Endotypic traits and lung function in patients with obstructive sleep apnea comorbid with obesity hypoventilation syndrome.

PURPOSE: Obesity hypoventilation syndrome (OHS) frequently coexists with obstructive sleep apnea (OSA) and is linked to lung function impairment and poor outcomes. However, its underlying pathology remains unclear. This study compares lung function and endotypic traits in obese OSA patients with and without OHS. METHODS: We prospectively collected polysomnographic data from 186 obese patients with OSA who underwent lung function and blood gas tests. Eighteen patients were identified as having OHS, as defined by body mass index ≥ 30 kg/m² with daytime hypercapnia (PaCO₂ ≥ 45 mmHg). Endotypic tr

Navigating autism and mental health: A qualitative study of therapists' experiences of psychotherapy for comorbidities in autistic adults.

In this study, we explored the psychotherapeutic process for autistic adult clients without intellectual impairment with comorbid psychological disorders, drawing on the perspectives of highly specialized psychotherapists with extensive clinical experience in autism-focused practice. Within the context of the ongoing shift from deficit-based interventions toward neurodiversity-affirming approaches, we investigated how therapists conceptualize comorbidities in autism and how this conceptualization informs their therapeutic work. We employed a qualitative research design and collected data throu

Herbal Medicine and Metabolic Syndrome: Phytochemicals Targeting the Molecular Causes of Insulin Resistance and Related Comorbidities.

Metabolic Syndrome (MetS) is a global health concern characterized by a cluster of interrelated metabolic abnormalities, including insulin resistance, central obesity, dyslipidemia, hypertension, and chronic low-grade inflammation. These factors significantly increase the risk of developing Type 2 Diabetes Mellitus (T2DM), Cardiovascular Disease (CVD), and other complications. In recent years, plant-derived natural products and herbal medicines have garnered increasing attention for their potential to modulate the underlying mechanisms of Metabolic Syndrome (MetS) safely and cost-effectively.

Immunological Mechanisms of the Gut-brain-cardiovascular Axis in Coronary Heart Disease with Comorbid Anxiety and Depression.

Coronary heart disease (CHD) is frequently accompanied by anxiety and depression, conditions that markedly worsen cardiovascular outcomes. Increasing evidence indicates that immune dysregulation, driven by gut microbiota alterations, plays a central role in linking psychological disorders with cardiovascular pathology through the gut-brain-cardiovascular axis. This narrative review systematically summarizes recent advances in the immunological mechanisms underlying CHD complicated by anxiety and depression. We focus on gut microbiota-immune interactions, inflammatory signaling pathways, immune

Treatments for COVID-19 and acute respiratory infections are associated with gender and comorbidities in an Italian online survey.

Ministries of health need to know and understand factors affecting medical treatment choices of population subgroups, to tailor official recommendations. This study aimed to identify, quantify and compare treatments used by patients with an acute respiratory infection with and without COVID-19 during the first year of the pandemic by accounting for important factors potentially affecting treatment outcomes. With an online questionnaire, retrospective information on treatments used during events of acute respiratory infections were gathered in Italy. Logistic regression was used to detect signi

Ultrafine particles and stroke hospitalizations: New insights of exposure-outcome threshold, daily peaks, and comorbidities.

Stroke is a leading cause of death and morbidity worldwide. Prior research has found that exposure to air pollution increases the risk of stroke, but few studies have focused on the ultrafine particles (UFPs)-stroke relationship. We investigated the association between UFPs and stroke hospitalizations and disparities by stroke subtypes, demographics, and seasonality. In this time-stratified case-crossover study, stroke hospitalizations (mainly ischemic stroke and hemorrhagic stroke) were identified using principal diagnosis from New York State hospital discharge data. UFPs and covariates, incl

Prevalence and representation of comorbidities and multimorbidity in randomised controlled trials in sepsis or septic shock: a systematic review.

BACKGROUND: Multimorbidity is prevalent among critically ill patients with sepsis yet remains underreported in critical care randomised controlled trials (RCTs). Inadequate reporting limits the generalisability of findings and the ability to understand whether chronic disease burden modifies treatment effects. We aimed to systematically map and evaluate how comorbidities and multimorbidity are represented, reported, and analysed in RCTs involving Intensive Care (ICU) patients with sepsis or septic shock. METHODS: We searched MEDLINE, Embase, CENTRAL, LILACS, Web of Science, medRxiv and clinic

Embitterment symptoms and comorbid psychopathology in an anxiety outpatient clinic: frequency, clinical distribution, and psychosocial correlates.

OBJECTIVE: Posttraumatic embitterment disorder (PTED) has been proposed as a clinical construct characterized by persistent feelings of injustice, resentment, helpless anger, and intrusive embitterment-related cognitions following negative life experiences perceived as unjust, humiliating, or irreversible. The present study aimed to determine the frequency and clinical distribution of clinically relevant embitterment symptoms and PTED-compatible presentations. METHODS: This cross-sectional study included 344 participants, consisting of 315 consecutive patients attending an anxiety outpatient

Post-COVID Syndrome in Patients With Comorbid Hypertension or Diabetes: A Narrative Review of Long-Term Outcomes.

Post-COVID syndrome (PCS), or long COVID, refers to a cluster of enduring symptoms that extend beyond the acute phase of the initial SARS-CoV-2 infection. Acute infection predominantly impacts the respiratory tract, but there is growing evidence for the multisystem involvement, such as cardiovascular, metabolic, and neurological, to be responsible for the prolonged presentation in PCS. Underlying cardiometabolic vulnerability may contribute to a high degree of susceptibility in patients with comorbidities like hypertension (HTN) and diabetes mellitus (DM). This narrative review summarizes curr

Retained Epidural Catheter Fragment Requiring Surgical Removal in a Patient With Multiple Comorbidities: A Case Report.

Epidural analgesia remains an important technique in many clinical settings; although generally considered safe, it is not free of complications. We present the case of a 69-year-old woman with poorly controlled diabetes mellitus and hypertension who, during an epidural block for a left open hepatectomy, experienced the fracture and retention of a 10-cm epidural catheter fragment within the spinal canal at the T12-L1 level. Despite the patient being asymptomatic, surgical removal was required due to the fragment's location and length and the potential risk of long-term systemic complications a

Atypical cerebral fat embolism syndrome in an elderly patient with comorbidities following fracture: A case report.

RATIONALE: Cerebral fat embolism (CFE) is a rare but serious complication of fractures, most commonly affecting young males. However, atypical cases in elderly patients with underlying comorbidities are easily misdiagnosed. PATIENT CONCERNS: We report a case of a 61-year-old male patient with diabetes and hypertension who developed impaired consciousness and fever on the 2nd day after sustaining multiple fractures in a traffic accident. His condition deteriorated after receiving local treatment. DIAGNOSES: CFE was diagnosed based on the characteristic "starry sky sign" on brain magnetic reso

Environmental enrichment suppresses colorectal cancer progression with comorbid depression by modulating the immunosuppressive tumor microenvironment.

BACKGROUND: Depression compromises treatment efficacy across multiple cancer types, including colorectal cancer (CRC), largely through mechanisms that suppress antitumor immunity. Conversely, depression alleviation has been associated with improved cancer outcomes. Environmental enrichment (EE), a non-pharmacological lifestyle intervention, has been shown to ameliorate depressive-like behaviors; however, its ability to inhibit tumor growth in depression-comorbid CRC and the underlying mechanisms remain poorly defined. Here, we investigated the effects of EE on chronic unpredictable mild stress
