Carbon footprint of common procedures in inflammatory bowel disease.

BACKGROUND: The aim of this study was to assess the environmental impact, primarily the carbon footprint of the most common procedures in inflammatory bowel disease (IBD). METHODS: In this study, all processes and products used during a total of eight laparoscopic ileocecal resections (ICRs) in patients with Crohn's disease (CD), eight laparoscopic subtotal colectomies (STCs) for ulcerative colitis (UC), and eight ligation of the intersphincteric fistula tract (LIFT) procedures in patients with Crohn's perianal fistula (PAF) (all in adults ≥ 16 years) between March 2023 and May 2024 were coll

Structural and genomic insights into erythromycin and clindamycin resistance of group B Streptococcus isolates in rural West Virginia, United States.

INTRODUCTION: Group B Streptococcus (GBS) is a leading etiological agent of neonatal sepsis and meningitis, as well as invasive infections that predominately affect adults over 65 and people with comorbidities. The Centers for Disease Control and Prevention's (CDC) nationwide surveillance has identified rising clindamycin and erythromycin resistance as a concerning level threat for invasive GBS infections. West Virginia (WV) is a rural state outside the CDC catchment area, therefore, we investigated 65 WV-GBS isolates identified in clinical specimens from various sources for serotype distribut

In Silico Development of a Chimeric Multi-Epitope Vaccine Targeting Helcococcus kunzii: Coupling Subtractive Proteomics and Reverse Vaccinology for Vaccine Target Discovery.

Background: Helcococcus kunzii, a facultative anaerobe and Gram-positive coccus, has been documented as a cunning pathogen, mainly in immunocompromised individuals, as evidenced by recent clinical and microbiological reports. It has been associated with a variety of polymicrobial infections, comprising diabetic foot ulcers, prosthetic joint infections, osteomyelitis, endocarditis, and bloodstream infections. Despite its emerging clinical relevance, no licensed vaccine or targeted immunotherapy currently exists for H. kunzii, and its rising resistance to conventional antibiotics presents a grow

Profiles of Peripheral Neuropathy and Risk Factors in People With Treatment-Naive Type 2 Diabetes Mellitus and Prediabetes.

BACKGROUND: Diabetic peripheral neuropathy (DPN) is the most common complication of both type 2 diabetes mellitus (T2DM) and type 1 diabetes mellitus (T1DM) occurring in a majority of the affected people. DPN is the key initiating factor for the development of diabetic foot ulceration and the most common cause of non-traumatic lower-limb amputations. Whether chronic hyperglycaemia per se or altered levels of intermediate conventional risk factors in these people are responsible for the increased risk of DPN, is a matter of debate. There is little data about the profile of DPN in people with tr

miR-200b-3p accelerates diabetic wound healing through anti-inflammatory and pro-angiogenic effects.

The poor healing characteristics of diabetic foot ulcers are partially attributed to diabetes-induced pro-inflammatory wounds. Our previous study reported that both miR-146a-5p and miR-200b-3p decrease endothelial inflammation in human aortic endothelial cells and db/db diabetic mice. Although miR-146a-5p has been reported to improve diabetic wound healing, the role of miR-200b-3p is not clear. This study compared the roles of these miRNAs in diabetic wound healing. Two 8-mm full-thickness wounds were created in 12-week-old male db/db mice on the left and right back. After surgery, 100 ng miR-

Effect of Medical Ozone Therapy in Preventing Compromised Nasal Skin in Revision Rhinoplasty.

BACKGROUND: Ozone is often used as an additive therapy for skin conditions like infectious diseases, wound healing, diabetic foot, and pressure ulcers. The viability of the nasal skin has crucial importance in revision rhinoplasty cases. The study investigates the potential benefits of medical ozone therapy in healing the nasal skin in multiple-operated cases. METHODS: The study retrospectively examined 523 revision rhinoplasty patients operated by the first author from January 2017 to January 2024. Patients consenting to ozone therapy received 3 major autohemotherapy sessions post-surgery. P

Prognostic Nutritional Index as a New Prediction Tool for All-Cause Mortality in Patients with Chronic Limb-Threatening Ischemia Undergoing Endovascular Therapy.

OBJECTIVES: Chronic Limb-Threatening Ischemia (CLTI) represents a complex manifestation of peripheral artery disease distinguished by symptoms such as ischemic rest pain, non-healing ulcers on the lower limb or foot, and the development of gangrene. CLTI is associated with a high risk of limb amputation, decreased quality of life, and substantial morbidity and mortality. The Prognostic Nutritional Index (PNI), which is calculated using albumin and lymphocyte levels, reflects the immunological and nutritional status. The objective of this study was to investigate the correlation between PNI lev

Parallel disease activity of Behçet's disease with renal and entero involvements: a case report.

BACKGROUND: Behçet's disease (BD) is a systemic inflammatory vasculitis with both autoimmune and autoinflammatory properties. Renal involvement in BD and its spontaneous remission have been rare. We herein describe a case of parallel disease activity of BD with entero and renal involvements, followed by a spontaneous remission without corticosteroid treatment. CASE PRESENTATION: A 54-year-old woman who had a 4-year history of BD, maintained with colchicine treatment, suffered abdominal pain, hemorrhagic stool and diarrhea. Physical examination revealed strong tenderness in the entire abdomen.

