When Overdose Strikes: Successful Use of Hemodialysis in Valproic Acid Toxicity.

Valproic acid (VPA) toxicity is a serious and potentially life-threatening condition that can arise from overdose or impaired drug metabolism. While mild cases may present with central nervous system depression, severe toxicity can lead to coma, multi-organ failure, and death. This case report discusses a 21-year-old male with a history of schizoaffective disorder, polysubstance abuse, and suicidal ideations who presented with altered mental status after a suspected overdose of VPA. His serum valproate level was found to be 879 µg/mL, significantly exceeding the therapeutic range. Despite init

Predictors of intensive care unit admission rates in patients with acute cholangitis.

Acute cholangitis (AC) is a heterogeneous disease with considerable variation in clinical presentation and high medical costs. Although the overall mortality rate is decreasing. However, the mortality in severe AC ranged from 10%-30% due to sepsis, multi-organ failure, and systemic inflammatory response syndrome. Assessing the predictors of poor outcomes, including mortality, intensive care unit (ICU) admission, and hospital stay, is vital for early and effective intervention. We assessed the predictors of mortality and ICU admission in patients with AC. We conducted a literature search in Pub

Superficial temporal artery-middle cerebral artery bypass for progressive cerebral infarction in invasive aspergillosis-induced total occlusion of internal carotid artery: A rare case and literature review.

Central nervous system (CNS) aspergillosis is a life-threatening infection primarily affecting immunocompromised patients and may lead to severe cerebral infarction through vascular invasion. However, there is limited data on the treatment options for aspergillosis-induced cerebral infarction especially surgical treatments such as superficial temporal artery (STA)-middle cerebral artery (MCA) bypass surgery. Herein, we present a case of cerebral infarction in a 59-year-old male with progressive right eye ptosis. Specifically, he had ipsilateral MCA stenosis originating from paranasal sinusitis

Prognostic variables predict clinical outcome after decompressive craniectomy: A single institute experience; A retrospective study.

Decompressive craniectomy (DC) is a well-established neurosurgical intervention in patients with high intracranial pressure who fail to respond to medical treatment. Data on predictive factors for functional outcomes in patients with DC who have malignant middle cerebral artery (MCA) infarction as opposed to intracranial hemorrhage (ICH) are scarce. Eighty-four patients who underwent DC treatment for ICH and malignant MCA infarction were examined. All patients underwent surgery in the Bahrain Salmaniya Medical Complex Neurosurgery Unit between January 2017 and June 2021. To determine whether a

Neurological Complications of Cannabinoids.

Cannabinoid use, particularly for recreational purposes, is increasing exponentially across all age groups, especially in younger populations, due to its perceived low risk and legalization. While cannabinoids may be largely considered as safe, there is mounting evidence of increased risk of systemic and neurological complications through their interaction with the poorly understood endocannabinoid receptor network within the central nervous system and other organ systems. Acute cannabinoid exposure can cause neuropsychiatric symptoms in addition to altering cerebral blood flow, leading to cer

Rare Delayed Ifosfamide Encephalopathy: A Case Report of Chemotherapeutic Neurotoxicity.

INTRODUCTION: Neurotoxicity is a well-documented side effect of ifosfamide chemotherapy. The presentation includes hallucinations, seizures, disorientation, coma, and death. Treatment with methylene blue can shorten the duration and severity of symptoms. Ifosfamide neurotoxicity almost always happens during or shortly after drug infusion and so is usually immediately recognized. Here, we describe a case of ifosfamide neurotoxicity with onset 14 days after treatment started. CASE PRESENTATION: A 25-year-old woman with round cell sarcoma of the jaw presented to the emergency department with 2 d

Prognostic Factors of Hospital Mortality After Near Hanging: A Retrospective two-Center French Study.

Introduction: suicide is a global public health issue, with over 800 000 people taking their own lives every year. However, most suicide attempts do not result in death. Hanging is the most common method used in France, often leading to post-hanging coma (PHC). The prognosis for patients admitted in intensive care unit (ICU) following PHC is poor, yet predictive criteria of mortality have been poorly evaluated. Methods: we retrospectively collected prehospital and in-hospital data from 65 patients hospitalized in 2 French ICU for PHC, between first March 2010 and first August 2023, and compare

Exertional Heatstroke Encephalopathy With Chronic Neurological Deficit.

Heat exhaustion progresses to heat stroke and then on to heatstroke encephalopathy, a serious illness. Extreme hyperthermia (over 40.5 °C), central nervous system failure, multiorgan dysfunction, and hypovolemic shock are the hallmarks of the clinical presentation of heatstroke. A 27-year-old male was presented to the causality with loss of consciousness followed by altered sensorium while working outdoors (outside temperature was 40°C), low blood pressure (70/50 mmHg), hyperthermia (41°C), tachycardia, focal seizures (1 episode), hypovolemic shock. At presentation, the Glasgow Coma Scale was

Vasogenic Edema Covering the Brain Surface in a Case of Severe Meningoencephalitis Due to Varicella-Zoster Virus Infection.

Meningoencephalitis caused by varicella-zoster virus (VZV) is a serious condition requiring prompt antiviral treatments, but magnetic resonance imaging (MRI) findings are often normal, limiting early diagnostic utility. We report a case of severe VZV-associated meningoencephalitis characterized by diffuse T2 hyperintense lesions covering the brain surface on MRI, presumed to be vasogenic edema. An immunocompetent 78-year-old Japanese woman presented with a disturbance of consciousness preceded by seven days of headache. On admission, she was in a semi-coma with intermittent convulsive seizures

Clinical features, pathogenesis, pathology, neuroimaging, clinical course and outcome of measles inclusion-body encephalitis: a systematic review of published case reports and case series.

Measles inclusion-body encephalitis (MIBE) is rare, with insights largely from case studies. We systematically analyzed subacute Sclerosing Panencephalitis (SSPE) cases in immunocompromised patients, identifying distinctive clinical and neuroimaging features. These findings could facilitate MIBE diagnosis without the need for brain biopsies. Our systematic review on MIBE and HIV-related SSPE adhered to PRISMA guidelines and was registered with PROSPERO. We searched multiple databases and followed a detailed inclusion process with independent reviews and quality assessment. Data on patient demo

Liquorice Toxicity: A Comprehensive Narrative Review.

BACKGROUND: Renowned since ancient times for its medical properties, liquorice is nowadays mainly used for flavoring candies or soft drinks. Continuous intake of large amounts of liquorice is a widely known cause of pseudo-hyperaldosteronism leading to hypertension and hypokalemia. These manifestations are usually mild, although in some cases may generate life-threatening complications, i.e., arrhythmias, muscle paralysis, rhabdomyolysis, and coma. In addition, liquorice has an important estrogenic-like activity. METHODS: We summarized the current knowledge about liquorice and reviewed 104 ca
