Conservative management of haemorrhagic cholecystitis, haemobilia with obstructive jaundice: a case report.

Haemorrhagic cholecystitis with haemobilia is a rare disease associated with high rates of morbidity and mortality if perforation and necrosis occur. Haemorrhagic cholecystitis and intrahepatic gallbladder perforation becomes hard to diagnose clinically and can be life threatening. This is a case of haemorrhagic cholecystitis, haemobilia with obstructive jaundice and intrahepatic gallbladder perforation in a COVID pneumonia patient. Emergency cholecystectomy in elderly (>70 yrs) is associated with high complication and mortality rates, therefore the decision whether or not to perform surgery s

Endocrine mucin-producing sweat gland carcinoma of the cheek.

Endocrine mucin-producing sweat gland carcinoma (EMPSGC) is a low-grade adnexal malignant neoplasm. We report a 90-year-old man who had a hard, dome-shaped tumor approximately 9 mm in diameter on the left cheek. Dermoscopy showed an overall, non-uniformly light-pink tumor with crust. The diagnosis of EMPSGC is made histologically from excisional biopsy. No signs of recurrent disease were evident at 42 months postoperatively.

Closure of a Difficult-to-Manage Chronic Pressure Injury with the Use of Autologous Platelet-Rich Plasma (APRP).

Pressure injury (PI) corresponds to a skin damage of ischemic aetiology that affects the integrity of the skin and is produced by prolonged pressure or friction between a hard internal and external surface. Treatment can be challenging when there is no resolution with usual care. The use of autologous platelet-rich plasma (APRP) gel arises as a therapeutic possibility in the presence of chronic pressure injuries. The case of a patient with chronic PI who has been treated with APRP is presented, achieving resolution of the lesion.

Mimicking uterine malignancy: Pelvic actinomycosis with giant uterine leiomyoma.

Pelvic actinomycosis is a rare disease which is hard to be distinguished from other diseases such as malignant tumors and tuberculosis due to its nonspecific clinical signs and symptoms. If pelvic actinomycosis can be diagnosed preoperatively, the patients can be cured with antimicrobial therapy avoiding surgery. It is especially of concern to distinguish pelvic actinomycosis from pelvic mass, if there is a history of intrauterine device use. We report a case of pelvic actinomycosis that was diagnosed after the postoperative pathology of a suspected uterine malignancy.

Technology You Can Swallow: Moving Beyond Wearable Sensors, Researchers Are Creating Ingestible Ones.

Around 6 p.m. each evening, the streets of Boston's suburbs come alive with the physically fit and those aspiring to be. They are runners, bikers, walkers, and scooter riders of all different body shapes and ages who would seem to have little in common except one thing-an electronic band wrapped around their wrist. For many of these people, it's hard to imagine life without the daily nagging from a personal health device to meet their daily prescript of 10,000 steps.

Self-injury in schizophrenia as predisposing factor for otomycosis.

Ear self-mutilation have been reported in schizophrenia, Mechanical damages to the auditory canal, like cleaning ear with hard and unsterile objects are predisposing factors of otomycosis. We present a case of repeated self-induced auricular trauma in schizophrenic patient. Aural swab were collected and examined by direct microscopy and culture, revealing Aspergillus flavus. A traumatized external ear canal skin can present favourable condition for fungal growth in the psychiatric population.

Fungus ball in the nasal cavity mimicking a rhinolith.

A 63-year-old man presented to the outpatient department of our tertiary care hospital, with unilateral left nasal obstruction, foul smelling nasal discharge and occasional mild epistaxis. On examination, a hard greyish black mass was noted in the anteroinferior portion of the inferior turbinate, with extension posteriorly into the inferior meatus with surrounding unhealthy granulation tissue. Provisional diagnosis of rhinolith was surprisingly reversed by histopathology, which suggested the presence of an Aspergillus fungus ball with Actinomyces colonisation.

Low Grade Fibromyxoid Sarcoma of the Palate: A Case Report.

Low-grade fibromyxoid sarcoma (LGFMS) are rare soft-tissue sarcomas that usually come to attention as slow growing masses. Males and females are affected approximately equally and typically arising in the soft tissue of the proximal extremities. Primary occurrence within the head and neck is exceedingly rare. We report a case arising from the hard palate of a 40-year-old male and according to our knowledge this is the first ever case of LGFMS involving the palate to be reported.

[Multidisciplinary treatment on vulvodynia--where is the evidence?].

Vulvodynia is a chronic pain syndrome in vulva with a prevalence of nearly 15%. Due to poorly understood pathophysiology and lack of efficient treatment frustration is vastly spread among affected women and their physicians. Behavioural, medical and surgical interventions are widely used, but hardly any of them show significance compared to placebo effect. A multidisciplinary treatment is considered to be the new state of the art treatment, but evidence for such a model is hard to find. This article is a review and evaluation of the many treatment models and their evidence.