When Limb Surgery Has Become the Only Life-Saving Therapy in FOP: A Case Report and Systematic Review of the Literature.

Fibrodysplasia ossificans progressiva (FOP) is a rare disease in which heterotopic ossification (HO) is formed in muscles, tendons and ligaments. Traumatic events, including surgery, are discouraged as this is known to trigger a flare-up with risk of subsequent HO. Anesthetic management for patients with FOP is challenging. Cervical spine fusion, ankylosis of the temporomandibular joints, thoracic insufficiency syndrome, restrictive chest wall disease, and sensitivity to oral trauma complicate airway management and anesthesia and pose life-threatening risks. We report a patient with FOP suffer

Causes of Mortality and Lesions Observed Post mortem in European Moles (Talpa europaea) in Cornwall, South-west England.

European moles (Talpa europaea) are a common species in the UK, but there are few published data on the causes of mortality in these animals. An opportunistic post-mortem study was carried out on 44 moles found dead or dying in Cornwall, UK. Assessment of muscle condition and fat reserves, where possible, indicated that most (27 of 37, 73%) were in good nutritive condition. The majority had died of trauma (n = 40, 91%), the principal cause of which was predation (n = 36, 81.8%) by foxes and domestic cats and dogs. The carcases were in a variable state of preservation, but 28 cases were suitabl

Evaluation of a muscle pump-activating device for non-healing venous leg ulcers.

This evaluation involves an innovative muscle pump-activating device (geko™) as an adjunctive therapy with best practices for non-healing venous leg ulcers (VLUs). Stimulating the common peroneal nerve (at the fibular head), the geko™ device creates a response that acts as foot and calf muscle pumps, increasing venous, arterial and microcirculatory flow. The aim was to evaluate and determine if the geko™ is effective in this population and if it should be added to the medical supply formulary. In all, 12 patients with 18 recalcitrant VLUs (defined as less than 30% reduction in wound size in 30

Combination therapy accelerates diabetic wound closure.

BACKGROUND: Non-healing foot ulcers are the most common cause of non-traumatic amputation and hospitalization amongst diabetics in the developed world. Impaired wound neovascularization perpetuates a cycle of dysfunctional tissue repair and regeneration. Evidence implicates defective mobilization of marrow-derived progenitor cells (PCs) as a fundamental cause of impaired diabetic neovascularization. Currently, there are no FDA-approved therapies to address this defect. Here we report an endogenous PC strategy to improve diabetic wound neovascularization and closure through a combination therap

Evaluation of patients with peripheral vascular disease.

Approximately 10% of people age >55 years have asymptomatic peripheral atherosclerotic disease (PAD), 5% have intermittent claudication (IC, the cardinal symptom), and 1% critical leg ischemia (rest pain or gangrene). In patients with IC, worsening occurs in approximately 16%, bypass surgery is required in approximately 7%, and amputation in approximately 4%. Disease progresses more frequently in diabetics and in tobacco smokers. Patients with PAD suffer increased cardiovascular morbidity related mainly to coronary artery disease with a mortality rate of approximately 30% after 5 years. Compar

Negative pressure wound therapy: an evidence-based analysis.

OBJECTIVE: This review was conducted to assess the effectiveness of negative pressure wound therapy. CLINICAL NEED: TARGET POPULATION AND CONDITION Many wounds are difficult to heal, despite medical and nursing care. They may result from complications of an underlying disease, like diabetes; or from surgery, constant pressure, trauma, or burns. Chronic wounds are more often found in elderly people and in those with immunologic or chronic diseases. Chronic wounds may lead to impaired quality of life and functioning, to amputation, or even to death. The prevalence of chronic ulcers is difficult

In vivo study of the anti-inflammatory effect of Rhus toxicodendron.

BACKGROUND: Homeopathic Rhus toxicodendron (Rhus tox) is used in various inflammatory conditions. We screened its effect compared to succussed ethanol controls and appropriate active controls. METHOD: We initially experimented with Rhus tox 6, 12, 30 and 200 cH, using carrageenan-induced paw oedema in rats. The 6 cH dilution appeared most effective and was used in subsequent assays. We used pre-treatment and single treatment regimes in Wistar rats, and mice. RESULTS: We found significant reductions compared to control in carrageenan-induced paw oedema, vascular permeability, writhing induced

Combination therapy for footpad lesions in a captive Bengal tiger (Panthera tigris tigris).

A captive adult male Bengal tiger (Panthera tigris tigris) was treated for chronic footpad ulcers associated with pacing on concrete cage surfaces. Combination therapy of oral fluoxetine and acepromazine administration to diminate pacing behavior, daily application of moisturizing ointment, and oral vitamin E supplementation resulted in complete resolution of all footpad lesions. Subsequent clinical episodes completely resolved with similar treatments.

Early experience with cryopreserved arterial allografts in below-knee revascularization for limb salvage.

BACKGROUND: Below-knee revascularization for limb salvage in the absence of a suitable autogenous saphenous vein is a frequent challenge associated with a high amputation rate. The aim of this prospective study was to evaluate the usefulness of cryopreserved arterial allografts in such cases. METHODS: Arterial allografts were harvested from multiple organ donors and cryopreserved at -80 degrees C. From March 1993 to December 1997, 35 cryopreserved arterial allografts were used as below-knee bypasses for repeated limb salvage in 32 patients. There were 15 men and 17 women with a mean age of 75
