India's participatory approach to developing a national AI strategy for health: lessons for collective learning.

National AI health strategies are proliferating, yet development methodologies remain largely undocumented, limiting collective learning. India's Strategy for AI in Healthcare (SAHI) was developed through a three-phase participatory process spanning situational analysis, regional consultations across four geographic zones, and structured synthesis engaging over 300 stakeholders. Documenting that process architecture for examination and adaptation by other countries is the methodological contribution of this paper. Consultation surfaced five system-level constraints recurring across emerging AI

Gaze direction as framing for interaction in sign language-interpreted routine postnatal visits.

This study aims to gain insight into participants' use of gaze in sign language-interpreted consultations in health centres for babies and toddlers in the Norwegian context. The study investigates how gaze direction and eye contact frame the interaction between participants comprising mothers who are deaf and health personnel who are hearing. The empirical material is based on five video recordings of interpreted health consultations, and multimodal interaction analysis is used to examine the participants' use of gaze in these interactions. The analysis demonstrates differences in eye contact

When Premedication Fails: Anaphylaxis to Iodinated Contrast Media in the Cardiac Catheterization Lab Despite Premedication.

BACKGROUND: IgE-mediated anaphylaxis to iodinated contrast media (ICM) is rare but potentially life threatening. Unlike non-IgE-mediated reactions, cases are often resistant to corticosteroid and antihistamine premedication. CASE SUMMARY: A patient with a history of anaphylaxis to Visipaque contrast was referred for invasive coronary angiography. Despite premedication, she developed hypotensive anaphylaxis requiring immediate case cessation and emergent treatment with epinephrine. Subsequent allergy evaluation with skin testing identified an alternative contrast agent, allowing successful per

Aboriginal Community Controlled Organisations Leading the Way in Child Health Research.

Historically, Aboriginal and Torres Strait Islander research in Australia has adhered to Western research paradigms and contributed to the adverse impacts of colonisation. However, recent developments driven by Aboriginal and Torres Strait Islander people and scholars, and development of ethical guidelines for research, have promoted a more inclusive and collaborative research landscape. In this study, published papers and internal documents arising from a long-term partnership between Marninwarntikura Women's Resource Centre (MWRC) and the University of Sydney (USYD) from 2009 to 2023 were an

Durvalumab‑induced type 1 diabetes mellitus in lung adenocarcinoma: A case report and literature review.

Immune checkpoint inhibitor-induced type 1 diabetes mellitus (ICI-T1DM) is a rare adverse reaction associated with durvalumab. Among the adverse reactions to durvalumab, the incidence of new-onset diabetes is relatively rare, occurring in ~0.2% of cases. The present study reports the case of a 62-year-old woman who developed ICI-T1DM following two cycles of durvalumab, presenting with thirst, polydipsia and polyuria. Laboratory examinations (glycated hemoglobin and glutamic acid decarboxylase antibody), along with consultations from an endocrinologist, led to the patient being diagnosed with I

[Case report of a cystic lung disease: From a rarity to the discovery of an unknown genetic variant].

INTRODUCTION: Cystic lung diseases are rare, with numerous differential diagnoses. Iconographic discovery consequently necessitates medical examinations in view of proposing an etiological orientation. CASE REPORT: A 57-year-old woman consulted in pulmonology following fortuitous detection of a cystic lung disease on an abdominal CT scan. Complementary medical examinations did not allow orientation towards a particular diagnosis. During a follow-up consultation, the patient informed her pulmonologist of the recent detection of a monoallelic variant of a FAT4 gene in one of her daughters, who

Development and Evaluation of a Health Behavior Change Clinic in Primary Care: An Interdisciplinary Partnership.

Providing effective healthy behavior change interventions within primary care presents numerous challenges. Obesity, tobacco use, and sedentary lifestyle negatively impact the health quality of numerous medical patients, particularly in underserved patient populations with limited resources. Primary Care Behavioral Health (PCBH) models, which incorporate a Behavioral Health Consultant (BHC), can offer point-of-contact psychological consultation, treatment, and also provide opportunities for interdisciplinary psychologist-physician clinical partnerships to pair a BHC's health behavior change ex

IoT-based disease prediction using machine learning.

