Dynamic profiling in inflammatory Bowel disease: A manifesto for personalized care.

Inflammatory bowel disease (IBD), encompassing Crohn's disease and ulcerative colitis, is a heterogeneous chronic condition whose clinical management has been advanced by the availability of molecularly targeted therapies. Despite this progress, marked inter- and intra-patient variability limits the effectiveness of a one-size-fits-all treatment approach. Precision medicine is a promising solution, but its implementation in routine care is limited. In this manifesto, we propose a multidimensional conceptual framework for the dynamic clinical profiling of IBD patients to be used in real-world c

Coupling DNA intercalation with redox catalysis: selective killing mechanism of glioblastoma by daidzin.

Glioblastoma (GBM) is recognized as one of the most treatment-resistant malignancies, owing to its reinforced DNA repair systems and limited drug accessibility across the blood-brain barrier. This study, identifies, daidzin (DZN), a naturally derived isoflavone, as a potent redox-active DNA intercalator that intrinsically combines physical intercalation with chemical reactivity to breach this resistance. Unlike traditional intercalators, DZN autonomously triggers destabilization of DNA helices by inducing torsional strain, thereby producing convergent strand and base lesions through photo-inde

Heart failure in the elderly: epidemiology, mechanisms, and management.

There is no consensus on an age cut-off for being considered elderly, but the majority of patients with heart failure (HF) have an advanced age. The lifetime risk for developing HF is ∼25%, with a sharp increase in incidence after the age of 70. The lifetime risk for men and women is almost equal, but women exhibit a higher propensity towards developing HF with preserved ejection fraction, whereas men are more prone to HF with reduced ejection fraction. During the biological ageing process, several systemic and local pathophysiological alterations impact the myocardium, including impaired auto

What matters most to the patient - a qualitative study of older patients in a geriatric ward.

INTRODUCTION: The ageing of the population is expected to increase the costs and consumption of health care. Care should be patient-centred. Patient-centred care (PCC) has been shown to increase the quality of life for older patients living with frailty, as well as reduce health care costs. As one of the core principles of PCC, it is important to investigate and consider patients' wishes regarding their care. AIM: The aim of this study was to investigate and identify factors and information that older patients find important when planning for their care. METHODS: Sixteen patients at the geri

Deciding between conservative kidney management and dialysis in older people with kidney failure: a narrative review.

The global population is ageing, and chronic kidney disease is becoming more common. As a result, increasing numbers of older adults with multimorbidity and frailty are progressing to kidney failure. This requires all healthcare professionals involved in the care of people with kidney failure to be able to provide careful guidance with regards to available treatment options. Choosing a treatment pathway for frail patients may be challenging, particularly for those considered ineligible for kidney transplantation. For such individuals, the remaining options are dialysis and conservative kidney

Indy reduction decreases aging-related dysbiosis in Drosophila.

Reduction in the Indy (I'm not dead yet) gene, a plasma membrane citrate transporter, in Drosophila and its homolog in worms extends lifespan by promoting metabolic homeostasis. Indy reduction delays the onset of aging-associated pathology in the fly midgut, including preservation of intestinal barrier integrity and intestinal stem cell homeostasis. Gut microbiota has broad impacts on host metabolism, health, and aging. Age-related dysbiosis impairs intestinal barrier function and drives mortality. However, the underlying mechanisms that link increased microbial load to frailty and negative ef

Beyond Histology: Tensiomyography as an Integrated Measure of Muscle Function in Osteoporotic and Osteoarthritic Patients.

Background/Objectives: Osteoporosis and osteoarthritis are age-related musculoskeletal disorders with a high socio-health burden, affecting both healthcare systems and individuals' quality of life. Both conditions are generally accompanied by a concomitant decline in muscle mass and strength, referred to as sarcopenia. In this context, tensiomyography emerges as a novel, non-invasive potential diagnostic strategy for assessing muscle quality, as this parameter influences the progression of both conditions. Methods: Histomorphometric and immunohistochemical analyses were performed on vastus lat

Amino Acid Intakes and Dietary Sources in a Nationally Representative Sample of Older Adults in Ireland: Findings from the National Adult Nutrition Survey (NANS).

