Saccadic trajectory in Huntington's disease.

Trajectories of saccadic eye movements can be modulated by the presence of a competing visual distractor. It is proposed that the superior colliculus (SC) controls the initial deviation through competitive lateral interactions. Given the ramifications of connections between basal ganglia (BG) thalamo-cortical circuitry and the SC, it was anticipated that this modulation would be differentially effected in those with Huntington's disease, which in its early stages is primarily a disorder of the BG. Horizontal deviation was determined for exogenously driven and endogenously driven vertical sacca

Ischemic orbital compartment syndrome as a complication of spinal surgery in the prone position.

OBJECTIVES: To report a patient with ischemic orbital compartment syndrome as a complication of spinal surgery in the prone position. DESIGN: Interventional case report. METHODS: An 80-year-old man underwent a prolonged lumbar decompression laminectomy for spinal stenosis, under general anesthesia in the prone position. Several hours later, the patient complained of left periocular pain and reduced vision. Examination revealed significant facial edema, left proptosis, and a tight orbit, as well as no light perception and elevated intraocular pressure in the left eye, with complete internal a

[Benzodiazepine and hiccup: three case reports].

Hiccup is a sudden contraction of the inspiratory muscles, followed by an abrupt closure of the glottis, thus producing a characteristic sound. In the literature, some drugs have been reported to induce hiccup. We discuss three case reports after administration of benzodiazepine to healthy young subjects during two clinical trials. In the first study (a bioequivalence trial of two forms of lormetazepam, tablets and oral solution), 12 subjects were included in an open controlled crossover study with two periods separated by a washout of 7 days. Two subjects presented with hiccup after administr

Swallowing disorders in patients with blepharospasm.

Blepharospasm is a focal dystonia characterized by involuntary eye closure due to abnormal contraction of orbicular eyelid muscles. When blepharospasm is associated to the presence of involuntary oromandibular movements, it is termed Meige syndrome. The aim of this study was to investigate the presence of deglutition alterations in patients with concurrent blepharospasm and Meige syndrome. Twenty consecutive patients were studied by video fluoroscopy using a barium technique. The 4 stages of deglutition were investigated. Ninety percent of patients (18 cases) presented deglutition disorders. T

[Caloric stimulation in infants].

Caloric stimulation is one of few clinically proven tests assessing the function of each vestibule separately in neonates. Its results represent the continuity of vestibulo-ocular reflex, beginning in the lateral vestibular canal. Vestibular disorders are diagnosed in 20 to even 70% neonates with sensorineural hearing loss with the prevalence of individuals with profound and acquired deafness. 58 high risk of hearing defect infants were included in the study. Their age ranged from 3 to 6 months. A group of 27 healthy controls with negative history concerning sensorineural hearing loss risk fac

Multiple intraorbital neurofibromas: a rare cause of proptosis.

BACKGROUND: Orbital neurofibromas are rare, accounting for 0.5 to 2.4 % of all orbital tumors. Generally, they manifest as slowly progressive proptosis, in a young adult or middle-aged person, and are usually solitary lesions. Sometimes, they can be associated with type 1 neurofibromatosis. We present a case of proptosis related to multiple intraorbital neurofibromas in an 82-year-old woman without type 1 neurofibromatosis. HISTORY AND SIGNS: An 82-year-old woman was referred for slowly progressive left proptosis associated with an ocular burning sensation. Neuro-ophthalmic examination reveal

[Distribution and abundance of Caiman crocodilus in the Caño Negro National Wild Life Refuge, Costa Rica].

The distribution and abundance of a population of Caiman crocodilus fuscus were estimated by monthly counting of eye-shines at night, from February 1999 to March 2000 in six transects of Río Frío in the Caño Negro National Refuge (RNVSCN), Northern Costa Rica. March was the month with the greatest abundance of caimans observed. The visible fraction of the population (PV2 index) fluctuated between 42.59% to 54.71% during the wet season and 35.49% to 53.93% in the dry season. The transects of river with greater abundance of caimans were Terrón-Sabogal and Sabogal-Playuela. Significant difference

A case of Joubert's syndrome with extensive cerebral malformations.

Joubert's syndrome is a relatively rare, autosomal-recessive syndrome defined by vermis hypoplasia, hypotonia, developmental delay and at least one of two additional manifestations: abnormal breathing pattern or abnormal eye movements. Detailed descriptions of the neuropathological findings in this syndrome are scarce. We present a radiological and pathological correlation of a case of Joubert's syndrome in which, apart from the classic vermis aplasia and some malformations of the brain stem and the spinal cord, extensive malformations of the cerebrum were found. The dentate nuclei were broken

Blindsight in subjects with homonymous visual field defects.

Brain damage in the visual system can lead to apparently blind visual areas. However, more elaborate testing indicates that some visual ability may still exist for specific stimuli in the otherwise blind regions. This phenomenon is called 'blindsight' if subjects report no conscious awareness of visual stimuli but when forced to guess, nevertheless perform better than chance. It has mainly been suggested that secondary visual pathways are responsible for this phenomenon. However, no published study has clearly shown the neural mechanism responsible for blindsight. Furthermore, experimental art

[Atypical optic neuritis in systemic lupus erythematosus (SLE)].

HISTORY AND CLINICAL DATA: A 67-year-old woman experienced acute unilateral visual loss accompanied by pain with eye movements. There was a marked relative afferent pupillary defect and a nerve fiber bundle defect in the upper half of the visual field. Optic discs were normal. After 4 days vision worsened to motion detection and only a temporal island was left in the visual field. The optic disc margin was blurred. Since thirty years she had been suffering from renal insufficiency. Immunoserologic examination revealed elevated ANA and DS-DNA antibody titers. An optic neuritis in systemic lupus

Post-emergency epidemiological surveillance in Iraqi-Kurdish refugee camps in Iran.

