Neurobehavioural deficits after severe traumatic brain injury (TBI).

OBJECTIVES: Debilitating neurobehavioural sequalae often complicate traumatic brain injury (TBI). Cognitive deficits, particularly of attention, memory, information-processing speed and problems in self-perception, are very common following severe TBI. METHOD: The Neurobehavioural Rating Scale (NRS) is a multi-dimensional clinical-based assessment instrument designed and validated to measure neurobehavioural disturbances following TBI. This study examined 41 patients who were admitted to the intensive care unit of the Department of General Neurosurgery at Cologne University Hospital after sev

Use of cognitive behavior therapy for functional hypothalamic amenorrhea.

Behaviors that chronically activate the hypothalamic-pituitary-adrenal (HPA) axis and/or suppress the hypothalamic-pituitary-thyroidal (HPT) axis disrupt the hypothalamic-pituitary-gonadal axis in women and men. Individuals with functional hypothalamic hypogonadism typically engage in a combination of behaviors that concomitantly heighten psychogenic stress and increase energy demand. Although it is not widely recognized clinically, functional forms of hypothalamic hypogonadism are more than an isolated disruption of gonadotropin-releasing hormone (GnRH) drive and reproductive compromise. Plasma and cerebrospinal fluid alpha1-antichymotrypsin levels in Alzheimer's disease: correlation with cognitive impairment.

alpha-1-Antichymotrypsin (ACT) is present in neuritic plaques in which it participates in the inflammatory cascade of Alzheimer's disease (AD). Reports of blood ACT levels in AD, and its usefulness as a disease biomarker, have been conflicting. In an effort to clarify this, we measured plasma ACT levels in 516 white subjects including 359 subjects with probable or possible AD, 44 subjects with other late-life dementias, and 113 nondemented people. Subjects with systemic inflammatory diseases or who were taking antiinflammatory medications were excluded. All patients underwent extensive medical

Seasonal variations of household costs of illness in Burkina Faso.

This paper assesses the seasonal variations of the time and financial costs of illness for rural households in Burkina Faso. It is based on a multiple round survey of 566 households, which included a time allocation study. The economic parameters of households which influence health seeking behavior changed substantially between the dry and rainy seasons: revenues fell in the rainy season and were exceeded by expenditures. Household production was at its peak in the rainy season resulting in significantly higher opportunity costs of time. At the same time illness perception changed: in the rai

Neuropathological correlates of memory dysfunction in the Wernicke-Korsakoff syndrome.

The Wernicke-Korsakoff syndrome is a neuropathological term which encompasses two clinical syndromes in thiamine-deficient alcoholics, Wernicke's encephalopathy and Korsakoff's psychosis. Wernicke's encephalopathy is characterised by eye and gait disorders and mental confusion, and can lead to the profound and permanent amnesia known as Korsakoff's psychosis. Despite this specific difference, both conditions appear to have identical neuropathology with haemorrhages and other lesions around the ventricular system. The memory deficit has been attributed to a number of brain lesions, including a

The genesis of alcoholic brain tissue injury.

1. Acetaldehyde has been implicated in the pathogenesis of alcohol-related liver damage by two mechanisms. Adduct formation with many tissue constituents, especially proteins, makes them immunologically foreign or reduces enzyme activity and formation of cytotoxic free radicals from acetaldehyde metabolism. Adduct formation damage to microtubule associated proteins and to hepatocyte membranes impedes protein movement into, out of and around the cell. 2. Evidence that these mechanisms also have a role in alcoholic brain damage includes raised blood acetaldehyde in alcoholics, especially in thos

Technostress and learning burnout among nursing students: The protective roles of academic self-efficacy and psychological resilience - A cross-sectional study.

BACKGROUND: The rapid integration of digital modalities within contemporary nursing curricula has exposed student cohorts to unique psychological strains, prominently manifested as technostress. While technostress is known to accelerate learning burnout, the complex boundary conditions and moderating mechanisms through which internal cognitive assets-specifically academic self-efficacy-alter or shape this negative relationship remain insufficiently quantified. OBJECTIVE: This study established a hierarchical predictive framework and moderation model to quantify the impact of technostress on l

Intranasal HSV‑1 infection drives region‑specific interferon‑dominant microglial remodeling.

Herpes simplex virus type 1 (HSV-1) is a neurotropic pathogen capable of invading the central nervous system (CNS) and is associated with chronic neuroinflammation, cognitive impairment, and neurodegenerative disease. While microglia orchestrate the initial immune response to HSV‑1, the molecular mechanisms that regulate their sustained neuroinflammatory activity in vivo remain poorly understood. To define the transcriptional and epigenetic mechanisms that shape microglial responses during acute HSV-1 infection in vivo, we have, for the first time, integrated single‑nucleus RNA sequencing, chr

Comparison of patient preference for and interpretability of video-based pain-expression scenarios and the numerical rating scale for postoperative pain assessment: a prospective observational study.