Background/Objectives: The global population is ageing rapidly, with projections indicating that there will be over two billion individuals aged ≥60 years by 2050. Sarcopenia and frailty are major age-related syndromes associated with loss of muscle mass, reduced strength, and increased vulnerability, for which adequate protein and amino acid intake are key preventive factors. However, nationally representative data on dietary amino acid intakes and sources among older adults are lacking, particularly in Europe. Methods: This study aimed to address this gap by updating the Irish Food Compositi

The Association between Polypharmacy, Low Appendicular Skeletal Muscle Mass Index, and Physical Performance: A Cross-Sectional Study.

INTRODUCTION: In response to the rapidly growing elderly population, geriatric syndromes such as frailty, polypharmacy, and sarcopenia have gained significant attention due to their impact on aging-related health outcomes. To prevent functional decline, it is crucial to understand the relationship between physical performance and other potential confounding factors. This study aims to investigate how polypharmacy and other factors relate to physical performance. METHODS: This cross-sectional study included 445 participants aged over 20, surveyed via questionnaire. Polypharmacy was defined as

Colorectal cancer in Greenland.

INTRODUCTION: Distal radius fractures (DRFs) are common, with over 14,000 annual cases in Denmark. Most cases are treated non-surgically and may require closed reduction prior to casting. National guidelines for anaesthesia and reduction technique for closed reduction of DRFs are lacking, impacting clinical practices and potentially optimal care. This survey aimed to describe Danish clinical practice for initial treatment and closed reduction of DRFs. METHODS: Between January and April 2025, the survey was distributed online to the 23 physicians responsible for all 44 emergency departments an

[Expert consensus on the "Four-Level and Dual-Axis" comprehensive diagnostic and therapeutic system for prosthetic rehabilitation of oral diseases].

Traditionally, prosthodontics has adhered to a clinical diagnosis and treatment model centered on "tooth defects, partial edentulism, and complete edentulism", which show insufficient alignment with the modern "biopsychosocial" medical model. The demand of high-quality medical services continues to increase with the nation's growing prosperity and the continuous extension of average life expectancy. Facing these heightened medical demands, particularly the challenges of oral frailty and multimorbidity in an aging society, the traditional diagnostic and therapeutic system for prosthodontics inc

Immune activation from M. tuberculosis screening tests predicts mortality.

Impaired immune responses are a key feature of aging; however, there are few laboratory tests that link these responses to clinical outcomes. Interferon-gamma release assays (IGRAs) for tuberculosis screening quantify release of interferon-gamma by T-cells, and the difference between unstimulated and mitogen-stimulated T-cells is assessed for test validity. We assess this measure's relationship with all-cause mortality. We obtained the most recent negative and indeterminate outpatient IGRAs from a large health system along with demographics, frailty, lymphocyte count, and inflammatory markers.

Association of glucose-lowering agents with readmission risk in patients with rheumatoid arthritis and comorbid type 2 diabetes mellitus.

This study evaluated the clinical value of a "immuno-metabolic co-management" strategy by investigating the association between glucose-lowering agents (metformin, insulin, and acarbose) and readmission risk in patients with rheumatoid arthritis (RA) and comorbid type 2 diabetes mellitus (T2DM). This study included up to 10 years of longitudinal follow-up data from 616 patients with RA and comorbid T2DM. A shared Gamma frailty model was utilized to analyze unplanned readmission. Furthermore, inverse probability of treatment weighting (IPTW) was employed to control for confounding, followed by

Bone as a Biomarker of Ageing and Senescence: Connecting Musculoskeletal Health to Longevity via Diet, Nutrition and Exercise.

Advancing age is often accompanied by a gradual decline in musculoskeletal (MSK) health and integrity, making the skeleton not just a structural framework; but a dynamic indicator of biological ageing. Bone tissue undergoes continuous remodelling through tightly regulated interactions among osteocytes, osteoblasts, osteoclasts, progenitor cells or mesenchymal stem/stromal cells (MSCs) and growth factors. With advancing age, this balance shifts toward increased resorption, increased adipose tissue formation, reduced bone formation, and deterioration of bone microarchitecture. This eventually ca
