[Osteosynthesis using perpendicular pins in the treatment of fractures and malunions of the neck of the 5th metacarpal bone].

At the same time as the paper by P. Vives in July 1981, we proposed, in June 1981, a fixation technique with two perpendicular K-wires for correction of rotated malunions of fractures of the neck of the fifth metacarpal. Longitudinal rotation of the distal fragment, either in pronation or in supination, interferes with thumb-little finger opposition and grip. It cannot always be corrected by orthopaedic reduction and osteosynthesis is sometimes necessary: an intramedullary axial K-wire with a curved tip maintains the proximal fragment and reduces its angulation in flexion. A transverse K-wire,

Use of miniplates in the treatment of jaw fractures.

The use of miniplate-osteosynthesis in fracture treatment has been evaluated in 47 patients. In 25 of the patients plate fixation of the fracture was used without intermaxillary fixation and in 22 a supplementary IMF was used. Nine of the 47 patients had alcohol or drug addiction problems. The most common cause of fracture was assault and traffic accidents. The rate of complications including sensibility disturbances, occlusal disturbance and infection was evenly distributed among the two groups. The frequency of postoperative sensibility disturbances was however high, fifteen of the patients,

Distal humerus fractures treated with the anconeus-triceps lateral flap approach: a case series of 10 consecutive patients.

BACKGROUND: The surgical management of distal humeral fractures remains challenging. Posterior approaches involving the triceps mechanism are crucial for achieving stable fixation and functional recovery but are associated with potential approach-related complications. Traditional approaches often involve olecranon osteotomy to achieve adequate exposure, but this technique is associated with specific complications. The purpose of the present study is to describe the clinical outcomes and surgical rationale of the anconeus-triceps lateral flap (ATLF) approach applied to open reduction and inter

Bilateral talus fracture dislocation: is avascular necrosis inevitable?

Bilateral simultaneous fracture dislocation of the talus is a very rare injury. It occurs usually following high-velocity injuries. We report a 45-year-old man who presented with bilateral talar fracture dislocation following a heavy stone falling on him. On his right side, he had open talus dislocation with lateral process fracture and on his left side he had closed talar neck fracture dislocation. He underwent immediate debridement and external fixator application for his right side and open reduction and internal fixation for his left talus. At the end of 34 months' follow-up, he was able t

Time Reduction by Prebending Osteosynthesis Plates Using 3D-Printed Anatomical Models, In Patients Treated With Open Reduction and Internal Fixation.

INTRODUCTION: The incidence of facial bones fractures is 18 to 32 for each 100,000 inhabitants. The most affected population are young working people. Fractures are most commonly caused by assaults and motor vehicle accidents. Its cost of care reaches 1.06 billion dollars. Premodeling osteosynthesis plates with anatomical models can decrease surgical time, bleeding, and increase patient satisfaction. This study aims to evaluate the impact of premodeled osteosynthesis plates, using anatomical models in patients with facial fractures. MATERIAL AND METHODS: Patients with facial fractures treated

[Arthroscopically assisted treatment of ankle fractures].

Acute ankle fractures are one of the most common fractures in adults with an incidence of 0.1-0.2 % per year. Operative treatment by open reduction and internal fixation (ORIF) is the standard method of treatment for unstable or dislocated fractures. The main goal of the operation is the anatomical realignment of the joint and restoration of ankle stability; nevertheless, anatomical reduction does not automatically lead to favorable clinical results. According to several studies the mid-term and in particular the long-term outcome following operative treatment is often poor with residual sympt

Distal end radius fractures: evaluation of results of various treatments and assessment of treatment choice.

OBJECTIVE: The rapid expansion of knowledge regarding the functional anatomy of hand and wrist, increasing functional demands of senior citizens and improved methodologies of achieving and maintaining anatomic restoration of distal radius fractures has generated a renewed interest in addressing these fractures in a more precise manner. The purpose of our study was to evaluate the difference in patients function among those treated by 1) closed reduction and Plaster of Paris cast, 2) distractor application, or 3) open reduction and internal fixation with a volar plate, and to assess the treatme

Severe aseptic synovitis of the knee after biodegradable internal fixation. A case report.

Biodegradable rods made of polyglycolide (Dexon, Biofix) or lactide-glycolide copolymer (Vicryl) have been used for the past 5 years for internal fixation of a variety of fractures and osteotomies (Böstman et al. 1989, 1990b). Experience from using such rods for fixation of intraarticular osteochondral lesions seems to be less extensive. We report a case of severe synovial reaction to biodegradable rods used for fixation of osteochondritis dissecans of the knee and discuss possible reasons for the increased risk of foreign-body reactions when these rods are used intraarticularly.