In 1991 a computerized, comprehensive epidemiological surveillance system was developed to monitor health trends in approximately 25,000 acutely displaced Kurds in Nowsood and Saryas refugee camps, Bakhtaran region, Northwestern Iran. In addition, community-based surveys offered information unobtainable from health facilities. Weekly population movements, attack rates, point-prevalence estimates, and case fatality ratios were calculated, and the data were analysed and compared. The overall crude mortality rate (CMR) in the camps under study was still 9 times higher than the reported CMR for Ir

Angelman syndrome.

BACKGROUND: Angelman (happy puppet) syndrome is a neuro-developmental condition characterized by an ataxic gait with puppet-like limb movements, paroxysmal bouts of laughter and severe mental retardation. Although considered a rare condition, over 140 cases have been documented since its designation in 1965. To date, only one study has been published investigating the ocular defects of Angelman syndrome. METHODS: In this paper we report the cognitive, motor, systemic, and oculo-visual findings of a 3 year old child with Angelman syndrome. RESULTS: The oculo-visual findings include choroidal

Behavioural and neural correlates of vestibular compensation.

Sudden complete loss of input from one labyrinth results in a massive change in behaviour. A vigorous horizontal ocular nystagmus occurs together with postural changes. These dramatic changes are short-lived and within about a week they have almost disappeared. This very rapid recovery has been the basis for the postulation that vestibular compensation is a textbook model for the study of neural plasticity in the central nervous system. Whilst the behavioural recovery is dramatic, quantitative testing reveals the loss and the permanent asymmetry of the system (Table 1). Recordings from single

Stereopsis, accommodative and vergence facility: do they relate to dyslexia?

The relation between visual functions and reading performance has been the subject of conflicting opinion over the years. The purpose of this study is 2-fold: first, to examine factors other than visual acuity, i.e., stereopsis, accommodative facility and vergence facility, which might show a greater impact on reading achievement; and second, to utilize well defined dyslexic and normal controls to ensure that the reading deficits were not caused by overt neurological, psychological, educational, or environmental factors. Thirteen normal readers and 13 dyslexics were carefully matched by rigoro

General remarks on the role of the vestibular system in weightlessness.

Different methods are described to experimentally achieve weightlessness. Since the function of the otolith system depends on the presence of contact forces opposing gravity, it is disabled in weightlessness and may send misleading positional information to the brain. Without the contributions of the otolith system it is difficult in space to distinguish self-motion from object motion. Furthermore, the disintegration of information from the neck position receptors from those of the otolith system can lead to additional illusory positional sensations. Since the function of the semicircular cana

[Oculomotor and visual sensory changes in the neurologically lesioned child].

Pathological changes in the visual system are often present in children with neurological handicaps. Strabismus, refractive errors, amblyopia, optic atrophy are the more frequent findings. The ophthalmological evaluation in these children includes: eliciting of pupillary light reflex; occlusion test; cover-test; observation of symmetry of corneal light reflexes; retinoscopy and ophthalmoscopy. Strabismus is the most frequent alteration. The so-called "caotic" strabismus, with very variable angle of squint and abnormalities of the vertical movements, is peculiar in children with neurological ha

Eye-tracking reading patterns across different interventions in glaucoma patients versus controls.

To evaluate reading performance patterns – specifically saccades, fixations, and reading speed – under three visual interventions (font size, line spacing, and contrast) in glaucoma patients compared to control subjects, using an eye tracker. This cross-sectional study included 111 patients (57 patients with glaucoma and 54 controls), with no ocular diseases other than glaucoma, and collected demographic and ophthalmologic data. Glaucoma was defined as progressive changes on fundus stereophotographs and/or documented compatible visual fields defects. All patients had at least best-corrected vi

Through the Eyes: A Case of Ocular Syphilis.

Syphilis is a bacterial disease caused by Treponema pallidum and is sexually transmitted via vaginal, anogenital, or orogenital contact. Vertical transmission between mother and unborn child is also possible, but transmission via skin-to-skin or blood contact is rare. The objective of this case is to highlight this rare ocular manifestation of syphilis as it manifests as a multisystemic disease affecting many organ systems. This is a case of a 46-year-old male with vision loss who was referred to the emergency department by an ophthalmologist. Two days before the presentation at the emergency

The influence of dopaminergic medication on balance automaticity in Parkinson's disease.

BACKGROUND: Studies have shown that dual-task standing balance in Parkinson's disease (PD) is significantly diminished. Additionally, it is well accepted that dopaminergic medication improves dynamic balance (Berg Balance Scale, mini-BESTest), but standing balance (force platform posturography) may suffer. What remains unknown is how dopaminergic medication influences standing balance automaticity in PD. RESEARCH QUESTION: Does dopaminergic medication improve standing balance automaticity during a phoneme monitoring dual-task in PD? METHODS: This was a cross-sectional study. Sixteen subjects

The association between primary open-angle glaucoma and fall: an observational study.

PURPOSE: Falls are among the most serious public health concerns for the elderly. Information conveyed via the visual sense is relevant to postural balance and movement, and proper visual function is essential to avoid falls. Here we investigated the prevalence of injurious falls among patients with primary open-angle glaucoma (POAG) who were more than 45 years old, compared with comparably aged healthy subjects. METHODS: This is a cross-sectional study. Consecutive patients who visited the Tanabe Eye Clinic, Yamanashi, Japan between January 1 and March 30, 2009 were screened for eligibility
