Hydrocephalus in CINCA syndrome treated with anakinra.

INTRODUCTION: Chronic infantile neurologic, cutaneous, articular (CINCA) syndrome is a rare congenital autoinflammatory disease characterized by neonatal-onset chronic meningitis, hydrocephalus, sensorineural hearing loss, persistent urticarial rash, deforming arthritis, and recurrent fever. This clinical entity is believed to result from dysregulation of cytokine production. No recommended treatment protocol exists so far for CINCA syndrome. CASE REPORT: We report a 7-year-old child affected with CINCA syndrome in whom no therapy had resulted effective. Anakinra, an interleukin-1-receptor an

Cortical cognitive potentials in elderly persons.

The purpose of this study was to analyze the changes in the acoustically evoked cortical cognitive potentials N 200, P 300, and N 3 with age. There were 232 participants, who were 60 years or older, and each was examined using a battery of audiological tests including a questionnaire, otomicroscopy, pure-tone audiometry, and cortical cognitive potentials, measured at Fz using an auditory oddball paradigm. N 200 was elicited in 46.9%, P 300 in 45.1%, and N 3 in 52.2% of the elderly participants. The most significant predictors for presence of cortical responses were the participant's age and he

Circling, deafness, and yellow coat displayed by yellow submarine (ysb) and light coat and circling (lcc) mice with mutations on chromosome 3.

We describe here two mouse mutants, yellow submarine (Ysb) and light coat and circling (Lcc). Ysb arose as the result of insertions of a transgene, pAA2, into the genome. Lcc is an independent, radiation-induced mutation. Both mutants are characterized by recessive circling behavior and deafness, associated with a non-segregating, semi-dominant yellow coat color. Complementation tests showed that Ysb and Lcc are allelic. We attribute the yellow coat in Ysb and Lcc mice to the absence of black awl overhairs, increased agouti zigzag underhairs, and the presence of agouti awls with long subapical

[Tinnitus in systemic diseases].

Permanent or temporary degenerative changes in the internal ear causing tinnitus may occur with particular intensity in patients suffering from systemic disease (diabetes, hypertension, rheumatic diseases, kidney and thyroid gland diseases). Pathomechanisms of hearing impairment and the risk of tinnitus and its character in particular cases are discussed in the paper. The research was carried out on 1200 patients treated in the Laryngological Rehabilitation Centre in Poznań between 1.1.1998 and 04.2001 due to tinnitus. The analysis included the diagnosis of general health condition, general la

Screening in child health.

Screening programmes in child health have evolved on the basis of individual enthusiasm and professional consensus, rather than being based on objective evidence of benefit. Three reviews have been carried out in the UK over the past 10 years. The only programmes which show robust evidence of effectiveness are those for PKU and hypothyroidism. The value of screening for hearing loss and vision defects is widely accepted, but there are many unresolved issues. Programmes for detection of congenital dislocation of the hip, congenital heart disease and growth disturbances are of doubtful value. Ea

Transient hypopituitarism.

Spontaneous recovery of pituitary function in patients with hypopituitarism is rare. We report the case of a 30-year-old man in whom hypopituitarism and sudden onset of bilateral hearing loss developed after a viral infection. No evidence of a mass lesion was detected on computed tomography or magnetic resonance imaging of the pituitary gland. The patient received a 3-week course of treatment with a high dose of glucocorticoids; in a period of 6 weeks, he felt clinically well, and biochemical tests showed spontaneous return of normal anterior pituitary function. We speculate that an immune res

Sound perception induced by extracranial magnetic stimulation in deaf patients.

Two profoundly hard-of-hearing and deaf patients were examined by non-invasive extracranial magnetic stimulation (EMS) in an effort to determine whether EMS could evoke auditory sensations. The patients were fitted with standard earplugs and were stimulated at the auricle, the mastoid and the temporal lobe area. The threshold of auditory sensation (TAS) was determined at each stimulus position and found to be approximately 20-40% of the maximum EMS level (2.0 Tesla). The TAS was generally lowest in mastoid stimulation, but was variable, and dependent on the angle and position of the stimulatin

Autoimmune endolymphatic hydrops: five-year review.