Presurgical management of unilateral cleft lip and palate in a neonate: a clinical report.

A cleft lip and palate consists of fissures of the upper lip and/or palate, and is the most commonly seen orofacial anomaly that involves the middle third of the face. Early treatment of patients with a cleft lip and palate is important because of esthetic, functional, and psychological concerns. Nasoalveolar molding provides excellent results when started immediately after birth. This clinical report describes the presurgical management of an infant with a complete unilateral cleft of the soft palate, hard palate, alveolar ridge, and lip.

Bronchial dentures as a cause of airway actinomycosis.

A 65-year-old man was referred to the respiratory clinic with recurrent chest infections on a background of stage 3 chronic obstructive pulmonary disease. On examination, there was wheeze bilaterally more marked on the left lower lobe. Subsequent imaging revealed an obstruction of the left main bronchus that was concerning for malignancy. Initially, on flexible bronchoscopy, a hard mass was found and multiple biopsies were positive for actinomycosis. Subsequent rigid bronchoscopy was undertaken and a set of dentures were removed from the airway.

Closed hollow bulb obturator--one-step fabrication: a clinical report.

A method is described for the fabrication of a closed hollow bulb obturator prosthesis using a hard thermoforming splint material and heat-cured acrylic resin. The technique allowed the thickness of the thermoformed bulb to be optimized for weight reduction, while the autopolymerized seal area was covered in heat-cured acrylic resin, thus eliminating potential leakage and discoloration. This technique permits the obturator prosthesis to be processed to completion from the wax trial denture without additional laboratory investing, flasking, and processing.

Report of a deaf child with Tourette's disorder.

This is a case study of a deaf child with Tourette's Disorder (TD). Hearing parents and mental health clinicians unfamiliar with typical behaviors of deaf children may have difficulties differentiating the clinical presentation of symptoms of TD from the effects of deafness, as well as in implementing appropriate interventions. This case study reports the history, symptoms, diagnostic process, and treatment interventions. This is relevant for furthering the clinical knowledge of mental health professionals working with Deaf, deaf, and hard-of-hearing children and adolescents.

Diagnostic considerations of calcified lymph nodes.

Dentists are responsible for the diagnosis of abnormalities involving both hard and soft tissues in and around the oral cavity. In addition to a clinical examination, the radiographic evaluation will often highlight abnormalities within the radiographic field. A relatively uncommon radiographic abnormality is calcification of lymph nodes, which may be associated with a number of pathologic processes, both local and systemic. We present and discuss lymph node calcifications and the diagnostic considerations associated with them in order to assist practitioners in managing their patients appropr

Meconium periorchitis: a rare cause of fetal scrotal cyst--MRI and pathologic appearance.

A case of meconium periorchitis detected by fetal MRI and misdiagnosed during pregnancy as inguinoscrotal hernia is reported for the first time. A full-term black boy presented at birth with an asymptomatic, 'stony-hard', scrotal mass suggestive of an in utero testicular torsion or testicular/paratesticular tumor. Early surgical treatment resulted in the removal of paratesticular yellowish amorphous material. Histology was consistent with the diagnosis of meconium periorchitis, a rare and benign condition resulting from healed intrauterine bowel perforation.

Is One Dimension Enough? A Response to Simonton's Varieties of (Scientific) Creativity (2009).

We admire Simonton's valiant attempt to bring all of creativity under a single dimension. We offer a view that is sometimes complementary and sometimes distinct from Simonton's model: Our own multidimensional amusement park theoretical model, which tries to integrate domain-specific and domain-general points of view. We eagerly await future research on Simonton's model and are curious of how his hard-soft dimension can be included into the creativity pantheon.

[Cervicofacial actinomycosis: two case reports].

Infectious diseases transmitted by actinomycosis species cause severe destructive lesions. This rare and specific infection is mainly found in the orofacial regions. Causes of any hard tissue swelling in the jaw have, thus, to be assessed carefully. When actinomycosis is identified, a surgical intervention with curettement, draining and long-term antibiosis is required. The aim of the current article is to describe two clinical cases and to show the necessity of both, microbiological and histological laboratory diagnostics, to hedge the clinic diagnosis.

Soft tissue recession around implants: is it still unavoidable?--Part I.

When treatment with dental implants is indicated, an accurate diagnosis must be made to evaluate the clinical parameters and determine the optimal time for immediate or delayed (ie, early or late) implant placement and loading following tooth extraction. It is also important to identify complications and their implications on the aesthetic outcome. This article explains the behavior of the hard and soft tissue around the implant, evaluates the timing of implant placement after extraction, and reviews various parameters that influence tissue marginal remodeling.
