Otolaryngology--head and neck surgery : official journal of American Academy of Otolaryngology-Head and Neck Surgery1980eng
The direct route to the cerebellopontine (CP) angle through the ear (retrolabyrinthine approach) represents a significant advance in neuro-otologic surgery. After incising the dura between the endolymphatic sac and the lateral venous sinus, a self-retaining retractor is used to collapse the lateral sinus, which provides a wide exposure of the CP angle, with visualization of the 5th through 11th cranial nerves. The retrolabyrinthine approach is a safe and direct route to the CP angle and has advantages over the suboccipital approach. Vestibular neurectomy through the retrolabyrinthine approach

Complement activation, as shown by increased amounts of complexes composed of C1r-C1s-C1 IA, and abnormal complexes of C1r-C1s were demonstrated in serum from patients with acute pneumococcal and chronic otitis media, serous or mucoid respectively. C1q binding substances were shown in middle ear effusions and in sera from patients with chronic serous otitis media. Presence of immune complexes and/or bacterial products capable of binding c1q results in formation of C1r-C1s-C1 IA complexes and may also cause the generation of C1r-C1s complexes. Such a dissociation of the C1 component will compro

BACKGROUND: Chronic suppurative otitis media (CSOM), sometimes referred to as chronic otitis media (COM), is a chronic inflammation and infection of the middle ear and mastoid cavity, characterised by ear discharge (otorrhoea) through a perforated tympanic membrane. The predominant symptoms of CSOM are ear discharge and hearing loss. Topical antiseptics, one of the possible treatments for CSOM, inhibit the micro-organisms that may be responsible for the infection. Antiseptics can be used alone or in addition to other treatments for CSOM, such as antibiotics or ear cleaning (aural toileting). A

Background The external ear, due to its prominent anatomical position and complex three-dimensional (3D) cartilaginous framework, is particularly susceptible to traumatic injuries. There has been a rising incidence of partial and total auricular deformities resulting from road traffic accidents, assaults, human and animal bites, burns, and accidental falls. These injuries are often aesthetically conspicuous and psychologically distressing. Reconstruction of post-traumatic auricular defects poses significant challenges because of the need to restore bilateral symmetry, the limited availability

BACKGROUND: the Bonebridge hearing implant is an active transcutaneous bone conduction implant suitable for various types of hearing loss. It was first launched in 2012 as the BCI 601, with a newer internal part (BCI 602) released in 2019. With the new size and shape, the BCI 602 can be used in patients previously excluded due to insufficient anatomical conditions, especially in patients with congenital defects of the outer and middle ear.
OBJECTIVES: the purpose of this study is to evaluate the objective and subjective benefits of the new Bonebridge BCI 602 in children who have hearing impai

Cholesteatoma is a destructive ear condition requiring complete surgical removal. One major problem lies in the frequent occurrence of residual cholesteatoma caused by squamous epithelium remaining in the middle ear. Our aim is to develop a laser treatment that is selectively directed against residual cholesteatoma cells and can be performed after cholesteatoma surgery in the same session. In a first trial, we studied the photodynamic effect of argon (AL) and diode lasers (DL) on cholesteatoma tissue. Intraoperatively harvested monolayer-cultured cholesteatoma cells were stained in vivo with d

BACKGROUND: Mandibulofacial dysostosis with microcephaly (MFDM, OMIM# 610536) is a rare monogenic disease that is caused by a mutation in the elongation factor Tu GTP binding domain containing 2 gene (EFTUD2, OMIM* 603892). It is characterized by mandibulofacial dysplasia, microcephaly, malformed ears, cleft palate, growth and intellectual disability. MFDM can be easily misdiagnosed due to its phenotypic overlap with other craniofacial dysostosis syndromes. The clinical presentation of MFDM is highly variable among patients.
METHODS: A patient with craniofacial anomalies was enrolled and eval

The Körner's septum (KS), persistent petrosquamosal lamina, is a bony lamina (developmental remnant) that extends from the articular fossa to the mastoid apex, above the middle ear, and runs inferiorly and laterally to the facial nerve canal. The petrous and squamous bones meet at this septum. The anatomical structure of KS, which most frequently occurs at the level of the head of the malleus and/or the anterior semicircular canal, is described in depth in this work. The embryological elements of the temporal bone development that result in the formation of KS are taken into consideration. Cli

INTRODUCTION: Hemiparesis caused by otogenic brain abscess in children is an unusual complication of Chronic Suppurative Otitis Media. Complications can occur when the disease is not treated properly so that the infection in the middle ear spreads intracranially.
CASE PRESENTATION: We report a case of Chronic Suppurative Otitis Media with cholesteatoma in a 14-year-old boy with complications of right hemiparesis caused by an otogenic brain abscess. His management included open craniotomy, drainage of the abscess, radical mastoidectomy and intravenous antibiotics according to the result of the