This article reviews the authors' experience with autoimmune endolymphatic hydrops over the past 5 years. Twenty-seven of 52 patients with diagnosed autoimmune inner ear disease (52%) manifested endolymphatic hydrops, usually bilateral. Treatment consisted of low salt diet, diuretic, vestibular suppressant, and usually prednisone. Cytotoxic drugs and lymphocytoplasmapheresis were reserved for refractory cases. Audiometric followup (average, 23 months) was available in 12 patients. Using American Academy of Otolaryngology--Head and Neck Surgery guidelines, hearing improved or stabilized in at l

[Need for rheologically active, vasoactive and metabolically active substances in the initial treatment of acute acoustic trauma].

Two rheologically active and 8 vasoactive and metabolically active substances were compared in eight independent studies, some of which were randomised and double blind, on 400 patients who had suffered acute acoustic trauma. The control group was given saline. Spontaneous recovery was excluded as far as possible. The following substances were tested: Dextran 40, hydroxyethyl starch 40/0.5, naftidrofurylhydrogenoxalate, Vinpocetin, betahistine, pentoxifylline, flunaricine, Regeneresen AU 4 and 0.9% saline. All groups showed superior results to the control group in both long-term and short-term

Autoimmune reactivity in Ménière's disease: a preliminary report.

Autoimmune inner ear disease was first described by McCabe in 1979. The diagnosis is supported by cell mediated immune responses to inner ear membrane antigen stimulation. Meniere's disease consists of episodic vertigo, fluctuating sensorineural hearing loss, tinnitus and often aural pressure. Although most clinicians feel that Meniere's disease has no known cause, recently several researchers have suggested that some cases of Meniere's disease may be autoimmune in origin. In the present study, 10 patients with bilateral Meniere's disease were studied prospectively for autoimmune inner ear dis

Association between metabolic syndrome and hearing loss: The mediating role of retinol - A cross-sectional analysis of NHANES 2007 to 2018 (excluding 2013-2014).

Emerging evidence suggests associations between metabolic syndrome (MetS) and hearing loss, though population heterogeneity remains understudied. Given the high susceptibility of the inner ear to oxidative damage, fat-soluble antioxidants such as vitamins A, E, and carotenoids may exert protective effects. This study aimed to examine the MetS-hearing loss association among the U.S. adults and assess the potential mediating roles of these antioxidants. This cross-sectional study analyzed data from the U.S. National Health and Nutrition Examination Survey (2007-2018, excluding 2013-2014). Weight

Speech-in-noise difficulties in aminoglycoside ototoxicity reflects combined afferent and efferent dysfunction.

OBJECTIVES: Patients with Cystic fibrosis (CF) often receive aminoglycosides (AGs) to manage recurrent pulmonary infections, placing them at risk for ototoxicity. Chronic AG use can lead to complex cochlear damage affecting inner and outer hair cells, the stria vascularis, and spiral ganglion neurons. The greatest damage is typically in the basal cochlear region, which encodes high-frequency hearing, with additional involvement of more apical regions. While extended-high-frequency (EHF) hearing loss (EHFHL; 9-16 kHz) is often the earliest sign of AG ototoxicity, speech in noise (SiN) effects a

The Relationship Between Linguistic Input and Communication Outcomes in Children With Unilateral Hearing Loss: The Impact of Device Fitting.

