The hidden cost of catheter related blood stream infections in patients on parenteral nutrition.

BACKGROUND & AIMS: Parenteral nutrition (PN) is a valuable and life-saving treatment for patients with intestinal failure. While its use is increasing, it has been demonstrated to be a risk factor for intravenous catheter-related blood stream infection (CRBSI) - a significant, serious and potentially fatal complication of PN use. CRBSI can have serious secondary consequences for patients, though, there is a paucity of literature describing these. The aim of this study is to audit the incidence of, and evaluate the consequences of, complications associated with CRBSI. METHODS: Medical records

The interRAI Suite of Mental Health Assessment Instruments: An Integrated System for the Continuum of Care.

The lives of persons living with mental illness are affected by psychological, biological, social, economic, and environmental factors over the life course. It is therefore unlikely that simple preventive strategies, clinical treatments, therapeutic interventions, or policy options will succeed as singular solutions for the challenges of mental illness. Persons living with mental illness receive services and supports in multiple settings across the health care continuum that are often fragmented, uncoordinated, and inadequately responsive. Appropriate assessment is an important tool that healt

Attending to the Emotional Well-Being of the Health Care Workforce in a New York City Health System During the COVID-19 Pandemic.

The COVID-19 pandemic has placed an enormous strain on health care workers, and its potential impact has implications for the physical and emotional well-being of the workforce. As hospital systems run well over capacity, facing possible shortages of critical care medical resources and personal protective equipment as well as clinician deaths, the psychological stressors necessitate a strong well-being support model for staff. At the Mount Sinai Health System (MSHS) in New York City, health care workers have been heroically providing frontline care to COVID-19 patients while facing their own a

A flexible format LAMP assay for rapid detection of Ebola virus.

BACKGROUND: The unprecedented 2013/16 outbreak of Zaire ebolavirus (Ebola virus) in West Africa has highighted the need for rapid, high-throughput and POC diagnostic assays to enable timely detection and appropriate triaging of Ebola Virus Disease (EVD) patients. Ebola virus is highly infectious and prompt diagnosis and triage is crucial in preventing further spread within community and healthcare settings. Moreover, due to the ecology of Ebola virus it is important that newly developed diagnostic assays are suitable for use in both the healthcare environment and low resource rural locations.

Lay Field-worker-Led School Health Program for Primary Schools in Low- and Middle-Income Countries.

BACKGROUND AND OBJECTIVES: School health programs are frequently attempted in low- and/or middle-income countries; however, programmatic scope and reach is limited by human resource constraints. We sought to determine if trained community members could implement a school health program that improved outcomes in rural primary schools in India. METHODS: This was a mixed-methods, stepped-wedge, cluster-controlled study of schools pragmatically assigned to receive a multicomponent, comprehensive school health program delivered by lay field-workers. RESULTS: All students in 22 primary schools (9

High‑magnesium waters and soils: Emerging environmental and food security constraints.

Food insecurity and declining availability of freshwater and new productive land in water-scarce areas and countries necessitate effective use of marginal-quality waters and underperforming soils. High‑magnesium waters and soils are emerging examples of water quality deterioration and land degradation leading to environmental and food security constraints in several irrigation schemes. A ratio of magnesium-to-calcium > 1 in irrigation waters and an exchangeable magnesium percentage > 25% in soils are considered high enough to result in soil degradation and impact crop yields negatively. These

Agents of change: The role of healthcare workers in the prevention of nosocomial and occupational tuberculosis.

Healthcare workers (HCWs) play a central role in global tuberculosis (TB) elimination efforts but their contributions are undermined by occupational TB. HCWs have higher rates of latent and active TB than the general population due to persistent occupational TB exposure, particularly in settings where there is a high prevalence of undiagnosed TB in healthcare facilities and TB infection control (TB-IC) programmes are absent or poorly implemented. Occupational health programmes in high TB burden settings are often weak or non-existent and thus data that record the extent of the increased risk o

Talking about human papillomavirus and cancer: development of consultation guides through lay and professional stakeholder coproduction using qualitative, quantitative and secondary data.

BACKGROUND: High-risk human papillomaviruses (HPVs) cause all cervical cancer and the majority of vulvar, vaginal, anal, penile and oropharyngeal cancers. Although HPV is the most common sexually transmitted infection, public awareness of this is poor. In addition, many clinicians lack adequate knowledge or confidence to discuss sexual transmission and related sensitive issues. Complex science needs to be communicated in a clear, digestible, honest and salient way. Therefore, the aim of this study was to coproduce with patients who have cancer appropriate resources to guide these highly sensit

Breast Imaging Second Opinions Impact Surgical Management.