BACKGROUND: Postoperative pain assessment relies primarily on numeric self-report tools, such as the numerical rating scale (NRS); these tools require abstract numerical cognition and may be challenging for some patients in the immediate postoperative period. Thus, in this prospective observational study, we aimed to evaluate a novel video-based pain-expression method in postoperative adults, with the primary objective of assessing patient preference for the video-based pain-expression method compared with that for the NRS. METHODS: This prospective single-center observational study was condu

Right inferior frontal contributions to visual spatial suppression and visuomotor performance.

BACKGROUND: Elite racket sports demand visuomotor responses at the millisecond level, requiring rapid prioritization of task-relevant information while suppressing competing or redundant signals. Such perceptual filtering is reflected in spatial suppression, which declines with aging and in clinical cognitive disorders. This study investigated whether visual spatial suppression in expert table tennis players (TTPs) was linked to visuomotor reaction time, and whether prefrontal-visuomotor dynamics were associated with this link. METHODS: We examined 28 expert TTPs and 31 healthy controls using

Translation and psychometric validation of the symptom catastrophizing scale in cardiac surgery patients: a cross-sectional study.

BACKGROUND: Symptom catastrophizing reflects maladaptive cognitive and emotional responses to physical symptoms and may worsen symptom burden, distress, and recovery. However, no validated Chinese instrument is available for cardiac surgery patients, limiting patient-centered assessment and early risk identification. OBJECTIVE: To translate and culturally adapt a symptom-neutral catastrophizing measure derived from the original Post-concussion Symptom Catastrophizing Scale and evaluate its psychometric properties in Chinese cardiac surgery patients. METHODS: Following Brislin's translation m

Health Information Seeking by People Navigating Masculinizing Transition Pathways in Switzerland: Communication Inequalities and Adaptive Approaches.

Gender-diverse people considering transition pathways often navigate health information environments shaped by fragmented healthcare structures, uneven provider competence, and cisnormative health communication. This qualitative study explores how trans men and non-binary/agender people pursuing masculinizing transition pathways in Switzerland identify, evaluate, and use health information while navigating communication inequalities, institutional fragmentation, and cisnormative healthcare structures. Sixteen semi-structured interviews were analyzed using reflexive thematic analysis informed b

Continuous blood pressure and autonomic activity during road-traffic noise: a multimodal time-course analysis.

Noise-induced stress is a prominent risk to well-being. Despite its relevance to cardiovascular load, blood pressure is rarely used in noise studies because conventional cuff-based measurements have low temporal resolution. Continuous non-invasive blood pressure (cNIBP) enables beat-to-beat monitoring, resolving short-lived, phase-dependent dynamics (latency, peak response, recovery) that are averaged out in intermittent protocols. This work records a detailed timeline of the stress response to realistic road-traffic noise during a cognitively demanding task, evaluating cNIBP alongside the est

Strategic modulation to avoid radiation toxicities for brain integrity (SMART-BRAIN).

BACKGROUND: Patients with medulloblastoma require craniospinal irradiation (CSI) to optimize disease control; however, cranial irradiation is associated with significant morbidity. We sought to develop a novel target volume (CTV_2340 cGy) and explore dosimetric benefits to minimize cognitive toxicity. METHODS: We conducted a literature review to confirm patterns of failure for average risk medulloblastoma to develop the CTV_2340 definition approach. Five patients (ages 4-16 years) were identified and proton and photon plans were generated for the new CTV_2340. Planning target goals included V

Dementia incidence trends in rural Spain: Ten-year cohort findings from NEDICES cohort.

PURPOSE: To estimate dementia incidence in the rural population enrolled in the Neurological Diseases in Central Spain (NEDICES) cohort and to evaluate the impact of ascertainment strategies on temporal trend estimates. METHODS: Residents aged ≥65 years were examined at baseline (1994-1995), in 1997-1998, and in 2004-2005. Incident dementia was identified through clinical reassessment, medical record review, and, for decedents between waves, via linkage to the Spanish National Death Registry. Person-time accrued from baseline to dementia, death, or censoring; incidence rates (per 1000 person-

Connection density affects the behavior of functional brain network metrics.

BACKGROUND AND OBJECTIVE: Many neuropsychiatric disorders are associated with alterations in functional brain network (FBN) connectivity. Functional network metrics reveal alterations in FBNs by quantifying changes in network segregation and integration. In general, calculating functional network metrics requires a reasonable thresholding or binarization of the network, which is a challenging task. We observe that the same functional network metric yields conflicting conclusions across different studies: some report higher values in patient groups compared to healthy groups, while others repor
