Osteonecrosis of the jaws associated with use of risedronate: report of 2 new cases.

Use of various bisphosphonates has been associated with the development of osteonecrosis of the jaws (ONJ). At least 865 cases of ONJ attributed to these agents have been reported in the English-language literature. Approximately 96% of these published cases were seen with administration of the intravenous agents pamidronate and zoledronate, whereas only 26 cases have been associated with oral bisphosphonates, 25 of them with alendronate. Only a single case of ONJ associated with the oral bisphosphonate risedronate has been previously cited. We report 2 cases of ONJ attributed to risedronate a

An unusually large asymptomatic periapical lesion that presented as a random finding on a panoramic radiograph.

OBJECTIVE: This report presents an asymptomatic case of a large mandibular periapical lesion that was discovered as a random finding on a panoramic radiograph. SUMMARY: A panoramic radiograph was asked for removal of wisdom tooth's roots. The radiograph revealed an unusually large lesion extending from lower premolar to opposite lower canine. Upon clinical examination a discolored lower central incisor and an elevation of the vestibule were visible. The discolored tooth did not respond to thermal and electrical vitality tests and was not tender to percussion. No history of trauma was recorded

Misdiagnosed fibrosarcoma of the mandible mimicking temporomandibular disorder: a rare condition.

The jawbones can be sites of various neoplastic conditions. Given the variety of processes affecting this particular anatomical area, formulation of a precise diagnosis can be challenging to clinicians. Limited jaw movement, pain, and facial asymmetry are common signs among patients, especially those with temporomandibular disorder (TMD). This paper reports a case of primary fibrosarcoma affecting the mandible and surrounding structures in a 14-year-old girl presenting signs and symptoms similar to TMD. Her condition was misdiagnosed, and she was treated for TMD over an extended period before

Spindle cell hemangioma (hemangioendothelioma) of the head and neck: case report of an unusual (or underdiagnosed) tumor.

Spindle cell hemangioma (SCH), also known as spindle cell hemangioendothelioma, is a unique vascular tumor with combined microscopic features of both a cavernous hemangioma and Kaposi sarcoma. It almost exclusively affects the dermis and subcutis of the distal extremities. A review of the literature disclosed only 5 cases of SCH reported in detail in the soft tissues of the head and neck. An additional case of SCH manifesting as a submucosal nodule in the upper lip of a 29-year-old woman is presented. Because this is the second case reported from the same institution, it is suggested that SCH

Cicatricial pemphigoid with severe gingival and laryngeal involvement in an 18-year-old female.

Cicatricial pemphigoid (CP), also known as mucous membrane pemphigoid, is an autoimmune vesiculobullous disease occurring mostly in elderly people and seldom occurring in individuals under the age of 20 years. It predominantly affects the mucosal surfaces, primarily the oral and conjunctival mucosa. Uncommonly, the upper aerodigestive tract is involved, which can lead to life-threatening complications. We present the case of an 18-year-old girl with desquamative gingivitis and severe laryngeal webbing and stenosis, caused by cicatricial pemphigoid. Airway management necessitated a tracheostomy

Case report: B-cell lymphoma of the maxillary sinus.

The radiographic manifestation of malignant lesions of the maxillary sinus on dental radiographs may be nonspecific, making it difficult to differentiate the lesion from disease of odontogenic origin or more benign sinus pathoses. A radiopaque mass in the maxillary sinus, resulting from a malignant neoplasm growing within or extending into the sinus, can be easily confused with the mass of a mucous retention pseudocyst. Similarly, a malignant growth in the early stages of development can produce radiographic patterns in the alveolar process that may resemble inflammation of odontogenic origin.

False-positive xerostomia following radioactive iodine treatment: case report.

Radioactive iodine (131I), used in the treatment of differentiated thyroid carcinoma, is known to cause both short-term and long-term radiation damage to the salivary glands. The injury appears as glandular swellings and/or decreased salivation with 131I dosage and passage of time playing significant roles. A case report is presented to alert the profession to the existence of patients who have received 131I therapy and who complain shortly thereafter of xerostomia, but following a thorough examination are found to represent a group of false-positives. Emphasis is placed on the diagnostic tech

Malignant mucosal melanoma of the right maxillary sinus: a case report.

