Optic nerve head edema among patients presenting to the emergency department.

OBJECTIVE: To determine the frequency of and predictive factors for optic nerve head edema (ONHE) among patients with headache, neurologic deficit, visual loss, or elevated blood pressure in the emergency department (ED). METHODS: Cross-sectional analysis was done of patients with ONHE in the prospective Fundus Photography vs Ophthalmoscopy Trial Outcomes in the Emergency Department (FOTO-ED) study. Demographics, neuroimaging results, management, and patient disposition were collected. Patients in the ONHE and non-ONHE groups were compared with bivariate and logistic regression analyses. RES

Method for retrospective estimation of natural head movement during structural MRI.

BACKGROUND: Head motion during brain structural MRI scans biases brain morphometry measurements but quantitative retrospective methods estimating head motion from structural MRI have not been evaluated. PURPOSE: To verify the hypothesis that two metrics retrospectively computed from MR images: 1) average edge strength (AES, reduced with image blurring) and 2) entropy (ENT, increased with blurring and ringing artifacts) could be sensitive to in-scanner head motion during acquisition of T1 -weighted MR images. STUDY TYPE: Retrospective. POPULATION/SUBJECTS/PHANTOM/SPECIMEN/ANIMAL MODEL: In al

Thirty-day readmission in patients undergoing head and neck microvascular reconstruction.

BACKGROUND: Characteristics of 30-day unplanned readmissions after head and neck microvascular reconstruction remain poorly understood. METHODS: We conducted a retrospective cohort of patients who underwent head and neck microvascular reconstruction between 2010 and 2015. RESULTS: The 30-day unplanned readmission rate was 13.0% (64/493). The most common readmission diagnoses were dehiscence, surgical site infection, or fistula (45.3%; n = 29). Of the readmissions, 46.9% (30/64) occurred within 7 days of discharge from the hospital. Risk factors for readmission on multivariable analysis inclu

Neck Kinematics and Electromyography While Wearing Head Supported Mass During Running.

BACKGROUND: Advanced combat helmets (ACH) coupled with night-vision goggles (NVG) are required for tactical athletes during training and service. Head and neck injuries due to head supported mass (HSM) are a common occurrence in military personnel. The current study aimed to investigate the effects of HSM on neck muscle fatigue that may lead to chronic stress and injury of the head and neck. METHODS: Subjects wore an ACH and were affixed with electromagnetic sensors to obtain kinematic data, as well as EMG electrodes to obtain muscle activations of bilateral sternocleidomastoid, upper trapezi

Radial Head Fracture Fixation Using Tripod Technique With Headless Compression Screws.

Radial head and neck fractures are one of the most common elbow fractures, comprising 2% to 5% of all fractures, and 30% of elbow fractures. Although uncomplicated Mason type I fractures can be managed nonsurgically, Mason type II-IV fractures require additional intervention. Mason type II-III fractures with 3 or fewer fragments are typically treated with open reduction and internal fixation using 2 to 3 lag screws. Transverse radial neck involvement or axial instability with screw-only fixation has historically required the additional use of a mini fragment T-plate or locking proximal radius

Ulnar head arthroplasty with posterior interosseous nerve neurectomy: A case report.

BACKGROUND: Distal Radioulnar joint (DRUJ) arthropathy can occur as a consequence of multiple mechanisms, including trauma, inflammatory or degenerative arthritis or even congenital anomalies. Ulnar head arthroplasty has been proposed as a viable therapeutic option, potentially restoring wrist function and improving pain control. This can also be achieved by complete wrist neurectomy but it has several risks associated with the procedure. A selective neurectomy may be more effective. CASE PRESENTATION: We report the case of a 51-year-old man, Tool-Colector, who suffered from constant diffuse

[Pathophysiology of aseptic femoral head necrosis: Pathogenesis and histopathological differential diagnosis].

BACKGROUND: Aseptic osteonecrosis is characterized by a complete death of the tissue (necrosis), more specifically, an ischemic necrosis of the lamellar bone tissue. The denotation aseptic refers to causal pathogenesis; therefore, it is not a matter of an infectious, septic-induced bone necrosis as in the case of acute infectious osteomyelitis. Formal pathogenesis leads to either (1) a hypoperfusion of the lamellar bone in the sense of an ischemic necrosis or (2) to directly induced damage of osteocytes and osteoblasts, which causes aseptic osteonecrosis. CAUSE: The causes of hypoperfusion/is

Insights About Head Lice Transmission From Field Data and Mathematical Modeling.

