Anterior cervical decompression and fusion for Samartzis Type I Klippel-Feil syndrome complicated by cervical radiculopathy with radiographic spinal cord compression: a case report of sandwich-type interposed mobile segment.

BACKGROUND: Klippel-Feil syndrome (KFS) is a rare congenital cervical malformation caused by embryonic segmentation defects leading to vertebral fusion, which alters cervical biomechanics and predisposes to premature spinal degeneration. The "sandwich-type interposed mobile segment" refers to a solitary mobile cervical segment trapped between congenital bony fusion and iatrogenic surgical fusion. It bears concentrated mechanical stress and carries a high risk of early adjacent segment degeneration (ASD). Current epidemiological data for postoperative ASD are mostly derived from general spinal

Comparative efficacy and safety of core decompression, cell-based therapy, hyperbaric oxygen therapy, extracorporeal shock wave therapy, and combined regimens for osteonecrosis of the femoral head: a network meta-analysis.

BACKGROUND: Osteonecrosis of the femoral head (ONFH) is a progressive, disabling disorder affecting young and middle-aged adults. Joint-preserving treatments-core decompression (CD), cell-based therapy, extracorporeal shock wave therapy (ESWT), hyperbaric oxygen therapy (HBO), and combined regimens-are increasingly used in early-to mid-stage ONFH, but their comparative efficacy and safety remain uncertain. Cellular therapies, varying in source, preparation, dose, and delivery, were analyzed as a single node to preserve network connectivity. METHODS: This PRISMA-NMA-compliant network meta-anal

Association between early postoperative posterior displacement of the spinal cord and postoperative C5 nerve root dysfunction after anterior cervical decompression and fusion: a retrospective imaging cohort study using standardized measurements.

BACKGROUND: C5 nerve root palsy is a challenging complication following anterior cervical decompression and fusion (ACDF) or anterior cervical corpectomy and fusion (ACCF) with a reported incidence of 1-5% (1). While posterior displacement of the spinal cord with resultant nerve root tethering is a leading hypothesis, high-quality evidence quantifying early postoperative cord migration and its association with C5 palsy risk, based on standardized imaging, remains scarce. OBJECTIVE: To investigate the association between early postoperative posterior displacement of the spinal cord, measured u

Limitations of minimally invasive posterior cervical foraminotomy-a decompression method of posteriorly shifting the nerve root-in cases of large anterior osteophytes in cervical radiculopathy: A retrospective multicenter cohort study.

BACKGROUND: Posterior cervical foraminotomy against anterior osteophyte is an indirect decompression procedure but less invasive compared to anterior cervical discectomy and fusion. Residual compression to the nerve root may lead to poor surgical outcomes. Although clinical results of posterior cervical foraminotomy for osteophytes are not considered better than those of disk herniation, osteophyte size and the association of the decompression area with poor surgical outcomes remain unclear. This study aimed to identify the limitations of minimally invasive posterior cervical foraminotomy for

A Comprehensive Review of Novel Interventional Techniques for Chronic Pain: Spinal Stenosis and Degenerative Disc Disease-MILD Percutaneous Image Guided Lumbar Decompression, Vertiflex Interspinous Spacer, MinuteMan G3 Interspinous-Interlaminar Fusion.

Spinal stenosis is the compression of nerve roots by bone or soft tissue secondary to the narrowing of the spinal canal, lateral recesses, or intervertebral foramina. Spinal stenosis may have acquired or congenital origins. Most cases are acquired and caused by hypertrophy of the ligamentum flavum, enlarged osteophytes, degenerative arthritis, disk herniations, and various systemic illnesses. The ligamentum flavum (LF) is a highly specialized elastic ligament that connects the laminae of the spine and fuses them to the facet joint capsules. There are a number of treatment options available for

Full Endoscopic Uniportal Unilateral Laminotomy for Bilateral Decompression in Degenerative Lumbar Spinal Stenosis: Highlight of Ligamentum Flavum Detachment and Survey of Efficacy and Safety in 2 Years of Follow-up.

OBJECTIVE: Conventional laminectomy for spine decompression involves inherent complications resulting from perioperative traumatization of posterior structures. Minimally invasive decompression was developed to address these issues. Full endoscopic spine surgery has shifted the paradigm from disc pathology to spinal stenosis by overcoming the limitation of bony procedures with evolving endoscopic instruments. However, a steep learning curve restricts the development of endoscopic decompression. The purpose of this study is to describe full endoscopic uniportal unilateral laminotomy for bilater

Progressive cord compression secondary to thoracic disc lesions in Scheuermann's kyphosis managed by posterolateral decompression, interbody fusion and pedicular fixation. A new approach to management of a rare clinical entity.

During a brief period of 21 months (November 1988 to August 1990) the neurospinal unit of our institution registered three cases of Scheuermann's kyphosis with herniation of the thoracic discs. Local and intercostal pain, progressive spasticity with gait disturbance and urinary hesitancy were the most common presenting symptoms. All of the patients were surgically managed by posterolateral decompression, interbody fusion (posterolateral approach) and pedicular screw plate fixation. The average length of follow-up was 25 months. Results were excellent in two patients and good in one. All patien
