Behavioral strategies to improve treatment participation and retention by pregnant drug-dependent women.

The present study examined the utility of behavioral incentives for improving early treatment participation and retention in a sample of 91 pregnant opiate and/or cocaine dependent women enrolled in an urban, university-based drug user treatment program between 1996 and 1997. An escalating voucher incentive system was compared to standard care. The relationship between treatment participation and retention and maternal and infant outcomes were examined using logistic regression, chi-square analyses, and t-tests. Behavioral incentives did not decrease rates of very early dropout from residentia

Identification of cocaine analytes in fingernail and toenail specimens.

Fingernail and toenail specimens were obtained from 18 suspected cocaine users. The nails were cut, heated under methanolic reflux, and the methanolic extracts were purified by solid-phase extraction. Gas chromatography/mass spectrometry was utilized for the qualitative and quantitative analysis of nine cocaine analytes. Comparison of conventional postmortem analysis of blood and urine with nail analysis revealed a marked increase in the detection of cocaine use by nail analysis. Cocaine analytes were present in 14 (82.3%) subjects utilizing nail analysis. Out of those 14 subjects, only 5 (27.

Existing evidence for the use of psychedelics in patients with cancer and other serious illness: A narrative review.

OBJECTIVES: Mood disorders and existential distress impact those with cancer or a serious illness at higher rates than the general population. There have been limited pharmacological advances in recent years, and available psychological interventions vary in degree of impact and durability as a treatment modality in this population. A recent renaissance in psychedelic research has suggested that this class of medications might offer an alternative treatment model for anxiety, depression, and existential and psychological distress that often accompanies the diagnosis of a serious illness. Biofabrication of pheochromocytoma and paraganglioma tumor organoids and assessment of response to systemic therapy.

Introduction Pheochromocytoma (PCC) and paraganglioma (PG) research is limited due to rarity of disease. We utilized patient-derived tumor organoids (PTOs) to explore personalized treatment options for these patients. Tumors were obtained from patients with PCC and PG who underwent operative resection and processed to biofabricate PTOs utilizing a collagen-based hydrogel. Blood was also obtained from patients to generate immune-enhanced PTOs (iPTOs). Organoids underwent treatment with chemotherapy, immunotherapy, or tyrosine kinase inhibitors. Cell viability was determined using multiple assay

Therapeutics targeting Bcl-2 in hematological malignancies.

Members of the B-cell lymphoma 2 (BCL-2) gene family are attractive targets for cancer therapy as they play a key role in promoting cell survival, a long-since established hallmark of cancer. Clinical utility for selective inhibition of specific anti-apoptotic Bcl-2 family proteins has recently been realized with the Food and Drug Administration (FDA) approval of venetoclax (formerly ABT-199/GDC-0199) in relapsed chronic lymphocytic leukemia (CLL) with 17p deletion. Despite the impressive monotherapy activity in CLL, such responses have rarely been observed in other B-cell malignancies, and pr

Medication nonadherence: an unrecognized cardiovascular risk factor.

Nonadherence with prescribed drug regimens is a pervasive medical problem. Multiple variables affecting physicians and patients contribute to nonadherence, which negatively affects treatment outcomes. In patients with hypertension, medication nonadherence is a significant, often unrecognized, risk factor that contributes to poor blood pressure control, thereby contributing to the development of further vascular disorders such as heart failure, coronary heart disease, renal insufficiency, and stroke. Analysis of various patient populations shows that choice of drug, use of concomitant medicatio

Real-world study of medication-related osteonecrosis of the jaw from 2010 to 2023 based on Food and Drug Administration Adverse Event Reporting System.

Medication-related osteonecrosis of the jaw (MRONJ) is a rare and severe adverse drug reaction (ADR) commonly seen in people taking drugs prescribed for metastatic cancer and osteoporosis. Prior studies only analyzed this ADR utilizing the public database Food and Drug Administration Adverse Event Reporting System (FAERS) by different stages (2010-2014, 2015-2021); a more comprehensive study is needed to analyze the MRONJ cases and the associated drugs from a longer time span. We conducted a retrospective pharmacovigilance analysis for all reported MRONJ cases between 2010 and 2023 in FAERS, u

CBr4-Catalyzed Substituent-Dependent Michael Addition/Paal-Knorr Cyclization of Indole with α,β-Unsaturated Ketones.

We report CBr4 catalyzed Michael addition of indole to α,β-unsaturated ketones for the synthesis of β-indolylketones through halogen bonding catalysis. This reaction is compatible with a diverse range of chalcones, including drug-derived chalcones containing sensitive functional groups such as amides, yielding the addition products in good yields. Additionally, 3-indolyl furanoid motifs have been synthesized through the Michael addition followed by Paal-Knorr cyclization by utilizing various unsymmetrical 1,4-enediones in a one-pot process with good yields.

Reducing the frequency of COPD exacerbations with antimicrobial therapy.

Chronic obstructive pulmonary disease (COPD) exacerbations are a leading cause of hospitalization in the United States, incurring a large financial burden on the healthcare system and placing significant strain on patient quality of life. Adding empiric antimicrobial agents, specifically macrolides, to the medical regimen of patients with severe COPD has been shown to reduce the frequency of COPD exacerbation. Although prophylactic macrolide therapy may relieve COPD burden, providers should consider the consequences of long-term antimicrobial utilization before prescribing these drugs.