The case of a 74-year-old female is reported. The patient was under long-term review for melanosis affecting the mucosa of the upper right alveolar ridge. The patient developed a primary mucosal melanoma at the site 5 years later and underwent a right maxillectomy and a modified right neck dissection to level 5 nodes. The defect was covered with an obturator; the patient is at present clinically disease free. Mucosal melanomas account for 1% of all melanomas reported. This is a more aggressive tumor than the cutaneous melanoma, which is reflected in the low survival rate of 3 years (approximat

Sonographic diagnostic criteria for screening Sjögren's syndrome.

OBJECTIVE: The objective of this study is to establish readily applied sonographic diagnostic criteria for Sjögren's syndrome. STUDY DESIGN: Sonographic images of 79 cases of previously suspected Sjögren's syndrome (including 43 actual cases) were analyzed retrospectively for the following characteristic features: (1) multiple hypoechoic areas, (2) multiple hyperechoic lines or spots, (3) multiple hypoechoic areas surrounded with hyperechoic lines or spots, and (4) obscuration of the gland configuration. Logistic regression analysis was used to extract valuable sonographic findings. Sonograph

Symmetry of the submandibular glands in humans--a postmortem study assessing the linear morphometric parameters.

BACKGROUND: The analysis of morphometric right-left symmetry of the salivary glands is important for assessing unilateral changes discovered in these organs during diagnostic imaging. STUDY DESIGN: A study was carried out on 18 adult human autopsy neck and maxillofacial specimens from both sexes (14 males, 4 females), with age range of 17 to 73 years (mean age 49.9 years). The submandibular duct was injected with contrast medium and images were made in two projection planes. RESULTS: Morphometry performed using the obtained radiographs included: (a) caliber (mean proximal, middle, and distal

Epithelial-myoepithelial carcinoma of the submandibular gland with a high uptake of 18F-FDG: a case report and image diagnosis.

Epithelial-myoepithelial carcinoma (EMC) is a rare low-grade malignant salivary gland neoplasm typically found in the parotid gland. This report describes a case of EMC that arose from the submandibular gland, and gives special emphasis to the preoperative diagnostic value of positron emission tomography (PET) in combination with computed tomography (CT) and magnetic resonance imaging (MRI). Although a preoperative diagnostically specific image of EMC could not be established, the malignant nature of this tumor was detected by these combined examinations.

Salivary duct carcinoma in the mandible: a case report.

Salivary duct carcinoma (SDC) is a distinctive and aggressive neoplasm. The most frequent site of origin is the parotid gland, followed by the submandibular gland. SDC originating in the minor salivary glands, particularly in the ectopic glands within the mandible, is extremely rare. We describe a 62-year-old man with SDC in the mandible, who presented with a painless lump in the right submandibular region (later identified as lymph node metastasis) and ipsilateral mental nerve palsy. Histologic examination after ablative surgery revealed SDC originating in the mandible and cervical nodal meta

Immunoprofile of a carcinosarcoma of the submandibular gland.

Carcinosarcomas are a very rare group of true malignant tumors of the salivary gland. As the name indicates, the tumor is composed of an epithelial and a mesenchymal component, both malignant. We report a case of carcinosarcoma of the submandibular gland in an 86-year-old woman. The epithelial component showed a squamous carcinoma phenotype, whereas the mesenchymal component was morphologically similar to a fibrosarcoma. The epithelial component was strongly positive for CK13, CK14, and AE1/AE, and groups of positive cells were seen for CK19 and vimentin. The whole mesenchymal component was po

A case of carcinosarcoma arising in the submandibular gland.

A 54-year-old man presented with an 8-year history of a hard asymptomatic mass of the left submandibular area. Total excision of the left submandibular gland with radical neck dissection was performed under a diagnosis of a submandibular tumor, probably a malignant mixed tumor. The pathologic diagnosis was carcinosarcoma consisting of carcinomatous and sarcomatous elements. The epithelial component was composed of squamous cell carcinoma, undifferentiated carcinoma, and adenocarcinoma. The nonepithelial component was composed of chondrosarcoma, osteosarcoma, spindle cell sarcoma, rhabdomyosarc

Effect of proliferating tissue on transplanted teeth in dogs.

OBJECTIVE: The objective of this study was to investigate the effect of transplanted proliferating tissue on the regeneration of periodontal ligaments (PDL) in tooth transplantation. STUDY DESIGN: Two weeks after removing the alveolar bone, proliferating tissues were excised and examined for expression of basic fibroblast growth factor (bFGF) and alkaline phosphatase (ALP) using a LightCycler. Next, the teeth to be transplanted (first premolars) were extracted and PDLs were removed by scaling. After that, the first premolar teeth and proliferating tissues for the experimental group were trans
