surgical techniques, outcomes remain variable, highlighting the need for a more integrated management approach. Traditional decision-making has focused primarily on choosing between craniotomy and decompressive craniectomy, without fully incorporating postoperative recovery and rehabilitation. A narrative synthesis of literature published between 2010 and 2025 was conducted using PubMed, Embase, and Scopus databases. Studies were i

Managing Communication Challenges in Recommending Aids and Adaptations to Activities of Daily Living in Physiotherapy and Occupational Therapy Assessment Consultations: A Study in a Palliative Care Setting.

Often, occupational and physiotherapeutic assessment consultations involve patients discussing their problems in activities of daily living (ADL) and therapists trying to find suitable solutions. These conversations can present challenges since patients frequently reject therapists' recommendations, which can jeopardize shared decision-making, bring interactional and relational tensions, and can mean patients can lose out on potential benefit. This study, in a palliative care setting, is aimed at improving our understanding of how therapists handle these communicative challenges. Based on audi

Toward causal artificial intelligence for biologic treatment response in rheumatoid arthritis: current evidence and future directions.

Rheumatoid arthritis (RA) is a heterogeneous autoimmune disease with substantial variation in response to biologic therapy. Artificial intelligence (AI) is increasingly used to predict treatment response, but most existing models are association-based and offer limited interpretability for clinical decision-making. To critically review the role of causal artificial intelligence in predicting and explaining biologic treatment response in RA and assess its potential to improve interpretability and individualized treatment selection. A narrative review with an integrative approach was conducted,

Machine learning or traditional statistical methods for predictive modelling in perioperative medicine: A narrative review.

Prediction of outcomes in perioperative medicine is key to decision-making and various prediction models have been created to help quantify and communicate those risks to both patients and clinicians. Increasingly, machine learning (ML) is being favoured over more traditional techniques to improve prediction of outcomes, however, the studies are of varying quality. It is also not known whether any increase in predictive performance using ML algorithms transpires into a clinically meaningful benefit. This coupled with the difficulty in interrogating ML algorithms is a potential cause of concern
