Black children with Lyme disease compared with children of other races were less likely to have an erythema migrans lesion diagnosed (adjusted odds ratio, 0.34; 95% confidence interval, .14-.79) but more likely to have a swollen joint (adjusted odds ratio, 3.68; 95% confidence interval, 2.13-6.36) after adjustment for age and local Lyme incidence.

Mycosis fungoides, the most common cutaneous T-cell lymphoma, is known to exhibit varied clinical presentations and mimic many other dermatoses. This morphological heterogeneity can often lead to initial misdiagnoses resulting in significant delays from symptom onset to diagnosis. We report a case of mycosis fungoides imitating clinical and histopathological features of erythema annulare centrifugum.

Human Immunodeficiency Virus (HIV) has profoundly affected the clinical practice of dentistry since the early 1980s. Acute lesions such as linear gingival erythema (LGE) and necrotizing ulcerative periodontitis (NUP) were described as HIV-related oral lesions. The behaviour of chronic gingivitis and periodontitis as well as other practice-related issues are discussed in this paper.

A 74-year-old woman developed a generalized pustular dermatosis with erythema-multiforme-like and vesiculobullous lesions 10 days after beginning a treatment by nadoxolol. Despite the clinical polymorphism of the eruption, histology constantly revealed the presence of subcorneal pustules, even within the epidermis overlying subepidermal bullae. The eruption spontaneously healed after discontinuation of nadoxolol.

Daffodils (Narcissus species) are the most common cause of irritant contact dermatitis among florists. Calcium oxalate crystals contained in the sap of the daffodil plants lead to irritant contact dermatitis on the skin. Daffodil rash commonly presents with fissuring, scaling, and erythema of the fingertips, hands, and forearms. The best preventative measure is to wear appropriate protective gloves and clothing.

A 49-year-old man, who presented with new onset dyspnea on exertion and fatigue without clear history of a typical erythema migrans rash, was found to have complete atrioventricular (AV) heart block with positive serology for Lyme disease. A complete stepwise reversal of the AV block was achieved shortly after the initiation of intravenous antibiotic treatment.

A 54-year-old woman presented to the emergency department with progressive proximal muscle weakness and a symmetric skin rash. Physical examination demonstrated a heliotrope rash, Gottron lesions, mechanic's hands and symmetrical erythema of the face, neck and upper legs. The diagnosis 'dermatomyositis' was established. Subsequently, the patient was successfully treated with prednisolone 1 mg/kg.

Erythromelanosis follicularis faciei et colli is a rare disease of unknown etiology characterized by symmetric erythematous brownish hyperpigmentation on the frontal, malar and temporal areas associated with involvement of the pilary follicle. It is common to present pilar keratosis on the shoulders and neck. The three main clinical characteristics are: erythema (with or without telangiectasias), discrete follicular papules and brownish hyperpigmentation.

Thalidomide is the drug of choice in the treatment of severe erythema nodosum leprosum (ENL) in men. It has recently been associated with deep venous thrombosis (DVT) when used in treatment of refractory multiple myeloma along with combination chemotherapy. We report a case of DVT in a patient treated for ENL with corticosteroids and thalidomide, and suggest a possible mechanism for the association.

This short paper reviews the nature of rosacea emphasizing the possibility of a solar cause. The sites of involvement and the physical signs of rosacea including the flushing, the erythema and the telangiectasia as well as the intermittent episodes of inflammation with swelling and papules may all be explained by UVR induced damage to dermal connective tissue. The dermal damage permits vaso-dilation and vascular pooling.

Shortly after birth, a five-week-old female infant developed small blisters and erythema that followed the lines of Blaschko on the upper and lower extremities as well as the abdomen. Histological examination confirmed the clinical presumptive diagnosis of incontinentia pigmenti. We discuss the dinical features, diagnosis, and the molecular genetic basis of this rare inherited skin disorder.

Leprosy is a chronic infection of the skin and nerves caused by Mycobacterium leprae. Erythema nodosum leprosum (ENL) is a reactive state in lepromatous leprosy. Thalidomide has been used to treat ENL since the 1960s. One of its mechanisms of action is anti-inflammatory through selective inhibition of the pro-inflammatory cytokine TNF-alpha produced by monocytes.

Solar urticaria is a rare disorder characterized by erythema, pruritus, and urticaria occurring minutes after exposure to a light source. It is one of several photosensitive conditions, such as phototoxic reaction, photoallergic reaction, systemic lupus erythematosus, and porphyria, that can cause photosensitivity. Herein we report a case of solar urticaria and review the rational approaches to its diagnosis and treatment.

The three cutaneous reaction patterns of erythema multiforme, Stevens-Johnson syndrome, and toxic epidermal necrolysis share many clinical features. Each is multifactorial in etiology; together they account for most severe cutaneous drug eruptions. In some respects, they can represent a progression in the severity of drug eruptions. Controversy continues to cloud the clinical delineation, etiology, and treatment.

The more common paraneoplastic skin changes associated with internal malignancies (acanthosis nigricans, dermatomyositis, secondary ichthyosis) are discussed. The rare glucagonoma syndrome, consisting of alpha cell tumor of the pancreas, migratory necrolytic erythema, and diabetes mellitus, is described. The importance of recognition of these skin lesions may lead to early detection of the underlying cancer with possible cure.

Toxic epidermal necrolysis is one of the rarest events -- a dermatologic emergency. The clinical presentation is characteristic -- disseminate erythema and widespread bullous necrosis of the epidermis and mucous membranes. Toxicity is universal and mortality is substantial even with prompt and intensive therapy. Drugs are the most commonly incriminated agents and therapy remains controversial, the majority favoring high-dose systemic corticosteroids.