The usage of finite element method techniques gives a possibility to replace time-consuming experiments or imitate physical process in the ear by numerical simulation. Especially, the research of spatial motion of ossicular chain in the middle ear is of high interest for the oto-surgeons and engineers. It is known that the most affected bone from the ossicular chain is the incus. After the cholesteatoma operation and tympanoplasty, the affected incus is removed or sacrificed; thus, the possibility of transducing noise lays on the stapes, new titanium or other material prosthesis. In this case,

The aim of this study was to investigate the prevalence of tinnitus among adolescents, and the factors known or hypothesized to be associated with tinnitus. Based on random sampling of school types and regions, a nationwide hearing survey of the first-year middle and high school students of South Korea was performed. The subjects underwent an otologic examination followed by pure tone audiometry up to 8 kHz. Questionnaires about the factors associated with hearing and tinnitus were completed by the students and their parents. Among the 1,593 subjects who completed the questionnaire and underwe

INTRODUCTION: Foreign bodies in the external auditory canal are fairly common, and tend to be seeds, toys, or insects. However, foreign bodies in the middle ear are rarer and are generally accidental or iatrogenic. Although there are numerous reports regarding fascinating foreign bodies in the ear, the same cannot be said about foreign bodies in the Eustachian tube (ET).
CASE REPORT: A 20-year-old male patient presented with right-sided active squamosal chronic otitis media (COM) persisting for 10 years. An incidental finding of a hyperintense foreign body (FB) located near the isthmus of the

A hearing aid using cartilage conduction (CC) has been proposed as an alternative to bone conduction (BC) hearing aids. The transducer developed for this application is lightweight, requires a much smaller fixation force than a BC hearing aid, and is more convenient to use. CC can be of great benefit to patients with fibrotic aural atresia. Fibrotic tissue connected to the ossicles provides an additional pathway (termed fibrotic tissue pathway) for sound to reach the cochlea by means of CC. To address the function of fibrotic tissue pathway, BC and CC thresholds were measured in six ears with

CONCLUSIONS: The model appears relevant for studies on sequelae after acute otitis media (AOM), and may be the seed of a new, chronic tympanic membrane perforation model in the gerbil.
OBJECTIVES: To evaluate an experimental model for abortive otitis media and to assess the structural and functional changes of the tympanic membrane in the resolving phase.
MATERIALS AND METHODS: The middle ears of 16 Mongolian gerbils were inoculated with type 6a Streptococcus pneumoniae. Half of the animals were treated with antibiotics on days 4-6, when otoscopy was performed as well. After 1, 2, 3 or 4 wee

INTRODUCTION: The aim of this study was to quantify the amount of deterioration in hearing and to document the trajectory of hearing loss in early identified children with unilateral hearing loss (UHL). We also examined whether clinical characteristics were associated with the likelihood of having progressive hearing loss.
METHODS: As part of the Mild and Unilateral Hearing Loss Study, we followed a population-based cohort of 177 children diagnosed with UHL from 2003 to 2018. We applied linear mixed models to examine hearing trends over time including the average amount of change in hearing.

We report an attempt to obtain images of the basal cerebral arteries and to measure the quantitative flow velocity in these arteries from outside the skull via the color Doppler imaging method. In 25 healthy volunteers, 22-41 years old, the transducer was positioned just superior to the zygomatic arch and anterior to the external ear canal. The absolute flow velocity was calculated by dividing the measured velocity by the cosine of the incident angle. The middle cerebral artery was readily identified in all subjects, but the anterior cerebral artery was seen in only 7/50, and the posterior cer

PURPOSE: The maturation of auditory pathways in newborns, particularly in pre-terms, is a crucial process influencing the accuracy and timeliness of early audiological diagnosis. The aim of this study was to investigate the effect of postnatal auditory pathway maturation on hearing loss diagnosis by comparing early and later ABR assessments in pre-term and full-term newborns.
METHODS: Seventy-eight newborns (156 ears) were enrolled and divided by gestational age. All underwent initial screening, clinical Auditory Brainstem Response (ABR), tympanometry, and evaluation of diagnostic timing. Sta

Congenital cholesteatoma accounts for 25% of cholesteatoma cases in children. Transcanal Endoscopic Ear Surgery (TEES) is ideal for these patients because it offers a wide endoscopic view of the middle ear and a minimally invasive approach. The two main limitations are the loss of one operative hand and a narrow external auditory canal in younger children. Here, we present the case of a 3-year-old patient with a Potsic stage III congenital cholesteatoma adherent to the incus and branches of the stapes. A robotic-assisted TEES procedure was performed, during which a robotic arm with 6 degrees o