OBJECTIVES: Although the benefits of hearing devices in children with bilateral hearing loss are well established, evidence in unilateral hearing loss (UHL) remains limited. This study aimed to (i) examine whether device fitting influences the relationship between linguistic input and communication outcomes (language and functional listening) in young children with UHL, and (ii) assess whether consistency of device use affects this relationship. DESIGN: A longitudinal study included 43 device-fitted and 61 non-fitted children with UHL at 12 mo (mean worse-ear hearing level: 80.9 versus 86.7 d

Synergistic role of blood-labyrinth barrier permeability and endolymphatic hydrops: a comparative perspective in Ménière's disease and vestibular migraine.

OBJECTIVE: This study compares the delayed gadolinium-enhanced MRI characteristics of Ménière's disease (MD) and vestibular migraine (VM) to develop a multiparametric model that incorporates endolymphatic hydrops (EH), blood-labyrinth barrier permeability, and their asymmetry. Additionally, it investigates the correlations between these imaging features and hearing loss across various frequencies. METHODS: A total of 79 patients-47 with MD and 32 with VM-were enrolled in the study between June 2023 and April 2025. All participants underwent a comprehensive medical history assessment, neurotol

Impact of Low-Level Blast Exposure From Military Training and Career Cumulation on Hearing Outcomes.

INTRODUCTION: Occupational trades expose military service members to unique sound exposure profiles, distinct in frequency and amplitude to those commonly studied in industrial settings. Characteristics of the energy exposure incurred through blast exposure exercises a distinct mechanism of injury in the auditory system that yield acute and cumulative health impacts. The following paper considers physiological and self-reported data attributed to repeated low-level blast exposure. We focused our analysis to examine objective audiological metrics of blast overpressure damage to quantify the imp

A Novel Method to Determine the Maximum Output of Individual Patients for an Active Transcutaneous Bone Conduction Implant Using Clinical Routine Data.

OBJECTIVES: The maximum output provided by a bone conduction (BC) device is one of the main factors that determines the success when treating patients with conductive or mixed hearing loss. Different approaches such as sound pressure measurements using a probe microphone in the external auditory canal or a surface microphone on the forehead have been previously introduced to determine the maximum output of active transcutaneous BC devices that are not directly accessible after implantation. Here, we introduce a method to determine the maximum output hearing level (MOHL) of a transcutaneous act

The Potential Anticancer Activity of 5-Fluorouracil Loaded in Cellulose Fibers Isolated from Rice Straw.

BACKGROUND: Over-the-counter hearing aids (OTC HAs) are approved for adults with mild-to-moderate hearing loss (HL) without professional assistance, but research comparing their effectiveness to professional-fit is limited. PURPOSE: We compared the effectiveness of OTC HA fittings between self-fit and professional-fit groups and examined how the group effects depend on the age, sex, technology proficiency, HL, and HA history. RESEARCH DESIGN: At a university audiology clinic, 71 adults were randomly assigned to self-fit or professional-fit groups and fitted bilaterally with OTC HAs. The self

The Effect of Cochlear Damage on the Sensitivity to Harmonicity.

OBJECTIVES: A sum of simultaneous pure tones with harmonic relationships (i.e., simple frequency ratios) is normally heard as a single sound, with a single pitch, even when its components are fully resolved in the auditory periphery. This perceptual phenomenon called "harmonic fusion" is thought to play an important role in auditory scene analysis as listeners often have to segregate simultaneous harmonic sounds with different fundamental frequencies. The present study explored the consequences of mild or moderate cochlear hearing loss for the sensitivity to harmonicity and the detection of in

Vibrant Soundbridge for hearing restoration after chronic ear surgery.

INTRODUCTION: Middle ear surgery is primarily concerned with resolving the discharging pathology, in the case of chronic otitis media (COM), or with complete eradication, in case of cholesteatoma. Either of these procedures may require repeated surgeries, often resulting in severe mixed hearing impairment. A middle ear implant may be indicated in these cases instead of a hearing aid because the anatomical conditions in such cases often impede an adequate acoustic coupling. The objective of this study was to evaluate MED-EL Vibrant Soundbridge (VSB) implantation in patients with severe conducti