COVID-19 disrupted lives and livelihoods and affected various sectors of the economy. One such domain was the already overburdened healthcare sector, which faced fresh challenges as the number of patients rose exponentially and became difficult to deal with. In such a scenario, telemedicine, teleconsultation, and virtual consultation became increasingly common to comply with social distancing norms. To overcome this pressing need of increasing 'remote' consultations in the 'post-COVID' era, the Internet of Things (IoT) has the potential to play a pivotal role, and this present paper attempts t

The diagnostic dilemma in a patient with neuroleptic malignant syndrome during the COVID-19 pandemic: A significant increase in acute phase reactants.

KEY CLINICAL MESSAGE: In some patients, neuroleptic malignant syndrome is accompanied significant high levels of erythrocyte sedimentation rate (ESR), C-reactive protein (CRP). ABSTRACT: Neuroleptic malignant syndrome (NMS) is an idiosyncratic life-threatening adverse reaction and usually triggered in response to antipsychotic drugs. In addition, leukocytosis and increased muscle enzymes levels (especially creatine phosphokinase) are observed in NMS. In addition, a transient increase in different types of acute phase reactants in NMS has been mentioned. This article describes a woman treated

UK survey of colorectal surgeons on the management of acute obstetric anal sphincter injuries.

AIM: The role of colorectal surgeons in the management of acute obstetric anal sphincter injury (OASI) is an ongoing debate. Their expertise in operating in the anorectal region lends itself to assisting in OASI repair. The aim of this study was to establish the current involvement and recommended management of acute OASI by colorectal surgeons. METHOD: An online survey of consultant colorectal surgeons was sent to members of the Pelvic Floor Society to assess current involvement in acute OASI management and repair. RESULTS: Forty completed surveys were collated and analysed. Sixty-five per

Increased Prevalence of Alcohol-Related Gastrointestinal and Liver Diseases During the COVID-19 Pandemic.

BACKGROUND: Higher prevalence of alcohol-related gastrointestinal (GI) and liver diseases (ARGLDs) were anecdotally reported during the COVID-19 pandemic, but little published evidence exists. METHODS: A healthcare system audit of inpatient GI consults was performed during the pandemic's lockdown phase (3/23/2020-5/10/2020, n=558) and reopening phase (6/1/2020-7/19/2020, n=711) with comparison to those timeframes in 2019. RESULTS: Consult volume decreased by 27.7% during the lockdown, but the proportion of ARGLDs increased by 59.6% (p=0.03). This trend continued during reopening, with potent

Which patient and surgeon characteristics are associated with surgeon experience of stress during an office visit?

OBJECTIVE: To determine clinician and patient factors associated with the surgeon feelings of stress, futility, inadequacy, and frustration during an office visit. METHODS: A survey-based experiment presented clinical vignettes with randomized patient factors (such as symptom intensity, the number of prior consultations, and involvement in a legal dispute) and feeling behind schedule in order to determine which are most related to surgeon ratings of stress, futility, inadequacy, and frustration on 11-point Likert scales. RESULTS: Higher surgeon stress levels were independently associated wit

ECG Changes in a Case of Posterior Myocardial Infarction in the Presence of Right Bundle Branch Block.

A 70-year-old male with hypertension and diabetes presented to the emergency department with a 1-hour history of chest pain. Initial 12-lead ECG revealed a right bundle branch block (RBBB) and ST depression (STD) in V2-V4. The anterior STD prompted a 15-lead ECG in which there was no evidence of ST elevation (STE). With a positive troponin, cardiology was consulted and the patient was admitted as a high-risk non-ST-elevation myocardial infarction (NSTEMI). Subsequently, his chest pain returned without further ST changes, regardless the patient went for emergency coronary angiography, which fou

Genetic causes of fractures and subdural hematomas: fact versus fiction.

Genetic disorders are in the differential diagnosis when young children present with unexplained fractures or intracranial hemorrhage. For medical and legal reasons, it is imperative to make the correct diagnosis and provide clear, evidence-based explanations of how alternative diagnoses were ruled out. A genetics consultation in cases of suspected child physical abuse should synthesize the history of present illness, medical history, family history, physical examination, and radiologic and laboratory findings in consultation with other specialists. The medical geneticist highlights how these

A framework for scabies control.

Scabies is a neglected tropical disease (NTD) that causes a significant health burden, particularly in disadvantaged communities and where there is overcrowding. There is emerging evidence that ivermectin-based mass drug administration (MDA) can reduce the prevalence of scabies in some settings, but evidence remains limited, and there are no formal guidelines to inform control efforts. An informal World Health Organization (WHO) consultation was organized to find agreement on strategies for global control. The consultation resulted in a framework for scabies control and recommendations for map
