Do minimally displaced, closed tibial fractures in children need monitoring for compartment syndrome?

INTRODUCTION: Acute compartment syndrome (ACS) may be seen following tibial fractures in adults. Although the risk of this complication is thought to be lower in children (especially in those under the age of 12 years) it is routine practice in many units to admit all children with this injury for observation. The aim of this study was to ascertain whether all children under the age of 12 presenting with fractures of the tibia merited admission and to provide recommendations on how to manage these patients based on our experience with this injury. PATIENTS AND METHODS: We retrospectively stud

Operative treatment of humeral shaft fractures. Comparison of plating and intramedullary nailing.

Plate and screw fixation (PSF) has always been the more common surgical treatment of humeral shaft fractures. However, intramedullary nailing (IMN) of the humerus has gained in popularity over the last two decades. The purpose of this retrospective study was to evaluate the clinical outcome of plate fixation versus intramedullary nailing of midshaft humeral fractures. The study included 91 patients treated at the department of Trauma Surgery of the University hospital of Leuven; 42 fractures had been treated by plate fixation and 49 by IM nailing. Union, functional outcome, possible complicati

The effect of ulnar styloid fractures on patient-rated outcomes after volar locking plating of distal radius fractures.

PURPOSE: Ulnar styloid fractures commonly occur with distal radius fractures (DRFs). Ulnar styloid fractures that involve the insertion of the radioulnar ligaments can cause distal radioulnar joint (DRUJ) instability, and the literature suggests that these fractures should be treated with open reduction internal fixation (ORIF). However, in the absence of DRUJ instability, the effects of ulnar styloid fractures are unknown. The purpose of this study is to evaluate the outcome of ulnar styloid fractures without DRUJ instability on patient-rated outcomes after DRF ORIF. METHODS: Between 2003 an

Nomenclature of frontobasal trauma: a new clinicoradiographic classification.

BACKGROUND: High-velocity accidents cause significant injury to the cranial and facial skeleton. Frontobasal fractures include fractures of the upper third of the face and anterior skull base. The pattern and classifications of these fractures are poorly understood at present. The authors have attempted a new comprehensive classification based on detailed clinicoradiographic evaluation. METHODS: A prospective study was undertaken in a tertiary care hospital in 1 calendar year (July of 2000 to June of 2001). All 63 patients with frontobasal fractures had detailed clinical and radiographic eval

Noncontiguous fractures of the femoral neck, femoral shaft, and distal femur.

BACKGROUND: Multifocal fractures of the femur are uncommon injuries and present unique management challenges. Combined ipsilateral fractures of the femoral shaft and femoral neck or the femoral shaft and distal femur have been described. The combination of noncontiguous ipsilateral femoral neck, femoral shaft, and distal femoral articular injuries, however, has not been described. The purposes of this report are to document the incidence and characteristics of this injury and to present a treatment rationale. METHODS: This was a retrospective study conducted at a Level I trauma center. RESUL

[Treatment of proximal ulnar comminuted fractures--a challenge].

INTRODUCTION: Comminuted fractures of the proximal ulna are severe injuries often complicated by involvement of the elbow joint and damaged soft tissues. The treatment of these fractures is very demanding and the results often mediocre due to of ancillary injuries (Dislocation in the humero-ulnar joint and associated radial head fractures). We analyzed the results of the treatment of these fractures and investigated whether other factors than the severity of fracture type influenced the final outcome. METHOD: Retrospective analysis of 18 patients with comminuted proximal ulna fractures. These

Combined Dorsal Spanning and Volar Plating for Management of Highly Comminuted Distal Radius Fractures.

BACKGROUND: Comminuted intraarticular distal radius fractures remain a surgical challenge, with no consensus on the optimal fixation method. Conventional techniques such as volar plating or external fixation may be insufficient to restore anatomical alignment and articular congruity in highly comminuted cases. This study evaluated the clinical and radiographic outcomes of combined dorsal spanning plate (DSP) and volar locking plate (VLP) fixation for management of comminuted distal radius fractures. METHODS: This prospective cohort study was conducted at a level I university trauma center bet

The Clinical and Functional Outcome of Extra-articular Distal Humerus Fracture Treated with Locking Compression Plate.

BACKGROUND: Extra-articular distal humerus fractures account for approximately 16% of all humerus fractures and 3% of adult fractures. These injuries pose significant treatment challenges due to the complex three-dimensional anatomy and the biomechanical demands placed on the distal humerus. OBJECTIVE: The objective of this study was to evaluate the clinical and functional outcomes of extra-articular distal humerus fractures treated with locking compression plates. MATERIALS AND METHODS: A prospective study was conducted on 90 patients with extra-articular distal humerus fractures who underw

A Rare Encounter: Unravelling the Enigma of a Bilateral Humerus Shaft Fracture With a Unique Mode of Injury.

Humerus bone fractures make up 4-6% of all adult fractures, of which proximal humerus shaft fracture is only six percent. Simultaneous occurrences of bilateral humerus shaft fractures are infrequently encountered in clinical practice. Precise statistics regarding these injuries are lacking, with scant documentation in the existing literature concerning the subject matter. These fractures may arise due to convulsions triggered by incidents like an electric shock, epilepsy, alcohol withdrawal, and hypoglycemia, which typically give rise to sudden and excessive muscular contractions. Such fractur

Comparing outcomes in the treatment of pediatric and adolescent intraarticular radial head fractures.

Intraarticular radial head (IARH) fractures are uncommon pediatric injuries with unpredictable yet poor outcomes. The aim of this study was to evaluate clinical outcomes of IARH fractures in pediatric and adolescent patients, by testing our hypothesis that surgically managed fractures would have less risk for an unplanned second surgery and better elbow range of motion at the final follow-up. A retrospective review of 53 IARH fractures was performed. Demographic and clinical data were recorded. Concomitant and associated injuries were documented. Initial management and any attempted reduction

Prescribing braces after forearm fractures does not decrease refracture rate.

Refracture is one of the most common complications of pediatric forearm fractures. One way to decrease this risk is to extend immobilization with a brace after the cast has been removed to allow for a range of motion exercises. The purpose of this study was to examine whether prescribing a brace after casting was discontinued decreased the risk of refracture. A retrospective, cohort study was performed at one level I trauma center. Girls under 10 years and boys under 12 years who sustained a forearm fracture from January 2013 to December 2018 were included. Patients with open fractures, fractu

Intramedullary fixation of distal fibular fractures: a systematic review of clinical and functional outcomes.

BACKGROUND: Ankle fractures are extremely common and represent nearly one quarter of all lower-limb fractures. In the majority of patients, fractures involve the distal fibula. The current standard in treating unstable fractures is through open reduction and internal fixation (ORIF) with plates and screws. Due to concerns with potentially devastating wound complications, minimally invasive strategies such as intramedullary fixation have been introduced. This systematic review was performed to evaluate the clinical and functional outcomes of intramedullary fixation of distal fibular fractures u

Early results with the new internal fixator systems LCP and LISS: a prospective study.

The Locking Compression Plate (LCP) and the Less Invasive Stabilisation System (LISS) are new implants with angular stability developed by the AO/ASIF. They offer an alternative for internal fixation in complex intra-articular fractures and high-energy diaphyseal fractures of the long bones. The new system was used in a prospective study to treat 113 patients (76 women, 37 men; average age: 53.1 years) with 122 injuries (multifragment shaft fractures or complex intra-articular, delayed/ non-union, malunion) from October 2002. Of these, 13 affected the humerus, 20 the radius, 2 the ulna, 21 the
