[The consequences of a nurse's negative evaluation of a blood pressure reading. Implications for the etiology of white-coat hypertension].

The aim of this study was to show how a nurses negative evaluation of a blood pressure (BP) reading can cause an increase in BP in the following reading. Twenty-five subjects with normal pressure who had been informed that they were to participate in an epidemiological BP study took part and they were divided into two groups. After taking the first BP reading, participants from the experimental group were informed that their pressure was normal but high for their age, whereas those in the control group were told that their pressure was normal for their age. The results showed that this comment

Obstetricians' attitudes towards 'isolated' oligohydramnios at term.

Oligohydramnios at term in the absence of maternal and fetal complications poses a dilemma in management. It is one of the major indications for antenatal surveillance and induction of labour. There is a growing body of evidence that oligohydramnios in the absence of maternal or fetal risk factors ('isolated' oligohydramnios) is not associated with an adverse perinatal outcome. In this study, we surveyed the management of isolated oligohydramnios at term in a tertiary referral maternity unit. Out of 92 women who were scanned at term, 22 (24%) women had isolated oligohydramnios. A greater tende

Preparedness for a smallpox outbreak: comparing metrics for assessing levels of vaccination among health-care workers by state.

By mid-2005, less than 17% of smallpox vaccine doses distributed to American states for health-care workers (HCWs) during the CDC campaign had been used. To understand how states responded, vaccination patterns were studied. Metrics were calculated to compare the level of preparedness for a smallpox outbreak in terms of absolute numbers of HCWs vaccinated compared to the percentage of doses distributed to each state, the rate of vaccination per capita population, and the percentage of HCWs vaccinated compared to the number the CDC recommended. States were then ranked. Results showed that ranki

Vesiculobullous eruption from intense pulsed light treatment.

BACKGROUND: Intense pulsed light (IPL) systems emit non-coherent, polychromatic light and are increasingly used for various dermatologic indications. Although generally regarded as safe therapy, IPL is not without risk. OBJECTIVE: We report a 21-year-old woman who experienced a severe blistering eruption after IPL treatment by a nonphysician. MATERIALS AND METHODS: Case report. RESULTS: The patient reported a 10-year history of persistent redness on her medial and lateral cheeks. She had tried no previous oral or topical therapies. She was diagnosed with rosacea and was treated with IPL. Th

[Transsexualism as an interdisciplinary phenomenon].

The authors express their criticism on the currently prevailing Polish diagnostic and therapeutic criteria of transsexualism. Relying on their clinical experience and expertise (opinions for the court) as well as current literature, they point to the necessity of a discussion between specialists from various medical fields (psychiatry, sexology, urology, surgery, endocrinology, genetics) and humanistic sciences (psychology, sociology, law, ethics) on this interdisciplinary phenomenon.

Doctors' attitudes and practices regarding smoking cessation during pregnancy.

OBJECTIVE: To investigate the current smoking cessation practices and attitudes of doctors working in the public antenatal services, as well as their perceived barriers to addressing the issue in the context of routine care. STUDY DESIGN: The study was qualitative, consisting of 14 semistructured, one-to-one interviews with doctors purposefully sampled from 5 public sector hospitals in Cape Town, South Africa. RESULTS: The doctors in this study regarded HIV, poor nutrition, alcohol abuse and psychosocial stress as equal or higher risks to pregnant women than smoking. They tended to underesti

Smoking among Japanese nursing students: nationwide survey.

AIM: This paper reports a study estimating the nationwide prevalence of and attitudes towards smoking among Japanese nursing students. BACKGROUND: The World Health Organization established "World No Tobacco Day" in 1987, and has been promoting antismoking measures worldwide since then, with annual themes. It has emphasized that health care professionals, including nurses, as role models for healthy living, should not smoke, and that as promoters of health education they should not seem to justify or condone their patients' smoking. To promote antismoking measures among nurses, it will be nece

Scope for improving congenital cataract blindness prevention by screening of infants (red reflex screening) in a New Zealand setting.

OBJECTIVE: To assess the coverage and quality of routine red reflex screening in a single region and to consider the results in the context of the current New Zealand guidelines. METHODS: All the health practitioners partaking in routine Well Child - Tamariki Ora checks in the Nelson-Tasman region were asked to complete a simple one-page questionnaire on red reflex screening. RESULTS: Out of 127 providers potentially involved in routine infant examinations, the response rate was 92%. Red reflex screening was not being performed at the appropriate routine infant check with an ophthalmoscope b

Vaccinations for adult solid organ transplant recipient: current recommendations.

The need for immunization in solid organ transplant recipient can arise from three factors: immune deficit owing to underlying disease, rejection of the organ graft, and immunosuppressive therapy given after transplantation. As a general rule, primary immunizations should be given as early as possible before transplantation because the immune response to vaccines is decreased in patients with end-stage organ disease. There are three categories of vaccines: Live vaccines--oral polio, vaccinia, bacillus Calmette-Guerin, live oral typhoïd, and intranasal influenza vaccines--are contraindicated in

Physician attitudes regarding bacille Calmette-Guérin vaccination and treatment of latent tuberculosis infection.

OBJECTIVE: To investigate the attitudes of international medical graduates (IMGs) about treatment of latent tuberculosis infection (LTBI). DESIGN: Anonymous survey among physicians in training, all IMGs from TB endemic countries. Attitudes were assessed through hypothetical scenarios regarding the protective effect of BCG and LTBI treatment. RESULTS: Of 77 participants, 72.4% were male, the mean age was 31.4 years, and mean years post-graduation 8. Positive tuberculin skin tests (TSTs) were reported among 64.0%, and 89.6% had received BCG vaccine. Over a quarter of IMGs (27.4%) believed that

Healthcare worker safety is a pre-requisite for injection safety in developing countries.

Unsafe injection practices, including the re-use of unsterile needles and syringes, are commonplace in developing country health settings, and contribute substantially to the global burden of blood-borne viral disease. Unsafe injection practices place at risk not only patients, but also healthcare workers, who practice universal precautions inconsistently and are commonly exposed to blood in the course of their work. Global awareness of the link between unsafe injection practices and the burden of blood-borne viral disease was slow to emerge but has grown in the recent years. In 1999, the Worl

Specific immunotherapy with a standardized latex extract versus placebo in allergic healthcare workers.

BACKGROUND: The prevalence of allergy to natural rubber latex proteins has increased over recent years among healthcare professionals but also in children undergoing multiple operations. Exposure to the antigen mainly occurs through the respiratory mucosa and the percutaneous route. Clinical manifestations range from urticaria to angioedema, rhinoconjunctivitis, bronchial asthma, or anaphylactic shock. Preventive measures have been proposed to reduce the risk of sensitization by using only powder-free or synthetic gloves and latex-free material in operating units, but this is not always possib

Alison Bell Memorial Award. Latex sensitivity and allergy: fact and fiction.

Health care workers (HCW) have a higher incidence of latex sensitivity than car maintenance workers! The wearing of latex gloves over several years increases the risk of developing latex allergy! All glove related skin reactions are due to latex! People with spina bifida have a high incidence of latex sensitivity! There is a relationship between allergies to natural latex rubber and avocados, chestnuts and bananas! We should all stop using latex products! Powdered gloves are as safe as unpowdered ones, as long as they are rinsed well!