Head lice infest millions of school-age children every year, both in developed and developing countries. However, little is known about the number of lice transferred among children during school activities, because direct methods to study this are almost impossible to implement. This issue has been addressed following an indirect method, which consist in collecting data of real infestation from several children groups and using a mathematical model of lice colonies to infer how the infestation observed might have evolved. By determining the events that would most likely lead to infestations a

Ninety Day Mortality After Radical Radiotherapy for Head and Neck Cancer.

AIMS: Treatment for head and neck cancers using definitive radiotherapy, with or without chemotherapy, is associated with significant acute toxicity. Our aim was to assess 90 day mortality after radical radiotherapy. A further aim was to identify patient, tumour or treatment factors associated with early death after treatment and whether these could be used to predict outcomes. MATERIALS AND METHODS: In total, 1116 patients with squamous cell pharyngeal and larynx cancer between January 2011 and December 2015 were included. Patients with T1 larynx cancer were excluded. Patients were treated u

What makes a good head positioner for preventing occipital pressure ulcers.

Patients who are stationary endure prolonged soft tissue distortions and deformations at contact areas between their body and the support surface, which may lead to the onset of pressure ulcers (PUs) over time. A novel technology for patient positioning employs innovation in materials science, specifically viscoelastic materials with shape memory properties that compose the Z-Flo™ head positioner (Mölnlycke Health Care, Gothenburg, Sweden). Head positioners are generally known to reduce the occurrence of PUs in scalp tissues and the ears, but quantitative assessments of their biomechanical eff

Biological dose-escalated definitive radiation therapy in head and neck cancer.

OBJECTIVE: To compare treatment outcome of patients with head and neck (HN) tumours treated with definitive radiation therapy that, mainly owing to differences in the fractionation scheme used with simultaneous integrated boost techniques, resulted in a different biological dose. METHODS: 181 patients with HN cancer, prescribed to about 70.2 Gy in the primary tumour, were included in this study. Population cohort was divided into Group <70 and Group ≥70 when the mean dose converted to a 2 Gy fractionation in the primary tumour was less or higher than 70.2 Gy, respectively. The probability of

Robust head CT image registration pipeline for craniosynostosis skull correction surgery.

Craniosynostosis is a congenital malformation of the infant skull typically treated via corrective surgery. To accurately quantify the extent of deformation and identify the optimal correction strategy, the patient-specific skull model extracted from a pre-surgical computed tomography (CT) image needs to be registered to an atlas of head CT images representative of normal subjects. Here, the authors present a robust multi-stage, multi-resolution registration pipeline to map a patient-specific CT image to the atlas space of normal CT images. The proposed registration pipeline first performs an

Osteolysis in total hip arthroplasty after head and inlay revision surgery.

Wear debris seems to be the most likely reason for osteolysis in THA. The aim was to show the influence of head and acetabular liner revision surgery on osteolytic zones around the femoral component. Can head and inlay revision surgery reduce the size of the osteolytic zones around the femoral component? Which patients could benefit? 47 patients (51 hips), who had undergone THA head and inlay revision surgery between 1999 and 2011, were reviewed. The mean lifetime for the prosthesis was 15.1 years (8-26, standard deviation 4.5) and the mean follow-up for head and inlay was 39.2 months (12-113,

Predictors of long-term neurological outcomes in non-accidental head injury.

BackgroundNon-accidental head injury (NAI) is an inflicted injury usually on a child, often resulting in long-term neurological impairment and occasionally death. This study aimed to investigate the predictive values of acute findings, especially ocular, for long-term neurological outcomes.MethodsMedical records including retinal images of all children who attended the local Children's hospital with a diagnosis of NAI from over a period of 5 years were reviewed and data collected via the electronic patient record system. Patient demographics, injuries sustained, wide-field digital retinal imag

Increased Aldosterone Release During Head-Up Tilt in Early Primary Hypertension.

BACKGROUND: Hyperaldosteronism is well known cause of secondary hypertension. However, the importance of aldosterone for the much larger group of patients with primary hypertension is less clear. We hypothesized that in young subjects with primary hypertension, the rise of plasma aldosterone levels in response to head-up tilt testing as a stress stimulus is exaggerated. METHODS: Hemodynamics (blood pressure (BP), heart rate (HR), cardiac index (CI), and total peripheral vascular resistance index (TPRI), all by TaskForce monitor) and hormones (plasma renin activity (PRA), angiotensin II (Ang I