Open Trimalleolar Fractures Treated with Ilizarov External Fixator.

BACKGROUND: Open trimalleolar fractures are some of the most severe fractures of the ankle joint region and some of the most difficult in trauma surgery. Repositioning with internal fixation is the most common surgical treatment method. There is little published data on primary repositioning and external fixation. MATERIAL AND METHODS: Five patients with open trimalleolar were fitted with Ilizarov external fixators in single-stage procedures. RESULTS: Full weight bearing was possible following surgical treatment in all five patients. None of the patients developed malunion, infections or vei

Fracture surgery of the extremities with the intra-operative use of 3D-RX: a randomized multicenter trial (EF3X-trial).

BACKGROUND: Posttraumatic osteoarthritis can develop after an intra-articular extremity fracture, leading to pain and loss of function. According to international guidelines, anatomical reduction and fixation are the basis for an optimal functional result. In order to achieve this during fracture surgery, an optimal view on the position of the bone fragments and fixation material is a necessity. The currently used 2D-fluoroscopy does not provide sufficient insight, in particular in cases with complex anatomy or subtle injury, and even an 18-26% suboptimal fracture reduction is reported for the

Radiological and Clinical Effectiveness of a Novel Calcaneal Fracture Brace after Intra-articular Calcaneal Fracture Surgery.

BACKGROUND: After calcaneal fracture surgery, a short leg splint and cast are typically applied. However, these restrict joint exercises, which is inconvenient for patients. In addition, there is a risk of complications, such as pressure ulcers or nerve paralysis with a short leg cast. In this study, we evaluated clinical and radiological outcomes of the use of a specially designed calcaneal brace after calcaneal fracture surgery. METHODS: From among patients who underwent open reduction and internal fixation for calcaneal fracture between July 9, 2013 and May 31, 2017, 102 patients who wore

Retromandibular transparotid approach for subcondylar mandibular fracture: A retrospective study.

INTRODUCTION: The retromandibular transparotid approach provides the shortest and the most direct access to mandibular sub-condylar fractures. However, this approach is less preferred due to the fear of facial nerve injury. The aim of this study was to evaluate the safety and morbidity of the retromandibular transparotid approach for open reduction and internal fixation of sub-condylar mandibular fractures. METHODS: A retrospective cohort study of 29 patients with 35 sub-condylar mandibular fractures who underwent open reduction and internal fixation through the retromandibular transparotid a

Good radiological and functional results after intramedullary nailing of distal radius fractures.

To evaluate the results after fixation of distal radius fractures using Micronail internal fixation, a prospective cohort study was performed, including displaced fractures of the distal radius (AO type A 2.2, A3.1, and A3.2). In total, 60 patients were included and 44 patients were available for 1-year follow-up (radiological and clinical examination) and 39 for 5-year follow-up (telephone interview). It was found that all fractures healed, with an average radial volar tilt of 2°, radial length of 11 mm, and a radial inclination of 22°. The functional result was excellent-to-good in 93% of th

Distal Phalanx Fracture of the Toe With Sesamoid Entrapment in the Interphalangeal Joint: A Case Report.

Phalangeal fractures are common among foot injuries, with the majority amenable to conservative treatment. This case involved a fracture of the toe combined with intra-articular sesamoid entrapment. To prevent nonunion, surgical intervention was recommended. We present a rare case of distal phalanx fracture of the toe with interphalangeal joint sesamoid entrapment, review the intraoperative challenges encountered, and summarize postoperative insights with recommendations. We report a 41-year-old female construction worker who presented with a crush injury to her right great toe. Diagnosis was

Intraoperative Tibia Fractures During Primary Total Knee Arthroplasty.

Intraoperative tibia fractures during primary total knee arthroplasty (TKA) are a known complication, however, the management options and outcome are poorly represented in the orthopedic literature. We performed a thorough literature review and detailed two cases of intraoperative tibia fractures during primary TKA at our institution to determine the mechanism behind these injuries and the optimal treatment strategy. We describe one case of intraoperative tibia fracture during primary posterior stabilized (PS) TKA and one case during primary cruciate retaining TKA. Open reduction and internal

Emerging Options for Biologic Enhancement of Stress Fracture Healing in Athletes.

In an era of continual single-sport specialization and year-round training, overuse injuries, including stress injuries of bone, are increasingly common. These injuries can be season- or even career-ending. For many elite and professional athletes, the traditional treatment strategy of immobilization and extended rest from sports participation is often not practical or acceptable. An understanding of modern strategies for evaluating and treating stress fractures is paramount for maintaining athletic participation and optimal athletic performance. This begins with the ability to categorize and