BACKGROUND: Breast surgeons often see women for second opinions for abnormalities found on breast imaging. For second opinions, these images are submitted for review and interpretation by dedicated breast imagers. This study evaluated the conformity of results among interpretation of imaging submitted from outside hospitals both from tertiary care centers, as well as community programs, in an attempt to evaluate the utility of this practice for the sake of clinical management and resource utilization. METHODS: A retrospective chart review was conducted on all breast patients that submitted ou

Deriving and validating a risk estimation tool for screening asymptomatic chlamydia and gonorrhea.

BACKGROUND: There has been considerable interest in the development of innovative service delivery modules for prioritizing resources in sexual health delivery in response to dwindling fiscal resources and rising infection rates. METHODS: This study aims to derive and validate a risk scoring algorithm to accurately identify asymptomatic patients at increased risk for chlamydia and/or gonorrhea infection. We examined the electronic records of patient visits at sexual health clinics in Vancouver, Canada. We derived risk scores from regression coefficients of multivariable logistic regression mo

A multinational, multidisciplinary consensus for the diagnosis and management of spinal cord compression among patients with mucopolysaccharidosis VI.

Cervical cord compression is a sequela of mucopolysaccharidosis VI, a rare lysosomal storage disorder, and has devastating consequences. An international panel of orthopedic surgeons, neurosurgeons, anesthesiologists, neuroradiologists, metabolic pediatricians, and geneticists pooled their clinical expertise to codify recommendations for diagnosing, monitoring, and managing cervical cord compression; for surgical intervention criteria; and for best airway management practices during imaging or anesthesia. The recommendations offer ideal best practices but also attempt to recognize the worldwid

Transcriptome landscape of the human placenta.

BACKGROUND: The placenta is a key component in understanding the physiological processes involved in pregnancy. Characterizing genes critical for placental function can serve as a basis for identifying mechanisms underlying both normal and pathologic pregnancies. Detailing the placental tissue transcriptome could provide a valuable resource for genomic studies related to placental disease. RESULTS: We have conducted a deep RNA sequencing (RNA-Seq) study on three tissue components (amnion, chorion, and decidua) of 5 human placentas from normal term pregnancies. We compared the placental RNA-Se

Coordination between formal providers and informal caregivers.

BACKGROUND: The increased "outsourcing" of care-related tasks to patients and their informal caregivers is part of a broader trend in service industries toward engaging customers as "coproducers" of service outcomes. As both quasi-patients and quasi-providers, caregivers may play a critical role in successful coproduction, but they require coordination with care providers to play this role effectively. When tasks are highly interdependent, uncertain, and time constrained, as they often are in health care, relational forms of coordination are expected to be most effective. PURPOSES: This study

The poison center role in biological and chemical terrorism.

Nuclear, biological and chemical (NBC) terrorism countermeasures are a major priority with municipalities, healthcare providers, and the federal government. Significant resources are being invested to enhance civilian domestic preparedness by conducting education at every response level in anticipation of a NBC terroristic incident. The key to a successful response, in addition to education, is integration of efforts as well as thorough communication and understanding the role that each agency would play in an actual or impending NBC incident. In anticipation of a NBC event, a regional counter

Nutrition and reproduction.

Adequate nutrition is essential for normal reproductive function in man. Menarche occurs at a critical level of 'fatness' and it appears that the hypothalamic events leading to pubertal development and the achievement of reproductive competence may be triggered by metabolic/endocrinological changes due to an increase in fat. The attainment and maintenance of ovulatory cycles demands a minimum degree of body fat (about 22%). Undernutrition results in weight loss and a reduction in fat tissue. This alteration in body composition in turn precipitates the hypothalamic changes that cause impaired g

Are there more healthcare workers in our community? A phenomenological exploration of factors influencing local hospital and primary care worker re-engagement in rural Australia.

INTRODUCTION: The local healthcare workforce is critical to rural communities, which face more complex health conditions than urban counterparts. Workforce shortages exacerbate strain on rural healthcare systems and create service access barriers. To date, retention and recruitment strategies have struggled to resolve staffing issues. This study investigated an overlooked local resource: hospital and primary healthcare workers previously employed or working at less than full-time capacity in rural Queensland, Australia. We explored barriers and enablers to increasing their working hours or ret
